1.Comprehensive Application of AHP-CRITIC Hybrid Weighting Method, Grey Correlation Analysis and BP-ANN in Optimization of Extraction Process of Qizhi Prescription

ObjectiveBased on analytic hierarchy process(AHP)-criteria importance through intercriteria correlation(CRITIC) hybrid weighting method, grey relational analysis and backpropagation artificial neural network(BP-ANN), to optimize the water extraction process of Qizhi prescription, so as to provide an experimental basis for optimization of the preparation process of this prescription and the establishment of quality standards. MethodsL9(34) orthogonal test was employed, and the AHP-CRITIC hybrid weighting method was utilized to determine the weight coefficients of the quality fractions of various components, including astragaloside Ⅳ, polygalaxanthone Ⅲ, calycosin-7-O-β-D-glucoside, tenuifolin, and 3,6′-disinapoylsucrose, as well as the dry extract yield. The comprehensive score of each factor level combination in the orthogonal test were calculated as evaluation indicator to select the optimal extraction process parameters. The effects of extraction times, extraction time, and solvent dosage on the aqueous extraction process of the formula were investigated through intuitive analysis, variance analysis, and grey relational analysis. Meanwhile, a BP-ANN model was established to reverse-predict the optimal extraction process parameters of Qizhi prescription, and the optimized process parameters were validated. ResultsThe weight coefficients of the five index components(astragaloside Ⅳ, tenuifolin, calycosin-7-O-β-D-glucoside, polygalaxanthone Ⅲ, and 3,6′-disinapoylsucrose) and dry extract yield were 25.7%, 20.82%, 16.41%, 12.45%, 15.96% and 8.67%, respectively. The optimized extraction process parameters were extracted 3 times with 8, 6, 6 times the amount of water, each time for 1 h. The network prediction results of BP-ANN test samples were consistent with the orthogonal test results, and the mean square error(MSE) of the predicted and measured values of the network was <1%. The water extraction process of Qizhi prescription analyzed and predicted by relevant mathematical models was stable and feasible, which could effectively improve the extraction efficiency of the active ingredients of Astragali Radix and Polygalae Radix, and the average comprehensive score of the validation test was 90.85 with the relative standard deviation(RSD) of 1.55%. ConclusionThis study establishes a water extraction process for compound Qizhi granules, and the optimized extraction process can effectively improve the extraction efficiency of active ingredients, which provides useful references for the optimization of preparation process and the establishment of quality standards for other clinical experience formulas.
2.The impact of metformin on marginal bone loss at the edge of implants in patients with type 2 diabetes mel-litus and exercise habit

Objective:To study the effects of metformin on marginal bone resorption of implants in patients with type 2 diabetes melli-tus(T2DM)and exercise habit.Methods:63 cases with 73 implants were included.Among them,there were 41 cases(47 implants)without T2DM in group N,10 cases(13 implants)with T2DM and without exercise habit in group M,12 cases(12 implants)with T2DM and exercise habit in the MR group.The patients were followed up at 6 months,1 and 2 years after implantation.The marginal bone loss(MBL).Implantation success rate and peri-implantitis incidence rate were compared among the groups.Results:The bone resorption of the proximal and median margins of the long-term bone level of the implants in the N and MR groups were significantly lower than that in the M group(P=0.001 and P=0.000 5,respectively).The implant success rates of group N,MR and M were 95.74%,100%and 76.92%,respectively.The incidence of peri-implantitis of the three groups was 2.13%,0 and 15.38%,respec-tively.Conclusion:Metformin is more effective in the improvement of the long-term marginal bone resorption of implants,increase the success rate of implants,and reduce the incidence of peri-implantitis in patients with T2DM and exercise habit in the mandibular first molar area.
3.Meta-synthesis of qualitative research on self-management experience in patients with coronary heart disease

