1.Detection of serum allergens in children with allergic rhinitis in Hangzhou

Objective To analyze the distribution of common allergens in children with allergic rhinitis(AR)in Hangzhou,and to provide reference for its prevention and treatment.Methods From January 2020 to December 2022,13 521 children who were diagnosed with AR and underwent in vitro serum specific immunoglobulin E(sIgE)detection in Affiliated Hangzhou First People's Hospital,School of Medicine,Westlake University were selected as the research objects.The positive rates of allergens in children with AR of different genders,ages and seasons were compared.Results Among 13 521 children,the positive rate of sIgE was 54.49%(7367/13 521),of which 64.82%were allergic to only one allergen.Among the 19 common allergens,the top three overall positive rates were household dust mite(44.27%),milk(14.62%),and mixed grass(4.82%).The positive rates of allergens of house dust mites,cat hair dander,mulberry,mold combination,tree pollen combination,amaranth,cockroach,milk,shrimp,crab,pineapple,shellfish and mango in male children were significantly higher than those in female children(P<0.05).The positive rates of allergens of house dust mite,mixed grass,cat hair dander,mulberry,mold combination,egg white,shrimp,crab,pineapple,cashew nuts and shellfish in different age groups were statistically significant(P<0.05).July and August were the two months with the highest allergen positive rate.The allergen positive rates of house dust mite,dog and cat hair dander were the highest in summer,and the allergen positive rate of plants was consistent with the flowering season.The sIgE concentration of house dust mite(77.87%)was mainly grade 3 and above.The sIgE concentration of other allergens was mainly grade 1 and 2.Conclusion Dust mites and milk are the most common allergens in children with AR in Hangzhou,and the distribution of allergens varies with gender,age and season.Allergen detection can provide a more accurate plan for the diagnosis,prevention and treatment of AR.
2.Mechanism underlying the effect of Liuwei Dihuang Pill on osteolysis and osteogenesis induced by titanium particles

BACKGROUND:At present,a large number of studies have found that Liuwei Dihuang Pill can be used to treat osteoporosis,but there are few related studies on the differentiation and mineralization of osteoblasts induced by wear particles using Liuwei Dihuang Pill. OBJECTIVE:To investigate the positive effect of different concentrations of Liuwei Dihuang Pill-containing serum on titanium particle-induced mouse MC3T3-E1 osteoblast in vitro osteolysis model. METHODS:Drug-containing serum was extracted after oral administration of Liuwei Dihuang Pill.The best concentration of Liuwei Dihuang Pill-containing serum and titanium particles on the viability of MC3T3-E1 cells was screened.MC3T3-E1 cells were divided into three groups.The blank group was given osteoblastic differentiation culture.The model group was given titanium particles(5 μg/mL)ossification culture.The drug-containing serum group was given titanium particles(5 μg/mL)+ Liuwei Dihuang Pill-containing serum(10%,15%and 20%doses).Osteoblast viability was detected by CCK-8 assay.Cell alkaline phosphatase activity was detected by alkaline phosphatase staining.Cell mineralization was detected by silver nitrate(Von Kossa)and alizarin red staining.Expression levels of bone differentiation-related genes Runx-2,Osterix,Ocn,Axin,Alp,and Opn were detected by qRT-PCR.Wnt/β-catenin signaling pathways β-catenin,p-GSK-3β,GSK-3β,Runx2 and Osterix protein expression levels were detected by western blot assay. RESULTS AND CONCLUSION:(1)Liuwei Dihuang Pill-containing serum culture reversed the decrease in alkaline phosphatase activity of MC3T3E-1 cells induced by titanium particles,increased the alizarin red staining and calcification of MC3T3E-1 cells,increased the expression of osteogenesis-related genes in MC3T3E-1 cells,and increased the expression of proteins related to the Wnt/β-catenin signaling pathway.(2)These findings indicate that Liuwei Dihuang Pill-containing serum can reverse the inhibitory effect of titanium particles on the differentiation and mineralization of osteoblasts,upregulate the expression of osteogenesis-related genes,and its mechanism is related to the regulation of Wnt/β-catenin signaling pathway,suggesting that Liuwei Dihuang Pill is expected to become an effective drug for preventing aseptic loosening of artificial joints.

