Objective:To construct the inflammation score(IS)and nutrition-immunity score(NIS)for patients with multiple myeloma(MM),and to verify their prognostic stratification effects and significance. Methods:The clinical data of 129 newly diagnosed MM patients admitted to our hospital from August 2011 to September 2022 were retrospectively analyzed.Univariate and multivariate Cox regression analysis of overall survival(OS)were comducted on clinical parameters, including inflammatory indicators such as red blood cell volume distribution width(RDW)and platelet count(PLT),nutritional-immune indicators such as albumin(ALB),absolute lymphocyte count(ALC),and suppressed immunoglobulin count(S-Ig count). To construct IS and NIS for prognosis,X-tile software and multivariate Cox regression analysis were used to verify the prognostic stratification role and significance of IS and NIS.The time-dependent receiver operating characteristic(ROC)curve,C-index curve, calibration curve, and decision curve analysis(DCA)were used to evaluate the discrimination, accuracy, and clinical net benefit of IS and NIS in predicting overall survival(OS),and compared to the international staging system(ISS).Results:IS was constructed based on the scores of RDW and PLT,and NIS was constructed based on the scores of ALB,ALC,and S-Ig count.According to X-tile analysis and multivariate Cox regression analysis,IS and NIS can divide the patients into three risk strata respectively:low, medium and high IS and NIS groups. The differences in OS and hazard ratio(HR)between the low, medium, and high strata were statistically significant(P<0.05).IS and NIS are both independent prognostic predictors for MM.The area under the ROC curve(AUC)and C index of IS and NIS for predicting 1-to 7-year OS were greater than those of ISS,and both were greater than 0.7.The prediction results of IS and NIS for 1-,3-,and 5-year OS rates were well consistent with the actual observed results. The DC A curves of IS and NIS for predicting 1-,3-,and 5-year OS were higher than that of ISS in a wide range of threshold probability intervals. Conclusion:IS and NIS have independent predictive significance for OS in MM patients.Their predictive discrimination, accuracy, and clinical net benefit are higher and better than ISS,and they may have potential application value in MM prognosis.

OBJECTIVE To explore the active components of Curcumae Rhizome as well as its potential value for the treatment of chronic pancreatitis(CP)using a combination of network pharmacology and bioinformatic approaches.METHODS Network pharma-cology methods were used to screen the active ingredients of Curcumae Rhizome and potential therapeutic targets for CP,and their ex-pression abundance and distribution in different cell types of CP were further analyzed in combination with CP tissue RNA sequencing data from publicly available databases.Molecular docking was performed to analyze the binding of the active components of Curcumae Rhizome to CP-related targets. Finally, the role of these core targets in pancreatic stellate cell(PSC)activation and related pathways was analyzed by single-cell RNA-sequencing to assess the anti-inflammatory and anti-fibrotic potential of the active ingredients of Curcumae Rhizome in CP treatment.RESULTS The most effective component of Curcumae Rhizome,Hederagenin, was identified by network pharmacological analysis, and its two therapeutic targets associated with CP were identified:LYZ and Rxra.Molecular doc-king results demonstrated that Hederagenin had an extremely strong binding capacity to the Rxra protein(affinity score=-7.392 kcal·mol-1),a core target of CP.Single-cell RNA-sequencing analysis further demonstrated that the hub target Rxra gene was closely associated with PSC activation and played an important role in PTN and TGF-β signaling pathways, the activation of which played a crucial role in the progression of chronic inflammation and fibrosis.CONCLUSION Curcumae Rhizome may provide new clues for the treatment of CP by inhibiting PSC activation.