Objective:To summarize the self-management experience of patients with coronary heart disease, so as to provide a basis for medical and nursing staff to build and improve the self-management system of coronary heart disease.Methods:Qualitative studies on the self-management experience of patients with coronary heart disease were retrieved by computer in PubMed, Embase, Web of Science, CINAHL, Cochrane Library, Joanna Briggs Institute (JBI) , China National Knowledge Infrastructure, WanFang Data, VIP, China Biomedical Literature Database from database establishment to June 30, 2021. The articles were evaluated using the Quality Evaluation Standard for Qualitative Research (2016) of Australia JBI Evidence-based Health Care Center, and the results were summarized using the Meta-synthesis.Results:A total of20 articles were included, and 59 research results were extracted. A total of 12 new categories were formed by induction and combination, which were integrated into 3 integrated results, namely the driving factors of self-management, the blocking factors of self-management, the trajectory and path of self-management.Conclusions:Self-management of patients with coronary heart disease is a tortuous process influenced by multiple factors. Collaboration and support from families, healthcare systems, society, and governments are needed to promote patients' adherence to long-term self-management and improve their prognosis and quality of life.
4.The impact of joint line alteration on patients underwent primary medial pivot total knee arthroplast

Objective:To investigate the influence of joint line alteration on the mid-term clinical effects of primary medial pivot total knee arthroplasty (MP-TKA).Methods:Data of a total 189 patients (233 knees) with 30 males (30 knees) and 159 females (203 knees) with age 66.42±6.12, diagnosed as knee osteoarthritis and underwent primary MP-TKA from January to December 2015, were retrospectively analyzed. All cases were followed up for at least 4 years. The patients were divided into four groups according to the extent of alteration, namely <-4 mm, -4- mm, 0- mm and >4 mm. Range of motion (ROM), Knee Society's Knee Scoring System (KSS), the Western Ontario and McMaster University (WOMAC) osteoarthritis index score were compared preoperatively and at 4 years postoperatively.Results:The postoperative follow-up duration was 52.20± 2.94 months (48-59 months). A total of 133 knee (57%) had joint line raised and 100 cases (43%) joint line distal shifted. The overall height of joint line before surgery was 16.56±3.54 mm (7.33-25.32 mm) and 17.34±2.92 mm (7.25-25.43 mm) postoperatively. The average alteration was 0.78±3.17 mm (-6.00-9.91 mm) with statistically significant difference ( t=-2.596, P=0.010). There was no statistical difference among the four groups in terms of gender, body mass index, pre-operative tibia slope and post condylar offset. The overall preoperative ROM of the all groups was 90.45°±20.44°, which increased from baseline to 108.95°±12.55° at 4 years postoperatively ( t=-22.381, P<0.001). There was statistical difference in term of ROM among each group ( F=7.101, P<0.001). The ROM in group 0- mm and >4 mm was significantly higher than that in group <-4 mm and -4- mm ( P<0.05). The KSS function score and clinic score at 4 years postoperatively were significantly higher that those before surgery, the WOMAC score was statistically lower at 4 years postoperatively ( F=0.723, P<0.001; F=1.138, P<0.001; F=2.502, P<0.001). There was no significant difference among the four groups in terms of KSS score and WOMAC ( P>0.05). One case in >4 mm group had gasket prolapse at 6 month after surgery and 3 cases with immediate anterior knee pain occurred in <-4 mm group. Conclusion:MP-TKA can lead to significant joint line alteration. The overall ROM after surgery is positively correlated with the quantity of joint line alteration. However, there was no correlation between quantity of joint line alteration and the joint function scores. Thus, MP-TKA can achieve good mid-term clinical effects.
5.Preparation, in vitro imaging and antitumor activity of the second near-infrared imaging drug-loaded liposomes

