1.Analysis of risk factors and construction of a prediction model for herpes zoster complicated by kidney dysfunction

Objective:To analyze the risk factors for herpes zoster complicated by kidney dysfunction and construct a prediction model.Methods:A retrospective analysis was conducted on the clinical data of 150 patients with herpes zoster who were admitted to Hangzhou Third People's Hospital from January 2022 to December 2023. The patients were divided into two groups based on the presence of kidney dysfunction: kidney dysfunction group ( n = 30, with kidney dysfunction) and control group ( n = 120, without kidney dysfunction). The risk factors for herpes zoster complicated by kidney dysfunction were analyzed. The logistic prediction model was constructed. The effectiveness of this prediction model in predicting herpes zoster complicated by kidney dysfunction was assessed using the receiver operating characteristic curve. Results:The differences in gender, body mass index, comorbidities (hypertension and hyperlipidemia), and sites of onset between the two patient groups were not statistically significant (all P > 0.05). The average age of the kidney dysfunction group was (67.74 ± 6.71) years, which was significantly older than that in the control group [(62.32 ± 5.58) years, t = 4.56, P < 0.001]. In the kidney dysfunction group, the proportions of patients with comorbid diabetes, severe pain, large lesion area, high-dose antiviral medication use, and hemoglobin levels < 120 g/L were 46.67% (14/30), 63.33% (19/30), 56.67% (17/30), 46.67% (14/30), and 40.00% (12/30), respectively. These values were all significantly greater than those in the control group [14.17% (17/120), 34.17% (41/120), 31.67% (36/120), 19.17% (23/120), and 10.00% (12/120), χ2 = 15.46, 2.60, 2.10, 9.76, 16.07, P < 0.001, 0.002, 0.035, 0.002, < 0.001). Logistic regression analysis revealed that older age ( OR = 3.023), comorbid diabetes ( OR = 4.315), severe pain ( OR = 3.623), large lesion area ( OR = 3.481), high-dose antiviral medication use ( OR = 4.030), and hemoglobin levels < 120 g/L ( OR = 0.151) were all significant risk factors for herpes zoster complicated by kidney dysfunction (all P < 0.05). Based on these risk factors, the following logistic prediction model was developed: Logit( P) = -8.753 + 1.106 × age + 1.462 × diabetes + 1.287 × pain severity + 1.247 × lesion area + 1.394 × high-dose antiviral medication use + (-1.889) × hemoglobin. The receiver operating characteristic curve analysis indicated that the area under the curve for predicting herpes zoster complicated by kidney dysfunction was 0.904 (95% CI: 0.859-0.937), with a sensitivity of 84.62% and a specificity of 89.04%. Conclusions:The risk factors for herpes zoster complicated by kidney dysfunction include older age, comorbid diabetes, severe pain, large lesion area, high-dose antiviral medication use, and hemoglobin levels < 120 g/L. Based on these factors, the construction of a logistic prediction model can provide a reliable basis for clinical prediction of the occurrence of kidney dysfunction.
2.Efficacy of high-flux hemodialysis in the treatment of stage 5 chronic kidney disease and its effects on nutritional status

