1.Analysis of cochlear reimplantation surgery and factors influencing postoperative auditory and speech function.

Objective:The aim of this study was to present an institution's experience with cochlear reimplantation(CRI), to assess surgical challenges and post-operative outcomes and to increase the success rate of CRI. Methods:We retrospectively evaluated data from 76 reimplantation cases treated in a tertiary center between 2001 and 2022. Clinical features include caused of CRI, type of failure, surgical issues, and auditory speech performance were analyzed. Categorical Auditory Performance (CAP) and Speech Intelligibility Rating (SIR) scores were used to evaluate pre-and post-CRI outcomes. Our center's consecutive cohort of 1 126 patients had seven patients, while 69 patients were from other cochlear implant centers. Device failure was the most common cause of CRI(68/76), with the remaining cases including flap complications(3/76), magnet displacement(3/76), secondary meningitis(1/76), and foreign bodies around the implant(1/76). Postoperative auditory and speech outcome improved in 31.6%(24/76) of patients, remained unchanged in 63.2%(48/76), and decreased in CAP and SIR scores in 5.2%(4/76) of patients. Postoperatively, the seven patients with cochlear ossification and fibrosis scored lower on the overall CAP and SIR scale than non-ossification individuals, which is a significant factor in surgical success rates and auditory-speech outcomes. Conclusion:CRI surgery is a challenging but relatively safe procedure, and most reimplanted patients experience favorable postoperative outcomes. Medical complications and intracochlear damage are the main causes of poor postoperative results. Therefore, minimally invasive CI has a positive significance for reducing the difficulty of CRI surgery and improving the CI performance.

2.Comparative study of six visual disability-related standards in China based on ICF and ICD-11

Objective To compare six existing Chinese standards related to visual disability,including Disability Assessment and Code for Life Insurance(Insurance Standard),Specification for Ability Assessment of Older Adults(Elderly Stan-dard),Grading of Disability due to Human Body Injury(Judicial Standard),Standard for Identify Work Ability—Gradation of Disability Caused by Work-related Injuries and Occupational Diseases(Work Injury Standard),Standard for Assessment of Disability Grades of Military Personnel(Military Standard),and Classification and Grading Criteria of Disability(Disability Classification Standard).Methods Based on the theoretical framework of International Classification of Functioning,Disability and Health(ICF)and International Classification of Diseases,the 11th Revision(ICD-11),and with reference to World Report on Vision(WRV),this study analyzed and compared five key dimensions of visual disability across the six stan-dards:definition,classification,grading,assessment methods and coding systems.Results In terms of definition,the insurance,work injury,military,and disability classification standards emphasized vi-sual structure and function,focusing on visual acuity and visual field;the insurance and elderly standards incor-porated activities and participation,reflecting the concept of functional vision;the judicial standard lacked a dedi-cated definition of visual disability.In terms of grading,each standard graded visual disability into different lev-els acoording to different functional indicators.In terms of coding,only the insurance standard adopted the ICF framework,while others used non-ICF numeric codes or lacked standardized coding.In terms of assessment tools,the elderly standard employed a customized perceptual and participation scoring system,while others re-lied primarily on vision charts.Conclusion Most current Chinese standards focus on impairments in visual function,with limited attention to activity limitations and participation restrictions.It is needed for incorporating the concept of functional vision,aligned with ICF and WRV,to establish a unified conceptual framework,terminology,coding system for visual disability,and comprehensive assessments that reflect not only impairments but also the impact on daily functioning and en-vironmental interaction.
3.Significance of KL-6 combined with CRP and blood routine in evaluating early lung injury in patients with influenza A

Objective To explore the diagnostic value of combined detection of Krebs Von den Lungen-6(KL-6),C-reactive protein(CRP),and some indicators in blood routine for lung injury in patients with influ-enza A virus infection.Methods A total of 92 patients with influenza A admitted to the hospital in December 2023 were selected as the observation group.Based on whether there were obvious texture changes such as patchy and ground glass opacities in the lungs on X-ray examination,the patients were divided into a lung inju-ry positive group(T1 group,30 cases)and a lung injury negative group(T2 group,62 cases).Additionally,53 healthy individuals who underwent physical examinations during the same period were selected as the control group.Peripheral blood routine,CRP,and KL-6 levels were detected.Peripheral blood routine,CRP,and KL-6 levels were compared between the observation group and control group,and between T1 group and T2 group.receiver operating characteristic(ROC)curve were used to evaluate the diagnostic efficacy of individual indica-tors and combined detection for patients with lung injury who infected with influenza A.Results Compared with the control group,white blood cell count,neutrophil ratio,monocyte ratio,neutrophil/lymphoid ratio(NLR),monocyte/lymphoid ratio(MLR),platelet/lymphoid ratio(PLR),CRP and KL-6 levels in the obser-vation group were increased,while the lymphocyte ratio and platelet count decreased(P<0.05).Compared with the T2 group,the CRP and KL-6 levels increased and the incidence of anemia increased in the T1 group(P<0.05).The results of binary Logistic regression analysis showed that blood routine,KL-6,and CRP were all influencing factors of lung injury in patients with influenza A(P<0.05).The area under the curve of single detection of KL-6,CRP,blood routine,the combined detection of KL-6 and CRP,and the combined detection of KL-6,CRP and blood routine were 0.679,0.641,0.604,0.727 and 0.781.Conclusion KL-6 combined with CRP and blood routine testing has high diagnostic efficacy for lung injury in patients with influenza A and has significant clinical significance.
4.Relationship between serum miR-141-3p expression and the efficacy and prognosis of platinum based chemotherapy in patients with non-small cell lung cancer

