1.Predictive value of preoperative CT-based diagnosis of sarcopenia on the prognosis of patients with pancreatic cancer

Objective:Exploring the predictive value of sarcopenia based on preoperative CT diagnosis for the prognosis of patients with pancreatic cancer.Methods:A retrospective analysis was conducted on the clinical,pathological,and imaging data of 488 patients who underwent radical resection for pancreatic cancer,to analyze the correlation between sarcopenia and the prognosis of patients with pancreatic cancer.Results:Among the 488 patients included,281 had sarcopenia before surgery and 207 did not.The COX proportional hazards regression model showed that sarcopenia was an independent risk factor for overall survival in pancreatic cancer patients[hazard ratio 1.43(95%confidence interval:1.02-1.99);P=0.04],while preoperative sarcopenia was not associated with postoperative disease-free survival(P>0.05).Conclusion:Sarcopenia diagnosed based on preoperative CT is significantly correlated with the overall survival of patients with pancreatic cancer after surgery and is an independent risk factor for the overall survival of patients with pancreatic cancer after surgery.
2.Effect of early warning mechanisms based on nosocomial infection surveillance system on precise prevention and control of spread of multidrug-resistant organisms in neurosurgery department of a hospital

OBJECTIVE To observe the effect of early warning mechanisms based on nosocomial infection surveil-lance system on precise prevention and control of spread of multidrug-resistant organisms(MDROs)infections in intensive care unit of neurosurgery department.METHODS The dynamic monitoring was performed in the ICU of neurosurgery department of a three-A general hospital by using real-time surveillance system of nosocomial infec-tion from Jan.2023 to Dec.2023.The warning information was sent to the doctor' station immediately once the MDROs infections were detected,the detection of drug-resistant bacteria was carried out for the relevant personnel and their surroundings,and the precise prevention and control measures were implemented.The impact on the iso-lation rate of MDROs,isolation rate of MDROs from the relevant personnel and surroundings,incidence of noso-comial MDROs infections and incidence of surgical site infection was observed after the measures were taken.RESULTS The carrying rate of MDROs was decreased from 21.80%to 5.74%among the relevant personnel after the precise prevention and control measures were taken(P<0.05),which was improved remarkably among the doctors(decreasing from 21.43%to 6.07%),nurses(decreasing from 23.68%to 3.98%),nursing workers(decreasing from 25.15%to 8.65%),and cleaning staff(decreasing from 25.49%to 5.05%)(P<0.001).The isolation rate of MDROs was decreased from 20.77%to 7.12%among the surroundings(P<0.05),which was decreased most remarkably among head cushions(decreasing from 32.28%to 12.13%),bed rails(decreasing from 16.70%to 3.27%),ventilator panels(decreasing from 23.00%to 6.95%)and bedside floors(decreasing from 31.99%to 9.96%)(P<0.001).The incidence of nosocomial MDROs infections declined from 3.84%to 1.71%(P<0.05),with incidence of incision infections decreasing from 4.00%to 1.45%(P<0.05),the inci-dence of intracranial infections decreasing from 2.38%to 0.56%(P<0.05).CONCLUSION The detection of drug-resistant bacteria is carried out immediately and precisely for the patients with MDROs infection,relevant personnel and their surroundings by using the real-time surveillance system of nosocomial infection,which achieve remarkable effect on prevention of transmission of MDROs and reduction of incidence of hospital-associated infec-tions and surgical site infections.

4.Changes in S100A8/9 and NLRP3/Caspase-1/interleukin-1β pathway in kidney-aging rats induced by D-galactose