Objective:To investigate the efficacy of high-flux hemodialysis in the treatment of stage 5 chronic kidney disease and its effects on nutritional status.Methods:A retrospective study was conducted on 64 patients with stage 5 chronic kidney disease who received treatment at Hangzhou Third People's Hospital from February 2023 to February 2024. These patients were divided into an observation group and a control group according to different blood purification methods, with 32 patients per group. The control group underwent conventional hemodialysis treatment, while the observation group underwent high-flux hemodialysis. Renal function, calcium and phosphorus metabolism, inflammatory factors, and nutritional indicators were compared between the two groups before and after treatment.Results:After treatment, the observation group showed significantly lower levels of serum creatinine [(238.18 ± 64.37) μmol/L] and blood urea nitrogen [(8.35 ± 1.43) μmol/L] compared with the control group [(343.12 ± 75.28) μmol/L, (10.97 ± 1.64) μmol/L, t = 5.99, 6.81, both P < 0.05]. The observation group had lower levels of calcium [(1.65 ± 0.19) mmol/L] and phosphorus [(1.54 ± 0.28) mmol/L] compared with the control group [(2.16 ± 0.23) mmol/L, (2.03 ± 0.36) mmol/L, t = 9.67, 6.08, both P < 0.05]. After treatment, the levels of C-reactive protein in the observation group [(9.72 ± 2.65) mg/L] were lower than those in the control group [(14.98 ± 3.76) mg/L, t = 6.47, P < 0.001]. However, there were no significant differences in hemoglobin, albumin, and prealbumin levels between the two groups ( t = -0.47, -0.16, -0.48, all P > 0.05). Conclusions:High-flux hemodialysis is effective in patients with stage 5 chronic kidney disease. It significantly alleviates the inflammatory state, but it has no notable effects on nutritional status. This approach merits consideration in clinical practice.

COVID-19 is an emerging infectious disease caused by SARS-CoV-2. After the infection of the virus, the host immune system is stimulated to produce multifarious specific antibodies to decrease or eliminate effects of the pathogen. Study of the specific antibodies dynamic characteristics in patients with COVID-19 is very important for the understanding and diagnosis of the disease, research and development of vaccine, and planning of prevention and control strategy. This paper reviews and summarizes the domestic and oversea research on dynamic characteristics of specific antibodies of COVID-19 patients, including the antibody producing, duration and level, and its possible influencing factors in order to improve the understanding of the immunological characteristics of COVID-19.
6.Analysis on duplicated reporting of brucellosis in China, 2005-2017

Objective:To analyze the current status of duplicated reporting of brucellosis in China during 2005-2017, to understand the impact of duplicated reporting on the incidence of brucellosis, analyze the possible causes of duplicated reporting, and put forward propose solutions to further improve the brucellosis surveillance data quality.Methods:Data were from brucellosis case reporting cards in National Notifiable Disease Report System (NNDRS), Excel 2010 software was used to establish the database, the duplicated reporting was identified by using Spark DataFrame aggregation analysis in Python 3.2.Results:During 2005-2017, a total of 499 577 cases of brucellosis were reported, but the duplicated reporting rate was 75.76/10 000 (3 785/499 577). The duplicated reporting rate in same year was 37.21/10 000 (1 859/499 577), the duplicated reporting rate in other year was 39.87/10 000 (1 992/499 577) and the overall duplicated reporting rate increased year by year. The duplicated reporting rate in same institution was 33.59 % (1 271/3 785) and the duplicated reporting rate by other institutions was 66.41 % (2 514/3 785). Hospitals had the highest number of duplicated reporting (2 757), and community healthcare centers had the highest of duplicated reporting rate (281.27/10 000) (123/4 373). The reported incidence of brucellosis was 2.926 4/100 000 in 2017, the corrected incidence was 2.844 8/100 000 after removing the duplicated reporting, which was the maximum decrease from 2005 to 2017, the corrected incidence decreased by 2.79 %. From 2005 to 2017, the average reported incidence of brucellosis in Xinjiang Uygur autonomous region was 14.712 0/100 000, and the corrected incidence was 14.060 0/100 000 after removing the duplicate reporting, with a decrease by 4.43 %, the biggest decline in China. The average reported incidence of brucellosis in Ningxia Hui autonomous region was 13.987 1/100 000, and the corrected incidence of brucellosis was 13.662 2/100 000, with a decrease of 2.32 %, ranking second in China. The reasons for duplicated reporting of brucellosis included repeated medical care seeking in same or different medical institutions in same year or other year, including the repeated medical care seeking in other areas. Conclusions:During 2005-2017, the number of duplicated reporting of brucellosis cases was on the rise in China. Hospital and community health service centers had highest case number and rate of duplicate reporting respectively, the duplicated reporting was mainly caused by re-reporting by other institutions or in other years. The duplicated reporting of brucellosis had greatly affected the accuracy of surveillance data of brucellosis in some provinces, such as Xinjiang and Ningxia. It is recommended to further strengthen brucellosis reporting management, improve data quality of NNDRS.
7.Epidemiological characteristics of fatal cases of hand, foot, and mouth disease in children under 5 years old in China, 2008-2018

