1.Research progress on the treatment of ischemia-reperfusion injury with human umbilical cord mesenchymal stem cells and their derivatives

Ischemia-reperfusion injury (IRI) can lead to organ dysfunction and tissue necrosis in the liver, kidney, myocardium and spinal cord, and there is currently a lack of effective treatment options. Human umbilical cord mesenchymal stem cell (HUC-MSC) and their derivatives have anti-inflammatory, anti-apoptotic, reactive oxygen species scavenging, mitochondrial and endothelial function improvement properties, and are ideal gene therapy carrier cells, providing new possibilities for the treatment of IRI in different organs. This article reviews the concept and mechanisms of IRI, the biological characteristics of HUC-MSC and their derivatives and their comparison with mesenchymal stem cells from other sources, and the mechanisms of HUC-MSC in treating IRI in different organs. It also summarizes and analyzes the advantages and disadvantages of HUC-MSC in protecting different organs from IRI, and prospects future research directions to explore more valuable research paths.
2.A qualitative study of self-management behavior maintenance dilemmas in patients with ischemic stroke/

Objective To explore the dilemma of self-management behavior maintenance in ischemic stroke patients,and to provide references for continuous self-management intervention design for ischemic stroke patients.Methods The descriptive qualitative research method was used to select samples by the purpose sampling method.From November to December 2023,18 patients hospitalised with stroke from the neurology ward of a tertiary general hospital in Luoyang City,Henan Province were selected for semi-structured interviews.Based on the Capability,Opportunity,Motivation-Behavior(COM-B)model,the data were analyzed by directed content analysis.Results A total of 3 themes and 12 sub-themes were identified,namely capability factors(poor level of health literacy,lack of self-control),opportunity factors(lack of medical resources impeding the maintenance of behaviors,social customs inducing the return of undesirable behaviors,difficult to satisfy the need for multiple emotional support,behavior being improved briefly after follow-up),and motivation factors(impulsive thinking,role conflict leading to forgetfulness,difficult to change previous bad habits,acceptance of the aging,weak self-management effect,symptoms inducing behavioral changes).Conclusion The maintenance of self-management behavior in ischemic stroke patients is affected by 3 aspects of obstacles,including capability,opportunity and motivation.On the premise of fully evaluating the specific characteristics of ischemic stroke patients,medical staff should meet the multidimensional needs of patients to promote the maintenance of self-management behaviors to improve their health outcomes.

6.Analysis on the results of external quality assessment for quality indicators of clinical laboratory in China

Objective:To analyze the implementation of the external quality assessment plan for quality indicators of clinical laboratories in China from 2016 to 2021, as well as that of the external quality assessment of 15 quality indicators in clinical laboratories, in order to provide reference for quality management of clinical laboratory specialties.Methods:The research data was collected from the external quality assessment plan for quality indicators, which was conducted by the National Center for Clinical Laboratories joining the clinical laboratory centers of 31 provinces (autonomous regions and municipalities directly). The essential information reported by each participating clinical laboratory from 2016 to 2021 and the external quality assessment data of 15 quality indicators in clinical laboratories were collected, followed by a descriptive analysis on the number of participating laboratories and the number of returns for each indicator. Median representation was used for the external quality assessment data of 15 quality indicators in clinical laboratories, and the TOPSIS method was applied to comprehensively evaluate the quality of the total testing process of participating clinical laboratories in each year.Results:From 2016 to 2021, the number of laboratories participating in the external quality assessment plan for quality indicators of clinical laboratory increased from 7 704 to 12 142. Quality indicators in pre-analytical phases: the incorrect sample type rate, incorrect sample container rater, and incorrect fill level rate had been decreasing year by year, reaching 0, 0, and 0.005 8% in 2021, respectively. The anticoagulant samples clotted rate had decreased from 0.068 6% in 2016 to 0.042 8% in 2021, and the blood culture contamination rate from 2017 to 2021 had been 0 without exception. The pre-examination turnaround time had been shortened from 28 minutes in 2016 to 2019 to 24 minutes in 2020 and 2021. Quality indicators in analytical phases: the intra-laboratory turnaround time had been extended from 45 minutes in 2016 to 2019 to 50 minutes in 2020 and 2021. Test covered by an IQC rate had been increasing year by year, reaching 60.61% in 2021. Test with inappropriate IQC performances rate was 0 in 2020 and 2021, the test covered by an EQA-PT control rate was 100%, and unacceptable performances in EQA-PT schemes rate from 2017 to 2021 was 0. The inter-laboratory comparison rate had increased from 1.56% in 2016 to 3.00% in 2021. Quality indicators in post-analytical phases: the incorrect laboratory reports rate, critical values notification rate and timely critical values notification rate had been 0, 100%, and 100%from 2016 to 2021 respectively. The comprehensive evaluation results of TOPSIS method showed that the overall quality level of clinical laboratory testing in 2020 was the highest, with Ci value of 0.850 5, while the lowest Ci value in 2016 was 0.143 6. Conclusions:The quality of clinical laboratory testing in China has been effectively improved. Clinical laboratories should continue to strengthen their monitoring of quality indicators, especially the intra-laboratory turnover time and the inter-laboratory comparison rate, for the purposes of identifying errors, analyzing causes and taking corrective measures to improve quality.

