This article summarizes Professor HAN Mingxiang's clinical experience in treating lung cancer based on the theory of "deficiency-phlegm-stasis-toxin". It is believed that depletion of healthy qi, together with binding of phlegm and stasis forming into toxin is the core pathogenesis of lung cancer occurrence and development. In clinical practice, the core treatment principle should be reinforcing healthy qi, resolving phlegm, removing stasis, and relieving toxins. The treatment should be given based on syndrome differentiation. For the syndrome of lung-spleen qi deficiency, the self-formulated Jianpi Huatan Zhike Decoction (健脾化痰止咳汤) can be applied to fortify the spleen and replenish qi, thereby reinforcing healthy qi. For the syndrome of deficiency of both qi and yin, the self-formulated Shenmai Yuzhu Decoction (参麦玉竹汤) is used to nourish yin and moisten the lungs, thereby clearing the source. For the syndrome of phlegm-dampness accumulating in the lungs, the self-formulated Sizi Wenfei Decoction (四子温肺汤) is applied to warm the lungs and resolve phlegm, thereby diffusing the lung qi. For the syndrome of qi stagnation and blood stasis, the self-formulated Yiqi Zhuyu Decoction (益气逐瘀汤) is used to activate blood and resolve stasis, thereby unblocking the collaterals. For the syndrome of binding of phlegm and stasis, the self-formulated Jiejie Decoction (结节方) is utilized to resolve phlegm and remove stasis, thereby dissipating nodules. For the syndrome of lung deficiency with toxin accumulation, the self-formulated Qiyu Sanlong Decoction (芪玉三龙汤) is applied to reinforce healthy qi and remove toxins, thereby combating cancer. Medications are flexibly prescribed based on syndrome differentiation, with an emphasis on reinforcing healthy qi and eliminating pathogenic factors, and maintaining the balance of zang-fu (脏腑) organ functions as well as qi, blood, yin and yang.

Objective To investigate the predictive value of D-dimer for in-hospital mortality and heart failure with reduced ejection fraction (HFrEF) in patients with acute myocardial infarction (AMI). Methods A total of 688 patients with AMI who were hospitalized at Zhongshan Hospital, Fudan University, from January 2020 to December 2023 were retrospectively enrolled. Demographic and clinical characteristics, laboratory findings, echocardiographic parameters, and in-hospital outcomes were collected. Patients were divided into four groups according to D-dimer quartiles, and trend tests were used to compare changes in each indicator with increasing D-dimer levels. The primary endpoints were in-hospital mortality and HFrEF. Multivariable logistic regression analysis was performed to assess the independent associations of D-dimer with in-hospital mortality and HFrEF. Receiver operating characteristic (ROC) curve analysis was used to evaluate its predictive performance. Results Differences and trend tests among four groups for age, creatinine, peak cardiac troponin T, N-terminal pro-brain natriuretic peptide (NT-proBNP), left ventricular ejection fraction, HFrEF incidence, and in-hospital mortality were statistically significant (P<0.05). Multivariable logistic regression analysis showed that elevated D-dimer was associated with an increased risk of in-hospital mortality in patients with AMI (OR=1.251, 95%CI 1.093–1.432, P=0.001), and HFrEF (OR=1.075, 95%CI 1.028–1.124, P=0.002). ROC curve analysis showed that the area under the curve (AUC) of D-dimer for predicting in-hospital mortality and HFrEF were 0.871 and 0.716, respectively. At a cutoff value of 0.47 mg/L, the sensitivity and specificity of D-dimer in predicting HFrEF were 81.9% and 52.1%, respectively; at a cutoff value of 2.18 mg/L, the sensitivity and specificity of D-dimer in predicting in-hospital mortality were 63.6% and 93.8%, respectively. Restricted cubic spline analysis demonstrated significant non-linear associations between D-dimer and the risks of in-hospital mortality and HFrEF (P<0.001). Conclusions Elevated D-dimer level is significantly associated with increased risks of in-hospital mortality and HFrEF in patients with AMI and shows acceptable predictive performance. As a simple and readily available laboratory biomarker, D-dimer may provide useful information for early risk stratification and clinical management in patients with AMI.

BACKGROUND:
Intervertebral disk (IVD) degeneration, which can cause lower back pain, is a major predisposing factor for disability and can be managed through multiple approaches. However, there is no satisfactory strategy currently available to reconstruct and recover the natural properties of IVDs after degeneration. As tissue engineering develops, scaffolds with embedded cell cultures have proved critical for the successful regeneration of IVDs.
METHODS:
In this study, an integrated scaffold for IVD replacement was developed. Through scanning electron microscopy and other mechanical measurements, we characterized the physical properties of different hydrogels. In addition, we simulated the physiological structure of natural IVDs. Nucleus pulposus (NP) cells and annulus fibrosusderived stem cells (AFSCs) were seeded in gelatin methacrylate (GelMA) hydrogel at different concentrations to evaluate cell viability and matrix expression.
RESULTS:
It was found that different concentrations of GelMA hydrogel can provide a suitable environment for cell survival. However, hydrogels with different mechanical properties influence cell adhesion and extracellular matrix component type I collagen, type II collagen, and aggrecan expression.
CONCLUSION
This tissue-engineered IVD implant had a similar structure and function as the native IVD, with the inner area mimicking the NP tissue and the outer area mimicking the stratified annulus fibrosus tissue. The new integrated scaffold demonstrated a good simulation of disc structure. The preparation of efficient and regeneration-promoting tissueengineered scaffolds is an important issue that needs to be explored in the future. It is hoped that this work will provide new ideas and methods for the further construction of functional tissue replacement discs.

