Osteopontin (OPN) is a secreted extracellular matrix glycoprotein that exists in body fluids and mediates multiple biological functions. The expression of OPN is particularly critical and active after ischemic stroke, and has dual significance for the body. The rapid increase of OPN has certain protective effects on the body, such as anti-inflammatory and promoting wound healing; if the rate of increase is relatively slower, it will have adverse effects on the body, such as promoting thrombosis and inducing inflammatory reactions. This article reviews the research progress of OPN in ischemic stroke.

Myocardial injury is the leading cause of death in uremic patients.PINK 1/Parkin-mediated mitochondrial autophagy is involved in the progression of myocardial injury.In recent years,pathogenic turbidity has been gradually accepted as a representative of a new type of toxic pathogens by the researchers of traditional Chinese medicine(TCM).This paper sorts out literature about pathogenic turbidity,analyzes the etiological and pathogenic characteristics of pathogenic turbidity,and suggests that the pathogenesis of uremia-induced myocardial injury can be more comprehensively clarified from the perspective of healthy-qi deficiency resulting in latent pathogenic turbidity.In the patients with uremia,the down-regulation of PINK1/Parkin causes the weakening of mitochondrial autophagy,which leads to the elevation of levels of reactive oxygen species(ROS)and inflammatory factors,and eventually causes the injury of myocardial cell.The above pathogenic mechanism is similar to the process of traditional Chinese medicine(TCM)in which the deficiency of the healthy-qi(in particular kidney deficiency)results in the production of the pathogenic turbidity(showing as dampness,blood stasis,phlegm,toxin and so on)and then causes the pathogenic turbidity hide in vessels and collaterals and gradually injure the heart vessels,and eventually results in the deficiency of heart vessels.The mitochondrial autophagy mechanism mediated by the PINK1/Parkin pathway is suitalbe for explaining the TCM pathogenesis of uremia-induced myocardial injury,characterized by healthy-qi deficiency resulting in latent pathogenic turbidity,and also is suitable for interpretating the principle of supporting healthy-qi to eliminate pathogenic turbidity for treating uremia-induced myocardial injury.Under the guidance of the theory of health y-qi deficiency and turbid pathogens in TCM,the development of specific PINK 1/Parkin agonists may expand the approach for the treatment of uremia-induced myocardial injury.

OBJECTIVE To explore the potential mechanism of action of Gexia zhuyu decoction in the intervention of metabolic-associated fatty liver disease （MAFLD） based on ferroptosis. METHODS With the help of network pharmacology， the central targets of Gexia zhuyu decoction intervening in ferroptosis of MAFLD were screened， then gene ontology， Kyoto Encyclopedia Gene and Genomes enrichment analysis and molecular docking were performed. Juvenile zebrafish with normal development at 3 d post-fertilization were randomly divided into control group， model group （5 mmol/L thioacetamide）， magnesium isoglycyrrhizinate group （positive control， 5 mg/mL）， and Gexia zhuyu decoction low- ， medium- and high- concentration groups （20， 40， 80 μg/mL， calculated by crude drugs）. After cultured for 72 h， the contents of alanine transaminase （ALT）， aspartate transaminase （AST）， total cholesterol （TC）， triglyceride （TG）， malondialdehyde （MDA）， superoxide dismutase （SOD）， glutathione （GSH）， reactive oxygen species （ROS） and Fe2+ were determined； the cellular structure of the liver tissues and hepatic steatosis were observed； the protein expression of silence information regulator 1 （SIRT1）， nuclear factor- erythroid 2-related factor 2 （Nrf2） and glutathione peroxidase 4 （GPX4） were detected. RESULTS The central targets of potential active ingredients of Gexia zhuyu decoction that act on ferroptosis in MAFLD included tumor proteins p53， SIRT1， Nrf2， etc.， which were enriched in biological processes such as positive/negative regulation of RNA polymerase Ⅱ promoter transcription， cellular components such as nucleus and cytoplasm， molecular functions such as protein binding， as well as signaling pathways such as ferroptosis and the cancer pathway， and they might be tightly linked to the main active ingredients. Compared with model group， the contents of ALT， AST， TC， TG， MDA， ROS and Fe2+ were all decreased significantly in each administration group， while the contents of SOD and GSH were increased significantly （P＜0.05）； the pathological damage of liver tissue cells had improved， and the accumulation of liver lipids had decreased. The protein expressions of SIRT1， Nrf2 and GPX4 had been significantly upregulated （P＜0.05）. CONCLUSIONS Gexia zhuyu decoction can regulate lipid metabolism， improve the level of oxidative stress， maintain Fe2+ homeostasis， and inhibit the process of ferroptosis in juvenile zebrafish with MAFLD， and the above effects may be related to the activation of the SIRT1/Nrf2/GPX4 axis.

