1.Treatment of the fibular fracture in Degree Ⅱ ankle supination-external rotation injury of the Dias-Tachdjian classification in children

Objective:To investigate the necessity of internal fixation of the fibular fracture in the treatment of degree Ⅱ ankle supination-external rotation injury by the Dias-Tachdjian classification in children.Methods:A retrospective study was conducted to analyze the data of 69 children with ankle fracture (degree Ⅱ ankle supination-external rotation injury by the Dias-Tachdjian classification) who had been treated at Department of Orthopedic Trauma, The People's Hospital of Weifang and Department of Pediatric Orthopedics, Tianjin Hospital from January 2015 to October 2021. There were 41 males and 28 females with an age of (10.1±1.4) years, and 37 left and 32 right sides affected. The patients were divided into 2 groups according to whether internal fixation of the fibular fracture was performed. Group A consisted of 21 cases with fibular internal fixation and group B of 48 cases without fibular internal fixation. The preoperative data, operation time and operation expenses were recorded and compared between the 2 groups. At the last follow-up, the anteroposterior and lateral radiographs of bilateral full length lower limbs and ankle joints were taken; the lateral distal tibial angle (LDTA) and the anterior distal tibial angle (ADTA) on the affected side, and the disparity between bilateral ankle tibiotalar angles were measured; ankle function was assessed according to the ankle-hindfoot score of American Association of Foot and Ankle Surgery (AOFAS); the occurrence of premature physeal closure (PPC) was recorded.Results:There was no significant difference in the preoperative general data between the 2 groups, indicating comparability ( P>0.05). All patients were followed up for (19.5±4.1) months. At the last follow-up, the LDTA on the affected side was 89.6° (87.9°, 90.5°) in group A and 88.6°±1.9° in group B; the ADTA on the affected side was 80.9° (79.0°, 81.4°) in group A and 80.0° (78.6°, 81.2°) in group B; the disparity between bilateral ankle tibiotalar angles was 1.1°±0.5° in group A and 1.2°±0.5° in group B; the AOFAS ankle-hindfoot score was (89.5±5.2) points in group A and 89.0 (87.0, 92.0) points in group B. There was no statistically significant difference between the 2 groups in the above items ( P>0.05). The incidence of PPC was, respectively, 14.3% (3/21) and 14.6% (7/48) in groups A and B, showing no statistically significant difference ( P>0.05). Conclusions:In the treatment of degree Ⅱ ankle supination-external rotation injury in children, internal fixation of the fibular fracture has no significant effect on the imaging angles or the function of the ankle joint. After anatomic reduction of the distal tibia, the fibular fracture can be treated without internal fixation to reduce operational trauma, shorten operation time and reduce operation expenses.
2.Analysis of surveillance data of foodborne disease from a level a tertiary general hospital in Shandong province from 2019 to 2021

