1.Study of association of sedentary time and physical activity with development of chronic obstructive pulmonary disease in non-smoking women aged 40 years and above in Songjiang District, Shanghai

Objective:To analyze the association of sedentary time and physical activity with the risk of developing chronic obstructive pulmonary disease (COPD) in non-smoking women aged 40 years and above in Songjiang District, Shanghai.Methods:Based on a natural population-based cohort in Songjiang, a total of 18 707 non-smoking women who were aged 40 years and above and without COPD at baseline survey were enrolled in the study. Cox proportional risk regression model was used to analyze the associations of the duration of sedentary behavior, physical activity with the risk for COPD at baseline survey, and the hazard ratio ( HR) of risk for COPD and its 95% CI were calculated. Stratified analyses were performed based on age, BMI, history of respiratory diseases and so on. Sensitivity analyses were conducted by excluding the cases diagnosed with COPD within one year after the baseline survey. Results:As of March 31, 2024, a total of 691 new COPD cases had been recorded after a median follow-up time of 6.96 years with an incidence density of 53.22 per 10 000 person-years. After adjusting for relevant confounders, in the tertile subgroups of sedentary time, the risk for COPD reduced by 17% in the short sedentary time group compared with the long sedentary time group ( HR=0.83,95% CI:0.70-0.99). Compared with the low physical activity level and long sedentary time group, the risk for COPD reduced by 24% in the high physical activity level and short sedentary time group ( HR=0.76, 95% CI: 0.61-0.95) and by 23% in the low physical activity level and short sedentary time group ( HR=0.77, 95% CI: 0.60-0.97). Compared with the non-physical exercise and long sedentary time group, the risk for COPD reduced by 28% in the non-physical exercise and short sedentary time group ( HR=0.72, 95% CI: 0.60-0.87). These associations remained when the cases diagnosed with COPD within one year of the baseline survey were excluded. Conclusions:Increasing physical activity and reducing sedentary time have beneficial effects to prevent COPD in non-smoking women, and reducing sedentary time alone may also reduce the risk for COPD if increasing physical exercise or other physical activity is not possible.

3.Study on the association of diet pattern with chronic obstructive pulmonary disease in population aged 40 years and above in Songjiang District, Shanghai

Objective:To explore the incidence of chronic obstructive pulmonary disease (COPD) in a cohort aged 40 years and above in Songjiang District, Shanghai, and to analyze the association of Mediterranean diet pattern and dietary approaches in stopping hypertension pattern (DASH) with the risk of developing COPD.Methods:Based on a natural population cohort in Songjiang District, Shanghai, 27 474 adults aged 40 years and above who did not have COPD at baseline were enrolled in the study. The Cox proportional risk regression model was used to analyze the association of baseline Mediterranean diet pattern score and DASH score with the risk of COPD, and the hazard ratio ( HR) of the risk and its 95% CI were calculated. Restricted cubic spline was used to analyze the nonlinear association between the two diet scores and the risk of COPD. Stratified analyses were performed according to gender, age, smoking status, etcetera. Sensitivity analyses were conducted by censoring cases diagnosed within one year after the baseline survey or people with a history of malignant tumor disease. Results:As of June 30, 2023, after a median follow-up time of 6.21 years, there were 1 089 (4.0%) new COPD cases with an incidence density of 64.00 per 10 000 person-years. After adjusting for relevant confounders, in the Mediterranean tertile subgroups under diet pattern score, the risk of developing COPD could be reduced by approximately 14% in the intermediate scoring group ( HR=0.86, 95% CI: 0.75-0.99) and 15% in the highest scoring group ( HR=0.85, 95% CI: 0.72-0.99) compared to the lowest scoring group. The association remained after censoring cases diagnosed within one year of the baseline survey ( HR=0.82, 95% CI: 0.70-0.95; HR=0.82, 95% CI: 0.68-0.97) or censoring people with a history of malignant tumor disease ( HR=0.84, 95% CI: 0.73-0.97; HR=0.84, 95% CI: 0.71-0.99). No statistical association was found between the DASH score and the risk of COPD. Conclusions:The Mediterranean diet pattern was associated with a lower risk of COPD. Increasing the intake of vegetables, fruits, legumes, and whole grains and decreasing the intake of red meat and others can reduce the risk of COPD. No association was found between the DASH dietary pattern and the risk of COPD in this community population.
4.Effect of Angiotensin Receptor-neprilysin Inhibitor on Acute Kidney Injury in Patients With Acute Decompensated Heart Failure