Pulmonary blast injury has become the main type of trauma in modern warfare, characterized by externally mild injuries but internally severe injuries, rapid disease progression, and a high rate of early death. The injury is complicated in clinical practice, often with multiple and compound injuries. Currently, there is a lack of effective protective materials, accurate injury detection instrument and portable monitoring and transportation equipment, standardized clinical treatment guidelines in various medical centers, and evidence-based guidelines at home and abroad, resulting in a high mortality in clinlcal practice. Therefore, the Trauma Branch of Chinese Medical Association and the Editorial Committee of Chinese Journal of Trauma organized military and civilian experts in related fields such as thoracic surgery and traumatic surgery to jointly develop the Clinical treatment guideline for pulmonary blast injury ( version 2023) by combining evidence for effectiveness and clinical first-line treatment experience. This guideline provided 16 recommended opinions surrounding definition, characteristics, pre-hospital diagnosis and treatment, and in-hospital treatment of pulmonary blast injury, hoping to provide a basis for the clinical treatment in hospitals at different levels.
8.Future challenges and solutions for safety in China: China CDC's exploration of injury prevention strategies

As an important component of worldwide injury prevention, China has made great contribution to the development. China is serving as a model for other countries in the world, especially for developing countries, and China's experiences have great implications for them. Besides the description of the present situation and trends of China's injury prevention work and review for its development history, this paper has also introduced the existing issues and the current challenges, and informed the efforts of the staff in related field to find solutions from different aspects. All of them jointly boosted the development of global injury prevention.
9.Serum sE-selectin positively correlates to cerebral microbleeds in patients with acute ischemic stroke

Objective Cerebral microbleeds (CMBs) are important indicators of cerebral small vessel disease .However, it is still unclear whether endothelial dysfunction is involved in CMBs .The aim of this study is to investigate the correlation between CMBs and soluble E-selectin (sE-selectin) in patients with acute ischemic stroke . Methods Based on the results of MRI (3.0 T) susceptibility weighted imaging , we divided patients with first acute ischemic stroke into a CMBs group ( n=63 ) and a non-CMBs group (n=63), and recruited another 45 volunteers with normal MRI findings as controls .We collected and conducted comparative a-nalysis on the demographic data , biochemical variables ( including the sE-selectin level ) , vascular risk factors , and the number of CMBs of the patients . Results Ordinal logistic regression analysis showed a significantly positive correlation between sE -selectin and the number of CMBs (OR=1.062, 95%CI:1.023-1.103, P=0.002), higher systolic blood pressure associated with more CMBs (OR=1.014, 95%CI:1.002-1.025, P=0.021). Conclusion Serum sE-selectin is significantly positively correlated with and can be used as a biological marker for the severity of CMBs .
10.Mycophenolate mofetil combined with low dose of hormone and lamivudine in the treatment of hepatitis B virus associated glomerulonephritis

Objective To assess the efficacy of mycophenolate mofetil (MMF) combined with low dose of hormone and lamivudine in the treatment of hepatitis B virus associated glomerulonephritis (HBV-GN).Methods The clinical data of 49 HBV-GN patients diagnosed by renal pathology was reviewed.They were treated with MMF( ≥ 12 years old,0.75 g,two times a day; < 12 years old,0.4 g/m2,two times a day),low dose of hormone [0.5 mg/(kg·d) ],lamivudine( ≥ 12 years old,100 mg/d; < 12 years old,3 mg/kg,two times a day).Results Among of 49 HBV-GN patients,clinical cure rate was 71.4%(35/49),the total effective rate was 81.6%(40/49),85.7%(42/49) patients' HBV DNA level decreased from 5.43 ×104 copies/ml to < 1000 copies/ml.The complete remission rate was 88.0% (22/25),the partial remission rate was 8.0% (2/25),the inefficiency was 4.0% (1/25) in membranous nephropathy (MN);the complete remission rate was 44.4% (4/9),the inefficiency was 55.6% (5/9) in mesangial proliferative glomerulone phritis (MsPGN) ; the complete remission rate was 70.0%(7/10); the partial remission rate was 30.0%(3/10)in membrane proliferative glomerulone phritis (MPGN) ;the complete remission rate was 40.0% (2/5),the inefficiency was 60.0%(3/5) in focal segmental glomerulosclerosts (FSGS).There was significant difference among the different pathological type (P<0.05).There were less adverse reactions.Conclusions MMF combined with low dose hormone and lamivudine is safe and effective in treating HBV-GN.The efficacy and pathological type has some relationship,MN has the best response,MPGN and MsPGN are second,FSGS is poor.
