This study explored whether Astragalus polysaccharide(APS)can regulate the VEGF/VEGFR signaling pathway to affect the proliferative activity of rat intestinal mucosal microvascu-lar endothelial cells(RIMMVECs).RIMMVECs were isolated from newborn rats, then purified and treated with APS at concentrations of 0.1,1.0,10.0,100.0,1 000.0,and 10 000.0 mg/L.MTT was used to determine the effect of APS on RIMMVECs proliferation and screen for the optimal concentration of APS.Subsequently, flow cytometry was used to detect the changes in cell cycle to evaluate the stage of action of APS on the cell cycle in RIMMVECs.Then, the ELISA was used to detect the changes of VEGFA in cell supernatant to evaluate the potential of cell proliferation and angiogenesis. The changes in fluorescence intensity of Fluo-8AM was observed using fluorescence microscopy to evaluate intracellular Ca2+levels.Finally,Western blot was used to detect the ex-pression of PERK in RIMMVECs to analyze the possible mechanism of APS.The results showed that 100 mg/L APS significantly enhanced the proliferative activity of RIMMVECs,increased the content of VEGFA in the cell supernatant, the intracellular Ca2+levels,and the expression of PERK protein, indicating that APS promotes the proliferation of RIMMVECs,which may be a-chieved by promoting the expression of VEGFA and activating the ERK pathway.

High-risk non-muscle invasive bladder cancer (HR-NMIBC) is associated with a higher risk of recurrence and progression. The standard treatment involves intravesical Bacillus Calmette-Guérin (BCG) instillation following transurethral resection of the bladder cancer. However, in cases of BCG unresponsiveness, radical cystectomy is typically recommended. Due to the high complication rates and significant impact on quality of life associated with this procedure, bladder-preserving treatment (BPT) strategies have emerged as an alternative. This review summarizes recent advances in BPT, including combined chemoradiotherapy, immune checkpoint inhibitors, targeted therapies, localized precision treatments, and novel immunomodulators. With ongoing technological and therapeutic innovations, BPT has become an effective alternative for patients who are unable or unwilling to undergo surgery, reshaping the treatment landscape of HR-NMIBC.

Journal of Modern Urology 2025;30(7):621-626
In recent years,patient-derived tumor organoid(PDTO)models have rapidly emerged as important tools in cancer research, thanks to their unique ability to preserve the characteristics of primary tumors. These models provide a reliable preclinical research platform for screening individualized chemotherapy and targeted therapies for patients. However, traditional PDTOs lack immune cells and cannot replicate the tumor immune microenvironment, which restricts their utility in evaluating immunotherapies. To address this challenge, researchers have developed composite models that incorporate both tumor cells and immune cells, known as patient-derived immunocompetent tumor organoids(PDITOs).PDITOs have shown excellent performance in the preclinical evaluation of immunotherapies, particularly with PD-1/PD-L1 immune checkpoint inhibitors, with some studies reporting up to 100%accuracy in predicting patient responses to immunotherapy. As a common malignancy of the urinary system, bladder cancer has benefited from the application of PDITOs in drug screening and personalized immunotherapy evaluation, demonstrating significant potential. This paper aims to review the rise and development of PDITOs,and compare the advantages and limitations of using different methods to construct PDITOs,so as to explore their application in the treatment of bladder cancer.

To establish a highly specific, sensitive, and efficient method for detecting antibodies a-gainst the Erns protein of classical swine fever virus(CSFV),and to distinguish CSFV vaccine strains from wild strains infections in combination with the E2 subunit vaccine. The purified Erns protein of the CSFV expressed by baculovirus was conjugated to carboxylated magnetic beads as a solid-phase carrier and horseradish peroxidase(HRP),separately.A double-antigen sandwich chemiluminescent enzyme immunoassay(CLEIA)was developed by optimizing various reaction parameters using a fully automated chemiluminescence analyzer. This method was then applied to quantitatively detect Erns protein antibodies in sera from pigs infected with prevalent strains and those immunized with the CSFV E2 subunit vaccine and challenged with field strains. The results showed that the optimal conditions for coupling protein-to-magnetic bead were as follows:coupling buffer pH of 8.0,a protein coupling amount of 2.5 mg/g,blocking solution of 10%BSA,serum sample volume of 20 μL.The optimal dilution of enzyme-labeled antigen was at 1:500 with a one-step reaction time of 15 minutes. The cutoff value of the established CLEIA method for detecting CSFV Erns protein antibodies was 5.83 U/mL and a diagnostic sensitivity of 1:128.No cross-reac-tivity was observed with positive sera against African swine fever virus, pseudorabies virus, porcine circovirus type 2,porcine epidemic diarrhea virus, porcine reproductive and respiratory syndrome virus,or porcine gastroenteritis virus.Additionally, the method yielded negative results with sera from pigs immunized with the E2 subunit vaccine. In repeatability tests, the intra-assay coefficient of variation(CV)ranged from 0.77%to 11.56%,and the inter-assay CV ranged from 10.30%to 14.55%,both below 15%.The positive and negative concordance rates with a commercial CSFV Erns protein antibody detection kit were 95.24%and 92.71%,separately,with an overall concord-ance rate of 93.23%.The double-antigen sandwich chemiluminescence method established in this study exhibits high sensitivity, excellent repeatability, and suitability for automated detection, making it applicable for serological differentiation between CSFV E2 subunit vaccination and infec-tion with prevalent strains.

Objective:To analyze the results of next generation sequencing (NGS) gene testing in liquid-based cytological specimens of lung adenocarcinoma cavity and evaluate the clinical efficacy of epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKI) treatment. Methods:Liquid based cytological specimens of 222 cases of lung adenocarcinoma with cavity effusion and 201 cases of metastatic lymph node biopsy were collected. Specimens were obtained from the Cytology Laboratory of the Cancer Hospital of the Chinese Academy of Medical Sciences. The collection period was from January 2018 to December 2022. The results of NGS gene detection were compared. The clinical efficacy of 91 patients treated with EGFR-TKI was evaluated, and the survival curve was analyzed by Kaplan-Meier and other statistical methods. Results:The mutation rates of cancer-related genes detected by NGS were 82.0% (182/222) vs 79.1% (159/201), ( P=0.455) in liquid-based cytological specimens and histological specimens of metastatic lymph node biopsy, respectively. However, the mutation rate of EGFR T790M was significantly higher in cavity effusion than in lymph node biopsy specimens [12.2%(27/222)>3.5%(7/201), P=0.001]. The results of gene mutation were identical in 10 of the 13 cases with cavity effusion and metastatic lymph node biopsy, and the agreement rate of EGFR was 84.6%(11/13). In 3 inconsistent cases, EGFR mutations were detected in 2 cavity effusion cases that were not detected by lymph node biopsy. Results of genetic analysis of fluid-based cytological samples of 91 patients with cavity effusion were evaluated after drug treatment with EGFR-TKI. The mean progression-free survival (PFS) of the patients was 11.4 months (95% CI: 9.9-12.9). The mean PFS of patients harboring EGFR mutation was 12.3 months (95% CI: 10.8-13.9), and the mean PFS of EGFR wild type was 4.1 months (95% CI: 2.1-6.2). Conclusions:The results of NGS gene detection in liquid-based cytological specimens of lung adenocarcinoma patients with cavity effusion show that the PFS time is similar to that of histological specimens after clinical treatment with EGFR-TKI, which proves the reliability of NGS gene detection results in liquid cytological specimens. NGS gene testing appears higher sensitivity in cavity liquid-based samples than in metastatic lymph node samples.
