1.Simvastatin alleviates kidney ischemia reperfusion injury by inhibiting ferroptosis

ObjectiveTo investigate the effect and mechanism of simvastatin pretreatment on kidney ischemia reperfusion injury (IRI) in mice. MethodsFifteen male C57BL/6 mice aged 6-8 weeks were divided into three groups: Sham operation group (Sham group), kidney IRI group (IR group), and simvastatin pretreatment+kidney IRI group (SIM group). Hematoxylin-eosin (HE) staining of kidney tissue and detection of serum creatinine (SCr) and lactate dehydrogenase (LDH) were used to evaluate kidney injury. The levels of superoxide dismutase (SOD), reduced glutathione (GSH), malondialdehyde (MDA) and reactive oxygen species (ROS) were detected to evaluate oxidative stress. The contents of ferrous iron (Fe2+) and ferric iron (Fe3+) in kidney tissue were detected, and the morphological changes of mitochondria were observed by transmission electron microscope. The relative expression levels of Kruppel-like factor 2 (KLF2), glutathione peroxidase 4 (GPX4), solute carrier family 7 member 11 (SLC7A11), and acyl-coa synthetase long chain family member 4 (ACSL4) protein in kidney tissue were detected. ResultsCompared with the IR group, the SIM group had significantly reduced renal tubular injury and decreased contents of Scr and LDH in serum (P < 0.001). It also showed increased expression of SOD and GSH and decreased expression of MDA and ROS (P < 0.01). Simvastatin pretreatment reduced the contents of Fe2+ and Fe3+ in the tissues (P < 0.01) and alleviated mitochondrial damage. It also promoted the expression of KLF2 (P < 0.01), up-regulated the expression of ferroptosis-related protective proteins GPX4 and SLC7A11, and down-regulated the expression of ferroptosis-related damage protein ACSL4 (P < 0.05). ConclusionSimvastatin pretreatment may inhibit kidney ferroptosis by promoting the expression of KLF2 to alleviate kidney IRI.
2.Relationship between ischemia-reperfusion injury and rejection

Ischemia-reperfusion injury of solid organs is a common complication during transplantation, but its mechanism remains unclear and there are no effective prevention and treatment methods. Rejection is a common immune response of the graft, which is classified into hyperacute rejection, acute rejection and chronic rejection. Rejection and ischemia-reperfusion injury of solid organs may share some common mechanisms, and further research on their correlation may provide strategies for the prevention and treatment of ischemia-reperfusion injury of solid organs. This article mainly reviews the association between rejection and ischemia-reperfusion injury of solid organs.
3.Experience of pedicled tunica vaginalis grafts in the treatment of 90 cases of longsegment anterior urethral stricture >2cm caused by male genital lichen sclerosus

Objective To summarize 20 years' clinical experience of urethroplasty with pedicled tunica vaginalis grafts in the treatment of long-segment anterior urethral stricture (>2 cm) caused by male genital lichen sclerosus (GLSc). Methods A retrospective study was conducted on the clinical data of 90 patients undergoing urethroplasty with pedicled tunica vaginalis grafts in our hospital. Patients were divided into three groups according to the length of stricture: 4-7 cm (n=25), >7-10 cm (n=44), and>10 cm (n=21). The surgical conditions, changes in preoperative and postoperative maximum urinary flow rate (Qmax), and improvements in the international index of erectile function (IIEF-5) scores were compared among the three groups. Results During the follow-up of 6-48 (average 22.0±2.7) months, 88 patients exhibited unimpeded urination, with a success rate of 97.8%. Retrograde urethrography showed the urethral lumen was well-dilated, the penis was not deformed or strictured, and the sexual intercourse was normal. The Qmax of the three groups was significantly improved 2, 6 and 12 months postoperatively (P<0.01), but with no statistically significant differences among the three groups (P>0.05). Similarly, the IIEF-5 scores showed no statistically significant differences among or within the groups (P>0.05). No serious complications occurred after operation. Urethral stricture occurred in 2 cases after operation, one of which had smooth urination after holmium laser urethrotomy, and the other achieved urination after anterior urethral incision. Conclusion Urethroplasty with pedicled tunica vaginalis grafts is suitable for the treatment of long-segment anterior urethral stricture caused by GLSc. It has the advantages of high surgical success rate, convenient graft harvesting, simple operation, few trauma and few complications. This surgical approach is recommended for primary hospitals.
4.Cervical Kyphosis Based on Cervical Rehabilitation Training System:A Biomechanical Model Study

Objective A biomechanical model of cervical kyphosis under the effects of axial traction load and lateral push load was establised,so as to provide a theoretical basis for the treatment of cervical curvature abnormalities,and formulate the most appropriate treatment plan for patients.Methods Based on the CT scan data of patients,the axial data of the cervical spine was extracted to fit the cervical curvature curve.Using the Timoshenko beam theory and the cervical rehabilitation training system,a mathematical model of cervical kyphosis was established for analytical calculations to obtain the recovery curve of the cervical spine under load and the total load required to cure cervical kyphosis,and its rationality was also verified.Results The biomechanical model of cervical kyphosis was established.Under the effects of axial traction load and lateral push load,the cervical spine effectively developed in the direction of physiological bending.The total axial load and lateral load were 353 N and 5 649 N,respectively,and the total axial load increased with the increase in traction angle.The therapeutic moment of the total lateral load decreased as the Bordon value increased.The therapeutic moment of the axial load was smaller than that of the lateral load in the range of normal Bordon value,confirming the rationality of the loads.Conclusions The established biomechanical model of cervical kyphosis can accurately simulate the biomechanical characteristics of the cervical spine,and the analysis results were valid,providing a mechanical theoretical basis for the design of treatment plans for patients.
5.The study on the correlation between cognitive impairment of cerebral small vessel disease and white matter hyperintensity grade and cerebral atrophy grade

