1.Pathogenic bacteria from children with vulvovaginitis and their antimicro-bial resistance

Objective To understand the pathogenic bacteria from children with vulvovaginitis and their antimi-crobial resistance.Methods Clinical data of children with vulvovaginitis and positive bacterial culture of vaginal se-cretion from Beijing Children's Hospital from January 1,2016 to December 31,2023 were analyzed retrospectively.Results Among 3 249 children with vulvovaginitis,a total of 3 389 pathogenic bacteria strains were detected,inclu-ding 1 730 strains of Gram-negative bacteria and 1 659 strains of Gram-positive bacteria,accounting for 51.05%and 48.95%,respectively.The top 5 detected pathogens were Haemophilus in fluenzae(n=1 201,35.44%),Strep-tococcus pyogenes(Group A)(n=694,20.48%),Staphylococcus aureus(n=375,11.06%),Streptococcus aga-lactiae(Group B)(n=183,5.40%),and Haemophilus parainfluenzae(n=179,5.28%).Among the isolated pathogens in pediatric patients across different age groups,Haemophilus in fluenzae accounted for 46.18%in chil-dren<7 years old,Streptococcus pyogenes(Group A)and Haemophilus in fluenzae accounted for 27.08%and 25.86%respectively in children aged 7-11 years old,Streptococcus agalactiae(Group B)and Staphylococcus au-reus accounted for 31.50%and 18.11%respectively in children>11-18 years old.The pathogens detected varied in different seasons.Among Gram-positive bacteria,Streptococcus pyogenes(Group A)had higher resistance rates to erythromycin and clindamycin,which were 93.52%and 92.94%,respectively.Among Gram-negative bacteria,Haemophilus in fluenzae had higher resistance rates to ampicillin,compound sulfamethoxazole,cefaclor,and cefu-roxime(all>34%).648 Haemophilus in fluenzae strains(53.96%)and 40 Haemophilus parain fluenzae strains(22.35%)produced β-lactamase.Conclusion The common pathogenic bacteria causing infectious vulvovaginitis in children are Haemophilus in fluenzae,Streptococcus pyogenes(Group A),and Staphylococcus aureus.The detec-tion rate of β-lactamase in Haemophilus influenzae is high.Understanding the pathogen detection and antimicrobial resistance status in hospitals can provide basis for clinician's empirical and rational selection of antimicrobial agents.

Postoperative infection of internal fixation of closed fractures the lower limbs in adults represents a devastating complication, characterized by diagnostic challenges, prolonged treatment duration and high disability rates. Current management of these infections faces multiple challenges, such as difficulties in early accurate diagnosis, and various controversies about the treatment plan, leading to poor overall diagnosis and treatment results. To address these issues, based on evidence-based medicine and principles with emphasis on scientific rigor, clinical applicability and innovation, the Trauma Branch of the Chinese Medical Association, Orthopedic Branch of the Chinese Medical Doctor Association, Orthopedics Branch of the Chinese Medical Association, and Trauma Orthopedics and Polytrauma Group of the Resuscitation and Emergency Committee of the Chinese Medical Doctor Association have collaboratively organized a panel of relevant experts to develop the Guideline for diagnosis and treatment of infection after internal fixation of closed lower limb fractures in adults ( version 2025). The guideline proposed 10 recommendations, aiming to provide a foundation for standardized diagnosis and treatment of postoperative infection in adults with closed lower limb fractures.
3.Pediatric salivary pleomorphic adenoma: report of 30 cases.

Objective:To summarize the clinical characteristics, diagnosis and treatment experience of salivary pleomorphic adenoma in children. Methods:Thirty patients with salivary pleomorphic adenomas treated in Beijing Childrens Hospital from January 2008 to December 2022 were retrospectively reviewed, including 11 boys and 19 girls, with the age ranging from 0.3 to 14.4 years(median age 10.4 years). Initial presentation, medical history, imaging workups, surgical approaches, complications, rates of recurrence were evaluated. Results:Major salivary gland lesions were most common(n=24, 80%); 53.3%(16 of 30) arising in the submandibular glands and 26.7%(8 of 30) in the parotid. Minor salivary gland lesions(n=6, 20%) were removed from the palate, tongue, face, trachea, nasopharynx, and upper mediastinumand. Preoperative imaging was reviewed in all patients and consisted of 26 ultrasound exams, 2 computerized tomography(CT) exams, and 15 magnetic resonance imaging(MRI) exams. Fine needle aspiration biopsy was performed in 12 patients. Surgical excision was performed in all patients. Postoperative complications included transient facial paresis(n=3), Pneumonia and pleural effusion(n=1). Average length of follow-up was 36.7 months; confirmed recurrence occurred in one patients. Conclusion:The symptoms of salivary gland pleomorphic adenoma in children are different according to the location of the tumor. The treatment is complete surgical resection, and a small amount of normal tissue around the tumor should be removed to reduce recurrence.