Objective Analyze the surveillance results of foodborne illness from a level A tertiary general hospital in Shandong Province from 2019 to 2021,to provide a realistic basis for the continuous development and improvement of foodborne disease surveillance.Methods The data were collected from a level A tertiary general hospital in Shandong Province from 2019 to 2021 in the food safety risk assessment business application platform of national health insurance information project of National Health Security Disease Control Information System,and stool/anal swab samples were collected as required for pathogen detec-tion.Results From 2019 to 2021,a total of 2 323 cases of foodborne diseases were reported and 682 samples were collected,with a sampling rate of 29.36%,and 134 positive cases were detected,with a detection rate of 19.65%.The difference between the detection rates of different years was statistically significant(P<0.05).The detection rate of norovirus was the highest(9.53%),followed by Vibrio parahaemolyticus(8.94%)and Salmonella(1.91%).The difference between the detection rates of different pathogens was statistically significant(P<0.05).The sex ratio of 2 323 cases was 1:1.28.Among the different age distributions,20-29 years old accounted for the highest proportion(29.19%),followed by 30-39 years old(27.21%)and<20 years old(11.71%).The highest number of cases was reported from June to September,the peak was reached from July to Au-gust,and the number of cases was gradually reduced in October and later.Among different occupational groups,housework and unemployed people accounted for the highest proportion(30.18%),followed by farmers(15.93%),students(13.65%)and commercial service workers(11.80%).The main symptoms of the digestive system were diarrhea in 2 093 cases(90.10%),abdominal pain in 1 741 cases(74.95%),nausea in 1 575 cases(67.8%)and vomiting in 1 401 cases(60.31%).The main diarrhea was watery stool in 1 954 cases(84.12%).The main systemic symptoms were fatigue in 454 cases(19.54%)and dehydration in 212 cases(9.13%).The main clinical diagnosis was acute gastroenteritis in 1 980 cases(85.23%)and infec-tious diarrhea in 199 cases(8.57%).The suspected exposed foods were mainly aquatic animals and their products(25.10%)and meat and meat products(19.37%).The main eating places of suspected exposed foods were families(57.34%),followed by restaurants(21.91%),other places(5.94%)and street food stalls(5.21%).Conclusion From 2019 to 2021,the pathogens of foodborne diseases detected were mainly norovirus and Vibrio parahaemolyticus,mainly in the age group of 20 to 39 years old,and the occupation distribution was mainly household and unemployed people,farmers,students and commercial serv-ice personnel,with digestive system symptoms such as nausea,vomiting,abdominal pain and diarrhea.The suspected exposed foods are mainly aquatic animals and their products and meat and meat products,and the main eating places are families,restau-rants and street food stalls.The surveillance and reporting of foodborne diseases should continue to be strengthened in the future.
3.Effects of transcranial magnetic stimulation on the swallowing and brain-stem auditory evoked potentials of dysphagic stroke survivors

Objective:To observe any effects of contralateral repeated transcranial magnetic stimulation (rTMS) of the swallowing motor cortex on the swallowing and brainstem auditory evoked potentials (BAEPs) of stroke survivors with dysphagia.Methods:A total of 83 stroke survivors with dysphagia were randomly divided into an ipsilesional stimulation group ( n=22), a contralesional stimulation group ( n=21), a bilateral stimulation group ( n=20), and a control group ( n=20). In addition to their conventional dysphagia training, those in the three stimulation groups received 3Hz rTMS while the control group was given fake stimulation. The treatment was administered daily for 20 minutes, 6 days a week, for 5 consecutive weeks. Before and after the treatment, swallowing function was assessed videofluoroscopically and using the Dysphagia Outcome and Severity Scale (DOSS). The oral and pharyngeal stages of swallowing were evaluated using the videofluoroscopic dysphagia scale (VDS). Brain stem conduction was assessed using BAEPs. Results:After treatment the average DOSS scores of all 4 groups were significantly better than before the treatment. The average DOSS scores of the contralesional and bilateral sti-mulation groups were then significantly better than those of the other two groups. The sub-item and total VDS scores of all 4 groups had decreased significantly, but the average score of the bilateral stimulation group was significantly lower than the control group′s average. Ipsilesional stimulation significantly improved the VDS sub-item scores for the triggering of pharyngeal swallowing, laryngeal elevation, and pharyngeal transit time compared with the control group. In the contralesional stimulation group the average total score and the VDS sub-item scores for apraxia, premature bolus loss, oral transit times, the triggering of pharyngeal swallowing, vallecular residue, laryngeal elevation, coating on the pharyngeal wall, and pharyngeal transit time were significantly lower than those of the control group, on average. After the treatment the latencies of BAEP waves I, III and V and the I-III, III-V and I-V interpeak intervals had decreased significantly in all four groups, but the average latencies and intervals of the bilateral and contralesional groups were significantly shorter than those of the control group. The latencies and intervals of the bilateral stimulation group were then significantly shorter than those in the ipsilesional stimulation group on average. The average latency of wave V in the bilateral stimulation group (6.53±0.73ms) was significantly shorter than that in the contralesional stimulation group after the treatment.Conclusion:Bilateral rTMS over the swallowing motor cortex combined with conventional dysphagia training can significantly improve the swallowing of dysphagic stroke survivors.
4.In Vitro Degradation Behavior of Absorbable Interface Screws.

