1.Historical Evolution and Key Information Research on Classic Formula Puji Xiaoduyin

Puji Xiaoduyin, a specialized formula for the swollen-head epidemic, was recorded in the Catalogue of Ancient Classical Formula (the Second Batch)-Han Medicine, published in September 2023. It had been inherited and developed by medical experts of successive generations and passed down to this day. This paper sorted out the historical evolution of this formula using bibliometric methods. It also comprehensively analyzed key information on the formula name, historical origin, drug dosage, herb origin, processing methods, decocting methods, function, and clinical applications. Additionally, this paper analyzed the application of this formula in both modern and ancient times. Results showed that the formula was first recorded as "Puji Xiaodu Yinzi" in LI Dongyuan's Proven Formulas written by LI Gao from the Jin dynasty. The medicinal composition and dosage were: Scutellariae Radix and Coptidis Rhizoma (20.65 g each), Ginseng Radix et Rhizoma 12.39 g, Scrophulariae Radix, Citri Reticulatae Pericarpium, and Glycyrrhizae Radix et Rhizoma (8.26 g each), Forsythiae Fructus, Arctii Fructus, Isatidis Radix, and Lasiosphaera Calvatia (4.13 g each), Bombyx Batryticatus and Cimicifugae Rhizoma (2.891 g each), Bupleuri Radix and Platycodonis Radix (8.26 g each). These medicines were grounded to fine powder. One dose, including 20.65 g of the powder, was mixed with 600 mL of water and decocted to 300 mL. After abandoning slag, the medicine should be taken warm frequently. In the formula, Bombyx Batryticatus is stir-fired. With the effect of dispersing wind and clearing heat, removing stagnation and dissipating mass, the formula is specialized in swollen-head epidemic, pestilence, red and swelling head, face, and neck, dry mouth and tongue, as well as other diseases resulting from toxic heat stagnated in the upper jiao. The formula is widely used in treating diseases involving the respiratory, dermal, ophthalmologic, otolaryngologic, and nervous systems. The formula is most frequently used for respiratory diseases, with a wide range of symptoms including parotitis/mumps (66 times), followed by tonsillitis (28 times). In conclusion, the broadly applied formula has accurate efficacy and great development value.

BACKGROUND:Studies have confirmed that Pterocarya hupehensis skan total flavonoids(PHSTF)can improve the level of collagen-induced arthritis in rats,but there is still a lack of research on the regulation of Wnt/β-catenin signaling pathway in fibroblast-like synoviocytes and its effect on related cell functions.OBJECTIVE:To investigate the effect and mechanism of PHSTF on lipopolysaccharide-induced proliferation,migration and apoptosis of fibroblast-like synoviocytes based on the Wnt/β-catenin signaling pathwayMETHODS:Fibroblast-like synoviocytes were divided into control group,lipopolysaccharide group,lipopolysaccharide+low-,medium-,and high-dose PHSTF groups(10,20,and 40 μg/mL),lipopolysaccharide+Wnt pathway inhibitor DKK1 group,and lipopolysaccharide+Wnt pathway inhibitor DKK1+high-dose PHSTF group(40 μg/mL).The cell counting kit-8 method was used to detect the effect of PHSTF on the viability of fibroblast-like synoviocytes,and the final drug concentration and time were screened.Flow cytometry was used to detect the apoptosis of fibroblast-like synoviocytes.Cell scratch assay,EDU staining and cell cloning assay were used to detect the migration and proliferation of fibroblast-like synoviocytes.Western blot assay was used to detect the protein expression levels of Wnt3a,β-catenin,tumorigenic genes,matrix metalloproteinase 2,matrix metalloproteinase 9,Bax and Bcl-2 in fibroblast-like synoviocytes.RESULTS AND CONCLUSION:(1)Compared with the control group,the cell viability decreased significantly when the concentration of PHSTF was>40 μg/mL(P<0.01).Therefore,the drug concentration of≤40 μg/mL was selected for subsequent experiments.(2)Compared with the lipopolysaccharide group,the wound healing rate,cell clone formation rate and the number of EDU-positive cells in the low-,medium-and high-dose PHSTF groups were significantly reduced,while the apoptosis rate was significantly increased(P<0.05-0.01).(3)Western blot results showed that compared with the lipopolysaccharide group,low-,medium-and high-dose PHSTF significantly inhibited cellular Wnt3a,β-catenin,cellular tumorigenic genes,matrix metalloproteinase 2,matrix metalloproteinase 9,and Bcl-2 protein expression,and promoted the expression of Bax protein(P<0.01).(4)Compared with the DKK1 group,the combination of DKK1 and high-dose PHSTF significantly inhibited the protein expression of Wnt3a,β-catenin,matrix metalloproteinase 2,matrix metalloproteinase 9 and Bcl-2 protein expression and promoted the protein expression of Bax(P<0.01).To conclude,PHSTF may inhibit the proliferation and migration of fibroblast-like synoviocytes and promote apoptosis by inhibiting the Wnt/β-catenin signaling pathway.