Objective To investigate the correlation between cognitive impairment of cerebral small vessel disease(CSVD)and white matter hyperintensity grade and cerebral atrophy grade.Methods The data of 213 patients with CSVD were analyzed retro-spectively.The semi-quantitative Fazekas visual score of brain MR was used to evaluate the white matter hyperintensity,and cerebral atrophy was evaluated by the whole cerebral cortex atrophy grade.Mini-mental state examination(MMSE)and Montreal cognitive assessment(Mo CA)were used to evaluate each patient and to classify according to the results.Results There was a statistically sig-nificant difference in age between male and female patients with CSVD(P=0.04,t=4.288).The mean age of males[(66.5±10.3)years]was less than females[(69.5±10.0)years].There was significant difference between patients with different gender and history of hypertension(P=0.02,t=5.424).There were statistical differences between white matter hyperintensity Fazekes grade and MMSE and Mo CA scores(P<0.001,χ2=5.639;P<0.001,χ2=5.843),and there were statistical differences between whole cerebral cortex atrophy grade and MMSE score(P=0.036,χ2=2.895).Conclusion There is a certain correlation between cognitive impairment of CSVD and head MR imaging evaluation grade.

8.A case report of reno-portal anastomosis liver transplantation for grade 4 portal vein thrombosis

The relevant clinical data were reviewed for a recipient of grade 4 portal vein thrombus undergoing reno-portal anastomosis liver transplantation on May 19, 2022. Liver function transaminase and bilirubin gradually normalized within 2 weeks after operation. An elevation of creatinine showed mild functional impairment at Day 5-7 post-operation and then recovered quickly. No portal vein thrombosis, gastrointestinal hemorrhage, ascites and other complications occurred within 2 years post-operation. The survival was excellent during 2-year follow-ups.
9.The impact of disease-related group payment methods on the diagnosis and treatment of inpatient medical insurance patients with neuromyelitis optica spectrum disorders in Xi'an and its improvement strategy

Objective:To preliminary investigate the impact of the diagnosis-related groups (DRG) payment method reform on the diagnosis and treatment of inpatient medical insurance patients with neuromyelitis optica spectrum disorders (NMOSD), and to propose potential improvement strategies.Methods:A single-center, retrospective study. From October 1, 2020, to September 30, 2022, 44 hospitalized medical insurance patients with acute-phase NMOSD diagnosed and treated at the First Affiliated Hospital of Northwest University (Xi'an First Hospital) were included in the study. Among them, there were 11 males and 33 females, with an average age of (40.8±20.2) years. According to the implementation time of DRG payment, patients were divided into two groups: group A, which consists of cases one year before the implementation of DRG payment from October 1, 2020 to September 30, 2021, and group B, which consists of cases one year after the implementation of DRG payment from October 1, 2021 to September 30, 2022, with 20 and 24 cases, respectively. Detailed information such as hospitalization duration, treatment methods, and hospitalization costs of the two groups of patients was collected. Comparative analysis was conducted on hospitalization costs and treatment methods between the two groups. For intergroup comparison, t-test was used for normally distributed data, and Mann-Whitney U test was used for skewed distributed data. Results:Among the 44 patients, 5 cases (5/24, 20.8%) received plasma exchange (PE) treatment, all of whom were in group B. The numbers of patients who received and did not receive intravenous immunoglobulin (IVIG) treatment were 9 and 11 in group A, respectively, and 7 and 12 in group B (except for 5 cases who received PE treatment), respectively. Compared with group A, there was no significant decrease in hospitalization duration ( t=0.004) and total hospitalization costs ( Z=0.036), as well as costs for western medicine ( Z=0.036), examinations ( Z=0.011), laboratory tests ( Z=0.040), treatments ( Z=0.017), and nursing ( Z=3.131) in group B, and the differences were not statistically significant ( P>0.05). For patients receiving PE treatment, except for the cost of western medicine ( Z=0.062, P=0.804), the other costs ( Z=8.288, 5.013, 11.400, 10.925, 9.126) were significantly higher than those of patients not receiving PE treatment, and the hospitalization duration ( t=20.474) was significantly prolonged, with statistically significant differences ( P<0.05). The total hospitalization costs of patients receiving IVIG treatment were significantly higher than those not receiving IVIG treatment in both group A and group B, with statistically significant differences ( Z=7.690, 10.314; P<0.05). There was no statistically significant difference in the comparison of total hospitalization costs between patients receiving IVIG treatment in group A and group B ( Z=0.137, P>0.05). Conclusions:There is no significant decrease in various hospitalization costs of NMOSD medical insurance patients in Xi'an after the implementation of DRG payment, especially for patients receiving PE treatment. It is suggested to optimize the rate stratification of NMOSD patients when implementing DRG payment methods.
10.Research progress of acetaldehyde dehydrogenase 2 in liver diseases

Acetaldehyde dehydrogenase 2 (ALDH2) is an important kind of aldehyde dehydrogenase in mitochondria, which has the function of eliminating acetaldehyde and other toxic aldehydes substances. Furthermore, it is abundant in liver and is closely related to the occurrence and development of a variety of liver diseases. ALDH2 genetic polymorphisms plays an important role in the occurrence of a variety of liver diseases in the human population.This paper mainly reviews the research progress of ALDH2 in liver diseases in recent years, with a view to provide theoretical basis for clinical prevention and treatment.
