Apical microsurgery is accurate and minimally invasive, produces few complications, and has a success rate of more than 90%. However, due to the lack of awareness and understanding of apical microsurgery by dental general practitioners and even endodontists, many clinical problems remain to be overcome. The consensus has gathered well-known domestic experts to hold a series of special discussions and reached the consensus. This document specifies the indications, contraindications, preoperative preparations, operational procedures, complication prevention measures, and efficacy evaluation of apical microsurgery and is applicable to dentists who perform apical microsurgery after systematic training.

2.The Clinical Mechanism of Improvement of Cognitive Impairment After Ischemic Stroke through Tongdu Xingshen Acupuncture by Regulating Gut Microbes

Objective To explore the mechanism of Tongdu Xingshen acupuncture,the clinical efficacy,systemic inflammatory response,blood-brain barrier and intestinal flora in patients with cognitive impairment after ischemic stroke(IS)were studied.Methods Thirty patients(3 cases shedding)with cognitive impairment after IS were included as the disease group,including patients before treatment as the disease group,patients after Tongdu Xingshen acupuncture treatment as the electroacupuncture group,and 30 healthy controls(3 cases shedding)were included as the healthy group.In the electroacupuncture group,on the basis of the basic treatment,Tongdu Xingshen acupuncture was applied,which was 30 min each time,once a day for 14 days.The MMSE,MoCA and MBI scores of the three groups were observed.The fecal and serum samples from all study subjects were collected,and 16S rDNA sequencing technology and ELISA were used to detect the changes of proinflammatory factors IL-6,IL-1β,TNF-α and S100β in serum in intestinal flora and feces.Results Compared with the healthy group,the MMSE,MoCA,and MBI score of patients in the disease group decreased significantly(P<0.05),serum proinflammatory factors and S100β protein content increased significantly(P<0.05),and the Shannon index(P<0.01)and Simpson index(P<0.001)increased significantly.Compared with the disease group,the MMSE,MoCA,and MBI score of the EA group increased significantly(P<0.05),the serum levels of proinflammatory factors and S100β decreased significantly(P<0.05),Shannon index and Simpson index decreased(P>0.05).The dominant bacterial flora in the healthy group mainly included Bacteroides,Bifidobacterium,Bacteroides,Faecalibacterium,Bifidobacteriaceae,Ruminococcaceae,and Bacteroides and other beneficial bacteria(P<0.05).The dominant flora in the disease group included Proteobacteria,Enterobacteriaceae,Escherichia,Klebsiella and other opportunistic bacteria(P<0.05),while the dominant flora in the EA group was consistent with the healthy group,the relative abundance of beneficial bacteria increased significantly(P<0.05),and the relative abundance of opportunistic bacteria decreased significantly(P<0.05).Spearman correlation analysis found that beneficial bacteria were positively correlated with clinical efficacy related indicators,but with serum proinflammatory factors and the content of S100β was negatively correlated.Conclusion Tongdu Xingshen acupuncture can regulate the diversity of intestinal flora to increase the abundance of Bacteroides,Bifidobacterium,Faecalibacterium,and other beneficial bacteria,regulate the intestinal microecological balance,Thereby regulating systemic inflammation and blood-brain barrier function,which plays a role in improving cognitive function.
3.Analysis of classroom environment hygiene in primary and secondary schools in Guizhou Province from 2022 to 2024