Endothelial-mesenchymal transition (EndMT) disrupts vascular endothelial integrity and induces atherosclerosis. Active integrin β1 plays a pivotal role in promoting EndMT by facilitating TGFβ/Smad signaling in endothelial cells. Here, we report a novel anthraquinone compound, Kanglexin (KLX), which prevented EndMT and atherosclerosis by activating MAP4K4 and suppressing integrin β1/TGFβ signaling. First, KLX effectively counteracted the EndMT phenotype and mitigated the dysregulation of endothelial and mesenchymal markers induced by TGFβ1. Second, KLX suppressed TGFβ/Smad signaling by inactivating integrin β1 and inhibiting the polymerization of TGFβR1/2. The underlying mechanism involved the activation of FGFR1 by KLX, resulting in the phosphorylation of MAP4K4 and Moesin, which led to integrin β1 inactivation by displacing Talin from its β-tail. Oral administration of KLX effectively stimulated endothelial FGFR1 and inhibited integrin β1, thereby preventing vascular EndMT and attenuating plaque formation and progression in the aorta of atherosclerotic Apoe-/- mice. Notably, KLX (20 mg/kg) exhibited superior efficacy compared with atorvastatin, a clinically approved lipid-regulating drug. In conclusion, KLX exhibited potential in ameliorating EndMT and retarding the formation and progression of atherosclerosis through direct activation of FGFR1. Therefore, KLX is a promising candidate for the treatment of atherosclerosis to mitigate vascular endothelial injury.

4.Transplantation of beard follicle units to treat androgenetic alopecia in men

Objective:To evaluate the safety and efficacy of transplanting beard to scalp using follicular unit extraction(FUE).Methods:Data obtained from hirsute patients with extensive alopecia who underwent hair transplantation between March 2017 and April 2014 at Nanfang Hospital were analyzed. Individual beard hair follicular units (FUs) were harvested under tumescence along the direction of beard growth, using a hollow acute punch with an inner diameter of 0.6-0.7 mm driven with a motor machine. The angle between the punch and the skin is adjusted according to the angle of the beard. The depth of the punch is usually between 2-4 mm and just breaks through the dermis of the skin. The rotation speed of punch is 2 500-3 000 r/min. The harvested FUs were implanted to the bald area. The total amount of FUs harvested and the rate of FUs transection were recorded for each patient. Follow-up examinations were scheduled at 3 days, 10 days, 1 month, 3 months and 9 months postoperatively to check for complications, wound healing and hair growth using portrait photograph and trichoscopy.Results:A total of 58 patients male patients with advanced androgenetic alopecia (AGA) (Norwood-Hamilton Ⅴ-Ⅵ) were included in this study. The average harvested FUs was 2 012±631 and the transection rate for beard FUs was(3.4±0.7)%. The incidence of donor folliculitis was 10.3%(6/58). Folliculitis was cured within 1-3 weeks after treatment. Although all the donor areas were healed normally, 62.1% (36/58) of the patients had remaining mild white spots under the trichoscopy. There was no visible hypopigmented scars observed in the bare areas post-operation. The satisfaction scores of both doctors and patients and the third party were 4.78, 4.40 and 4.76, respectively.Conclusions:Transplanting beard FUs to bald scalp using FUE is a safe and effective for severe androgenetic alopecia with abundant beard.

MicroRNA (miRNA or miR) is a class of highly conserved endogenous non-coding RNA of 21~25nt, which is widely existed in organisms. Currently, miRNA has been proven to be associated with cardiovascular diseases in clinical research, but it has not been reported in the field of forensic medicine. This paper highlights recent findings about miRNA and its application in cardiovascular diseases, and the application aspect of miRNA in sudden cardiac death in forensic science.
8.Intervention effects of the extract of Alismatis Rhizoma on streptozotocin-induced type 2 diabetic rats

To investigate the hypoglycemic effects of ethyl acetate extract of Alismatis Rhizoma (AREEA) on type 2 diabetes mellitus,the model of type 2 diabetic rats was induced by high-fat diet feeding for 4 weeks and then intraperitoneal injection of a low dose of streptozotoin (STZ).Rats without the above-mentioned treatment were selected as the normal control group.The diabetic rats were randomly divided into 5 groups:the model control group,low doses (20 mg/kg),medium doses (50 mg/kg),high doses (100 mg/kg) of AREEA groups and positive control-metformin group (100 mg/kg).After four weeks,oral glucose tolerance test (OGTT) and insulin tolerance test (ITT) were performed,respectively.12 hours after the last administration,the levels of serum glucose,glycated hemoglobin(HbA1 c),insulin (Ins),total cholesterol (TC),triglyceride (TG),high density lipoprotein (HDL-C),low density lipoprotein (LDL-C),superoxide dismutase (SOD),malondialdehyde (MDA),glutathione (GSH-Px),tumor necrosis factor-or (TNF-α),interleukin-6 (IL-6),insulin-mediated tyrosine of IRS-1 and Akt phosphorylation in adipose tissue were determined.In addition,the pathological changes of pancreas were examined.After administration for 4 weeks,all doses of AREEA significantly reduced the fasting blood glucose of type 2 diabetic rats (P <0.05).In the high doses group of AREEA,the levels of GLU,HbA1c,TC,TG,MDA,TNF-α and IL-6 in serum were decreased significantly,and the levels of SOD and GSH-Px were increased (P < 0.05).These results suggest that AREEA has the therapeutic effect on type 2 diabetes-related symptoms by metabolic regulation of glucose and lipids.
9.Early Diagnostic Value of Ischemia-Modified Albumin in Rat Acute Hemorrhagic Myocardial Ischemia Model