The composite material PLGA compounded with β-tricalcium phosphate (β-TCP) was prepared by melt blending method, and the absorbable interface screw was prepared by injection molding process. Prepare PBS buffer that simulates human body, conduct in vitro degradation experiments on interface screws according to relevant national and industry standards, then test and characterize interface screws at different time points for degradation of intrinsic viscosity, average molecular weight distribution, mass loss, mechanical properties and thermal properties. According to the degradation performance-time curve, determine the time node at which the interface screw loses the mechanical properties. In this paper, the in vitro degradation behavior of interfacial screws prepared from PLGA and β-TCP composites was studied in detail, providing a reference and basis for the degradation behavior of absorbable products prepared from PLGA and β-TCP composites.

5.Application of step-down thinking mode in safe transportation of patients after general anesthesia operation

Objective To explore the application of descending step-down mode in safe transportation of patients after general anesthesia operation. Methods A total of 25 nurses in the Operation Room of the First People's Hospital of Ji'ning in Shandong Province were trained from January 2017 to June 2017. The nursing ability of nurses in operation room before and after training and the incidence of adverse events were compared. Results After the training, the score of the operation room nurse's theory examination, the emergency test score and the patient's transshipment ability were (85.26±4.96), (86.77±4.77) and (88.98±5.02), which were all higher than those before training (62.33±3.56), (61.58±4.12) and (62.44±4.78) (P<0.05). The transportation time after training was (9.52±1.85) min, which was shorter than that before training (15.21±3.40) min (P< 0.05). The incidences of adverse events such as the dropping or blocking of the infusion tube, the interruption of oxygen supply, the equipment failure and the fluctuation of the blood pressure of the patients after the training were 2.50%, 2.50%, 3.75% and 1.25%, which were all lower than before training (8.75%, 7.50%, 12.50%, 10.00%) (P< 0.05). The patient satisfaction after training (95.00%) was higher than that before training (80.00%) (P< 0.05). Conclusions The reduced step thinking mode can effectively improve the nursing skills of nurses in operation room and the safe transport ability of patients after general anesthesia, and reduce the occurrence of adverse events in the course of patients' transshipment after general anesthesia and improve the patient satisfaction.
6.Effects of Yuanzhi San on Ethology and Cerebral Acetylcholinesterase Activity of Memory Disorder Mouse Model Induced by Scopolamine

Objective To observe the effects of Yuanzhi San (YZS) on the ethology and cerebral acetylcholinesterase(AchE) activity of mouse model of memory disorder induced by scopolamine. Methods Sixty mice were randomly divided into six groups, namely blank control group, model group, positive medicine group, and low-dose, middle-dose and high-dose YZS groups. Except for the blank control group and model group were given the normal saline, the mice in other groups were administered with the corresponding drugs for 10 days. And then, mice in the medication groups were given subcutaneous injection of scopolamine in the dose of 3mg/kg to induce memory disorder model. Morris water maze test and step-down test were adopted for the observation of the learning-memory ability of the mice, and at the end of the tests, the activity of AchE in mouse cerebral cortex was measured by a biochemical method. Results Compared with the model group, escape latency was decreased, and retention time and swimming distance in the effective area in Morris water maze test were prolonged in YZS groups (P<0.05); latency in step-down test was also prolonged (P<0.05). YZS had an effect on decreasing AchE activity in the cerebral cortex of model mice (P < 0.05 compared with the model group). However, the differences were insignificant among the medication groups(P>0.05). Conclusion YZS exerts certain effect on improving learning-memory ability of memory disorder mice induced by scopolamine , and the mechanism might be related with the inhibition of AchE activity in the cerebral cortex of model mice.
7.Trunk flexor and extensor strength and the kinematic parameters of gait among stroke survivors

Objective To correlate the isokinetic strength of stroke survivors' trunk flexors and extensors with the kinematic parameters of their gait.Methods Thirty stroke survivors and 30 healthy counterparts formed an observation group and a control group.The strength of everyone's trunk flexors and extensors was measured isokinetically,including the flexor peak torque (FPT),extensor peak torque (EPT) and the ratio of the two (F/E).The test velocities were 30°/s,60°/s and 90°/s.And the kinematic parameters of their gait,including the step length,stride,velocity,frequency,width and cycle time were measured using a 3D gait analysis system,as well as the percentage of stance phase,swing phase,double support phase and single support phase.Results No significant differences were found between the average FPT,EPT or F/E of the two groups at any of the three test velocities.At the 30°/s test velocity the average FPT,EPT and F/E of the observation group were all positively correlated with the average step length,stride,velocity,steps/min,width and cycle time,as well as with the stance phase,swing phase,double support phase and single support phase percentages.The strongest correlation was between F/E and velocity (r=0.57),followed by frequency (r=0.54).Conclusion litating the walking ability of stroke survivors and correcting their gait should focus on strengthening the trunk muscles and optimizing their coordination.
8.Sampling comparative analysis on military flying cadets by comprehensive assessment for two years

