1.Application of local pedicled mucosal flap combined with type a botulinum toxin injection in the treatment of refractory contact granuloma of the larynx.

Objective:This study aims to explore the clinical effectiveness of a novel treatment method for refractory laryngeal contact granuloma, involving CO2 laser excision with local pedicled mucosal flap transfer combined with type A botulinum toxin injection. Methods:A retrospective analysis was conducted on 18 patients with refractory laryngeal contact granuloma who visited Department of Otolaryngology Head and Neck Surgery, Nanjing Tongren Hospital, School of Medicine, Southeast University from January 2021 to June 2023. These patients underwent CO2 laser excision of the granuloma with local pedicled mucosal flap transfer combined with type A botulinum toxin injection. During follow-up, electronic laryngoscopy were performed at 1, 3, 6, and 12 months postoperatively, and local laryngeal mucosa, voice quality, and pharyngeal discomfort symptoms were evaluated. Results:Postoperative electronic laryngoscopy revealed the disappearance of granulomas in all 18 patients. Symptoms such as hoarseness, foreign body sensation in the throat, and cough were significantly improved. No complications were observed systemically or locally. No recurrence was observed during one-year follow-up. Conclusion:CO2 laser excision of granuloma with local pedicled mucosal flap transfer combined with type A botulinum toxin injection could eliminate the lesion, restore the integrity of the vocal fold lining, preserve the perichondrium, and provide a time window for mucosal flap recovery. This approach adheres to the principle of preserving normal mucosa, achieves a high cure rate, and is therefore worthy of widespread promotion and application in clinical practice.

2.Regulation of methyltransferase METTL3 on radiotherapy sensitivity of oral squamous cell carcinoma cells

Objective:To study the influence of methyltransferases like 3(METTL3)on the radiosensitivity of oral squamous cell carcinoma cells(OSCCs).Methods:The apoptosis level of OSCCs CAL27,SCC9 and SCC15 treated with X-ray radiation doses of 2,4 and 8 Gy respectively was compared by flow cytometry,the expression of methylated gene RNA and protein in the cells were examined by qRT-PCR and Western blot.m6A in the cells was quantified by LC/LC-MS method.qRT-PCR and Western blot were used to investigate the expression of methylated gene RNA and protein in the cells.Flow cytometry was used to examine the cell apoptosis level of CAL27 and SCC15 cells treated with METTL3 overexpression and knockdown respectively.The clone forma-tion of CAL27 and SCC15 cells after knockdown and overexpression of target genes followed by radiation treatment was observed by clonogenic assays.Results:The apoptosis rate of all the cell lines increased with the increase dose of radiation at each dose,CAL27 cells showed the highest and SCC15 showed the lowerst apoptosis rate.The RNA and protein expression levels of METTL3 in CAL27 were significantly lower than those of SCC15.m6A quantification showed that the methylation modification in CAL27 cells was lower than that in SCC15.The expression of METTL3 was increased in CAL27 and SCC15 cells after radiation treatment.Knockdown of METTL3 increaced the apoptosis rate and decreased the clonogenesity,overession of METTL3 the decreaced the ap optosis rate and increased the clonogenecity of the cells.Conclusion:Regulation of METTL3 can affect the radiotherapy sensitivity of OSCCs,METTL3 may become a new target for radiosensitization of OSCCs.
3.Application and efficacy of microscopic vocal cord shortening with retrodisplacement of anterior commissure in voice feminization surgery