4.The effect of GMDTC on attenuating cisplatin-induced toxicity in the treatment of tumor-bearing rats

Objective To investigate the protective effect of sodium (s)-2-(dithiocarboxylato((2R,3R,4R,5R,6R)-2,3,4,5,6-pentahydroxyhexyl) amino)-4-(methylthio) butanoate (GMDTC) against cisplatin-induced toxicity during antitumor treatment. Methods Specific pathogen-free female SD rats were inoculated with LLC-WRC-256 tumor cells to establish tumor-bearing models, which were randomly divided into the model control group, cisplatin control group, and low-, medium-, and high-dose GMDTC groups, with 10 rats in each group. Another negative control group with 10 rats was included. Rats in the cisplatin control group and the three GMDTC dose groups were injected intravenously with cisplatin at a dose of 5 mg/kg body mass for one time. After 2.0 hours, rats in the three GMDTC dose groups were injected intravenously with GMDTC at doses of 27, 54, and 108 mg/kg body mass, once per day for five consecutive days. Tumor volume, platinum levels in biological samples (whole blood, urine, kidney, and tumor tissue), serum creatinine (Cr) and urea nitrogen (BUN) levels were measured at different time points. The tumor mass was measured, the pathological changes of renal tissue were observed, and the complete blood count was tested. Results The dilation of renal tubules, cell necrosis and shedding, and the formation of renal tubule patterns in the kidneys of rats in the medium- and high-dose GMDTC groups were significantly reduced compared with those in the cisplatin control group. The tumor volume of rats in the cisplatin control group and the three GMDTC dose groups decreased on the 3rd, 5th and 7th days after cisplatin administration, and the tumor weight decreased on the 7th days compared with the model control group (all P<0.05). However, there was no significant difference in the above indexes among the four groups (all P>0.05). The levels of serum Cr and BUN of rats in the cisplatin control group on the 3rd, 5th, and 6th days after cisplatin administration, as well as the score of renal tubular injury degree on the 7th day, were higher than those in the negative control group and the model control group (all P<0.05). The serum Cr levels of rates on the 3rd and 5th days after cisplatin administration, the serum BUN levels on the 5th day in the medium- and high-dose GMDTC groups, the score of renal tubular injury degree, and renal platinum level on the 7th day decreased compared with the cisplatin control group (all P<0.05), while the serum Cr and BUN levels on the 6th day and the whole-blood platinum levels in the high-dose GMDTC group decreased (all P<0.05). The urinary platinum levels of rats in the three GMDTC dose groups increased on the 1st day after GMDTC administration (all P<0.05), but decreased on the 3rd day compared with the cisplatin control group (all P<0.05). The counts of white blood cells, neutrophils, lymphocytes and platelets of rats in the cisplatin control group were lower than those in the model control group (all P<0.05). There was no significant difference in the above indexes of rats between the three GMDTC dose groups and the cisplatin control group (all P>0.05). Conclusion Intravenous administration of GMDTC at doses of 54 or 108 mg/kg body mass effectively reduce the nephrotoxicity of cisplatin-treated rats with LLC-WRC-256 tumors without affecting the antitumor effect of cisplatin.

To formulate an expert consensus on the principles of preoperative hair removal and provide scientific guidance for standardized removal of hair before surgical procedures so as to reduce the incidence of surgical site infections.METHODS Led by the Hospital Management Institute of National Health Commission of the People's Republic of China,this consensus was reached with the joint efforts from the expects of relevant fields such as surgeries,interventional therapies,nursing,and infection prevention and control.The consensus facilitates the classification and evaluation of literatures by following the evidence grade formulated by Oxford Evidence-based Medicine Center and focuses on the association of preoperative hair removal with surgical site infection,it reaches the evidence grade of expert consensus and recommendation intensity by integrating with discussions on meetings and clinical experience of the expects from relevant fields.RESULTS A total of 6 items of consensus were reached by summarizing the latest evidence on the aspects including the indications for preoperative hair removal,tools,range,timing and places.CONCLUSION The consensus,to some extent,make supplements to and complete the exiting regulations and standards.It provides guidance for the medical institutions to carry out the preoperative hair removal.

