1.China - Africa cooperation for tropical diseases control: current status and future priorities

Tropical diseases, the transmission of which is affected by multiple natural and social factors, pose a great challenge to global public health, notably in African countries. During the past several decades, China and African countries have continuously collaborated for the control of neglected tropical diseases and malaria, which has become an important part of global South-to-South cooperation and global health governance. This article reviews the history of China-Africa cooperation for tropical diseases control, summarizes the experiences and achievements over the past decade, analyzes the current challenges in the coopera tion, and proposes future recommendations. The China-Africa cooperation has achieved significant progress in the control of tropical diseases, such as malaria, schistosomiasis, and filariasis, and established a China-Africa cooperation network for tropical diseases control. Through the "Three-Step" strategy of China-Africa cooperation, the effectiveness of China's integrated control strategies has been validated in Africa, and the application of China's tropical disease control technologies has been promoted in African disease-epidemic countries. Currently, China-Africa collaboration, however, still experiences multiple realistic challenges, such as insufficient resources, difficulty in technology transfer, and weak primary healthcare systems. In the future, both sides are recommended to further strengthen policy coordination, deepen technological cooperation, innovate cooperation models, aiming to continuously promote the high-quality development of China-Africa cooperation for tropical diseases control.
2.Prediction of postoperative pulmonary complications in video-assisted thoracic surgery for lung cancer based on cardiopulmonary exercise testing and machine learning

Objective To develop a predictive model for postoperative pulmonary complications (PPC) following video-assisted thoracic surgery (VATS) in lung cancer patients by integrating cardiopulmonary exercise testing (CPET) parameters and machine learning techniques. Methods A retrospective analysis was conducted on patients with early-stage non-small cell lung cancer who underwent CPET and VATS at Guangdong Provincial People’s Hospital between October 2021 and July 2023. Patients were divided into a PPC group and a non-PPC group. The least absolute shrinkage and selection operator (LASSO) regression was used to select important features associated with PPC. Six machine learning algorithms were utilized to construct prediction models, including logistic regression, support vector machine, k-nearest neighbors, random forest, gradient boosting machine, and extreme gradient boosting. The optimal model was interpreted using SHapley Additive exPlanations (SHAP). Results A total of 325 patients were included, with an average age of 60.36 years, and 55.1% were male. Significant differences were observed between the PPC and non-PPC groups in age, diabetes, coronary heart disease, surgical approach, forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC), FVC% predicted, peak oxygen uptake (peak VO2), anaerobic threshold (AT), and ventilatory equivalent for carbon dioxide slope (VE/VCO2 slope) (P<0.05). In the predictive model constructed by selecting 7 key features using LASSO regression, the random forest model demonstrated the best overall performance across various metrics, with an area under the receiver operating curve of 0.930, an F1 score of 0.836, and a Brier score of 0.133 in the training set. It also exhibited good predictive ability and calibration in the test set. SHAP analysis ranked feature importance as follows: peak VO2, VE/VCO2 slope, age, FEV1, smoking history, diabetes, and surgical approach. Conclusion Integrating CPET parameters, the random forest model can effectively identify high-risk patients for PPC and has the potential for clinical application.
3.Effect of Wei's Huoxue Tongluo Formula(韦氏活血通络方)on Visual Function and Fundus Blood Flow in Treating Atrophic-Stage Non-Arteritic Anterior Ischemic Optic Neuropathy with Qi Deficiency and Blood Stasis