The combination of diagnosis and treatment through nanotechnology is conducive to the development of cancer treatment. Fluorescent imaging in the second near-infrared window(NIR-II)developed rapidly in recent years due to its imaging advantages. In this paper, we prepared a novel nano drug system, DOX-IR1061-cationic liposome, in which NIR-II fluorescent probe IR1061 was loaded as imaging agents and doxorubicin was loaded as therapeutic agents. It also explores enhanced cellular uptake and cancer cell inhibition rate through octadecylamine. Our NIR-II performance test on liposomes showed that DOX-IR1061-cationic liposome has NIR-II imaging ability. Analysis of liposome cell uptake behavior and cancer cell inhibition experiments demonstrated that octadecylamine can promote liposome uptake by cells and synergize with DOX to enhance anticancer effects. This suggests that the DOX-IR1061-cationic liposome can be used to achieve imaging and therapy effect with further research value.
6. Analyzing the influencing factors of multisite work-related musculoskeletal disorders among workers in a railway vehicle manufacturing enterprise

OBJECTIVE: To analyze the prevalence and influencing factors of multisite work-related musculoskeletal disorders(WMSDs) of workers in a railway vehicle manufacturing enterprise. METHODS: A total of 366 male workers in the assembly and riveting workshop of a railway vehicle manufacturing enterprise were selected as the research subjects using the cluster sampling method. The Chinese Musculoskeletal Questionnaire was used to investigate the prevalence of multiple sites of WMSDs. Multiple logistic regression analysis was used to analyze the influencing factors. RESULTS: The total prevalence of WMSDs was 56.3%(206/366). The prevalence of WMSDs in all parts from high to low was as follows: lower back(35.5%), hand and wrist(27.6%), neck(23.2%), shoulder(21.0%), knee(19.9%), upper back(18.6%), hip and leg(18.0%), ankle/foot(15.8%) and elbow(12.3%)(P<0.01). The total prevalence of multisite WMSDs was 38.0%(139/366). The prevalence of WMSDs in different numbers of parts from high to low was as follows: 6 or more parts(12.0%), 2 parts(10.7%), 3 parts(6.6%), 5 parts(5.5%) and 4 parts(3.3%)(P<0.01). Multiple logistic regression analysis results showed that the overweight and obese workers had higher risk of multi site WMSDs than those with normal body mass index(P<0.05).Those with long-term low heads, frequent bending, long bending of the elbows, and higher frequency of work requirements, and less frequently communicated with the leader had higher risk of multi site WMSDs(P<0.05). CONCLUSION: The prevalence of multisite WMSDs in railway vehicle manufacturing enterprise is relatively high. The influencing factors include individual factors, adverse ergonomic factors and psychosocial factors.
7.Preparation and characterization of the second near-infrared window fluorescent/photoacoustic imaging liposomes

The second near-infrared window(NIR-II)fluorescent dye IR-1061 has the disadvantage of poorwater solubility and low quantum yield, which limits its application in biomedicine. We used liposomes as the carrier to increase their water solubility, and co-loaded a near-infrared region(NIR-I)fluorescent dye IR-780 into liposomes to enhance the fluorescence intensity of IR-1061 in NIR-II. Results showed that the liposomes exhibited significantly enhanced characteristic fluorescence under 980 nm laser irradiation. In addition, particle size, appearance, photothermal conversion efficiency, photoacoustic performance and cytotoxicity of liposomes were examined. This study showed that these combined liposomes exhibited potential as a NIR-II fluorescence/photoacoustic bimodal imaging system for the diagnosis and guidance of tumor photothermal therapy.
8.Expressions and clinical significances of CD68 and ABHD5 in gastric carcinoma