Aim To explore the mechanism by which dioscin regulates IL-17+γδT cells in the treatment of arthritis. Methods A collagen-induced arthritis(CIA)model was established in DBA/1 mice using bovine type Ⅱ collagen. The mice were randomly divid-ed into the CIA model group, methotrexate(MTX)positive control group, and dioscin low-dose(Dioscin-L),medium-dose(Dioscin-M),and high-dose(Dios-cin-H)groups.After intervention, the therapeutic effects were evaluated using scoring methods.Joint pathological damage was analyzed by hematoxylin and eosin(HE)staining. The levels of anti-collagen-spe-cific antibodies and the pro-inflammatory cytokine IL-17 were measured by ELISA.The expressions of γδT cells and their subtypes,as well as the secretion level of IL-17,were detected by flow cytometry. Results Dioscin significantly reduced the arthritis severity score in collagen-induced arthritis(CIA)mice, alleviated joint pathological damage, inhibited the production of IL-17 by splenic lymphocytes and the levels of anti-col-lagen-specific antibodies total IgG and IgG3,and de-creased the proportion of γδT cells in the lymph nodes, splenic γδT cells, and the Vδ4+T-cell subset. The level of IL-17 produced by the Vδ4 subtype in the lymph nodes of the intervention groups was lower than that in the model group, but the difference was not sta-tistically significant. Conclusion Dioscin has signifi-cant therapeutic effect on CIA,and its mechanism may be through the inhibition of γδT cells, but it is unlikely to be related to IL-17 derived from γδT cells.

Chinese Journal of Epidemiology 2025;46(1):26-32
Objective:To evaluate the association between hypertension duration and risk for chronic kidney disease in residents in China. Methods:Participants aged 18-74 years from the sixth round of China Chronic Disease and Risk Factor Surveillance in 2018 were included. The age/date at hypertension diagnosis was reported by them, and hypertension duration was calculated based on the age at diagnosis and the age at survey. The hypertension duration was calculated as 0 year (i.e., normotensive participants), 0.1- year, 5.0- years, 10.0- years, and ≥15.0 years. Serum creatinine, urinary albumin, and urinary creatinine levels were measured, and chronic kidney disease was diagnosed when glomerular filtration rate was <60 ml·min -1·(1.73 m 2) -1 and/or urine albumin-to-creatinine ratio was ≥30 mg/g. Multivariable logistic regression analysis, which took intra-group correlation into account, was used to evaluate the association of hypertension duration with chronic kidney disease. Results:A total of 140 662 residents were finally included in the analysis. After adjusting the confounders, including blood pressure, the odds ratio of chronic kidney disease was 1.16 (95% CI: 1.09-1.23), 1.33 (95% CI: 1.20-1.48), 1.33 (95% CI: 1.18-1.49), and 1.43 (95% CI: 1.29-1.60) in study subjects with hypertension durations of 0.1-4.9 years, 5.0-9.9 years, 10.0-14.9 years, and ≥15.0 years, respectively, in comparison with normotensive people. This result was further supported by the positive association between hypertension duration and chronic kidney disease in people with previously diagnosed hypertension. The results of restricted cubic spline suggested that the risk for chronic kidney disease showed a steep increase within 0.1- 4.9 years after hypertension diagnosis, then showed neither increase nor decrease. The above association seemed to be stronger in those with hypertension diagnosed age <45 years. Compared with those with hypertension duration of 0.1-4.9 years, the odds ratio was 1.38 (95% CI: 1.04-1.84), 1.22 (95% CI: 0.91-1.65), and 1.47 (95% CI: 1.04-2.07) in those with hypertension durations of 5.0-9.9 years, 10.0-14.9 years, and ≥15.0 year, respectively. In those with hypertension diagnosis at ≥45 years, the corresponding odds ratio was 1.08 (95% CI: 0.98-1.19), 1.08 (95% CI: 0.97-1.21), and 1.16 (95% CI: 1.02-1.32), respectively. Conclusions:Hypertension duration is positively associated with the risk for chronic kidney disease in residents in China, and this association is independent of blood pressure level. Early diagnosis of hypertension and long-term control of blood pressure are effective strategies for secondary prevention of hypertension-related chronic kidney disease.