Objective:
To understand classroom environmental conditions and their association with spinal curvature abnormalities among students of primary and secondary schools in Guizhou Province, so as to provide a basis for formulating school health policies and scientific prevention and control measures for abnormal spinal curvature in students.
Methods:
Using a stratified random sampling method, 471 schools (2 811 classrooms) were selected annually across the province from 2022 to 2024 in order to monitor and evaluate classroom environment according to national standards. Spinal examinations were conducted for 196 606 (2022), 194 876 (2023), and 195 048 (2024) students, and χ 2 test was used to analyze the correlation of desk and chair compliance with student spinal curvature abnormalities.
Results:
The qualified rates of classroom blackboard illuminance uniformity ( 85.41 %) and the blackboard size (82.24%) were the highest in primary and secondary schools in Guizhou Province, while the average blackboard surface illumination qualified rate (20.10%) was the lowest. The average desk illumination (50.11%-58.63%), desk illumination uniformity (61.92%-72.27%) and qualified noise rate (50.04%-51.94%) increased significantly in 3 years; in addition, the compliance rate of desks and chairs decreased from 25.00% to 13.52%, and the differences were all statistically significant ( χ 2=42.48, 80.93, 46.09, 129.72, all P <0.05). Middle school classrooms outperformed primary schools in terms of per capita area, average blackboard illuminance, blackboard illuminance uniformity, average desktop illuminance, desktop illuminance uniformity, and noise compliance ( χ 2=311.55, 12.41, 20.64, 40.76, 10.25, 52.47), but had lower compliance for blackboard size and reflectance ( χ 2=537.29, 7.59) (all P <0.01). Urban schools had higher compliance than suburban schools for per capita area, average blackboard illuminance, average desktop illuminance, and desktop illuminance uniformity ( χ 2=73.71, 17.68, 29.30 , 36.03), but lower compliance for desk-chair suitability, blackboard size, and blackboard reflectance ( χ 2=4.72, 26.02, 5.43 ) (all P <0.05). The spinal curvature abnormality detection rate was 0.83%. A significant association was found between abnormality detection and desk-chair non compliance ( χ 2=223.85, P <0.01).
Conclusions
Classroom environment hygiene in Guizhou schools is suboptimal. Strengthening school environmental hygiene infrastructure and greater attention to its impact on student s health are essential.
4.Epidemiological characteristics of spinal curvature abnormalities in children and adolescents

Objective:
To understand the epidemiological characteristics of abnormalities spinal curvature among children and adolescents in Karst landform in Guizhou Province, so as to provide a scientific basis for targeted comprehensive intervention.
Methods:
From September to December 2023, 194 875 children and adolescents aged 6-18 were selected from all countieldistricts of 9 cities (prefectures) in Guizhou Province by stratified random cluster sampling method according to the proportion of Karst landform area in Guizhou Province for carrying out spinal curvature abnormality screening, and a questionnaire survey was conducted on 139 449 students in the fourth grade and above of primary school. Binary Logistic regression was used for multivariate analysis.
Results:
The detection rate of abnormal spinal curvature among children and adolescents aged 6-18 in Guizhou Province was 1.13%. The detection rates of abnormal spinal curvature in areas with a high proportion of Karst landform, areas with a low proportion of Karst landform, intercalation areas, and nonKarst landform areas were 1.10%, 2.00%, 0.90%, and 0.60%, respectively. Among them, the detection rates of abnormal spinal curvature in female students (2.40%, 1.60%, 0.90%) in areas with a low proportion of Karst landform, intercalation areas and nonKarst landform were higher than those in male students (1.60%, 0.10%, 0.30) (χ2=12.66, 112.69, 30.22, all P<0.05). The detection rates of abnormal spinal curvature among senior high school students (2.00%, 4.30%, 1.40%, 1.30%) in different Karst landform distributions were successively higher than those among junior high school students (1.40%, 3.20%, 1.00%, 0.60%) and primary school students (0.70%, 0.80%, 0.60%, 0.30%) (χ2=306.11, 175.80, 14.24, 39.57, all P<0.05). The results of multivariate Logistic regression analysis showed that the detection rates of abnormal spinal curvature in both highproportion and lowproportion Karst landform areas were higher than those in nonKarst landform areas [OR(95%CI)=1.84(1.05-2.25), 1.60(1.23-2.09), both P<0.05].
Conclusion
The detection rate of abnormal spinal curvature in children and adolescents aged 6-18 in Guizhou Province is related to the distribution of Karst landform, so it is necessary to strengthen screening and appropriate comprehensive interventions.
5.Influencing factors of quality of early recovery after radical surgery for colorectal cancer in elderly and establishment of prediction modeling