Objective To investigate the relationship between serum microRNA(miR)-141-3p expression and the efficacy and prognosis of platinum based chemotherapy regimens in patients with non-small cell lung cancer(NSCLC).Methods A total of 154 NSCLC patients who received platinum based chemotherapy be-tween January 2018 and May 2021 were enrolled(NSCLC group),along with 78 healthy volunteers during the same period(control group)for comparison and analysis.Serum miR-141-3p expression was measured using real-time quantitative PCR,and its relationship with clinicopathological characteristics was analyzed.NSCLC patients were divided into an ineffective group and an effective group based on chemotherapy response,and further categorized into high miR-141-3p expression and low miR-141-3p expression groups based on the aver-age value of miR-141-3p expression.The relationship between miR-141-3p expression and the ineffectiveness of platinum based chemotherapy regimens and mortality in NSCLC patients was analyzed.The predictive effi-cacy of serum miR-141-3p expression for the ineffectiveness of platinum based chemotherapy regimens in NSCLC patients was analyzed by receiver operating characteristic curve.Kaplan Meier method was used to plot survival curves of NSCLC patients with different miR-141-3p expression levels.Results Serum miR-141-3p expression was significantly lower in NSCLC group compared to the control group(P<0.05).Patients with largest tumor size≥5 cm,poor differentiation,TNM stage Ⅲ-Ⅳ,lymph node metastasis,or distant me-tastasis showed lower serum miR-141-3p expression compared to those with largest tumor size<5 cm,mod-erate-to-high differentiation,TNM stage Ⅱ,and no lymph node or distant metastasis(P<0.05).Among 154 NSCLC patients,the inefficacy rate of platinum based chemotherapy was 59.74%(80/154).The expression of miR-141-3p in the ineffective group was lower than that in the effective group(P<0.05).Multivariate Logis-tic regression showed that high miR-141-3p expression was identified as an independent protective factor of the inefficacy of platinum based chemotherapy(P<0.05).The area under the curve for predicting the ineffi-cacy of platinum based chemotherapy using serum miR-141-3p expression was 0.816,with sensitivity of 77.50%and specificity of 68.92%.The 3-year survival rate for the 154 NSCLC patients was 55.19%(85/154).Patients with high miR-141-3p expression had a significantly higher 3-year survival rate compared to those with low miR-141-3p expression(P<0.05).Cox regression showed that high miR-141-3p expression was also identified as an independent protective factor of the mortality in NSCLC patients(P<0.05).Conclusion The low expression of miR-141-3p in the serum of NSCLC patients is closely related to pathologi-cal features,the efficacy and prognosis of platinum based chemotherapy regimens,and may become an auxiliary evalu-ation indicator for the efficacy and prognosis of platinum based chemotherapy regimens in NSCLC patients.
5.Effects of ovarian torsional reduction in young rats on ovary tissue structure and reproductive function on torsional side in adult stage

Objective To conduct the reductive reservation for torsional ovary in different states(.un-necrotic ovary,suspected necrotic torsional ovary and torsional necrotic ovary)after the ovarian torsion in pre-maturity SD rat,and to investigate its effect on the ovarian reproductive function in maturity stage.Methods A total of 32 SD rats with 3 weeks old were selected and randomly divided into 4 groups,8 cases in each group.The animal model of ovarian torsion was made by using the Turner method,which was similar to the animal model of testicular torsion,and either group was selected to undergo the sham operation as the con-trol group(CG group).The non-necrotic torsional ovary detorsion group(NNTOD group),suspected necrotic torsional ovary detorsion group(SNTOD group)and necrotic torsional ovary detorsion group(NTOD group)were established respectively.When the animals were fed to sexual maturity at 8 weeks of age,the experimen-tal animals were sacrificed by vertebral dislocation method,and the ovary tissues on the torsional side were cut for HE staining and transmission electron microscopy to examine the changes of ovarian histomorphology and mitochondrial structure.Ovarian cell apoptosis was detected by Tunel assay.ELISA method was used to de-tect the follicle-stimulating hormone(FSH),luteinizing hormone(LH)and anti-mullerian duct hormone(AMH)levels in centrifugal blood.Results The ovarian structure in the CG group and NNTOD group was clear,the ovarian follicles at different levels were developed well;the electron microscopy showed normal mi-tochondria.The ovarian partial structure in the SNTOD group was disorganized,the number of follicles at all levels of growth was decreased;electron microscopic examination showed a little damage of mitochondria.In the NTOD group,the arrangement of ovarian structure was obviously disordered,the number of growing folli-cles at all levels was significantly reduced;the electron microscopy showed that most of the mitochondria were obviously swollen and severely damaged.Compared with the CG group,the apoptosis rate in the SNTOD group and NTOD group was significantly increased(P<0.05).There was no statistically significant differ-ence between CG group and NNTOD group(P>0.05).Compared with the NTOD group,the apoptosis rate in the SNTOD group was decreased(P<0.05).Compared with the CG group,the levels of LH and FSH in the SNTOD and NTOD groups were increased,and the AMH level was decreased(P<0.05).The LH and FSH levels in the SNTOD group were lower than those in the NTOD group,the AMH level was higher than that in the NTOD group(P<0.05).Conclusion After reduction of the torsional suspected necrotic ovary in the prematurity rat,the ovarian reproductive function in the torsional side during sexual maturity period is slightly injured;while after reduction of torsional necrotic ovary restoration,its reproductive function is appar-ently damaged.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
8.Study on the diagnostic value of 3D ultrasound combined with 2D 2D-TVS on uterine arteriovenous fistula

