Gallbladder cancer is a highly aggressive malignancy of the biliary system, often diagnosed at the advanced stage due to its insidious early symptoms, leading to poor overall progno-sis. In recent years, the rapid advancement of artificial intelligence (AI) technologies and their inte-gration into medicine have opened new avenues for the early diagnosis and precision treatment of gallbladder cancer. Currently, AI incorporating deep learning algorithm has significantly improved diagnostic sensitivity and specificity in ultrasound, computed tomography, and pathological analysis. However, clinical translation of AI models remains limited by challenges such as insufficient annota-ted data and limited model interpretability. Future research should focus on establishing multi-center data-sharing mechanisms, developing interpretability tools, and optimizing multimodal data integration strategies, thereby promoting the transformation of AI technologies from an auxiliary diagnostic tool to a core component of clinical decision-making.

Non-alcoholic fatty liver disease(NAFLD)is a progressive liver disease caused by factors other than alcohol caused by heterotopic fat accumulation in the liver. In recent years, the incidence rate has been increasing, and there is no specific clinical drug.Studies have found that nuclear transcription factor κB(NF-κB)can activate inflammation and oxidative stress, which plays an important role in the pathogenesis of NAFLD.Traditional Chinese medicine is convenient to obtain and cheap. It can treat NAFLD through multiple channels with good clinical efficacy, and no obvious side effects have been found till now. Many studies have shown that traditional Chinese medicine can inhibit the occurrence and development of NAFLD by inhibiting the NF-κB signaling pathway. This article summarizes the research results of traditional Chinese medicine in treating NAFLD based on the NF-κB signaling pathway since 2021,for several traditional Chinese medicine extracts(flavonoids, terpenoids, polysaccharides, glycosides, alkaloids and phenolic compounds)and traditional Chinese medicine compound(Shugan Jianpi Fang,Erhuang Quzhi Granules,Fuzi Lizhong Decoction,Huangqin Decoction,Qinlian Hongqu Decoction,Jiangzhi Granules),which can inhibit the further development of NAFLD by improving liver inflammatory response, oxidative stress response, fibrosis, apoptosis, autophagy and pyroptosis through the NF-κB signaling pathway,in order to provide new ideas for future new drug development and clinical medication.

Objective::To examine the impact of hypertensive disorders of pregnancy (HDP) on offspring metabolomics.Methods::We searched five databases: PubMed, Ovid Embase, MEDLINE, Web of Science, and China National Knowledge Infrastructure, and included studies that reported metabolomics among human offspring born to HDP-complicated pregnancies.Results::Database search yielded 4054 articles, and after full-text screening, ten observational studies met inclusion criteria. Half of the studies had a sample size of less than 100 and were all observational studies in preeclampsia (PE) and gestational hypertension.Neonates were the most focused group in all included studies. Offspring born to HDP-complicated pregnancies exhibited statistically significant variations in blood metabolomics compared to their counterparts, characterized by amino acids, lipids, carnitine, and others (e.g., 1α,25-(OH) 2-D). Most studies reported a significant increase in differential metabolites of offspring born to HDP-complicated pregnancies. Four studies ( n = 1109) measured lipids-related metabolites, and all consistently showed that offspring born to PE-complicated pregnancies had significantly higher concentrations than non-PE exposed offspring. Conclusion::The existing evidence suggests an intergenerational effect of HDP on offspring metabolomics. Long-term follow-up studies are needed to advance the health effects of related adverse health outcomes and inform the prevention of offspring’s health.