Objective To explore the factors influencing early recovery quality after radical sur-gery for colorectal cancer in elderly patients and establish a prediction model.Methods A total of 182 elderly patients who underwent elective radical surgery for colorectal cancer at the Affiliated Hospi-tal of Yangzhou University between May 2023 and May 2024 were enrolled as study objects.Data such as gender,age,body mass index(BMI),American Society of Anesthesiologists(ASA)classification,albumin,serum creatinine,hemoglobin,and D-dimer levels at admission were collected.Surgical ap-proach,operative time,anesthesia time,length of hospital stay,and whether the patient was transferred to the intensive care unit(ICU)postoperatively were also recorded.Relevant patient information was compiled through the electronic medical record system to calculate the modified frailty index(mFI).The 15-item Quality of Recovery Scale(QoR-15)was used to assess patients'recovery quality three days postoperatively.Results A total of 163 patients had good recovery(QoR-15 score ≥120)and 19 had poor recovery(QoR-15 score<120).Preoperative mFI(≥0.27)and BMI(≥21.05 kg/m2)were identified as factors influencing early recovery quality after radical surgery for colorectal cancer in elderly patients.The area under the receiver operating characteristic(ROC)curve(AUC)for the prediction model of recovery quality after radical surgery for colorectal cancer in elderly patients was 0.816(95%CI:0.710~0.921),indicating good agreement between the models predicted recovery quality and ac-tual recovery quality,indicating high discrimination and accuracy.Conclusion Preoperative mFI(≥0.27)and BMI(≥21.05 kg/m2)are factors influencing recovery quality after radical surgery for colorectal cancer in elderly patients.Improving perioperative frailty status and appropriately regu-lating BMI levels can help reduce the risk of postoperative complications.

Colorectal cancer (CRC) is a prevalent malignant tumor often leading to liver metastasis and mortality. Despite some success with PD-1/PD-L1 immunotherapy, the response rate for colon cancer patients remains relatively low. This is closely related to the immunosuppressive tumor microenvironment mediated by tumor-associated macrophages (TAMs). Our previous work identified that a phosphoglycerate mutase 1 (PGAM1) allosteric inhibitor, HKB99, exerts a range of anti-tumor activities in lung cancer. Here, we found that upregulation of PGAM1 correlates with increased levels of M2-like tumor-associated macrophages (TAMs) in human colon cancer samples, particularly in liver metastatic tissues. HKB99 suppressed tumor growth and metastasis in cell culture and syngeneic tumor models. M2-polarization, induced by colon cancer cell co-culture, was reversed by HKB99. Conversely, the increased migration of colon cancer cells by M2-TAMs was remarkably restrained by HKB99. Notably, a decrease in TAM infiltration was required for the HKB99-mediated anti-tumor effect, along with an increase in CD8+ T cell infiltration. Moreover, HKB99 improved the efficacy of anti-PD-1 treatment in syngeneic tumors. Overall, this study highlights HKB99's inhibitory activity in TAM-mediated colon cancer progression. Targeting PGAM1 could lead to novel therapeutic strategies and enhance the effectiveness of existing immunotherapies for colon cancer.
8.Effects of Hedysari Radix Praeparata Cum Melle and vinegar processed Curcumae Rhizomaon short-chain fatty acid metabolism in colitis-related colon cancer mice

Objective To investigate the effects of Hedysari Radix Praeparata Cum Melle and vinegar processed Curcumae Rhizoma(HRCR)on short-chain fatty acids(SCFAs)in colitis associated colon cancer(CAC)mice.Methods Forty C57BL/6 mice were randomly divided into control group,model group,sulfasazine group(SASP)and HRCR group,with 10 mice per group.Control group was given the same amount of drinking water,and the other group was given azoxymethane combined with dextran sulfate sodium to establish an CAC model.The SASP and HRCR groups were administrated with 0.455 g·kg-1 sulfasalazine and 6.825 g·kg-1 HRCR from the beginning of the experiment.The content of SCFAs in colon contents of mice was detected by gas chromatography-mass spectrometry.Results The contents of acetic acid,propionic acid,isobutyric acid,butyric acid,valeric acid,and hexanoic acid in the colon contents of control group mice were(2.80±0.54),(0.47±0.26),(0.02±5.90 x 10-3),(0.82±0.24),(9.85±2.24)×10-3,(7.21±4.21)×10-3 mg·g-1 respectively.Compared with the control group,the contents of acetic acid,propionic acid,isobutyric acid,butyric acid and valeric acid in the colon contents of mice in the model group were significantly reduced[(0.71±0.36),(0.24±0.08),(6.08±2.54)× 10-3,(0.13±0.03),(3.64±2.14)×10-3 and(1.99±0.81)×10-3 mg·g-1,all P<0.01].Compared with the model group,the contents of acetic acid,propionic acid,butyric acid,valeric acid,and hexanoic acid in the colon contents of mice treated with HRCR were significantly increased after intervention[(1.58±0.25),(0.39±0.04),(0.34±0.08),(5.51±1.76)×10-3 and(6.67±1.73)×10-3 mg·g-1,P<0.05,P<0.01].Conclusion The metabolism of SCFAs in CAC mice can be regulated by decocted astragalus-vinegar Curcuma.
9.Characteristic and trend of global capsule endoscopy research based on bibliometrics

