1.Acute abdominal pain from ovarian endometriosis

Endometriosis is a common, estrogen-dependent, inflammatory, gynecologic disease process in which normal endometrial tissue is abnormally present outside the uterine cavity. [1] Endometriosis is a common cause of chronic pain, dyspareunia, dysmenorrhea, and infertility. Most commonly, endometriosis is found within the pelvis, specifically on the ovaries. Because of rupture, bleeding, infection, or torsion, ovarian endometriosis (OMA) may cause acute abdominal pain, which is similar to acute abdominal pain caused by other reasons and is not easy to diagnose.[2,3] Determining the clinical and pathological features of OMA is crucial for accurate assessment, diagnosis, and treatment.
2.Pachymic acid exerts antitumor activities by modulating the Wnt/β-catenin signaling pathway via targeting PTP1B

Objective: To determine the inhibitory effects of pachymic acid on lung adenocarcinoma (LUAD) cells and elucidate its underlying mechanism. Methods: CCK-8, wound healing, Transwell, Western blot, tube formation, and immunofluorescence assays were carried out to measure the effects of various concentrations of pachymic acid on LUAD cell proliferation, metastasis, angiogenesis as well as autophagy. Subsequently, molecular docking technology was used to detect the potential targeted binding association between pachymic acid and protein tyrosine phosphatase 1B (PTP1B). Moreover, PTP1B was overexpressed in A549 cells to detect the specific mechanisms of pachymic acid. Results: Pachymic acid suppressed LUAD cell viability, metastasis as well as angiogenesis while inducing cell autophagy. It also targeted PTP1B and lowered PTP1B expression. However, PTP1B overexpression reversed the effects of pachymic acid on metastasis, angiogenesis, and autophagy as well as the expression of Wnt3a and β-catenin in LUAD cells. Conclusions: Pachymic acid inhibits metastasis and angiogenesis, and promotes autophagy in LUAD cells by modulating the Wnt/ β-catenin signaling pathway via targeting PTP1B.

4.Role of PD-L1 on promoting epithelial-mesenchymal transition by activating NF-κB in endometrial cancer

Objective To investigate the effect of programmed death ligand-1(PD-L1)overexpression on epithelial mesenchymal transformation(EMT)in endometrial cancer cells and its possible mechanism.Methods Ishikawa/PD-L1,a PD-L1 stable overexpression cell line constructed in the laboratory and the control Ishikawa/EV were used.The efficiency of PD-L1 overexpression was detected by qPCR and Western blot assay.The cell migration ability and invasion activity were detected by scratch assay and Transwell assay.The EMT-related protein and the phosphorylation level of nuclear transcription factor-κB(NF-κB)were detected by Western blot.Results Compared with the control group,the migration ability and invasion activity of Ishikawa/PD-L1 were significantly enhanced,and the expression of E-cadherin was significantly down-regulated,whereas vimentin,N-cadherin and p-p65 were significantly increased.Conclusion PD-L1 can promote EMT by inducing the activation of NF-κB pathway,and thus enhance the migration and invasion ability of endometrial cancer cells.
5.Detection of serum allergens in children with allergic rhinitis in Hangzhou

Objective To analyze the distribution of common allergens in children with allergic rhinitis(AR)in Hangzhou,and to provide reference for its prevention and treatment.Methods From January 2020 to December 2022,13 521 children who were diagnosed with AR and underwent in vitro serum specific immunoglobulin E(sIgE)detection in Affiliated Hangzhou First People's Hospital,School of Medicine,Westlake University were selected as the research objects.The positive rates of allergens in children with AR of different genders,ages and seasons were compared.Results Among 13 521 children,the positive rate of sIgE was 54.49%(7367/13 521),of which 64.82%were allergic to only one allergen.Among the 19 common allergens,the top three overall positive rates were household dust mite(44.27%),milk(14.62%),and mixed grass(4.82%).The positive rates of allergens of house dust mites,cat hair dander,mulberry,mold combination,tree pollen combination,amaranth,cockroach,milk,shrimp,crab,pineapple,shellfish and mango in male children were significantly higher than those in female children(P<0.05).The positive rates of allergens of house dust mite,mixed grass,cat hair dander,mulberry,mold combination,egg white,shrimp,crab,pineapple,cashew nuts and shellfish in different age groups were statistically significant(P<0.05).July and August were the two months with the highest allergen positive rate.The allergen positive rates of house dust mite,dog and cat hair dander were the highest in summer,and the allergen positive rate of plants was consistent with the flowering season.The sIgE concentration of house dust mite(77.87%)was mainly grade 3 and above.The sIgE concentration of other allergens was mainly grade 1 and 2.Conclusion Dust mites and milk are the most common allergens in children with AR in Hangzhou,and the distribution of allergens varies with gender,age and season.Allergen detection can provide a more accurate plan for the diagnosis,prevention and treatment of AR.
6.Severe disseminated intravascular coagulation complicated by acute renal failure during pregnancy