Objective :To prove ischemia-modified albumin (IMA ) is an early index to adjust acute hemorrhagic myocardial ischemia with animal experiment .Methods :Twenty-four SD rats (group A) were bleeding with Hb dropped to 100 ,90 ,80 ,70 , 60 g/L .When serum IMA ascended obviously ,killed the rats and judged myocardial damage .Twenty-four SD rats (group B) were bleeding with Hb dropped to 100 ,90 ,80 ,70 ,60 g/L .When serum cTnT ,CK-MB ascended obviously ,killed the rats and judged myocardial damage .Twenty-four SD rats (control group) were bleeding with Hb dropped to 100 ,90 ,80 ,70 ,60 g/L ,and they were infused with albumin at the same time .When cTnT ,CK-MB of blood ascended obviously ,killed the rats and judged myocardial damage .Results :IMA of blood in SD rats of group A increased to (64 .2 ± 3 .1) U /mL when Hb was 80 g/L (P< 0 .05) .cTnT ,CK-MB ,NT-proBNP of blood had no changes at this time ,and no myocardial damage have been found .NT-proBNP of group B increased to (1175 .3 ± 274 .1) pg/mL when Hb was 60 g/L (P< 0 .05) ,cTnT of group B increased to (1 .48 ± 0 .33) ng /mL when Hb was 60 g/L (P< 0 .05) ,CK-MB of blood increased to (91 .8 ± 7 .0) U /L when Hb was 60 g/L (P< 0 .05) ,and rats had myocardial damage at this time point .IMA of blood in SD rats of intervention control group had no changes at each time point .NT-proBNP of blood climbed to (879 .8 ± 83 .5) pg/mL when Hb was 60 g/L (P<0 .05) .cTnT of blood climbed to (1 .03 ± 0 .02) ng/mL when Hb was 60 g/L (P< 0 .05) .CK-MB of blood climbed to (86 .2 ± 7 .0) U /L when Hb was 60 g/L (P< 0 .05) ,and rats had myocardial damage .Conclusions :The animal experiment had proved that ischemia-modified albumin is an early index to adjust acute hemorrhagic myocardial ischemia .
10.Mid-term results of Zweymüller SLR-plus? stem used in hip revision

BACKGROUND:The outcomes of SL-Plus? stem in primary total hip arthroplasty have been proved good, but whether SLR-Plus? revision stem can obtain a good outcome in revision hip arthroplasty needs more studies. OBJECTIVE:To observe the clinical results of revision hip arthroplasty with Zweymül er SLR-plus ? stem. METHODS:Revision hip arthroplasty of 41 hips in 39 patients was performed from November 1997 to May 2013 using SLR-plus ? stem. There were 26 hips (male) and 15 hips (female). They were at the age of 34-73 years old, 53 on average. In the 41 hips, 36 cemented and 5 uncemented femoral prostheses were used. Prosthetic changes on radiographs were observed. In accordance with Brooker classification, heterotopic ossification was classified to record the region and incidence of heterotopic ossification. Hip joint function was evaluated using Harris Hip Score, and survival rate of the prosthesis was analyzed. RESULTS AND CONCLUSION:A total of 31 patients (33 hips) were fol owed up for 1 to 16 years. The mean preoperative Harris hip score of 34 (range, 13-64) points improved to 85 (range, 55-94) points at the time of final fol ow-up. 32 hips (97%) had an excellent result. A 1 mm width radiolucent line was found in 1 femoral component without any symptom. Osteolysis and migration were seen in 1 hip, which needed re-revision. Heterotopic ossification developed in 9 hips, including 2 hips of Brooker grade 1, 4 hips of grade 2 and 3 hips of grade 3. No re-infection was found. Kaplan-Meier survivorship was 92%with radiographic loosening as the end point. Results demonstrated that the SLR-Plus? stem has sufficient immediate and long-term stability, which is reliable for patients undergoing hip revision surgery.