Objective To analyze temporal trends in infectious disease burden among Chinese elderly(≥60 years)using data from the Global Burden of Disease Study(GBD 2021),evaluate age-period-cohort effects on disease burden, predict trends through 2045,and propose evidence-based medication management strategies. Methods We conducted a threefold analysis of infectious disease burden from 1990 to 2021 using Joinpoint regression to identify temporal trends,Age-Period-Cohort(APC)modeling to disentangle epidemiological effects, and Nordpred projections for 2045 disease burden estimates. Results Over 32 years, all infectious disease categories except HIV/AIDS and sexually transmitted infections demonstrated significant declines.Enteric infections showed the most rapid reductions in mortality(AAPC=-7.85,P﹤0.001)and disability-adjusted life year rates(DALYR;AAPC=-7.18,P﹤0.001).We also found a significant decrease in the incidence of tropical diseases and malaria(AAPC=-6.77,P﹤0.001).APC analysis found that the age effect was mostly negative in terms of the annual percentage change in mortality and DALYR for each disease, except for HIV/AIDS,with an overall decline in period risk over time, and a generally higher risk of morbidity and mortality for the early birth cohort, and an overall decline in the risk of each disease as the year of birth progressed, but the risk of HIV/AIDS death and DALY period and cohort risks trended upward. While the overall risk for certain diseases was slightly higher in males compared to females, the trends were largely consistent across both sexes. In terms of projections, the absolute prevalence of respiratory infections and tuberculosis in terms of number of cases and age-standardized rates, and the growth of the disease burden of HIV/AIDS were particularly prominent, making them important health challenges for the future. The burden of disease in the elderly often results in issues such as potential polypharmacy, which must be addressed to improve medication management. Conclusions China has achieved remarkable progress in reducing infectious disease burdens among older adults, though HIV/AIDS and sexually transmitted infections present an escalating public health threat. These findings advocate for enhanced surveillance systems,age-specific prevention strategies, and precision medication protocols to optimize therapeutic outcomes in geriatric populations.

Objective Based on the traditional Chinese medicine theory of"simultaneous regulation of the liver and kidney,"this study integrated network pharmacology prediction, molecular docking, and animal experimental validation to analyze the multi-target regulatory network of Duzhong Xionghua(the male flower of Eucommia ulmoides)-Sanqi Hua(Sanchi flower)herb pair(hereinafter called"herb pair")in modulating glucolipid metabolic disorders in type 2 diabetes mellitus(T2DM),thereby elucidating its underlying molecular mechanism. Methods Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform,China National Knowledge Infrastructure,VIP Database for Chinese Technical Periodicals, and Wanfang Data were used to obtain the active ingredients of the male flower of Eucommia ulmoides and Sanchi flower.PubChem Compound,SwissTargetPrediction, and SuperPred were used to screen and predict the targets of the drugs; The Human Gene Database,The Online Mendelian Inheritance in Man,and Therapeutic Target Database were used to screen the key gene targets of T2DM.A"component-target-pathway"network diagram was constructed using Cytoscape software, and Gene Ontology functional annotation and Kyoto Encyclopedia of Genes and Genomes pathway enrichment analysis were employed to identify the functions of the relevant target genes and pathways.Molecular docking was used to verify the binding activities of the core components and the key targets. For animal experiments, spontaneous T2DM model mice were used,in which the normal group consisted of six mice(wild type)from the same litter, and the 24 successfully modeled mice were randomly divided into model,metformin(0.26 g/kg),high-dose herb pair(2.6 g/kg),and low-dose herb pair groups(1.3 g/kg)according to the blood glucose levels and body weights, with six mice per group. The drugs were administered by gavage daily for six consecutive weeks. The body weight and fasting blood glucose(FBG)levels were measured weekly, and an oral glucose tolerance test was performed in the fifth week.At the end of drug administration, body weight,naso-anal length, liver and bilateral epididymal adipose mass were measured;pathological changes in the liver were observed using HE staining;serum levels of aspartate transaminase(AST),alanine amino-transferase(ALT),total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),and high-density lipoprotein cholesterol(HDL-C)were detected using colorimetric assay;and liver tissue phosphoinositide 3-kinase(PI3K)/protein kinase B(AKT)/glycogen synthase kinase-3β(GSK-3β)signaling pathway protein expressions were determined using Western blotting. Results Network pharmacology screening identified 38 active components and 669 potential targets of the herb pair.Intersection analysis with 1,275 T2DM-related targets yielded 185 common targets.Protein-protein interaction network analysis and pathway enrichment revealed the PI3K/AKT signaling pathway as a key mechanism.Molecular docking confirmed the strong binding affinity of the core components to key targets such as AKT1,suggesting that the herb pair may activate the PI3K/AKT pathway and inhibit GSK-3β activity via beta-sitosterol etc.Animal experiments demonstrated that, compared with the model group, the metformin group exhibited reduced FBG,AST,and ALT levels(P<0.01),but failed to improve body weight,Lee's index,or epididymal fat coefficient. Both herb pair doses significantly lowered Lee's index, hepatic index, and the epididymal fat coefficient(P<0.01),with the low-dose herb pair group showing attenuated body weight gain in mice.In contrast, the high-dose herb pair group exhibited decreased FBG,improved glucose tolerance, reduced TC,TG,and LDL-C levels, and increased HDL-C level(all P<0.01).HE staining revealed that all metformin and the herb pair markedly restored hepatic structure and alleviated steatosis in model mice, with more pronounced effects in the high-dose group than in the low-dose group.Western blotting result indicated that in the low-dose herb pair group,phospho-PI3K(p-PI3K),AKT,and phospho-GSK-3β(p-GSK-3β)protein expressions significantly increased(P<0.05 or P<0.01),whereas GSK-3β decreased(P<0.05).The high-dose group exhibited enhanced PI3K,p-PI3K,AKT,phospho-AKT,and p-GSK-3β protein expressions(all P<0.01),accompanied by reduced GSK-3β expression(P<0.01).Conclusion The male flower of Eucommia ulmoides-Sanchi flower herb pair may ameliorate T2DM-related glucolipid metabolic disorders by modulating the PI3K/AKT/GSK-3β pathway.

