Objective To explore the effect of different pontic designs in resisting stress interruption effect and roller coaster phenomenon through three-dimensional finite element analysis, providing clinical guidance for the application of clear aligners in extraction cases. Methods Four three-dimensional finite element models of different pontic designs in the extraction area of clear aligners were established, including the pontic-free connection design, the conventional hollow pontic design, the partially solid-filled pontic design and the fully solid-filled pontic design. All four aligner models were individually assembled with the dental arch model.A 0.2 mm distal movement of the canine was simulated to observe the initial displacement and periodontal ligament stress distribution of canine and second premolar in each group of models. Results The initial displacement tendency diagram revealed that in all experimental conditions, both the canine and second premolar exhibited tipping movement patterns. However, significant variations in initial displacement magnitudes were observed across different pontic designs. The fully solid-filled pontic-connected clear aligner model exhibited the greatest initial displacement values(both crown and root)and the maximum initial displacement magnitude.Notably, this kind of design displayed movement characteristics closer to bodily movement, indicating superior control efficacy in tooth positioning.Periodontal ligament stress analysis revealed that the fully solid-filled pontic-connected clear aligner model generated the highest maximum principal stress in the periodontal ligament. Conclusion This three-dimensional finite element study reveals that the application of solid-filled pontic in clear aligner therapy could improve biomechanical control at extraction sites by minimizing aligner distortion, reducing stress interruption effect and preventing the roller coaster phenomenon.

Objective To explore the role and mechanism of RNA demethylase fat mass and obesity-associated protein(FTO)in the ferroptosis in testicular interstitial cells induced by di(2-ethylhexyl)phthalate(DEHP).Methods Forty 3-week-old C57BL/6 male mice were randomly divided into a control group(corn oil)and 3 dosed DEHP treatment groups(5,250 and 500 mg/kg),and received an intragastric infusion of corresponding agents for 35 d,respectively. After mouse testicular interstitial TM3 cells was treated with 0,100,200 and 400 μmol/L mono-2-ethylhexyl phthalate(MEHP)for 24 h,corresponding plasmids were transfected to construct Fto overexpressing TM3 cells.Serum testosterone level was detected by ELISA,expression of testicular proteins was detected with immunohistochemical assay, and contents of Fe2+,malondialdehyde(MDA)and lipid peroxides in the testicle were detected by colorimetry.Methylated RNA immunoprecipitation,RT-PCR,and Western blotting were used to detect the level of N6-methyladenosine(m6A)modification.Results In the mice exposed to 250 and 500 mg/kg DEHP,the serum testosterone level was significantly reduced(P<0.01),contents of Fe2+,MAD and lipid peroxides in testicular tissue were obviously increased(P<0.01),and protein levels of RNA demethylase FTO,and ferroptosis related molecules ferritin heavy chain 1(FTH1)and glutathione peroxidase 4(GPX4)were significantly down-regulated(P<0.05),while those of transferrin receptor(TFRC),ferroportin(FPN),cyclooxygenase-2(COX-2),and acyl-CoA synthetase long-chain family member 4(ACSL4)were notably up-regulated(P<0.05).MEHP treatment for 24 h resulted in remarkably decreased cell viability in the TM3 cells, increased production of intracellular reactive oxygen species(ROS),reduced mitochondrial membrane potential(MMP)(P<0.01),down-regulated mRNA and protein levels of Fto(P<0.01),and the changes in other ferroptosis related proteins were consistent with the trend in testicular tissue, indicating ferroptosis in testicular interstitial cells.Intervention with ferroptosis inhibitor Fer-1 or overexpression of Fto significantly inhibited MEHP-induced toxicity and ferroptosis in TM3 cells(P<0.05),and overexpression of Fto reduced the m6A modification of Gpx4 and Fth1 mRNA(P<0.05).Conclusion Abnormal m6A modification of Gpx4 and Fth1 caused by inhibiting FTO expression may be the mechanism of ferroptosis in testicular interstitial cells induced by DEHP.

Objective To explore the effect of attachment wear on tooth movement in clear Aligner treatment, and to provide references for improving the efficiency of orthodontic treatment. Methods The tooth cone bean computed tomography (CBCT) scanning data were obtained, and the tooth model was established and assembled with the worn and complete attachment, respectively. When the appliance applied 2° distal torsion, 0.25 mm distal displacement, 0.1 mm depression displacement and 0.1 mm elongation displacement to the teeth, the changes of tooth displacement, periodontal ligament stress and root equivalent stress in the worn and complete attachment groups were compared. Results When the clear Aligner was used to apply distal torsion and elongation displacement to the teeth, the maximum tooth displacement decreased significantly, and the worn attachment caused the teeth to move obliquely when the distal translation and elongation displacement occurred. When the tooth was subjected to distal torsion, distal translation, depression and elongation, the maximum principal stress of periodontal ligament and the equivalent stress of tooth root decreased when the worn attachment caused the tooth to be subjected to distal torsion, distal translation, depression and elongation. Conclusions Attachment wear will affect the efficiency of tooth rotation and elongation. When the attachments are obviously worn, they should be replaced in time to ensure normal function of the attachments.

Objective:To explore the effect of mobile health (mHealth) management system on dietary behavior of patients with coronary heart disease (CHD) .Methods:Using the convenient sampling method, CHD patients who were hospitalized in Department of Cardiology of Beijing Anzhen Hospital and Beijing Chaoyang Hospital of Capital Medical University were selected as the research objects from June 2019 to January 2020. The research objects were randomly divided into the intervention group and the control group using SAS software. Finally, 68 cases in the intervention group and 66 cases in the control group completed the study. The intervention group received mhealth management, while the control group received routine management, and they were followed up to 1 month after discharge. The proportion of patients with the low-salt and low-oil diet, the intake of salt, oil, staple food, vegetables, fruits, fish, shrimp, poultry and eggs, BMI and blood pressure were compared between the two groups. Results:At 1 month follow-up, the intervention group was better than the control group in terms of low salt and low oil diet, salt intake, oil intake and vegetable intake compliance, and the differences were statistically significant ( P<0.05) . At 1 month follow-up, there were no statistically significant differences in BMI and blood pressure between two groups ( P>0.05) . Conclusions:The mobile health system can effectively increase the proportion of patients with a low-salt and low-oil diet, and improve the intake compliance of salt, edible oil and vegetables in patients with CHD. In future studies, the intervention time should be extended to further explore the long-term intervention effect of mobile health and give full play to the role of mobile health in continuous behavior management of patients with CHD after discharge.