Objective:To understand the epidemiological characteristics and risk factors of fatal cases of hand, foot, and mouth disease (HFMD) in children under 5 years old in China from 2008 to 2018, and provide evidence for the development of targeted prevention and control measures and reduction of the incidence of fatal HFMD cases.Methods:The incidence data of reported HFMD cases in China during 2008-2018 were collected from the National Notifiable Disease Surveillance Reporting System of China for the analyses on the demographic characteristics, spatial distribution, diagnosis or reporting and pathogen spectrum of the HFMD cases. Then the risk factors causing deaths were analyzed by using logistic regression model.Results:From 2008 to 2018, a total of 3 646 fatal cases of HFMD in children under 5 years old were reported in China. There were more fatal HFMD cases in boys than in girls (1.82∶1), the death mainly occurred in age group 0 to 2 years (87.71%). Adjusted mortality rate of HFMD in children under 5 years old in China declined from 0.87 per 100 000 in 2010 to 0.11 per 100 000 in 2018 (APC=-23.20%). In the 2 523 laboratory-confirmed deaths, 2 323 (92.07%) were EV-A71 infections, but the constituents of CV-A16 and other enterovirus infections increased. The interval from onset to diagnosis M=2( P25-P75:2 -4)d. The interval from onset to death M=3( P25-P75:2 -4)d. Age between 0 and 1 years, EV-A71 infection, longer interval between onset and diagnosis, and living in rural area were the risk factors causing fatal HFMD cases. Conclusions:The number of the fatal cases, the rate of mortality and case fatality HFMD in China had shown downward trends since 2010. EV-A71 is still the main pathogen causing fatal cases, but we should pay more attention to gene pattern of the other enteroviruses except EV-A71 and CV-A16. To reduce the risk of the fatal cases we should strengthen the health education about the immunization of EV-A71 inactivated vaccines and reduce the interval from onset to diagnosis in young children in western provinces and rural areas.
8.Analysis of morbidity and mortality characteristics of the notifiable diseases reported in 2013 in China

Objective To learn the characteristics of morbidity and mortality of notifiable diseases reported in China in 2013.Methods Descriptive analysis method was used to analyze the morbidity and mortality of notifiable diseases in China in 2013,with Microsoft Excel 2010 and ArcGIS 10.0 used to develop statistical charts.Results In 2013,the morbidity of the nationwide notifiable diseases was 473.87/100 000,a decrease of 3% below the average of the recent 3 years,while the mortality was 1.23/100 000,an increase of 2% over the average of the recent 3 years.The rate of laboratory diagnosis of the reported cases was 38.4%.Top 5 diseases of the reported incidence were hand foot and mouth disease,other infectious diarrhea,hepatitis B,tuberculosis and syphilis.The death cases reported were mainly AIDS,tuberculosis and rabies.As classified by the transmission route analysis,intestinal infectious diseases accounted for 49% of the total incidence reported for the year,followed by the blood and sexually transmitted infectious diseases,respiratory infectious diseases,animal and vector borne infectious diseases.According to the pathogenic analysis,virus infectious diseases accounted for 68% of the total cases,higher than bacterial infectious diseases and animal-borne/vector-borne infectious diseases.In Shanghai,Zhejiang and Jiangsu,human infection with avian influenza A (H7N9) virus was emerging,as epidemic situation of measles,dengue fever and brucellosis were on the rise significantly nationwide,while the morbidity of infectious diseases decreased,namely pulmonary tuberculosis,hepatitis B among others.Morbidity of the top 5 provinces for notifiable infectious diseases were Hainan,Guangxi,Guangdong,Xinjiang and Zhejiang,respectively.Conclusion The proportion of laboratory confirmed cases among totals was still low in 2013.The morbidity of the infectious diseases was higher in western provinces and parts of south-eastern province,the mortality was higher in westem provinces.The emerging human infection with avian influenza A (H7N9) virus,and the high epidemic of measles,dengue fever and brucellosis in some areas had caught the society concerns.
9.Viral etiologies of hospitalized pneumonia patients aged less than five years in six provinces, 2009-2012.