Objective To analyze the situation of blood lipid and blood routine parameters in patients with pneumoconiosis, and construct a column chart diagnostic model to explore their diagnostic value for pneumo-coniosis.Methods A total of 456 patients with pneumoconiosis admitted to the First Affiliated Hospital of Chongqing Medical and Pharmaceutical College from January 2022 to January 2024 were selected as the pneu-moconiosis group, while 462 healthy subjects exposed to dust during the same period were chosen as the con-trol group.Serum lipids and blood routine parameters related to pneumoconiosis were measured and compared between two groups.Univariate and multivariate Logistic regression analyzes were conducted to examine ser-um lipids and blood routine parameters associated with pneumoconiosis.A risk prediction model was construc-ted using logistic regression in machine learning, and the diagnostic efficacy of the column chart diagnostic model was evaluated by calculating the C-index through receiver operating characteristic(ROC)curve and plotting the model calibration curve based on Hosmer Lemeshow goodness of fit.Decision curve analysis(DCA)was used to assess the clinical practicality of the column chart diagnostic model. Results The levels of serum high-density ester protein cholesterol(HDL-C),cholesterol(TC),red blood cell(RBC),hematocrit(HCT),hemoglobin concentration(HGB),lymphocyte number(LYM),and lymphocyte percentage(LYM%)in the pneumoconiosis group were lower than those in the control group(P<0.05).The levels of neutrophil-lymphocyte ratio(NLR),platelet-to-lymphocyte ratio(PLR),and systemic immune inflammation index(SII)were higher than those in the control group(P<0.05).Multivariate Logistic regression analysis showed that HDL-C,LYM%,PLR,and SII were independent influencing factors for pneumoconiosis(P<0.05).A column chart diagnostic model for the occurrence of pneumoconiosis was constructed using HDL-C,TC,LYM%,PLR,and SII as diagnostic factors. The ROC curve C-index of the diagnostic model was 0.84(95%CI:0.81-0.86),with sensitivity for diagnosing pneumoconiosis of 75.29%,specificity of 77.51%,posi-tive predictive value of 83.25%,and negative predictive value of 67.88%.Internal validation was conducted on the constructed column chart diagnostic model, with a validation set ROC curve C-index of 0.84(95%CI:0.80-0.87),sensitivity of 80.91%,specificity of 72.62%,positive diagnostic value of 79.46%,and negative diagnostic value of 74.39%.The calibration positive curve slope of the diagnostic model was close to 1,and in the fit test P>0.05.DCA analysis showed that the diagnostic model had clinical practical value for risk diag-nosis of pneumoconiosis. Conclusion HDL-C,TC,LYM%,PLR and SII are independent influencing factors for pneumoconiosis.A column chart diagnostic model for the occurrence of pneumoconiosis is successfully constructed based on machine learning principles, and it has been verified to have high diagnostic efficiency.