ObjectiveTo evaluate the efficacy and possible mechanism of Wei's Huoxue Tongluo Formula (韦氏活血通络方,WHTF) in treating atrophic-stage non-arteritic anterior ischemic optic neuropathy (NAION) with qi deficiency and blood stasis. MethodsA total of 82 atrophic-stage NAION patients with qi deficiency and blood stasis were randomly divided into a treatment group and a control group, with 41 cases in each group. The treatment group was given oral administration of WHTF twice a day plus acupoint injection of distilled water 2 ml at Taiyang (EX-HN5) once daily, while the control group received injection of compound anisodine injection 2 ml at Taiyang (EX-HN5) once daily and oral administration of WHTF placebo twice a day. Both groups received treatment for a course of 14 days. The best-corrected visual acuity (BCVA), optic disc perfusion density (PD), flux index (FI), macular superficial PD, vascular density (VD), and traditional Chinese medicine (TCM) syndrome scores were compared between groups before treatment and on day 7 and day 14 of treatment. Additionally, mean defect (MD) and mean sensitivity (MS) of visual fields were measured before treatment and on day 14, along with safety evaluation. ResultsAfter treatment, both groups showed significant improvement in BCVA, visual field MD and MS, and TCM syndrome scores (P<0.05 or P<0.01). On day 14 of treatment, the TCM syndrome score in the treatment group was significantly lower than that in the control group (P<0.05). There was no significant improvement in optic disc PD and FI, and macular superficial PD and VD after treatment in either group (P>0.05) except that on day 7 the macular superficial foveal PD in the control group was significantly better than that in the treatment group (P<0.05). During the treatment period, no serious adverse events occurred in either group. ConclusionWHTF can improve the visual function indicators including visual acuity and visual field, as well as TCM syndrome scores in atrophic-stage NAION patients with qi deficiency and blood stasis. It shows clinical safety, although it does not appear to have a significant effect on optic disc or macular blood flow.
4.Comparison of single and hybrid models for predicting the incidence of hand, foot, and mouth disease in Changsha

Objective To compare the performance of seasonal difference autoregressive moving average exogenous variable model (SARIMAX), backpropagation neural network (BPNN), long short-term memory network (LSTM), and various combination models in fitting and predicting the incidence of hand, foot, and mouth disease (HFMD), and to provide a reference for optimizing HFMD prediction and prevention and control decisions. Methods Taking the monthly HFMD incidence data in Changsha from 2010 to 2024 as the training set, and the monthly HFMD incidence data from January to August 2025 as the validation set, the single models, SARIMAX-BPNN, SARIMAX-LSTM and weight combination models were constructed respectively. The R², mean absolute percentage error (MAPE), mean absolute error (MAE), and root mean square error (RMSE) were used to evaluate the model fitting and prediction performance. Results Based solely on MAPE, the LSTM model showed the best fitting and prediction performance. A comprehensive analysis of multiple indicators including R², MAPE, MAE, and RMSE indicated that the combination models had superior prediction performance. Compared with the LSTM, RMSE predicted by equal weight combination, different weight combination and SARIMAX-BPNN decreased by 40.97%, 37.50%, and 25.00%, respectively, while MAPE increased by 124.84%, 96.90%, and 89.69%, respectively. The fitting effect was as follows: LSTM > unequal weight combination > equal weight combination > SARIMAX-LSTM > SARIMAX-BPNN > BPNN > SARIMAX. The prediction performance was as follows: SARIMAX-BPNN > unequal weight combination > equal weight combination > LSTM > SARIMAX-LSTM > BPNN > SARIMAX. Conclusion SARIMAX-BPNN combination or weighted combination of multiple different models are two more robust combination strategies for predicting the incidence trend of hand, foot and mouth disease in Changsha.
5.Clinical study on peripheral blood 25-hydroxy vitamin D, TBNK lymphocyte subsets, and cytokines levels in patients with brucellosis