Objective:To explore the the application and outcomes of microscopic vocal cord shortening with retrodisplacement of anterior commissure in voice feminization surgery.Methods:We conducted a retrospective analysis of 296 patients(including 3 cases of pseudohermaphroditism and 293 transgender women)who underwent this procedure at the Voice Center of Nanjing Tongren Hospital of Otolaryngology-Head and Neck Surgery between November 2014 and August 2023. The patients, aged 18-50 years (mean age of 28.3±14.5) underwent feminizing laryngoplasty via transoral microsurgery, involving resection of the superficial lamina propria of the anterior vocal cords and subglottic tissues of the anterior commissure, followed by vocal ligament plication to achieve vocal cords shortening and anterior commissure retrodisplacement. Patients adhered to a1-month voice rest postoperatively and subsequently underwent speech rehabilitation training. Subjective/objective voice analyses, aerodynamic evaluations, and stroboscopic examinations were performed at 3, 6, and 12 months postoperatively to assess the effectiveness of voice feminization.Results:All surgeries were performed successfully and 94.3% (279/296) of the patients reported satisfaction at the end of the one-year follow-up. For effective cases, fundamental frequency (F 0) increased by 57.42±16.72 Hz compared to baseline. Postoperative acoustic/aerodynamic parameters and stroboscopic findings of vocal cord mucosal wave motion remained within normal ranges. These results indicated that this voice feminization surgery combined with postoperative pronunciation training achieved natural and feminine vocal characteristics. Conclusion:The combination of vocal cord shortening and anterior commissure retrodisplacement, along with postoperative pronunciation training, represents an effective surgical strategy for achieving a natural feminine voice.

6.Current status of herpes zoster vaccination in an infectious diseases hospital from 2020 to 2023

OBJECTIVE To statistically analyze the data of herpes zoster vaccination in the past four years and sum-marize the current status of herpes zoster vaccination.METHODS The data of herpes zoster vaccination were col-lected from the Fifth Medical Center of PLA General Hospital between 2020 and 2023.The descriptive statistical analysis was performed for the status vaccination regarding to the age group and gender.The influencing factors for the vaccination were analyzed,and the questionnaire survey was conducted for adverse reactions after the vacci-nation.RESULTS A total of 495 people received herpes zoster vaccination in the hospital from 2020 to 2023,inclu-ding 290 female and 205 male,and the overall number of vaccination showed an upward trend.Among the popula-tions who received the vaccination,the proportion of the population aged between 50 and 60 years old was 47.68%,the population aged between 60 and 70 years old 39.19%,the population aged more than 70 years old 13.13%.Totally 353 cases had the previous history of herpes zoster virus infection,accounting for 71.31%.The major adverse reactions for the herpes zoster vaccination included slight swelling,pruritus,fatigue and headache with low-grade fever.The follow-up result showed that 4 of 376 vaccination people reported to be reinfected with herpes zoster virus.CONCLUSIONS The people aged between 50 and 60 years old are the major population for her-pes zoster vaccination,the vaccination rate declines year by year among the population aged more than 60 years old,and the rate of willing to vaccination is low.It is necessary for the epidemic prevention institutions to actively disseminate the knowledge of herpes zoster vaccine and carry out health education,especially the vulnerability to the virus and safety of the vaccine,so as to raise the public's willingness to the vaccination.

8.Application of microflap suture under self-retaining laryngoscope in benign vocal cord lesions

Objective To investigate the clinical effect of microflap suture under self-retaining laryngoscope on benign vocal cord diseases.Methods A total of 115 patients with benign vocal cord diseases who underwent surgical treatment in the Binhai People's Hospital Affiliated to Kangda College of Nanjing Medical University and Nanjing Tongren Hospital Affiliated to School of Medicine of Southeast University were enrolled in this retrospective study.According to the surgical treatment methods,the patients were divided into observation group(58 patients were treated with microflap suture under self-retaining laryngoscope)and control group(57 patients received conventional surgery).The treatment effect,Voice Handicap Index-10(VHI-10)score,the scales of overall grade(G),roughness(R),breathiness(B),asthenian(A)and strain(S),and voice related parameters were compared between the two groups.Results The overall treatment effect of the observation group was significantly better than that of the control group(P<0.05).There were no significant differences in the VHI-10 score functional score,physiological score,emotional score,VHI-10 score,GRBAS score,longest phonation time,recording fundamental frequency,fundamental frequency perturbation,amplitude perturbation,or voice harmonic ratio before treatment between the two groups(P>0.05).After treatment,the VHI-10 score functional score,emotional score,total score),GRBAS score,the value of fundamental frequency perturbation and voice harmonic ratio in the observation group were significantly lower than those in the control group(P<0.05);the longest phonation time in the observation group was significantly longer than that in the control group(P<0.05).Conclusion Microflap suture under self-retaining laryngoscope is an effective treatment for benign vocal cord diseases,and the voice recovery is better after surgery.
9.Effects of ambient temperature on metabolic syndrome and pathway analysis

