Following the reform of the professional title system for"Breaking Four One-sided Evaluation Criteria"—focu-sing on more than just papers,titles,academic qualifications,and awards-establishing a scientific,systematic,and comprehen-sive medical performance evaluation index system has become essential for developing medical talent teams and conducting profes-sional title assessments fundamentally.This study establishes a medical performance evaluation index system tailored for clinicians in various departments of a cancer hospital who are competing for senior professional titles.This system comprises six primary in-dicators and 18 secondary indicators,with results presented in a ranked format of medical performance.Additionally,we have al-so analyzed the corr-elation between clinicians'medical performance rankings and their professional title evaluation outcomes through practical application.The results indicate that clinicians with higher performance rankings have significantly higher suc-cess rates in evaluations(P＜0.05).This index system underscores clinical practice,enhances classification-based evaluations,and supports advanced information management and precision in hospital administration,thereby providing a solid foundation for strengthening the hospital's core competitiveness.

A 53-year-old female patient with multiple metastases of colon cancer was treated with regorafenib 120 mg/d orally (21 days of medication and 7 days of withdrawal was defined as 1 cycle). On the 7th day of regorafenib treatment, the patient developed intermittent pain in the right upper abdomen and waist, which was gradually aggravated. After 20 days of waist pain, renal ultrasound and abdominal CT examination showed a right perirenal subcapsular hematoma and laboratory tests showed hemoglobin 99 g/L and normal coagulation function. Spontaneous perinephric hematoma due to regorafenib was considered. Regorafenib was discontinued, the patient was instructed to stay in bed, and hemostasis and rehydration therapy was given. After 11 days of drug withdrawal, the patient′s lumbar pain was improved and hemoglobin returned to 102 g/L. After 2 months of drug withdrawal, abdominal ultrasound showed that the right perirenal hematoma was reduced and hemoglobin returned to normal.

Anti-VEGF therapies have been widely used in the treatment of age-related macular degeneration, diabetic macular edema, retinal vein occlusion with macular edema and other retinal diseases. It have achieved remarkable treatment effect with relatively high safety, but there are still reports of adverse reactions in cardio-cerebral vessels and eyes. There are many methods to measure retinal blood flow. Although the principles of these methods are different, the results are different, and there is no uniform standard, it has been observed that anti-VEGF drugs may cause some changes in retinal vessel diameter, arterial blood flow velocity and blood flow parameters. Especially after multiple injections, the effect may be more obvious.

A young girls took irregularly Vitamin D2 tablets (0.25 mg/tablet,each tablet contains 10 000 IU vitamin D2) purchased by her parents at a pharmacy since she was 1.5 years old.The girl took 1 tablet at a time,once to thrice daily,occasionally missing.Five months later,she developed polydipsia and polyuria.She drank more than 2 000 ml water every day,urinated once every 0.5-1 hour,and urinated more than 5 times every night.Her maximum daily urine volume could reach 3 400 ml,and her urine was transparent as clear water.Three weeks after the symptoms appeared,laboratory tests in another hospital showed that blood calcium was 4.34 mmol/L,blood phosphorus was 2.65 mmol/L,and 24-hours urine calcium was 8.0 mmol.Vitamin D poisoning was suspected and her parents were asked to stop feeding the girl vitamin D2 tablets.Rehydration,diuretics and other treatments were given.After 3 days,her blood calcium decreased to 3.60 mmol/L.After 6 days,she was transferred to Peking University First Hospital,where she continued to receive rehydration and symptomatic treatments with a low calcium and phosphorus diet.And after 8 days,the blood calcium was 2.47 mmol/L and 24-hours urine calcium was 2.7 mmol.Her symptoms of polydipsia and polyuria improved and she was discharged.Telephone follow-up 1 month after discharge,the girl's polydipsia and polyuria disappeared and her urine examination was normal.