Osteoporotic proximal humeral fracture (OPHF) is one of the common osteoporotic fractures in the aged, with an incidence only lower than vertebral compression fracture, hip fracture, and distal radius fracture. OPHF, secondary to osteoporosis and characterized by poor bone quality, comminuted fracture pattern, slow healing, and severely impaired shoulder joint function, poses a big challenge to the current clinical diagnosis and treatment. In the field of diagnosis, treatment, and rehabilitation of OPHF, traditional Chinese and Western medicine have accumulated rich experience and evidence from evidence-based medicine and achieved favorable outcomes. However, there is still a lack of guidance from a relevant consensus as to how to integrate the advantages of the two medical systems and achieve the integrated diagnosis and treatment. To promote the diagnosis and treatment of OPHF with integrated traditional Chinese and Western medicine, relevant experts from Orthopedic Expert Committee of Geriatric Branch of Chinese Association of Gerontology and Geriatrics, Youth Osteoporosis Group of Orthopedic Branch of Chinese Medical Association, Osteoporosis Group of Orthopedic Surgeon Branch of Chinese Medical Doctor Association, and Osteoporosis Committee of Shanghai Association of Integrated Traditional Chinese and Western Medicine have been organized to formulate Expert consensus on the diagnosis and treatment of osteoporotic proximal humeral fracture with integrated traditional Chinese and Western medicine ( version 2024) by searching related literatures and based on the evidences from evidence-based medicine. This consensus consists of 13 recommendations about the diagnosis, treatment and rehabilitation of OPHF with integrated traditional Chinese medicine and Western medicine, aimed at standardizing, systematizing, and personalizing the diagnosis and treatment of OPHF with integrated traditional Chinse and Western medicine to improve the patients ′ function.
10.Clinical analysis of 161 multicenter cases of ruptured abdominal aortic aneurysms over the past decade (2013—2022)

Background and Aims:Rupture is the most serious complication of abdominal aortic aneurysm,requiring rapid diagnosis,emergency surgery,and posing significant surgical challenges,with high mortality rates. Currently,there is very limited reporting on ruptured abdominal aortic aneurysm (rAAA) in our country,which presents numerous difficulties for the prevention and treatment of rAAA. This study collected the data of epidemiological characteristics,treatment outcomes,and prognosis of rAAA patients from multiple centers with a large sample size,analyzing the current status and trends of rAAA surgery in China over the past decade,aiming to provide reference for clinical practice.Methods:The clinical and follow-up data of 161 rAAA patients treated at five major vascular surgery centers (50 from Drum Tower Hospital Affiliated to the Medical School,Nanjing University;19 from the First Affiliated Hospital of Anhui Medical University;33 from the Second Affiliated Hospital of Nanchang University;31 from Qilu Hospital,Shandong University;and 28 from the First Affiliated Hospital of the University of Science and Technology of China) were retrospectively analyzed.Results:Among the 161 patients,124 (77.02%) were male and 37 (22.98%) were female,with an average age of 68.27 years. The median age at diagnosis was 70 years for males and 71 years for females. The overall mean aneurysm diameter was 7.11 cm,with males at 7.72 cm and females at 6.82 cm,showing a statistically significant difference (P<0.05). The main comorbidities were hypertension and coronary artery disease. Over 80% of patients presented with abdominal pain as the initial symptom,while 15% presented with low back pain,and 8 patients sought medical attention for dizziness or visual disturbances. Among the 161 patients,86 underwent open surgical repair (OSR),and 75 received endovascular aneurysm repair (EVAR). The proportion of EVAR has increased annually,reaching nearly 70% in the past five years,and up to 90% in patients aged over 70 years. All OSR procedures were performed under general anesthesia,while 20 EVAR cases used local anesthesia and 55 used general anesthesia. Compared to the OSR group,the EVAR group showed significant differences in operative time (231.77 min vs. 162.49 min),intraoperative blood transfusion volume (1578.56 mL vs. 843.87 mL),length of hospital stay (21.21 d vs. 15.34 d),ICU stay duration (8.28 d vs. 5.49 d),and hospitalization costs (108500 CNY vs. 132800 CNY) (all P<0.05). No significant differences were found between the EVAR and OSR groups in total complications or perioperative mortality rates (both P>0.05). The main causes of perioperative death included respiratory and circulatory failure,acute myocardial infarction,and severe infections. Postoperative follow-up was effectively conducted for 92 patients,with follow-up durations ranging from 10 to 142 months. Survival analysis revealed no significant difference in survival rate between the OSR and EVAR groups (P=0.3182).Conclusion:rAAA is a rare and high-risk disease,with certain clinical differences between male and female patients. The number of EVAR procedures has increased rapidly;however,EVAR has not shown a significant advantage over OSR in improving long-term survival rates.