Objective::The risk of acute kidney injury (AKI) is high in patients with acute decompensated heart failure (ADHF). The aim of this study is to analyze the role of urinary neutrophil gelatinase-associated lipocalin (uNGAL) in diagnosing AKI in patients with ADHF and evaluate the therapeutic effect of angiotensin receptor-neprilysin inhibitor (ARNI) on AKI.Method::Sixty patients with ADHF were enrolled at the First Affiliated Hospital of Kangda College of Nanjing Medical University from January 2020 to June 2021, and randomized into 2 groups (ARNI group: 30 patients treated with tablets of sacubitril valsartan sodium; and angiotensin-converting enzyme inhibitor (ACEI) group: 30 patients treated with benazepril). The uNGAL level was measured immediately after as well as 1, 2, 3, and 7 d after hospital admission. The serum creatinine (sCr) level and estimated glomerular filtration rate (eGFR) were measured immediately as well as 2 and 7 d after hospital admission. The urine volume, dose of loop diuretics, and duration of hospital stay (DoHS) were recorded.Result::The most valuable diagnostic metric for AKI in patients with ADHF was the uNGAL level 1 d after hospital admission, which had a sensitivity of 0.94, specificity of 0.84, and optimal cutoff of 125.62 μg/L. In the presence of AKI, during the first 2 d, patients in the ARNI-AKI and ACEI-AKI groups showed an increase in the sCr level and a reduction in the eGFR level, but there was no significant difference between the 2 groups ( P > 0.05). After 7 d of treatment, the sCr level decreased and the eGFR level increased in both groups, with a significantly greater changes being observed in the ARNI-AKI group than in the ACEI-AKI group ( P < 0.05, respectively). In the absence of AKI, the difference in the sCr level and eGFR between the 2 groups was not significant. The DoHS ((11.25 ± 2.38) d vs. (14.11 ± 2.89) d), urinary microalbumin level ((22.95 ± 6.04) mg/L vs. (31.91 ± 2.18) mg/L), and daily dose of loop diuretics ((19.03 ± 3.04) mg/d vs. (23.62 ± 4.46) mg/d) were significantly lower in patients with AKI in the ARNI group than in the ACEI group ( P < 0.05, respectively). Conclusion::In patients with ADHF, uNGAL measurement enables the diagnosis of AKI earlier than that using the sCr level by 1 to 2 d. ARNI treatment reduced the sCr level, facilitated eGFR recovery, reduced the daily dose of loop diuretics, and decreased the DoHS compared with that in patients receive ACEI treatment.

6.Genotyping of clinical Stenotrophomonas maltophilia isolates by pulsed field gel electrophoresis

Objective To identify the genotype of clinical stenotrophomonas maltophilia(SMA) isolates and investigate the characteristics of SMA in nosocomial infections.Methods Totally 165 strains of SMA were clinically isolated during the period of 2004 to 2007.Disc diffusion test(K-B method) was used for antibiotic susceptibility.qacE△1 gene was detected by polymerase chain reation(PCR).The gene homology in the SMA strains was analyzed by pulsed field gel electrophoresis(PFGE).Results Among the tested SMA strains 87.9% sourced from low respiratory tract infection.The antibiotics with more than 80% of sensitive rate against SMA were minocycline,trimethoprim-sulfamethoxazole and levofloxacin.The positive rate of qacE△1 gene was 13.3% in 60 tested strains.The analysis of gene homology for the 11 clinical strains showed that two genotypes from identical clone were found in both respiratory ICU and emergency ICU respectively.Conclusions SMA was an important pathogenic bacterium in nosocomial infections.The treatment for SMA infection is very difficult since its multi-drug resistance.More attention for effective sterilization and isolation of patients must be paid to prevent the transmission of SMA from same clone.
7.Observation of Antibacterial Activity of Galla Chinensis etc. 14 Kinds of Traditional Chinese Medicine against 112 Strains of Staphlococcus Aureus in Vitro

Objective To evaluate the antibacterial activity of Galla chinensis etc.14 kinds of TCM against 112 strains of Staphylococcus aureus in vitro. Method Deterimination of antibacterial activity of Galla chinensis etc.against 112 strains of Staphylococcus aureus was performed by using new agar dilution method. Results The antibacterial efficiency of Galla chinensis, Forsythia suspensa and Acacia catechu (L.) Willd against 112 strains Staphylococcus aureus was good, and the MIC90 was 0.102, 0.244, 1.19 mg/mL respectively. Conclusion Galla chinensis etc. 14 kinds of TCM had strong antibacterial activity agsinst 112 srains of Staphylococcus aureus.
8.Antibacterial Activity for Prunus Mume Sieb et Zucc against 308 Clinical Isolates

Objective To observe antibacterial efficiency of Prunus mume Sieb et Zucc against clinical isolates. Method Antibacterial activity of Prunus mume Sieb et Zucc against 308 strains of clinical isolates were determined by agar dilution method. Results MIC50 of Prunus mume Sieb et Zucc against staphylococcus aureus (112 strains), S.epidermidis (112 strains), and Enterococci (28 strains) were 0.72, 1.44, 0.72 mg/mL, and MIC90 were 1.44, 1.44, 0.72 mg/mL respectively. The MIC90 of Prunus mume Sieb et Zucc to Klebsiella pneumoniae and E. coli were 2.88, 1.44 mg/mL respectively. Conclusion Prunus mume Sieb et Zucc has good antibacterial activity against Gram positive cocci and some Gram negative bacilli.
9.Nutritional support by the way of peripheral vein in the patients with severe acute pancreatitis

Objectives:To investigate the effects of nutritional support by the way of peripheral vein(PV) in the patients of severe acute pancreatitis(SAP). Methods:The treatment results of nutritional support by the way of PV was observed retrospectively within the latest eight years in the patients of SAP. Results:31 cases were cared,and no complications relating to the use of PV in TPN happened. Conclusions:Nutritional support is an important and effective treatment in SAP patients,and the use of PV is an effective and safety route.