Objective To investigate changes in the pro-inflammatory mediator S100A8/9 and NLRP3/Caspase-1/IL-1β pathway in a rat kidney-aging model induced by D-galactose.Methods Twelve SD rats were divided into control and D-galactose groups,and injected subcutaneously in the back of the neck with D-galactose(150 mg/kg)to establish a rat model of kidney aging.Kidney samples were collected under anesthesia after 8 weeks.Kidneys were stained for senescence-associated beta-galactosidase(SA-β-Gal),mRNA expression levels of the aging-related genes p21,p16,and p53 were detected by quantitative reverse transcription-polymerase chain reaction(qRT-PCR),and histopathological changes were observed by hematoxylin-eosin(HE)and Masson staining.Serum urea nitrogen and creatinine,and catalase(CAT),glutathione peroxidase(GSH-PX),superoxide dismutase(SOD),and malondialdehyde(MDA)levels in the kidney tissues were detected.Reactive oxygen species(ROS)were detected by dihydroethdium staining and protein expression levels of collagen Ⅲ,α-smooth muscle actin(α-SMA),Protein expression of S100A8/9 was detected by immunofluorescence,and transforming growth factor(TGF)-β1 levels in kidney tissues and key factors in the NLRP3/Caspase-1/IL-1β inflammatory pathway were detected by Western Blot.A renal senescence model using HK-2 cells was constructed using H2O2 in vitro,and expression levels of the senescence proteins p21 and p16 and mRNA expression levels of the inflammatory factors IL-18 and tumor necrosis factor-α(TNF-α)were detected.Cell senescence was observed by SA-β-Gal staining.The effects of the S100A8/9 inhibitor paquinimod on expression levels of S100A8/9 and NLRP3/Caspase-1/IL-1β pathway-related proteins in the aging model were also detected.Results mRNA levels of the aging genes p21,p16,and p53 in kidney tissues were significantly increased in rats in the D-galactose group compared with the control group(P<0.01),and SA-β-Gal staining showed a significant increase in senescent cells(P<0.01).Serum blood urea nitrogen and creatinine levels increased(P<0.05),CAT,GSH-PX,and SOD activities decreased(P<0.01),while MDA activity increased in the D-galactose group(P<0.01).Collagen Ⅲ,α-SMA,and TGFβ1 expression and the ROS content in tissues increased(P<0.05).Glomeruli were atrophied or absent in the D-galactose group,the lumens of the renal sacs and renal tubules were enlarged,the nuclei were deeply stained and constricted,and numerous collagen fibers were deposited.Levels of S100A8 and S100A9 protein(P<0.01),as well as NLRP3,Caspase-1,and IL-1β increased(P<0.05).Paquinimod alleviated HK-2 cell senescence and decreased expression levels of the senescence proteins p21 and p16,and mRNA levels of the inflammatory factors IL-18 and TNF-α(P<0.05,P<0.01).The number of senile cells was also decreased,shown by SA-β-Gal staining(P<0.01).Paquinimod also inhibited the protein expression of S100A8 and S100A9(P<0.01)and NLRP3,Caspase-1,and IL-1β(P<0.05 or P<0.01).Conclusions S100A8/9 participates in the chronic inflammatory response by activating the NLRP3/Caspase-1/IL-1β pathway,thereby promoting D-galactose-induced renal aging.
5.Autophagy level of CD8+T cells in rat model of chronic obstructive pulmonary disease and correlation with its number

Objective:To investigate the autophagy level of CD8+T cells in rat model of chronic obstructive pulmonary disease(COPD)and correlation with its number,and to explore the role of autophagy in pathogenesis of COPD.Methods:Thirty-six 6-week-old male SD rats were divided into three groups,including normal control group,COPD group and COPD intervention group.COPD model was established by smoking,and intraperitoneal injection of 3-methyladenine(3-MA)was used for intervention.Lung function of rats was detected by small animal pulmonary function testing,and frequency of CD8+T cells,CD8+memory T cells,CD8+effector T cells in blood and lung of rats were detected by flow cytometry.CD8+T cells were sorted by immunomagnetic beads,and expressions of autophagy protein including LC3-Ⅱ/Ⅰ,Beclin-1 and p62 in CD8+T cells were detected by Western blot.The middle lobe of right lung was collected for histological observation.Results:Infiltration of inflammatory cells and the decline of lung function in COPD group were more obvious than those in normal group.Frequency of CD8+T cells,CD8+effector T cells and CD8+memory T cells in blood and lung of rats in COPD group were significantly higher than those in normal control group(all P<0.05).Level of autophagy protein,LC3-Ⅱ/Ⅰ and Beclin-1 of CD8+T cells in COPD group were significantly higher than that in normal control group,while level of p62 was lower than that in normal control group(P<0.01).CD8+T cells,CD8+effector T cells,CD8+memory T cells in COPD intervention group were significantly lower than those in COPD group(P<0.05).Correlation analysis showed that LC3-Ⅱ/Ⅰ expression level was positively correlated with frequency of CD8+T cells in blood(r=0.667,P<0.01).Expression level of Beclin-1 was positively correlated with frequency of CD8+T cells and CD8+effector T cells in blood(r=0.505,P=0.021;r=0.428,P=0.037).Expression of LC3-Ⅱ/Ⅰprotein was positively correlated with frequency of CD8+T cells and CD8+effector T cells in lung tissue(r=0.474,P=0.019;r=0.549,P=0.006).Expression of Beclin-1 was positively correlated with frequency of CD8+effector T cells in lung tissue(r=0.458,P=0.025).Conclusion:Level of CD8+T cell autophagy is correlated with its number.CD8+T cell autophagy may be involved in the chronic inflam-matory process of COPD,and 3-MA can inhibit COPD inflammation.