Background In recent years, the incidence of metabolic syndrome (MS) is increasing significantly in China. Some studies have found that temperature is related to single metabolic index, but there is a lack of research on associated mechanism and identifying path of the influence of temperature on MS. Objective Based on the data of Guangdong Province, to investigate the effect of temperature on MS and its pathway. Methods A total of 8524 residents were enrolled by multi-stage random sampling from October 2015 to January 2016 in Guangdong. Basic characteristics, behavioral characteristics, health status, and physical activity level were obtained through questionnaires and physical examinations, and meteorological data were obtained from meteorological monitoring sites. We matched individual data both with the temperature data of the physical examination day and of a lag of 14 d. A generalized additive model was used to explore the exposure-effect relationship between temperature and MS and its indexes, calculate effect values, and explore the effects of single-day lag temperature. Based on the literature and the results of generalized additive model analysis, a path analysis was conducted to explore the pathways of temperature influencing MS. Results The association between daily average temperature on the current day or lag 14 day and MS risk was not statistically significant. When daily average temperature increased by 1 ℃, the change values of fasting blood-glucose (FBG), systolic blood pressure (SBP), diastolic blood pressure (DBP), and high density lipoprotein cholesterol (HDL-C) were −0.033 (95%CI: −0.040-−0.026) mmol·L−1, −0.662 (95%CI: −0.741-−0.583) mmHg, −0.277 (95%CI: −0.323-−0.230) mmHg, and −0.005 (95%CI: −0.007-−0.004) mmol·L−1 respectively. The effects of average daily temperature on FBG, blood pressure, HDL-C, and waist circumference lasted until lag 14 day. The effects of daily average temperature on SBP and DBP were the largest on the current day. Daily average temperature of current day had direct and indirect effects on FBG and SBP. Temperature had an indirect effect on TG, and the intermediate variables were waist circumference and FBG, with an indirect effect value of −0.011 (95%CI: −0.020-−0.002). The indirect effects of daily average temperature on SBP, FBG, and TG were weak. Conclusion There is no significant correlation between temperature and risk of MS, and daily average temperature of current day could significantly affected blood pressure and FBG with a lag effect. Daily average temperature of current day has indirect effects on FBG and TG.
10.Practical Considerations for Converting Operating Rooms and Post-anaesthesia Care Units into Intensive Care Units in the COVID-19 Pandemic - Experience from a Large Singapore Tertiary Hospital.

COVID-19 has spread globally, infecting and killing millions of people worldwide. The use of operating rooms (ORs) and the post-anaesthesia care unit (PACU) for intensive care is part of surge response planning. We aim to describe and discuss some of the practical considerations involved in a large tertiary hospital in Singapore. Firstly, considerations for setting up a level III intensive care unit (ICU) include that of space, staff, supplies and standards. Secondly, oxygen supply of the entire hospital is a major determinant of the number of ventilators it can support, including those on non-invasive forms of oxygen therapy. Thirdly, air flows due to positive pressure systems within the OR complex need to be addressed. In addition, due to the worldwide shortage of ICU ventilators, the US Food and Drug Administration has granted temporary approval for the use of anaesthesia gas machines for patients requiring mechanical ventilation. Lastly, planning of logistics and staff deployment needs to be carefully considered during a crisis. Although OR and PACU are not designed for long-term care of critically ill patients, they may be adapted for ICU use with careful planning in the current pandemic.
