1.Clinical features and outcomes of ischemic stroke among young Filipino adults.

BACKGROUND AND OBJECTIVE
The incidence of ischemic stroke typically increases with age; however, recent studies have shown a concerning trend of stroke cases among adults under the age of 45. This neurologic condition is called “Stroke in the Young” (SITY). SITY poses public health concerns due to its long-term consequences on individuals and their families. Despite significant impact, published literature on SITY among Filipinos is scarce. Given the potential differences in genetic background and lifestyle, the clinical characteristics and outcomes of SITY Filipinos may vary considerably from other populations. Therefore, the aim of this study is to describe the clinical features and outcomes of ischemic SITY Filipinos.
METHODSThe study was a two-center, five-year retro- spective cohort design involving 19- to 45-year-old patients admitted between January 1, 2017, and December 31, 2022, diagnosed clinically and radiologically with ischemic stroke for the first time. Medical records were reviewed, including demographic data, stroke symptoms, cardiovascular or non-cardiovascular risk factors, and laboratory results. Ischemic stroke subtypes were categorized into cardioembolic, small artery occlusion, stroke of other determined causes, and stroke of undetermined cause through the Trial of ORG 10172 in Acute Stroke Treatment (TOAST) classification. Functional outcomes on hospital discharge were assessed by the Modified Rankin Scale (mRS). All data were analyzed using descriptive statistics in the Statistical Package for the Social Sciences (SPSS software, version 29).
RESULTSA total of 205 cases of ischemic SITY were chart reviewed. The mean age was 37.30, with a female predominance of 68.3%. The most reported cardiovascular risk factors were obesity (56.6%), hypertension (51.2%), heavy alcohol consumption (36.5%), and diabetes mellitus type 2 (19.5%). Concurrently, the non-cardiovascular risk factors identified were pregnancy, particularly in the postpartum period (4.8%), use of estrogen-containing pills (4.8%), and migraine without aura (4.4%). Based on TOAST classification, small vessel occlusion (42.1%) and large artery atherosclerosis (30.2%) were the most frequent ischemic stroke subtypes of SITY Filipino females. Mostly showed no symptoms of disability (35.1%) on hospital discharge.
CONCLUSIONThis study highlights the difference in the clinical profile of young Filipino adults with ischemic stroke. Contrary to previous studies, ischemic stroke was more predominant among young females. Aside from hypertension, obesity has emerged as the leading cardio- vascular risk factor for ischemic SITY. Moreover, noncardiovascular risk factors, specific to females (pregnancy, use of estrogen-containing pills, and migraine), were also identified in the study. With regards to stroke subtypes, small vessel occlusion and large artery atherosclerosis were frequently seen in young female patients. These f indings suggest a need for gender-specific approaches in the evaluation, management, and prevention of ischemic SITY.

2.Takayasu arteritis in a 41-year-old Male presenting as acute myocardial infarction and ischemic stroke: A case report.

INTRODUCTION
Takayasu arteritis (TA) is a rare chronic large vessel vasculitis that affects the aorta and its major branches with a median age of onset of 25 years. The disease has a worldwide incidence of 1-2 per million, primarily affecting females with a 9:1 ratio. It is considered as an autoimmune disease that leads to progressive vessel thickening and stenosis, or aneurysmal dilatation. Coronary artery involvement is observed in 5.9%-58.2% of TA cases. We present a case of TA in a Filipino male presenting concurrently with myocardial infarction (MI) and ischemic stroke.
CASE REPORTA 41-year-old Filipino male smoker with hypertension presented with chest pain, left-sided paresthesia and hemiparesis. Initial assessment revealed differential blood pressure between the arms, sensory and motor deficits, and abnormal ABI. Electrocardiogram confirmed anteroseptal ST-elevation MI and cranial computed tomography (CT) showed ischemic stroke. Arterial duplex scan had findings suggestive of hemodynamically significant lower extremity stenosis. A CT aortogram revealed multiple occlusions, including in the left subclavian artery, suggesting TA. Coronary angiography was attempted but was deferred due to peripheral arterial occlusion. A CT coronary angiogram revealed severe stenosis of the left anterior descending artery and moderate stenosis of the other coronaries. The patient was treated with dual antiplatelet therapy, statins, anticoagulation, corticosteroids and methotrexate. He experienced significant improvement in neurological symptoms and was chest pain-free upon discharge. At the 1-month follow-up, the patient remained asymptomatic.
DISCUSSIONCoronary involvement in TA can manifest as angina, MI, or other coronary lesions. The coexistence of MI and ischemic stroke in the same event is rare. Traditional risk factors for ischemic heart disease (IHD) in this patient such as hypertension and smoking may have contributed to the presentation, though TA itself is known to accelerate atherosclerosis. Limited vascular access hindered coronary intervention in this case and revascularization strategies remain challenging in active TA. The formation of extensive collateral arteries, along with early initiation of immunosuppressive therapy, likely contributed to the patient’s survival.
CONCLUSIONThis case illustrates a rare and complex case of TA in a male patient with concurrent MI and ischemic stroke. Although coronary revascularization was not pursued due to occluded access, immunosuppressive therapy successfully managed the patient’s condition. Extensive collateral artery formation and early therapeutic intervention were key factors in the patient’s favorable outcome.