Objective To report 1 case with pancreatic cancer after chemotherapy infected by influenza B virus, and complicated with atrial fibrillation, pulmonary infection. Methods Clinical data, treatment, outcome of this case were reviewed and the nursing emphasis was summarized. Results Through active treatment and nursing, with cured influenza, successful cardioversion of atrial fibrillation and controlled lung infection, the patient was transferred to general hospital to treat basic disease. During hospitalization, none infection occurred among medical staff and caregivers. Conclusions Key points of nursing of patient with pancreatic cancer after chemotherapy complicated with influenza B include disease observation of atrial fibrillation, lung infection and influenza B virus infection, nutritional status improvement, skin care, psychological care for patients and care-givers, disinfection and isolation to influenza B virus infection, and staff self-protection.

Objective To investigate the levels of cancer related fatigue (CRF) and the correlations between CRF and serum inflammatory factors and hypothalamus-pituitary-adrenal (HPA) axis in patients with gastrointestinal cancer.Methods The CRF level was assessed by brief fatigue inventory (BFI).The level of C-reactive protein (CRP) was measured by immunoturbidimetry, and the level of cortisol was measured by electrochemiluminesence.The levels of interleukin (IL)-6, IL-1β, tumor necrosis factor-α (TNF-α), adrenocorticotropic hormone (ACTH) and norepinephrine (NE) were measured by enzyme-linked immunosorbent assay (ELISA).Results The average total score of CRF was 3.15±1.93, and the degree was mild to moderate, which was positively correlated with the CRP (r=0.321, P=0.000), TNF-α (r=0.265, P=0.000), NE (r=0.174, P=0.015) and ACTH (r=0.257, P=0.000), but was not correlated with the cortisol (r=0.033, P=0.652).Eastern Cooperative Oncology Group (ECOG) score (t=8.081, P=0.000), education (t=-4.244, P=0.000), treatment (t=4.563, P=0.000), time from diagnosis to sampling (t=3.453, P=0.001) and CRP (t=2.837, P=0.006) were important factors of CRF.Conclusion The CRF status is common in gastrointestinal cancer patients.The CRF is correlated with the NE and ACTH of HPA axis.Medical staff should pay attention to the inflammatory factors and hormone levels to improve the fatigue status and the quality of patients.

Objective To describe the level of self-perceived burden (SPB) and analyze its influencing factors among the patients with advanced lung cancer, in order to provide a theoretical basis for individualized nursing care. Methods A total of 102 hospitalized patients with lung cancer from 3 hospitals in Shandong province were investigated by the Self-Perceived Burden Scale for Cancer Patient (SPBS-CP). Results The total score of the SPBS-CP was (60.31±17.06) points, which indicated at a moderate level. Those patients rated the highest score on the sub-scale of economic/family burden (22.04±6.72) points, the lowest score on the sub scale of care burden (10.28±3.62) points. Single factor analysis and regression results showed that different work conditions, different stages were related with SPB. Conclusions Patients with moderate and advanced lung cancer had a moderate or higher level of SPB. Staff nurses should focus on the psychological status of the patients and take positive interventions to release the negative emotional experiences.

Rabbit hemorrhagic disease (RHD) is contagious and highly lethal. Commercial vaccines against RHD are produced from the livers of experimentally infected rabbits. Although several groups have reported that recombinant subunit vaccines against rabbit hemorrhagic disease virus (RHDV) are promising, application of the vaccines has been restricted due to high production costs or low yield. In the present study, we performed codon optimization of the capsid gene to increase the number of preference codons and eliminate rare codons in Spodoptera frugiperda 9 (Sf9) cells. The capsid gene was then subcloned into the pFastBac plasmid, and the recombinant baculoviruses were identified with a plaque assay. As expected, expression of the optimized capsid protein was markedly increased in the Sf9 cells, and the recombinant capsid proteins self-assembled into virus-like particles (VLPs) that were released into the cell supernatant. Rabbits inoculated with the supernatant and the purified VLPs were protected against RHDV challenge. A rapid, specific antibody response against RHDV was detected by an ELISA in all of the experimental groups. In conclusion, this strategy of producing a recombinant subunit vaccine antigen can be used to develop a low-cost, insect cell-derived recombinant subunit vaccine against RHDV.