Objective To explore the effect of the orthokeratology lenses on the control of different types of aniso-metropia in myopic children.Methods A total of 99 myopic children aged 8 to 16 years who got the orthokeratology len-ses at the Department of Ophthalmology,Heping Hospital Affiliated to Changzhi Medical College from September 2020 to November 2022 with complete data were included.These children were divided into the simple myopic anisometropia group(monocular myopia,binocular diopter difference ≥ 1.00 D,n=39)and the compound myopic anisometropia group(binoc-ular myopia,binocular diopter difference ≥ 1.00 D,n=60).The children with higher anisometropia(binocular diopter difference ≥ 2.50 D)in the two groups were set as the high anisometropia subgroup(n=18 and 29,respectively),and chil-dren with lower anisometropia(1.00 D≤ binocular diopter difference＜2.50 D)were set as the low anisometropia subgroup(n=21 and 31,respectively).In each group,eyes with a higher diopter were set as the high diopter eyes,and the contra-lateral eyes with a lower diopter were set as the low diopter eyes.Diopter,corneal topography,intraocular pressure,cor-neal endothelium and axial length of children in the two groups were examined and recorded.The changes in axial length before and after wearing orthokeratology lenses for 1 year were compared between the two groups,analyzing the correla-tion between the degree of anisometropia and changes in the binocular axial length.Results After wearing orthokeratolo-gy lenses for 1 year,children in both groups had an increase in the axial length with a lower increase in the axial length of the high diopter eyes compared to the low diopter eyes;before and 1 year after wearing orthokeratology lenses,the axial length of high diopter eyes was greater than that of the low diopter eyes in both groups,and the differences were statistical-ly significant(all P＜0.05).Both groups of children showed a decrease in the binocular axial length difference after wear-ing the orthokeratology lenses for 1 year;before and 1 year after wearing orthokeratology lenses,the binocular axial length difference of children in the simple myopic anisometropia group was greater than that in the compound myopic anisometro-pia group,and the differences were statistically significant(t=4.903 and 2.670;both P＜0.05).The changes in binocular axial length difference before and after wearing the orthokeratology lenses of children in the high anisometropia subgroup and low anisometropia subgroup of the simple myopic anisometropia group were greater than those in the high anisometro-pia subgroup and low anisometropia subgroup of the compound myopic anisometropia group,respectively,and the differ-ences were statistically significant(both P＜0.05).In the simple myopic anisometropia and compound myopic anisometro-pia groups,the degree of anisometropia was positively correlated with the binocular axial length changes before and 1 year after wearing the orthokeratology lenses(r=0.423 and 0.510,both P＜0.05).Conclusion Orthokeratology lenses can effectively reduce the difference in binocular axial length of children with myopic anisometropia,and their control effect on simple myopic anisometropia is better than that of compound myopic anisometropia.

There have been breakthroughs in the development and clinical application of immuno-therapeutic agents in recent years.As the clinical targets of second-generation immune checkpoint inhibitors(ICIs),programmed death-1(PD-1)and its ligand(programmed death-ligand 1,PD-L1)have become one of the hot spots in drug discovery.The Food and Drug Administration of the USA and National Medical Products Administration in China have approved a variety of PD-1/PD-L1 monoclonal antibody drugs(such as nivolumab,pembrolizumab and atezolizumab),which are marketed for the treatment of small-cell lung cancer,rectal cancer,colon cancer,and melanoma,among other diseases.However,in the clinical application of PD-1/PD-L1 monoclonal antibody drugs,it is found that they can cause different degrees of immune-related adverse reactions in the lung,liver,kidney,gastrointestinal system,endocrine system and skin,and even the emergence of hyperprogressive disease.Effective monitoring and management of clinical applications of PD-1/PD-L1 monoclonal antibody drugs and rational use of some drugs can improve the immunotherapy process and reduce the effects of adverse reactions and hyperprogressive diseases in patients under immunotherapy.