BACKGROUND:Knee osteoarthritis is prevalent among the middle-aged and senior people in Asian countries, however, the epidemiology survey of total knee arthroplasty is rarely reported in China.
OBJECTIVE: To retrospectively analyze the data of patients undergoing total knee arthroplasty from 2008 to 2013 in Hefei City, and explore the distribution of age and gender of these patients.
METHODS: A retrospective analysis among patients undergoing total knee arthroplasty from January 1st 2008 to December 31st 2013 in Hefei City was performed. Data were extracted from the database of Medical Records Room of Relevant Hospitals in Hefei City, including the patient’s gender, age, disease duration, education level, body mass index and surgical site. In addition, more clinical information in one hospital were selected and analyzed, to compare the difference of clinical features between men and women.
RESULTS AND CONCLUSION:From 2008 to 2013, totaly 1 146 patients underwent total knee arthroplasty due to knee osteoarthritis. The rate of total knee arthroplasty increased over the 6 years and was much higher in women than in men. The single-centre registry data revealed that there was no difference in age, disease duration, education level, and body mass index and surgical site between men and women. Our findings indicate that, the rate of total knee arthroplasty is increasing steadily from 2008 to 2013 in Hefei City and is higher in women than in men. Risk factors that account for such disparity in total knee arthroplasty utilization need to be further investigated.

OBJECTIVETo investigate the clinical and hematological features and prognostic factors of primary myelofibrosis (PMF) patients in Shanghai and surrounding area of Shanghai, China.
METHODSThe clinical manifestations, laboratory parameters and survival were retrospectively analyzed in 75 PMF patients diagnosed from Jan, 1996 to Dec, 2013 in our hospital and were compared with those of Chinese subjects from Tianjin and foreign Caucasian patients, respectively. Comparison of categorical variables was performed by χ² test. Survival was estimated using the Kaplan- Meier method. Log- rank test was used to compare survival date. A Cox model was used for multivariate analyses.
RESULTSThe median age of the 75 patients was 56(19-81) years old. There were 51(68%) patients with HGB less than 100 g/L. The median value of HGB was 83 g/L. Similar with those from Tianjin, the patients in our study were significantly younger with higher proportion of severe anemia and lower platelet counts when compared with foreign Caucasian patients. Using IPSS and dynamic international prognostic scoring system (DIPSS) model, the survival curves of intermediate- 1, intermediate- 2 and high risk groups were significantly different. In univariate analyses, variables significantly correlated with poor prognosis were systemic symptoms, HGB<100 g/L, HGB<80 g/L, PLT ≤ 100 × 10⁹/L, WBC<10 × 10⁹/L and blood blasts ≥ 0.01. Multivariate analysis identified IPSS and HGB < 80 g/L as independent risk factors for survival.
CONCLUSIONChinese PMF patients may have characteristics of younger age at onset with more severe anemia. However, IPSS and DIPSS model are still suitable for Chinese patients to predict survival. To increase the weight of anemia severity may provide a better prognostic stratification for Chinese patients with PMF.

Objective:To evaluate the therapeutic effect and the prognosis of the laryngeal function preserving surgery (LFPS) and total laryngectomy in hypophngeal carcinoma.Method:Ninety-three cases of laryngopharynx carcinoma were retrospectively analyzed from September 1974 to September 2006.Among which 57 cases were primary pyriform sinus cancer,20 cases were postero-cricoid cancer and 16 cases were postero-pharyngeal wall cancer.53 cases were treated by LFPS,and 40 cases were treated by total laryngectomy. The treatment effectiveness, complication, survival rate and repair materials were analysed.Result:Adopting Kaplan-Meier survival analysis, the 3-year survival rates were 69.9%and 5-year survival rates were 43.0%.The 3-year survival rates of LFPS and Non-LFPS were 73.6% and 67.5%, and the 5-yenr survival rates of LFPS and Non-LFPS were 49.1% and 32.5%. There was no significant difference between the two groups(χ~2=2.566,P>0.05). Single element analysis indicated survival rates concerned with T-stage(χ~2=9.764,P<0.05), neck lymphatic transfer(χ~2=10.472,P<0.01) and the degree of pathological differentiation(χ~2=25.894,P<0.01).Cox regression analysis suggested that T-stage,the degree of pathological dfferentiaton and wheather going through the surgical operation were the independent elemant of the patient's prognosis. There was no significant difference between LFPS and No-LFPS in the neoplasms location, complication, neoplasms residuum and neoplasms recurrence. Conclusion:LFPS did not affect the survival rates and LFPS was feasible.LFPS can increase the living quality of laryngopharynx careinoma patients.