Objective:To explore the application value of three-dimensional ultrasound(3D-US)combined with two-dimensional trans vaginal sonography(2D-TVS)in diagnosing uterine arteriovenous fistula(UAVF).Methods:A total of 36 patients with suspected UAVF who admitted to Baoding Maternal and Child Health Care Hospital from January 2024 to December 2024 were retrospective selected.All of them underwent 3D-US,2D-TVS and combined examinations of 3D-US and 2D-TVS.The gynecological intravenous contrast-enhanced ultrasound was used as the"gold standard"to compare and analyze the sensitivity,specificity,accuracy rate,positive predictive value and negative predictive value of 3D-US,2D-TVS and the combined examination of them.The diagnostic efficacies of the three diagnostic methods were calculated by the four-grid table method and the analysis of the receiver operating characteristic(ROC)curve.The detection rates of 2D-TVS and combined examination of 2D-TVS and 3D-US for UAVF imaging signs were compared.Results:In 36 patients,gold standard confirmed 26 positive cases and 10 negative cases.The sensitivity,specificity,accuracy rate,positive predictive value and negative predictive value of 2D-TVS examination were respectively 69.23%,80.00%,72.22%,90.00%and 50.00%.These indicators of 3D-US examinations were respectively 84.62%,90.00%,86.11%,95.65%and 69.23%.These indicators of the combined examination were respectively 92.31%,90.00%,91.67%,96.00%and 81.82%.There were not statistically significant differences in these indicators among the three diagnostic methods(P>0.05).However,the sensitivity,accuracy rate of the combined examination were respectively higher than those of 2D-TVS examination,and the differences were statistically significant(x2=4.457,4.600,P<0.05).The detection rates of the sings included lake-like,multicolored Mosaic and blood flow spectrum of high-speed low-resistance in UAVF images of the combined examination were all higher than those of 2D-TVS examination,and the differences were statistically significant(x2=4.000,4.431,4.600,P<0.05).Conclusion:The combination of 3D-US and 2D-TVS can significantly improve the diagnostic sensitivity,accuracy and detection rate of imaging signs in the diagnosis for UAVF,which has important clinical application value.
9.Construction of an evaluation index system for the effectiveness of ideological and political education in the curriculum of professional master's degree nursing programs

Objective:To construct an evaluation index system for the effectiveness of ideological and political education in the curriculum of professional master's degree nursing programs, providing an evaluation tool for objectively assessing the teaching quality of ideological and political education in these programs.Methods:The initial draft of the index system was designed based on a review of domestic and international literature. From October 2022 to January 2023, 15 experts were selected using convenience sampling for three rounds of Delphi expert consultation, and the analytic hierarchy process was used to determine the weight of the indicators.Results:The positive response rate from the 15 experts in all three rounds of consultation was 100% (15/15), and the expert authority coefficient was 0.813. The Kendall's coefficient of concordance for the overall indicators in the third round was 0.116 ( P<0.01). The constructed evaluation index system includes 5 first-level indicators (goal and concept, faculty, course resources, teaching implementation, teaching effectiveness), 12 second-level indicators, and 49 third-level indicators. Conclusions:The evaluation index system for the effectiveness of ideological and political education in the curriculum of professional master's degree nursing programs demonstrates good scientific validity and reliability. It can provide objective and quantitative evaluation criteria for the development of ideological and political education in these programs.
10.Thyroid surgery in patients with uncontrolled hyperthyroidism: Perspectives from endocrinologists

Hyperthyroidism is a prevalent endocrine disorder commonly managed with antithyroid medications, radioactive iodine, or surgical intervention. Conventionally, thyroidectomy is performed only after achieving a euthyroid state to minimize intraoperative and postoperative complications. However, emerging evidence suggests that, under specific conditions, thyroidectomy may be safely conducted even in the absence of complete preoperative disease controlled. This review provides an expert endocrinologist′s perspective on the safety and feasibility of thyroid surgery in patients with uncontrolled hyperthyroidism, examines various preoperative optimization strategies, and underscores the critical role of perioperative management in ensuring optimal surgical outcomes.