Objective:To quantitatively analyze the primary research characteristics and keywords of international and domestic papers in the field of capsule endoscopy with bibliometric methods, and to illustrate the progression of capsule endoscopy research and offer insights for researchers in this domain.Methods:The Stork and visualized software CiteSpace were used to search and analyze the literature on "capsule endoscopy" in the English database of PubMed and the Chinese database of CNKI from 2002 to 2022.Results:A total of 6 011 English articles were identified in the PubMed database during the specified period. Both the number of publications and their citation frequencies displayed an upward trend on a global and domestic scale. The growth rates of global and domestic English publications were 5% and 15%, respectively, with major contributions coming from the United States, Japan, China, Italy and Germany. Key areas of global research interest included magnetic capsule endoscopy, colon capsule endoscopy, inflammatory bowel disease, small bowel capsule endoscopy, capsule endoscopy images, and obscure gastrointestinal bleeding, etc. A total of 2 027 articles were retrieved in the CNKI database, with top three research institutions including Department of Gastroenterology, Changhai Hospital, Naval Medical University; Department of Gastroenterology, Nanfang Hospital, Southern Medical University; School of Electronic Information and Electrical Engineering, Shanghai Jiao Tong University. Emerging research keywords encompassed artificial intelligence, deep learning, and guidelines.Conclusion:The study reveals a gradual increase in global research activities related to capsule endoscopy, with China demonstrating significant international academic influence in this field.
10.Envafolimab inhibits the growth of AGS gastric cancer cells by down-regulating SLC1A3/ERK/MAPK signaling pathway

Objective To investigate the effect of envafolimab on the proliferation,invasion and migration of AGS gastric cancer cells and its mechanism.Methods AGS cells were cultured in vitro and divided into envafolimab groups(2.5,5,10,20 and 40mg/ml of envafolimab,respectively)and control group(equal volume of normal saline).Cell viability was detected by cell counting kit-8(CCK8)assay,cell invasion and migration were detected by Transwell assay,and cell apoptosis was detected by flow cytometry.The differential proteins and related signaling pathways of the two groups were analyzed by proteomics,and the differential proteins and pathways found were verified by Western blot.Results Compared with the control group,the survival rate of AGS cells was significantly decreased in the envafolimab group(P=0.0060).It also significantly inhibited the invasion and migration of AGS cells(P=0.0052),and promoted the apoptosis of AGS cells(P=0.0030).Cluster analysis and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment revealed that solute carrier family 1 member 3(SLC1A3)protein expression was significantly down-regulated in the envafolimab group compared with the control group,which was related to the mitogen-activated protein kinase(MAPK)pathway.Western blot showed that the expression of SLC1A3 protein in AGS cells treated with envafolimab was significantly decreased(P=0.0041),and the phosphorylation levels of mitogen-activated extracellular signal-regulated kinase(MEK)and extracellular signal-regulated kinase(ERK)were also significantly decreased(P=0.0008).Conclusion Envafolimab can inhibit the growth of AGS gastric cancer cells possibly by down-regulating the expression of SLC1A3 and inhibiting the ERK/MAPK signaling pathway.Multiple antitumor mechanisms of action are possible for envafolimab.