5.Performance Evaluation and Resource Utilization Optimization of Multidisciplinary Team Model for Lung Cancer: A Real-World Study

To compare the performance differences between the multidisciplinary team (MDT) model and the conventional diagnostic and treatment model for lung cancer, and to explore a high-quality development pathway for optimizing lung cancer diagnostic and treatment resources. A retrospective analysis was conducted on electronic medical record data of lung cancer patients at Shanghai Chest Hospital from March 2025 to December 2025. Patients were divided into an MDT group and a conventional care group based on whether they were admitted to the integrated oncology ward. Statistical analyses were performed using the Mann-Whitney A total of 4, 758 patients with primary lung cancer were included, comprising 365 (7.7%) in the MDT group and 4, 393 (92.3%) in the conventional care group. After adjusting for confounding factors, the MDT model significantly reduced hospitalization frequency during the observation period by 48.8% ( The MDT model for lung cancer significantly reduces hospitalization frequency; however, its effect on cost per hospitalization is population-selective, with increased costs in early-stage (stage Ⅰ) patients and decreased costs in late-stage (stages Ⅱ and Ⅳ) patients. The implementation of the MDT model should adopt precise patient stratification management, prioritizing the optimal patient population to achieve the optimal allocation of medical resources.
6.Research progress on the mechanism and clinical application of the effective ingredients from Sijunzi decoction in the treatment of gastric cancer

Gastric cancer is a common malignant tumor of the digestive tract and can be classified as “fullness of the stomach”, “epigastric pain”, “noise” and other categories in the field of traditional Chinese medicine. Sijunzi decoction is composed of Panax ginseng, Poria cocos, Atractylodes macrocephala, and honey-fried Glycyrrhiza uralensis, and it has the effect of tonifying qi and strengthening the spleen. This article summarizes the active ingredients, mechanism of action, and clinical application research progress of Sijunzi decoction in treating gastric cancer. The results show that the main active ingredients of Sijunzi decoction include ginsenosides, atractylenolide, pachymic acid, glycyrrhizic acid, etc.; Sijunzi decoction and its effective ingredients can play an anti-gastric cancer role by inhibiting the proliferation of gastric cancer cell, inducing apoptosis of gastric cancer cell, enhancing gastric cancer cell chemotherapy sensitivity, and inhibiting invasion and metastasis of gastric cancer cell. In addition, Sijunzi decoction can enhance the efficacy of chemotherapy drugs, strengthen the immune function of the body and lower serum cancer marker levels during the clinical treatment of gastric cancer.
7.Critical role of mitochondrial dynamics in chronic respiratory diseases and new therapeutic directions.

Chronic obstructive pulmonary disease (COPD) and pulmonary hypertension (PH) are both chronic progressive respiratory diseases that cannot be completely cured. COPD is characterized by irreversible airflow limitation, chronic airway inflammation, and gradual decline in lung function, whereas PH is characterized by pulmonary vasoconstriction, remodeling, and infiltration of inflammatory cells. These diseases have similar pathological features, such as vascular hyperplasia, arteriolar contraction, and inflammatory infiltration. Despite these well-documented observations, the exact mechanisms underlying the occurrence and development of COPD and PH remain unclear. Evidence that mitochondrial dynamics imbalance is one major factor in the development of COPD and PH. Mitochondrial dynamics is precisely regulated by mitochondrial fusion proteins and fission proteins. When mitochondrial dynamics equilibrium is disrupted, it causes mitochondrial and even cell morphological dysfunction. Mitochondrial dynamics participates in various pathological processes for heart and lung disease. Mitochondrial dynamics may be different in the early and late stages of COPD and PH. In the early stages of the disease, mitochondrial fusion increases, inhibiting fission, and thereby compensatorily increasing adenosine triphosphate (ATP) production. With the development of the disease, mitochondria decompensation causes excessive fission. Mitochondrial dynamics is involved in the development of COPD and PH in a spatiotemporal manner. Based on this understanding, treatment strategies for mitochondrial dynamics abnormalities may be different at different stages of COPD and PH disease. This article will provide new ideas for the potential treatment of related diseases.