Objective:To learn about the levels of 25-hydroxy vitamin D (25-OH VD), TBNK lymphocyte subsets, and cytokines in peripheral blood of patients with brucellosis.Methods:A prospective design was adopted, one hundred patients with brucellosis admitted to the Department of Infectious Diseases, Beidahuang Industry Group General Hospital from May 2024 to February 2025 were selected as the brucellosis group, and one hundred healthy individuals who underwent physical examinations at the hospital during the same period were selected as the control group. The peripheral blood 25-OH VD levels were detected by chemiluminescence method. Further, 100 patients with brucellosis were divided into a brucellosis combined with osteoarthritis group (74 cases) and a brucellosis without osteoarthritis group (26 cases). Flow cytometry was used to detect the counts of peripheral blood TBNK lymphocyte subsets and cytokine levels. Meanwhile, Spearman rank correlation was used to analyze the correlation between peripheral blood 25-OH VD levels and TBNK lymphocyte subsets counts as well as cytokine levels in patients with brucellosis complicated by osteoarthritis.Results:The peripheral blood 25-OH VD level in the brucellosis group [20.31 (15.74, 24.35) ng/ml] was significantly lower than that of the control group [25.18 (21.13, 29.59) ng/ml], and the difference was statistically significant ( Z = - 5.07, P < 0.001). The peripheral blood 25-OH VD level [18.05 (13.79, 23.74) vs 22.43 (19.93, 28.25) ng/ml], CD4 + T cell count [(860 ± 275) vs (1 036 ± 376) cells/μl], and interleukin (IL)-6 levels [4.17 (2.14, 9.41) vs 7.83 (5.97, 11.34) ng/L] in the brucellosis combined with osteoarthritis group were significantly lower than those in the brucellosis without osteoarthritis group ( Z/t = - 2.88, 2.20, - 2.85, P = 0.004, 0.035, 0.004). Correlation analysis showed that the peripheral blood 25-OH VD level in patients with brucellosis complicated by osteoarthritis was positively correlated with the counts of CD45 +, CD3 + T, CD4 + T, CD8 + T, and natural killer cells ( r = 0.31, 0.26, 0.25, 0.25, 0.25, P = 0.007, 0.027, 0.032, 0.031, 0.032), and negatively correlated with IL-17A level ( r = - 0.40, P < 0.001). Conclusion:Patients with brucellosis have insufficient 25-OH VD, and those with osteoarthritis have lower 25-OH VD level, CD4 + T cell count, and IL-6 level than those without osteoarthritis.
6.Efficacy analysis of modified endoscopic autologous cartilage eustachian tube pharyngeal orifice tubo-plasty on patulous eustachian tube

Objective This study aimed to investigate the efficacy of modified endoscopic autologous cartilage eustachian tube pharyngeal orifice tuboplasty(MEACETT)in patients with patulous eustachian tube(PET).Meth-ods A retrospective analysis was conducted on the clinical data of 27 patients(30 ears)diagnosed with PET who underwent MEACETT.Autologous cartilage was used through the incision at the posterior end of the inferior turbi-nate and filled into the lateral wall of the pharyngeal orifice of the eustachian tube.Without affecting the movement function of the eustachian tube during swallowing,the collapse of the pharyngeal orifice was fully filled.Before and after the surgery,the visual analogue scale(VAS),the eustachian tube dysfunction questionnaire-7(ETDQ-7)and hospital anxiety and depression scale(HADS)was used for assessment to evaluate the surgical efficacy.Results There was no significant difference in depression scores before and after surgery(P>0.05).However,postopera-tive anxiety scores,ETDQ-7 scores,and VAS scores were significantly lower than preoperative scores(P<0.05).Among the 27 patients,9 showed significant symptom relief,13 exhibited partial relief,and 5 had no significant change compared to preoperative symptoms.The overall response rate of the treatment(significant relief and partial relief)was 81.48%(22/27).All surgeries were successfully performed.Except for secretory otitis media occurring in 2 cases,no major complications were observed.Conclusion MEACETT demonstrates significant symptom relief in PET patients,with high surgical safety and low complication rates,making it worthy of clinical promotion.
7.Research on the diagnostic efficiency of the"R"value of tubomanometry in detecting patulous eusta-chian tube