In this issue, we feature the Executive Summary of the 2024 Clinical Practice Guidelines on the Management of Acute Ischemic Stroke and Intracerebral Hemorrhage in the Philippines, an important resource for promoting evidence-based, patient-centered care and improving stroke outcomes.

Despite being the most common neurological disease, wide variations in management persist among clinicians. This CPG aims to provide evidence-based recommendations for the diagnosis and management of acute stroke.
METHODSA systematic synthesis of evidence was conducted to address the diagnosis and management of both acute hemorrhagic and ischemic stroke. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach was followed. The process included: (1) generation of critical questions and outcomes (2) retrieval of current and relevant evidence, (3) synthesis and appraisal of the evidence base, (4) formulation of draft recommendations, (5) deliberation by a multi-sectoral stakeholder panel on feasibility, values and preferences to assess the strength of recommendations and (6) planning for dissemination, implementation, impact evaluation and updating.
RESULTSSixteen clinical questions were addressed leading to 21 recommendations. Most were strong recommendations supported by moderate to high certainty of evidence. The recommendations span the spectrum of acute stroke care, including early diagnosis with imaging, admission to stroke units, reperfusion therapies, surgical options, prevention of complications and the role of adjunctive treatments.
CONCLUSIONThe recommendations in this CPG are intended to guide clinicians and other stakeholders in the diagnosis and management of acute stroke. They are meant to supplement, not replace, the sound clinical judgment of healthcare providers.

7.Recurrent Ischemic Stroke Two Decades After Craniospinal Radiotherapy for Suprasellar Germinoma: A Case of Suspected Radiation-Induced Cerebrovascular Disease

Introduction:
Cranial radiotherapy is an established treatment for
intracranial germinoma but may lead to late cerebrovascular
complications including radiation-induced vasculopathy,
cavernoma formation, and premature intracranial
atherosclerosis. These complications may manifest many
years after treatment and represent an important cause of
stroke in young adult survivors of brain tumors.
Case:
We report a 39-year-old male with a history of suprasellar
germinoma treated with transsphenoidal surgery followed
by craniospinal radiotherapy in 2006. He subsequently
developed panhypopituitarism requiring lifelong hormone
replacement therapy. In October 2024, he experienced
a cerebrovascular accident resulting in mild residual
right-sided weakness. Brain magnetic resonance imaging
demonstrated multifocal old infarcts, small vessel disease
(Fazekas grade 2), and multiple susceptibility artifacts
suggestive of radiation-induced cavernomas.In March 2026, he presented with new onset right-sided
weakness and facial asymmetry. Initial neurological
examination revealed right-sided motor power of 4/5.
Computed tomography of the brain showed no acute
intracranial hemorrhage but confirmed multiple old infarcts
and small vessel ischemic changes. Computed tomography
angiography revealed intracranial atherosclerotic disease
with 25–50% stenosis of the cavernous segments of both
internal carotid arteries and mild irregularities of posterior
circulation vessels without evidence of large vessel
occlusion or aneurysm.
Radiation-induced vasculopathy is a recognized delayed
complication of cranial irradiation, often presenting 5–25
years after treatment. Pathophysiological mechanisms
include endothelial injury, accelerated atherosclerosis, and
progressive arterial stenosis. In rare cases, cranial irradiation
may also lead to secondary Moyamoya syndrome,
characterized by progressive stenosis of the intracranial
internal carotid arteries with development of collateral
vessels. Although this patient’s vascular imaging did not
demonstrate the characteristic collateral network seen in
Moyamoya syndrome, it remains an important differential
diagnosis in young patients presenting with recurrent
stroke after cranial irradiation. In this patient, young age,
prior cranial irradiation, multifocal infarcts, and coexisting
radiation-induced cavernomas strongly suggest radiationrelated cerebrovascular disease as a major contributor to
recurrent stroke.
Conclusion
Long-term survivors of intracranial germinoma treated
with cranial radiotherapy remain at risk of delayed
cerebrovascular complications. Early recognition and
aggressive secondary stroke prevention, along with
continued neurological surveillance, are essential to reduce
morbidity in this population.