8.Preliminary study on the inhibitory effect of Opisthoplatia orientalis Burm. polypeptides on autophagy via the PINK1/Parkin signaling pathway in rat models of postherpetic neuralgia induced by resiniferatoxin

Objective:To investigate the analgesic effect of Opisthoplatia orientalis Burm. polypeptides (OOBP) on postherpetic neuralgia (PHN) induced by resiniferatoxin (RTX) in rat models, and its effect on the phosphatase and tensin homologue deleted on chromosome ten (PTEN) -induced kinase 1/Parkin (PINK1/Parkin) signaling pathway. Methods:Thirty-two special pathogen-free rats were randomly divided into 2 groups: a blank control group ( n = 8) receiving intraperitoneal injections of physiological saline (0.20 mg/kg) , and a model group ( n = 24) receiving intraperitoneal injections of RTX (0.20 mg/kg) to establish the PHN rat model. The rats' paw withdrawal mechanical threshold (PWMT) was measured on days 1, 4, 7, and 10 after RTX injections. After 10 days of RTX treatment, rat models were randomly assigned to 3 subgroups: PHN group, OOBP group, and gabapentin group, with 8 rats in each group. The OOBP group and gabapentin group were gavaged with OOBP (equivalent to 0.9 g raw drug per kg) and gabapentin (27 mg/kg) respectively, while the PHN group and control group were gavaged with physiological saline. All treatments lasted for 3 weeks, during which PWMT was continuously monitored. One hour after the final dose, rats were sacrificed, and spinal cord tissues and serum samples were collected. Hematoxylin-eosin (HE) staining was performed to observe spinal pathological changes, enzyme-linked immunosorbent assay (ELISA) to detect serum levels of interleukin-10 (IL-10) , tumor necrosis factor-α (TNF-α) , β-endorphin (β-EP) , and calcitonin gene-related peptide (CGRP) , and Western blot analysis to determine the expression of PINK1, Parkin, and ubiquitin-binding protein P62 in rat spinal cord tissues. The entropy weight method was applied to comprehensively evaluate the effect of OOBP on the above cytokines, proteins and other pharmacodynamic indicators in rat models of PHN. Results:From day 1 to day 10 after modeling, PWMT values in the model group were significantly lower than those in the blank control group (all P < 0.05) , and also significantly lower than baseline values prior to modeling (all P < 0.05) . Histopathological analysis of rat spinal cord tissues showed less pathological changes (such as Nissl body swelling and neuronal necrosis) but more normal Nissl bodies in both the gabapentin group and OOBP group compared with the PHN group. ELISA revealed significantly decreased serum levels of TNF-α and CGRP but significantly increased serum levels of β-EP and IL-10 in the OOBP group compared with the PHN group (all P < 0.05) . Western blot analysis showed that expression levels of PINK1 and Parkin were significantly lower in the gabapentin group (0.441 ± 0.061, 0.597 ± 0.180, respectively) and the OOBP group (0.666 ± 0.117, 0.481 ± 0.073, respectively) than in the PHN group (1.033 ± 0.085, 1.088 ± 0.040, respectively, all P < 0.05) ; in contrast, the P62 expression significantly increased in the gabapentin group (0.810 ± 0.086) and OOBP group (0.902 ± 0.153) compared with the PHN group (0.543 ± 0.082, both P < 0.05) . The entropy weight analysis showed that the comprehensive scores were 0.222 and 0.229 in the OOBP group and the gabapentin group respectively, suggesting a greater overall therapeutic effect of OOBP. Conclusion:OOBP may exert analgesic effects in rat models of PHN by downregulating the expression of inflammatory factors and pain-related factors and modulating the PINK1/Parkin signaling pathway, thereby inhibiting mitochondrial autophagy in spinal neurons and reducing inflammatory responses.
9.Analysis of prognostic influencing factors of isocitrate dehydrogenase wild-type glioma