Objective To analyze the change in qualification rate of comprehensive assessment , and to provide an effective theoretical basis for future selection and recruitment of military flying cadets .Methods Group A consisted of 400 cadets who were randomly chosen from the final selection with the physical examination conclusions of disqualifiedandqualified by comprehensive assessmentin a certain year via the cross-sectional investigation method , and group B was composed of 400 cadets who were chosen from the following year with the same method as group A .A questionnaire con-cerning demographic features was conducted among all cadets .A database was established with Epidata 3.02 software, and SPSS 16.0 software was used to analyze diseases which were qualified by comprehensive assessment.Results The qualification rate of comprehensive assessment in group A was higher than group B (45.00%vs 34.75%, P<0.01),there were differences between the two groups in overweight ( underweight ) ,deflection of nasal septum , gallbladder polyp , spinal malformation and undertall(P<0.05 or P<0.01).Conclusion The expert consensus which is made by the air force ex-perts through their work experience and the foreign air force standards has a positive application effect in the practical work . It is an efficient supplement to the physical examination standards and has an important effect on military aviator selection .
9.Psychosocial factors of suicide attitudes among new recruits in training camp

Objective To explore the psychosocial influencing factors of suicide attitudes among new recruits in training camp.Methods Cluster sampling was conducted and 5 200 new recruits in 11 training camps were investigated with suicide attitude questionnaire(QSA),symptom checklist 90(SCL-90) and self-designed questionnaire.Influencing factors associated with suicide attitudes were analyzed with muhiple linear regression analysis.Results The score of awareness of property of suicide behavior dimension of QSA were (3.71±0.58),and (2.77±0.47) of the attitude to the suicide dimension,(2.43±0.43) of the attitude to family members of the suicide dimension and (3.13±0.86) of the attitude to euthanasia dimension.The results of multiple linear regression analysis indicated that recruits who came from single parent family,had a history of family violence,had a introvert character,had poor emotional stability,had a history of psychiatrist visiting,took Sedatives more often,had more negative life events in past year,tended to use unhealthy cathartic method of emotion,had more difficulty in adjusting to the army life,had lower factor score of obsessivecompulsive symptoms in SCL-90,and higher factor score of depression and paranoia,tended to hold an admissive attitude to suicide behavior.Conclusion The recruits in training camp don' t accept suicide behavior as a whole,and their suicide attitudes were influenced by mental health states and many other psychosocial factors.Suicide prevention education should be targeted.
10.Value of Vienna Test System in psychological screening of flying cadets

Objective To study the applicability of Vienna Test System ( VTS) to psychological screening of flying cadets,and to establish a corresponding standard model for flying cadets .Methods VTS and pilot psychological selection system were used to evaluate the mental health of 134 cases of flying cadets , SPSS 13.0 was used to compare their consistency and relevance.Results The results of VTS included the raw score of overall mean duration [(46.68 ±21.06)s] on 2hand test, the correct reaction times (246.54 ±25.88) on DT test, the normal values of overall mean duration [(27.84 ±21.98)s] on 2 hand test,and the correct reaction times (60.57 ±18.61) on DT test and other indexes. Pearson coefficient was calculated as predictive validation .The composite score of VTS was significantly positively correla-ted with the first platform(r=0.46,P<0.0001), the second platform(r=0.27,P<0.05) and the psychological perform-ance of the pilot psychological selection system (r=0.31,P<0.05).Kappa-test showed that the two methods were highly consistent(Kappa=0.53).Conclusion The application of VTS to screening the mental health of flying cadets can yield and quick but accurate results , which are of high reliability and validity .It is undoubtedly of great significance for improving the level of aviation security ,objectivity of selection results and predictive validity .