Disseminated intravascular coagulation (DIC) is a clinical syndrome caused by various etiologies and characterized by systemic activation of blood coagulation, leading to vessel thrombosis, organ dysfunction, and severe bleeding.[1] DIC represents a life-threatening condition that is the endpoint of uncontrolled systemic activation of the disease. Once it enters the stage of malignant DIC, the patient’s death becomes unavoidable. DIC always occurs as a secondary disorder and is commonly associated with postpartum hemorrhage, followed by hypertensive disorders, acute fatty liver of pregnancy, sepsis, and amniotic fluid embolism (AFE).[2] In clinical obstetrics, DIC often occurs during delivery or postpartum, while cases of prenatal acute DIC as the first manifestation are uncommon. Patients with DIC have higher rates of multiple organ dysfunction syndrome, including acute renal failure (ARF), respiratory failure, and disturbance of consciousness.
7.Construction and Practice of Cough and Asthma Pharmacy Clinic

OBJECTIVE To explore the practice mode of cough and asthma pharmacy clinic in Affiliates Hangzhou First People's Hospital, Zhejiang University, School of Medicine. METHODS To set up a cough and asthma pharmacy clinic, equipped with corresponding hardware and software facilities, and establish a cough and asthma pharmaceutical care mode. From December 1, 2020 to August 31, 2022, 2 357 patients in the hospital cough and asthma pharmacy clinic were collected, and their general data were analyzed. The patients learned by themselves according to the inhalation video, and then were guided by pharmacists to evaluate the differences in the use skills of patients before and after the intervention of pharmacists. The 345 patients with chronic obstructive pulmonary disease who were treated in the cough and asthma pharmacy clinic from December 1, 2020 to July 31, 2021 were collected. They were divided into 30 d follow-up group(n=173) and 15 d follow-up group(n=172) according to the blind sampling method. The score difference of Morisky medication Compliance Scale(MMAS-8) at 1, 3, 6 and 12 months of follow-up was compared. RESULTS Among 2 357 patients, 1 213(51.46%) were males and 1 144(48.54%) were females, 1 309 cases(55.54%) aged ≥60 years old, 563 cases(23.89%) aged 40-59 years old, 402 cases(17.06%) aged 20-39 years old, 32 cases(1.36%) aged 14-20 years old, and 51 cases(2.16%) aged<14 years old. The main diseases of the patients were chronic obstructive pulmonary disease(55.38%) and asthma(20.73%). Budesonide and Formoterol Fumarate powder for inhalation(37.08%) and Budesonide, Glycopyrronium Bromide and Formoterol Fumarate Inhalation Aerosol(25.84%) were frequently used. On the basis of patients' video teaching, pharmacists' mastery of inhaler use skills was better than that of patients' self-learning through video. The score before intervention was 5.360±1.208 and that after intervention was 7.890±0.356. There was significant difference between the two groups(P<0.05). At a month of follow-up, the MMAS-8 score of the 15-day follow-up group was 6.310±1.079. The MMAS-8 score of the 30 d follow-up group was 5.250±0.977, there was a significant difference between the two groups(P<0.05), but there was no significant difference in the MMAS-8 score of the two groups at 3, 6, 12-month follow-up. CONCLUSION The establishment of cough and asthma pharmacy clinic promotes the integration of pharmacy and respiratory clinical discipline, and improves the medication compliance of patients.

We aimed to evaluate the effectiveness and safety of single-course initial regimens in patients with low-risk gestational trophoblastic neoplasia (GTN). In this trial (NCT01823315), 276 patients were analyzed. Patients were allocated to three initiated regimens: single-course methotrexate (MTX), single-course MTX + dactinomycin (ACTD), and multi-course MTX (control arm). The primary endpoint was the complete remission (CR) rate by initial drug(s). The primary CR rate was 64.4% with multi-course MTX in the control arm. For the single-course MTX arm, the CR rate was 35.8% by one course; it increased to 59.3% after subsequent multi-course MTX, with non-inferiority to the control (difference -5.1%,95% confidence interval (CI) -19.4% to 9.2%, P = 0.014). After further treatment with multi-course ACTD, the CR rate (93.3%) was similar to that of the control (95.2%, P = 0.577). For the single-course MTX + ACTD arm, the CR rate was 46.7% by one course, which increased to 89.1% after subsequent multi-course, with non-inferiority (difference 24.7%, 95% CI 12.8%-36.6%, P < 0.001) to the control. It was similar to the CR rate by MTX and further ACTD in the control arm (89.1% vs. 95.2%, P =0.135). Four patients experienced recurrence, with no death, during the 2-year follow-up. We demonstrated that chemotherapy initiation with single-course MTX may be an alternative regimen for patients with low-risk GTN.

Objective:
To investigate the incidence of postoperative pancreatic fistula (POPF) and its risk factors after radical gastrectomy.