Objective To evaluate the diagnostic efficacy of the R value in tubomanometry(TMM)for the di-agnosis of patulous eustachian tube(PET).Methods The clinical data of 58 patients with PET and 65 controls were retrospectively analyzed.TMM was performed on both groups under nasopharyngeal pressures of 30,40,and 50 mbar respectively.The diagnostic efficacy of the R value for PET was evaluated through receiver operating char-acteristic(ROC)curves.Results In the control group,the average R values under nasopharyngeal pressures of 30,40,and 50 mbar were 0.86±0.50,0.76±0.41,and 0.68±0.34 respectively.In contrast,the corresponding R values in the PET group were significantly lower,which were 0.56±0.38,0.50±0.36,and 0.46±0.38 respec-tively.According to the ROC curve analysis,the areas under the curve(AUC)at these pressures were 0.62,0.74,and 0.74 respectively.The specificity and sensitivity of the R value under nasopharyngeal pressures of 30,40,and 50 mbar were 76.90%and 54.30%,74.60%and 68.10%,86.90%and 54.30%,respectively.Under pressures of 30,40,and 50 mbar,the incidence rates of R>1 in the control group and the PET group were 29.23%(38/130)and 12.77%(12/94)(x2=8.69,P=0.003),20.00%(26/130)and 6.38%(6/94)(x2=7.20,P=0.007),10.00%(13/130)and 3.19%(3/94)(x2=2.87,P=0.09)respectively.Conclusion Although the low R value in TMM reflects the presence of PET to some extent,it does not provide adequate sensitivity and specificity to serve as an independent diagnostic criterion for PET.
8.Influence of scraping along meridians combined with thunder-fire moxibustion on motor function and hemorheology in patients with shoulder-hand syndrome after ischemic stroke

Objective:To analyze the effect of meridian scraping combined with thunder fire moxibustion in improving the motor function and hemorheology of patients with shoulder hand syndrome (SHS) after ischemic stroke, in order to provide reference for clinical intervention of SHS patients after ischemic stroke.Methods:A prospective study was conducted using a simple random sampling method to select SHS patients after ischemic stroke admitted to Shijiazhuang Hospital of Traditional Chinese Medicine from January 2020 to June 2023, and were randomly divided into two groups. The thunder-fire moxibustion group were given routine nursing and thunder-fire moxibustion therapy, and the combined group were additionally given scraping therapy along meridians on the basis of thunder-fire moxibustion group. The Upper Limb Fugl-Meyer Motor Function Assessment Scale (U-FMA) score, Visual Analogue Scale (VAS) score, Action Research Arm Test (ARAT) score, shoulder joint activity score, hemorheological indicators (hematocrit, plasma viscosity, platelet adhesion rate) and activities of daily living (Barthel Index), etc. were compared between two groups before and after intervention.Results:Finally, 86 patients with SHS after ischemic stroke were included, with 42 patients in the thunder-fire moxibustion group, including 25 males and 17 females, aged (59.37 ± 7.64) years; 44 patients in the combined group, including 24 males and 20 females, aged (59.52 ± 7.61) years. Before intervention, there was no statistical significance in U-FMA score, VAS score, ARAT score, shoulder joint activity score, hemorheology index level and Barthel Index between two groups (all P>0.05). After intervention, the U-FMA score, ARAT score and Barthel Index in the combined group were (48.21 ± 4.85), (40.22 ± 4.64), (56.43 ± 9.76) points, respectively, which were higher than (44.56 ± 4.23), (35.98 ± 4.37), (51.97 ± 8.31) points in the thunder-fire moxibustion group, the differences were statistically significant ( t =3.71, 4.36, 2.28, all P<0.05). The VAS score, shoulder joint mobility score, blood cell volume, plasma viscosity and platelet adhesion rate in the combined group were (2.44 ± 0.87) points, (1.23 ± 0.25) points, 0.44 ± 0.02, (1.35 ± 0.32) mPa·s, (53.56 ± 5.71)%, respectively, which were lower than (3.05 ± 0.99) points, (1.58 ± 0.22) points, 0.46 ± 0.02, (1.72 ± 0.36) mPa·s, (62.53 ± 5.94)% in the thunder-fire moxibustion group, the differences were statistically significant ( t values were 3.04-7.14, all P<0.05). Conclusions:Scraping along meridians combined with thunder-fire moxibustion therapy can achieve good intervention effect in patients with SHS after ischemic stroke, improve the upper limb motor function and hemorheology of patients, and enhance the activities of daily living.
9.Clinical diagnosis and treatment analysis of primary pancreatic signet-ring cell carcinoma