9.Preparation of washed mixed platelets by blood cell processing apparatus and its effect on biological activity

Objective To establish a new program of blood cell processing apparatus(NGL-BBS)for the preparation of washed mixed plateletsand to study the effect on biological activity of platelets compared with tradi-tional manual method.Methods Mixed concentrated platelets were separated and prepared from whole blood by white membrane method.Blood cell processing apparatus with new program set(experimental group)and manual method(control group)was used for thepreparation of washed mixed platelets.The expression rate of CD62P and CD63 in the two groups of washed mixed platelets was compared by flow-cytometry.Thrombus elastography(TEG)was used to measure and compare the MA value between the two groups.Results The expression rate of CD62P and CD63 in the experimental group was lower than that in the control group(t = 4.11,P<0.01;t = 10.78,P<0.01).TheTEG MA valueof the experimental group was higher than that of the control group(t = 6.67,P<0.01).Conclusion The present study demonstrates that the use of NGL-BBS for the preparation of washed mixed plate-lets has a lesser impact on biological activity compared to manual preparation methods.
10.Comparison of the biomechanical effects of cervical fusion surgery between using a cervical interfacet self-locking cage and a Zero-profile anterior cervical interbody fusion device

Objectives:To compare the biomechanical effects on the cervical spine between the insertion of a cervical interfacet self-locking cage(CILC)for anterior cervical interbody fusion and the insertion of a Zero-profile anterior cervical interbody fusion device(Zero-P)through the finite element method.Methods:Extracting the thin-layer cervical CT scan data of a 24-year-old male volunteer,and excluding any history of cervical spine trauma,surgery,cervical spondylosis,and imaging-diagnosed cervical spine deformities.Establishing a three-dimensional finite element model(blank model)of the normal lower cervical spine.The validity of the model was verified by comparing the range of motion(ROM)with those reported in previous studies.Based on this model,finite element models were constructed for single-segment posterior CILC insertion and fusion and Zero-P insertion and fusion.The surgical segment was set as the C4/5 segment,and CILC and Zero-P were implanted respectively to fuse and construct model.The lower endplate of the C7 vertebral body was fixed,and an axial load of 73.6N was applied to simulate the head's weight.A 1.0N·m torque was applied to the upper surface of the C2 vertebral body to simulate the overall movement of the C2-C7 finite element models,including flexion,extension,lateral bending,and axial rotation.The ROM of the segment and the stress changes in the adjacent segment intervertebral discs and facet joints were analyzed in all four motion directions for the blank model,CILC model,and Zero-P model.Results:The ROMs of the established three-dimensional finite element model of the lower cervical spine in all motion directions were consistent with previously published studies,therefore its validity was verified.Compared with the blank mod-el,the ROMs in all directions of the fusion segment was significantly reduced in both the CILC and Zero-P models.The ROM of the adjacent segments and the peak stress in the intervertebral discs and facet joints of the adjacent segments were higher in all motion directions compared with the blank model,with no significant difference in the degree of ROM increase between the CILC and Zero-P models.The peak stress increase in the intervertebral discs of adjacent segments in the CILC model was smaller than that in the Zero-P model.In the CILC model,the peak stress values of the C3/4 segment during flexion,extension,lateral bending,and rotation increased from pre-fixation values of 2.181,3.358,3.636,and 3.950MPa to post-fixation values of 2.532,3.881,4.463,and 4.917MPa,respectively.The peak stress values of the C5/6 segment during flexion,extension,lateral bending,and rotation increased from pre-fixation values of 1.558,3.996,3.778,and 3.660 MPa to post-fixation values of 1.864,4.131,4.183,and 4.266MPa,respectively.In the Zero-P model,the peak stress values of the C3/4 segment during flexion,extension,lateral bending,and rotation increased from preoperative values of 2.181,3.358,3.636,and 3.950MPa to 2.977,4.241,4.654,and 5.509MPa,respectively.The peak stress values of the C5/6 segment during flexion,extension,lateral bending,and rotation increased from pre-fixation values of 1.558,3.996,3.778,and 3.660MPa to post-fixation values of 2.314,5.214,4.469,and 4.739MPa,respectively.The peak stress increase in the adjacent facet joints in all motion directions was greater in the CILC model than in the Zero-P model.Conclusions:CILC insertion results in less impact on adjacent segment disc stress compared to Zero-P and provides reliable fixation,making it a suitable option for treating adjacent segment disease after cervical surgery.