8.Acute Ischemic Stroke Masking Underlying Hyperthyroidism: A Diagnostic Pitfall of Non-Thyroidal Illness Syndrome

Introduction:
Non-thyroidal illness syndrome commonly occurs during
acute systemic illnesses and is characterized by suppressed
or inappropriately normal thyroid-stimulating hormone
with low or low-normal thyroid hormone levels. This
biochemical pattern may obscure or delay the diagnosis
of underlying hyperthyroidism, especially in mild or
borderline cases. Acute stroke is a recognized trigger of
non-thyroidal illness syndrome, yet its masking effect on
hyperthyroidism is under-recognized in clinical practice.
Case:
A 58-year-old Malay male with cardiovascular risk factors
presented with an acute ischemic stroke involving the left
occipital lobe and right internal capsule. He developed
new-onset atrial fibrillation (CHA₂DS₂-VASc 3) and
mild left ventricular systolic dysfunction. Initial thyroid
function tests during the acute stroke phase showed mildly
suppressed thyroid-stimulating hormone (TSH) (0.30
mIU/L) with high-normal free T4 (21.2 pmol/L), interpreted
in the context of acute illness. The patient was clinically
stable without overt thyrotoxic features.
One month post-stroke, repeat testing revealed further TSH
suppression (0.09 mIU/L) and rising free T4 (27.6 pmol/L).
Detailed history uncovered prior Graves’ disease in 2019
with treatment default. Examination revealed a small
diffuse goiter without ophthalmopathy. The biochemical
evolution following recovery from acute illness confirmed
recurrent hyperthyroidism previously masked by nonthyroidal illness syndrome.
Conclusion
Acute stroke can induce cytokine-mediated suppression
of the hypothalamic-pituitary-thyroid axis and altered
peripheral deiodination, leading to misleading thyroid
function tests. In this case, non-thyroidal illness syndrome
blunted the biochemical severity of hyperthyroidism,
delaying recognition despite high-risk features such as atrial
fibrillation and prior Graves’ disease. Reliance on a single
thyroid function test during acute illness may therefore
result in underdiagnosis.

Acute ischemic stroke (AIS) is a cerebrovascular disease characterized by high morbidity, disability, and mortality, posing a significant threat to human health. Endovascular treatment has now been established as a key method for AIS management, in which stent retrievers that can mechanically remove blood clots play a key role in this technique. In recent years, stent retrievers have evolved in complexity and functionality to improve the ability of clot removing and surgical safety. However, the present instruments still have limitations on treatment efficiency, vascular adaptability, and operational precision, posing an urgent need for innovation in the design of stent retrievers. This paper systematically reviewed the structural features and working principles of AIS stent retrievers from the perspective of efficacy evaluation metrics, historical development, recent advancements in stent retrieval technology, and future prospects.

10.Research progress of small-molecule natural medicines for the treatment of ischemic stroke.

Stroke is the second leading cause of disability and mortality worldwide, imposing a substantial socioeconomic burden on individuals and healthcare systems. Annually, approximately 14 million people experience stroke, with ischemic stroke comprising nearly 85% of cases, of which 10% to 20% involve large vessel occlusions. Currently, recombinant tissue plasminogen activator (tPA) remains the only approved pharmacological intervention. However, its utility is limited due to a narrow therapeutic window and low recanalization rates, making it applicable to only a minority of patients. Therefore, there is an urgent need for novel therapeutic strategies, including pharmacological advancements and combinatory treatments. Small-molecule natural medicines, particularly those derived from traditional Chinese herbs, have demonstrated significant therapeutic potential in ischemic stroke management. These compounds exert multiple neuroprotective effects, such as antioxidation, anti-inflammatory action, and inhibition of apoptosis, all of which are critical in mitigating stroke-induced cerebral damage. This review comprehensively examines the pathophysiology of acute ischemic stroke (AIS) and highlights the recent progress in the development of small-molecule natural medicines as promising therapeutic agents for cerebral ischemic stroke.