Objective:To analyze the prognostic influencing factors in patients with isocitrate dehydrogenase (IDH) wild-type glioma, and further evaluate the value of surgical resection in prognosis.Methods:The clinical data and molecular pathological information of 647 patients with IDH wild-type glioma were retrieved from the Chinese Glioma Genome Atlas (CGGA) database (from 2006 to 2019). The clinical characteristics were recorded, including gender, age, initial symptoms, preoperative Karnofsky performance status (KPS) score, tumor location, tumor laterality, extent of resection, O6-methylguanine-DNA methyltransferase (MGMT) promoter methylation status and postoperative treatment. Kaplan-Meier survival curve was plotted to calculate overall survival (OS) and progression-free survival (PFS), and comparisons were performed using the log-rank test. Multivariate Cox regression analysis was used to identify the independent influencing factors of prognosis in patients with IDH wild-type glioma.Results:Among the 647 patients with IDH wild-type glioma, there were 120 cases of WHO grade Ⅱ, 115 cases of grade Ⅲ, and 412 cases of grade Ⅳ; the median OS was 20.3 months. There were statistical differences in age, initial symptoms, tumor location, preoperative KPS score, extent of resection, MGMT promoter methylation status and postoperative treatment among patients with different WHO grades (P<0.01), but there were no statistical difference in gender and tumor laterality (P>0.05). The median OS and PFS in patients with WHO grade Ⅱ were significantly longer than those in patients with WHO grade Ⅲ and WHO grade Ⅳ (169.9 months vs. 24.7 and 14.4 months, 138.8 months vs. 17.7 and 11.4 months), the indexes in patients with WHO grade Ⅲ were significantly longer than patients with WHO grade Ⅳ, and there were statistical differences (P<0.05). The median OS and PFS in patients with total resection were significantly longer than those in patients with subtotal resection and partial resection (37.2 months vs. 20.3 and 8.5 months, 29.5 months vs. 17.6 and 6.0 months), the indexes in patients with subtotal resection were significantly longer than patients with partial resection, and there were statistical differences (P<0.05). Among 153 patients with MGMT promoter methylation who received postoperative chemoradiotherapy, the median OS and PFS in patients with total resection (67 cases) were significantly longer than those in patients with subtotal resection (64 cases) and partial resection (22 cases): 28.8 months vs. 18.4 and 9.0 months, 25.6 months vs. 16.6 and 6.5 months), the indexes in patients with subtotal resection were significantly longer than patients with partial resection, and there were statistical differences (P<0.05). Among 202 patients with MGMT promoter non-methylation who received postoperative chemoradiotherapy, the median OS and PFS in patients with total resection (81 cases) were significantly longer than those in subtotal resection (81 cases) and partial resection (40 cases): 31.3 months vs. 21.9 and 14.0 months, 22.7 months vs. 16.8 and 9.7 months, and there were statistical differences (P<0.05), but there were no statistical difference in the indexes between patients with subtotal and patients with partial resection (P>0.05). Multivariate Cox regression analysis result showed that age, extent of resection, WHO grade (grade Ⅲ/grade Ⅳ) and postoperative treatment were independent OS and PFS influencing factors in patients with IDH wild-type glioma (OS: HR = 1.327, 3.295, 3.406/7.964 and 1.597; 95% CI 1.079 to 1.633, 2.627 to 4.132, 2.225 to 5.216/5.339 to 11.880 and 1.288 to 1.981; P<0.01. PFS: HR = 1.282, 2.655, 3.143/6.511 and 1.477; 95% CI 1.039 to 1.583, 2.123 to 3.322, 2.049 to 4.819/4.376 to 9.690 and 1.181 to 1.849; P<0.05 or <0.01).Conclusions:The age, extent of resection, WHO grade and postoperative treatment are independent prognosis influencing factors in patients with IDH wild-type glioma. Treatment strategies should fully consider the clinical and molecular pathological characteristics to achieve maximal safe tumor resection and optimal survival benefit.

Objective To investigate the levels and clinical significance of microRNA(miR)-17-5p and miR-141-3p in the serum of children with bacterial meningitis(BM).Methods A total of 111 children with BM ad-mitted to the First Affiliated Hospital of Xinjiang Medical University from May 2019 to May 2022 were in-cluded as the study group,and another 111 healthy children who underwent physical examinations were in-cluded as the control group.Real-time fluorescence quantitative PCR(qRT-PCR)was used to measure the ex-pression levels of serum miR-17-5p and miR-141-3p.Pearson correlation was used to analyze the correlation between serum miR-17-5p,miR-141-3p levels and inflammatory factors in children with BM.Multivariate Lo-gistic regression was applied to analyze the influencing factors of BM occurrence.Receiver operating character-istic(ROC)curve was applied to analyze the diagnostic value of miR-17-5p and miR-141-3p levels for BM.Re-sults The serum levels of miR-17-5p and miR-141-3p in the study group were obviously lower than those in the control group(P<0.05),while the serum levels of C-reactive protein(CRP),procalcitonin(PCT),inter-feron-γ(IFN-γ),tumor necrosis factor-α(TNF-α),interleukin(IL)-1β,and IL-6 in the study group were high-er than those in the control group(P<0.05).According to Pearson correlation analysis,miR-17-5p and miR-141-3p were negatively correlated with CRP,PCT,IFN-γ,TNF-α,IL-1β,and IL-6(P<0.05).According to multivariate Logistic analysis,CRP,PCT,IFN-γ,TNF-α,IL-1β,and IL-6 were risk factors affecting the occur-rence of BM(P<0.05),while miR-17-5p and miR-141-3p were protective factors affecting the occurrence of BM(P<0.05).According to the ROC curve,the area under the curve(AUC)of serum level of miR-17-5p for diagnosing BM was 0.756,and the AUC of serum level of miR-141-3p for diagnosing BM was 0.720.The AUC of the combination of the two for diagnosing BM was 0.819,which was larger than that of single detec-tion(Zcombination vs.miR-17-5p=2.278,Zcombination vs.miR-141-3p=2.425,P<0.05).Conclusion The expression levels of miR-17-5p and miR-141-3p in the serum of children with BM are reduced.The two are related to the levels of inflammatory factors,and their combined detection has a high diagnostic value for BM.