OBJECTIVETo analyze the viral etiologies of hospitalized pneumonia patients aged less than five years in six provinces during 2009-2012, and to describe the seasonality of the detected viral etiologies.
METHODSEight hospitals were selected in six provinces from a national acute respiratory infection surveillance network. Demographic information, clinical history and physical examination, and laboratory testing results of the enrolled hospitalized patients aged less than five years with pneumonia, including respiratory syncytial virus (RSV), human influenza virus, adenoviruses (ADV), human parainfluenza virus (PIV), human metapneumovirus (hMPV), human coronavirus (hCoV)and human bocavirus (hBoV) were analyzed. The viral etiology spectrum of the enrolled patients was analyzed by age-group, year, and seasonality of the detected viral etiologies were described.
RESULTS4 508 hospitalized children less than five years old, with pneumonia from 8 hospitals were included, and 2 688 (59.6%) patients were positive for at least one viral etiology. The most frequent detected virus was RSV (21.3%), followed by PIV (7.1%) and influenza (5.2%), hBoV (3.8%), ADV(3.6%) and hMPV(2.6%). The lowest positive rates in hCoV(1.1%). RSV, influenza, PIV, hBoV and hMPV all showed the nature of seasonality.
CONCLUSIONRSV was a most common viral etiology in the hospitalized young children less than 5 years of age with pneumonia. Prevention measures should be conducted to decrease its severe impact to the young infants and children in China.

10.Analysis of effect on infectious diseases outbreak detection performance by classifying provinces for moving percentile method.

OBJECTIVEProviding evidences for further modification of China Infectious Diseases Automated-alert and Response System (CIDARS) via analyzing the outbreak detection performance of Moving Percentile Method (MPM) by optimizing thresholds in different provinces.
METHODSWe collected the amount of MPM signals, response results of signals in CIDARS, cases data in nationwide Notifiable Infectious Diseases Reporting Information System, and outbreaks data in Public Health Emergency Reporting System of 16 infectious diseases in 31 provinces in Chinese mainland from January 2011 to October 2013. The threshold with the optimal sensitivity, the shortest time to detect outbreak and the least number of signals was considered as the best threshold of each disease in Chinese mainland and in each province.
RESULTSAmong all the 16 diseases, the optimal thresholds of 10 diseases, including dysentery, dengue, hepatitis A, typhoid and paratyphoid, meningococcal meningitis, Japanese encephalitis, scarlet fever, leptospirosis, hepatitis, typhus in country level were the 90(th) percentile (P90), which was the same as provincial level for those diseases.For the other 6 diseases, including other infectious diarrhea, influenza, acute hemorrhagic conjunctivitis, mumps, rubella and epidemic hemorrhagic fever, the nationwide optimal thresholds were the 80th percentile (P80), which was different from that by provinces for each disease. For these 6 diseases, the number of signals generated by MPM with the optimal threshold for each province was decreased by 23.71% (45 557), 15.59% (6 124), 14.07% (1 870), 9.44% (13 881), 8.65% (1 294) and 6.03% (313) respectively, comparing to the national optimal threshold, while the sensitivity and time to detection of CIDARS were still the same.
CONCLUSIONOptimizing the threshold by different diseases and provinces for MPM in CIDARS could reduce the number of signals while maintaining the same sensitivity and time to detection.