Objective:To investigate the diagnosis, treatment, and prognosis of primary pancreatic signet-ring cell carcinoma (SRCC).Methods:A retrospective analysis was conducted on clinical data of 11 patients with pathologically confirmed SRCC treated at the Women and Children's Hospital Affiliated to Qingdao University and the Affiliated Hospital of Qingdao University between 2002 and 2024. The cohort included 10 male and 1 female patients, aged (65.6±9.2) years. Data on gender, age, clinical manifestations, biochemical markers, tumor biomarkers, surgical approaches, postoperative pathology, adjuvant the-rapy, and treatment outcomes were recorded. Postoperative survival was assessed via outpatient records and telephone follow-ups.Results:Among the 11 patients, all 10 male patients had a long-term smoking his-tory. Initial symptoms included epigastric pain (5 cases), jaundice (2 cases), postprandial upper abdominal discomfort (3 cases), and persistent hyperglycemia (1 case). Elevated total bilirubin with increased alanine transaminase and aspartate transaminase was observed in 4 cases, while 7 cases had normal liver function. Tumor biomarker profiles showed elevated carcinoembryonic antigen (CEA) alone in 2 cases, elevated carbohydrate antigen 19-9(CA 19-9) alone in 6 cases, concurrent elevation of CEA and CA19-9 in 2 cases, and normal tumor markers in 1 case. Tumor locations included the pancreatic head (8 cases) and pancreatic body/tail (3 cases). All patients underwent radical resection without major perioperative complications. Immunohistochemistry revealed perineural invasion (+ ) in 10 cases and Ki-67 ≥ 50% in 9 cases. Six patients received postoperative adjuvant therapy. The median disease-free survival was 14 months, and the median overall survival was 18 months.Conclusion:SRCC lacks specific clinical manifestations and carries a poor prognosis. Radical surgical resection remains the cornerstone of treatment, while adjuvant therapy may improve survival outcomes.
10.Construction and effect of auricular acupressure in patients with cough with different syndrome types

Objective To construction and investigate the application effect of auricular acupressure technology in patients with cough with different syndrome types.Methods From December 2022 to March 2023,the Delphi method was used to consult experts to form a technical scheme for auricular pressure in patients with cough with different syndrome types in the early and middle stages.122 patients with stage Ⅰ and stage Ⅱ lung cancer cough from the oncology department of 3 tertiary hospitals of Traditional Chinese Medicine in Beijing between July 2023 to January 2024 were randomly divided into an experimental group(61 cases)and a control group(61 cases).The experimental group referred to the constructed auricular acupressure technique program to identify and take acupoints,and the control group used the original auricular acupressure program for 7 d of continuous intervention.The changes in the scores of the Visual Analog Scale(VAS)and the scores of The MD Anderson Symptom Inventory for lung cancer(MDASI-LC)were observed in the 2 groups before and after the intervention.Results The final scheme included 3 first-level items,12 second-level items,and 14 third-level items.The 2 rounds of expert authority coefficients were 0.88,indicating a high degree of authority,and the Kendall harmony coefficients of the 2 rounds of correspondence were 0.170 and 0.130(P<0.001),respectively,and the results tended to be consistent.A total of 122 patients completed the study.After intervention,the VAS score of cough severity in the experimental group was 3.0(2.0,3.0)points,which was lower than 3.0(3.0,4.0)points in the control group.The MDASI-LC score of the experimental group was 17.0(9.5,31.5)points,which was lower than 28.0(15.5,47.5)points in the control group,and the difference was statistically significant(P<0.05).Conclusion The constructed auricular acupressure technique program has certain scientificity,and it can effectively relieve cough symptoms of stage Ⅰ~Ⅱ lung cancer patients,and the severity of symptom clusters has been improved,which provides the basis for TCM nursing intervention in cough symptoms of lung cancer patients.