Objective To analyze the clinical application value of cinematic rendering(CR)reconstruction technology in acute aortic dissection(AAD),and to compare the imaging quality between CR and volume rendering(VR)reconstruction.Methods Patients with suspected A AD who underwent aortic computed tomography angiography(CTA)were analyzed retrospectively.All images were uploaded to Siemens Syngo.via post-processing workstation for VR and CR three-dimensional reconstruction,respectively.The optimized view angle,staining and transparency were selected and segmented by a radiologist to display the lesion to the full extent.All subjective evaluations of post-processing images were randomly evaluated on Siemens Syngo.via post-processing workstation by two radiologists.The two radiologists reached a consensus after consultation,and the results without consensus were evaluated by another senior radiologist.The 3-point scale was used in the subjective evaluation of post-processing images.The scores of rupture,endometrium,and true and false cavity were recorded.The diagnostic confidence was also recorded.Results A total of 21 ADD patients were enrolled,11 patients(52.3％)were Debakey Ⅲ type.The scores of rupture in CR and VR reconstruction were 2.952 points and 2.619 points,respectively,which had significant difference(P=0.016).For the endometrium of AAD,the score of all 21 patients in the CR reconstruction was 3 points,while only 7 patients(33.3％)in the VR reconstruction had 3 points,which showed significant difference between the both(P＜0.001).For the true and false cavity of AAD,only 1 patient(4.8％)in the VR reconstruction was 3 points,while all 21 patients in the CR reconstruction had 3 points(P＜0.001).The scores of CR reconstruction on the diagnostic confidence were significantly higher than those of VR reconstruction(P＜0.001).Conclusion CR reconstruction can provide photorealistic anatomical post-processing images,and can improve the display and evaluation of AAD.

Objective:To investigate the effect of complex decongestive therapy(CDT) on elephantiasis and skin texture improvement.Methods:The clinical data of patients with elephantiasis of lower limbs who were admitted to the Department of Plastic Surgery, Ninth People’s Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January 2016 to December 2018 were retrospectively analyzed. The patients with elephantiasis of different causes were treated with CDT for 1 course (4 weeks). The skin texture parameters (skin water content, transepidermal water loss, skin fibrosis), limb circumference and segmental edema were measured before and after treatment and analyzed by paired t test. Pearson correlation analysis was used to detect the correlation between segmental edema and skin texture parameters before and after treatment. The subjective feelings of patients were recorded during follow-up. A self-designed online questionnaire was used to follow up the patient’s disease control status, including the limb circumferences, changes in quality of life (life, work, social and emotion), the adaptability to bandages and elastic socks. Data were classified and analysed as excellent, good, medium and poor.Results:A total of 20 patients were included, including 16 females and 4 males. The average age was 50.45 years (ranged 9-70 years). 5 cases were primary lymphedema and 15 cases were secondary lymphedema. The duration of the disease ranged from 2 years to 27 years, with an average of 8.4 years. After 1 course of treatment, the skin texture was significantly improved. The skin water content decreased from 55.6%±7.4% before treatment to 42.1%±7.4% after treatment ( P< 0.001). The transepidermal water loss decreased from (14.981±5.699) g·m -2·h -1 before treatment to (9.312±2.590) g·m -2·h -1 after treatment. The skin fibrosis decreased from (0.087±0.042) N before treatment to (0.065±0.033) N after treatment ( P<0.001). The circumference of the affected limb decreased from (11.09±3.14) cm before treatment to (4.82±2.83)cm after treatment ( P<0.001). The segmental edema decreased from (4.00±2.14)L before treatment to (1.21±0.78) L after treatment. Segmental edema was positively correlated with skin water content, transepidermal water loss, and skin fibrosis ( r=0.447, r=0.429, r=0.751, P<0.05). The patients were followed up for 6 months after treatment. The circumference control was excellent in 13 cases, good in 5 cases and moderate in 2 cases. The quality-of-life changes were excellent in 16 cases, good in 3 cases, medium in 1 case. The adaptability of elastic material was excellent in 9 cases, good in 8 cases, medium in 2 cases and poor in 1 case. Conclusions:CDT can reduce subcutaneous edema, effectively reduce limb volume, decrease skin fibrosis, improve skin barrier function, improve skin texture, restore the appearance of the limb and improve patients’ quality of life.