Objective To evaluate the expressions of CD68 and alpha/beta hydrolase domain-containing protein 5 (ABHD5) in gastric carcinoma and their interrelation,and the relationship with the clinicopathological features of gastric carcinoma.Methods The immunohistochemistry staining was used to determine the expressions of CD68 and ABHD5 in 60 cases of gastric carcinoma and 30 cases of adjacent normal tissues.The relationships between CD68,ABHD5 and clinicopathological features of gastric carcinoma,and the correlation of them were analyzed.Results The expressions of CD68 and ABHD5 in gastric carcinoma were significantly higher than those in adjacent normal tissues,and the positive expression rates were 65.00% vs.26.67%(x2 =11.779,P =0.001) and 60.00% vs.30.00% (x2 =7.200,P =0.007) respectively,with significant differences.The expression of CD68 was related to TNM stage (x2 =7.959,P =0.005),lymph node metastasis (x2 =6.901,P =0.009) and serosal invasion (x2 =5.833,P =0.016),but it was not associated with pathological grade (x2 =0.028,P =0.866),gender (x2 =0.533,P =0.465) and age (x2 =0.060,P =0.807).The expression of ABHD5 was related to pathological grade (x2 =4.318,P =0.038),TNM stage (/x2 =4.051,P =0.044) and serosal invasion (x2 =5.275,P =0.022),but it was not associated with lymph node metastasis (x2 =0.545,P =0.460),gender (x2 =0.191,P =0.662) and age (x2 =1.358,P =0.244).Contingency correlation analysis showed that there was positive correlation between CD68 and ABHD5 in gastric cancer (c =0.328,P =0.010).Conclusion The expressions of CD68 and ABHD5 in gastric carcinoma are higher than those in adjacent normal tissues,which are correlated with the different clinicopathological features of gastric carcinoma.The expression of CD68 is positively correlated with the expression of ABHD5.The combined detection of CD68 and ABHD5 expressions in gastric carcinoma is helpful to evaluate the invasion,metastasis and prognosis of gastric carcinoma.
9. Effects of mental workload on work-related musculoskeletal disorders in railway vehicle manufacturing workers

OBJECTIVE: To investigate the correlation of mental workload and prevalence of work-related musculoskeletal disorders musculoskeletal disorders( WMSDs) in railway vehicle manufacturing workers.METHODS: A total of 362 male workers in assembling and welding workshop from a railway vehicle manufacturing enterprise were selected as study subjects by cluster sampling method.The level of mental workload and prevalence of WMSDs were investigated using a revised Subjective Workload Assessment Technique and China Musculoskeletal Questionnaire.RESULTS: The median score of mental workload was 67 and the prevalence rate of WMSDs was 56.9%.The multivariate logistic regression analysis results indicated that the higher the mental workload of railway vehicle manufacturers,the higher their risk for WMSDs after excluding the influence of confounding factors( P < 0.05).Workers in welding work showed a higher risk than those in assembling work( P < 0.01).Workers with fast work rhythm showed higher risk of WMSDs than those with regular working rhythm( P < 0.01).Workers with comfortable working environment and temperature showed lower risk of WMSDs than those with uncomfortable working environment and temperature( P < 0.01).CONCLUSION: The mental workload can increase the risk of WMSDs,with a dose-effect relationship in railway vehicle manufacturing workers.The type of work,work frequency and the temperature in working environment are also influencing factors of WMSDs.
10.Tree shrew umbilical cord mesenchymal stem cells:isolation, cultivation and osteogenic and adipogenic differentiation

BACKGROUND: Studies have shown that umbilical cord mesenchymal stem cells are ideal seed cells for tissueengineering research.OBJECTIVE: To isolate, culture and identify tree shrew umbilical cord mesenchymal stem cells, in order toestablish a standardized tree shrew umbilical cord mesenchymal stem cell lines.METHODS: Caesarean-isolated tree shrew umbilical cord samples were used to isolate and culture umbilical cordmesenchymal stem cells using tissue explant adherent method. Flow cytometry assay was used to detect cellsurface markers. Osteogenic and adipogenic induction media were used to induce umbilical cord mesenchymalstem cells to differentiate into osteoblasts and adipocytes.RESULTS AND CONCLUSION: The cultured umbilical cord mesenchymal stem cells expressed CD90 and CD105 with the positive rate of 99.9% and 99.8% respectively. Hematopoietic stem cell marker CD34 expression ratewas 0.0% and the endothelial cell marker CD31 expression rate was 0.7%, in line with the characteristics of umbilicalcord mesenchymal stem cell surface markers. Calcium nodules by alizarin red staining and lipid droplets by oil red Ostaining were observed in the induced cells. These experimental findings indicated that umbilical cord mesenchymalstem cells from tree shrews capable of osteogenic and adipogenic differentiation were successfully isolated and cultured.
