1.Study on the correlation between oxygenation index and monitored parameters of ventilator lung injury view

Objective:To explore the correlation between the oxygenation index and the monitored parameters of lung injury view in ventilator,so as to provide a basis for monitoring the change of oxygenation index by the parameters of lung injury view.Methods:A total of 28 patients who received mechanical ventilation treatment and admitted to respiratory department of the First Affiliated Hospital with Nanjing Medical University from May 14 to September 30,2024 were selected.The monitored parameters of lung injury views of them during mechanical ventilation treatment with invasive ventilators were collected,including platform pressure(Pplat),driving pressure(ΔP),mechanical energy(Emech),ratio of tidal volume to ideal body weight(TVe/IBW),etc..Then,the data of oxygenation index were obtained.Finally,135 valid data were screened out.The 84 pieces of data with oxygenation index≤300mmHg were included in low oxygen group,and 51 data with oxygenation index>300mmHg were included in normal group.The correlations between oxygenation index and Pplat,ΔP,Emech,TVe/IBW were conducted by pairwise analysis.Results:In the low oxygen group,the oxygenation index was[(196.302±74.522)mmHg]significantly negative correlation with Emech[(12.686±5.401)J/min](ρ=-0.425,P<0.05),but not was significant correlation with Pplat,ΔP,and TVe/IBW(P>0.05).In the normal group,there were not significant correlations between the oxygenation index and Pplat,ΔP,Emech and TVe/IBW(P>0.05).There was a significant negative correlation between Emech and rank of oxygenation index(β=-0.389,P<0.05)in the low oxygen group.Conclusion:The visualization function of the Emech of the monitored parameters in lung injury view of ventilator can provide help for monitoring the changes of oxygenation index of patients in the low oxygen group,and can improve the oxygenation of patients by optimizing mechanical energy.However,it is necessary to balance the contradiction between ventilation intensity and protection for lung.
2.Practice and effectiveness of the optional parasitology curriculum Two Sides of the Same Coin in Parasitic Diseases among international students for the Bachelor of Medicine and Bachelor of Surgery program

To promote convergence education among national and international students, Nanjing Medical University designed an optional curriculum Two Sides of the Same Coin in Parasitic Diseases among international students for the Bachelor of Medicine and Bachelor of Surgery (MBBS) program, and a research-based teaching method was employed to teach 31 international students. This curriculum contained three parts, including parasitology-related knowledge, basic knowledge and skills of scientific research, and frontier topics in scientific research advances, and the assessment contents included course sign-in, classroom activity participation and scientific research presentation. Curriculum evaluation showed 100.0% (8/8) of students in batch 2018—2021, 81.8% (9/11) in batch 2022, and 83.3% (10/12) in batch 2023 with a total score of 80 points and higher, with median scores (interquartile range) of 91.1 (7.0), 90.8 (5.7) points and 90.8 (5.7) points, respectively (H = 0.04, P > 0.05). In addition, a questionnaire survey was performed with a Likert scale to assess the interests in curriculum learning, and the curriculum practicality and importance among 31 international students, and a total of 27 valid questionnaires were recovered, with a recovery rate of 87.1%. Questionnaire survey showed that the median total scores (interquartile range) were 7.0 (0.8) points among batch 2018—2021 students, 6.4 (1.4) points among batch 2022 students and 6.0 (1.1) points among batch 2023 students (H = 2.64, P > 0.05). Collectively, these data demonstrate that this optional curriculum improves the interests in parasitology learning of the international students for MBBS program, as well as their capability of self-directed learning, teamwork and innovative.
3.Code of practice for reference dosimetry in MR-guided high energy photon radiotherapy

The presence of magnetic fields in a magnetic resonance accelerator (MR-linac) can affect the reference dosimetry, and thus the existing Code of Practices (CoPs) are inadequate for MR-linac. In this article, the characteristics of adsorbed dose to water and ionization chamber response in the presence of magnetic fields were introduced and a formalism for reference dosimetry in MR-linac was developed based on the existing CoPs, aiming to provide reference for dosimetric quality control and research work of MR-linac in China.

6.Effect of long-term aspirin treatment on aneurysm sac after endovascular aortic repair of infrarenal abdominal aortic aneurysms: a propensity score matched analysis

Objective:To investigate the effect of long-term oral aspirin on the changes in the aneurysm sac and persistent type Ⅱ endoleak after endovascular aortic repair (EVAR) of infrarenal abdominal aortic aneurysms based on propensity score-matched analysis.Methods:A retrospective cohort study was used to analyze the clinical data of 133 patients with infrarenal abdominal aortic aneurysms treated with EVAR from January 2019 to December 2021 in the Department of Vascular Surgery, Nanjing Drum Tower Hospital. There were 113 males and 20 females, aged (74.8±7.2) years (range: 59 to 95 years). Patients were divided into the group receiving aspirin ( n=80) and the group not taking aspirin ( n=53) based on whether they took aspirin regularly for a long time after surgery. The two groups were matched in a 1∶1 ratio using propensity score matching and the caliper value was 0.05. Cumulative probability curve was plotted using the Kaplan-Meier method and the Log-rank test was used to compare the differences in primary endpoint events (enlargement of the aneurysm sac, occurrence of persistent type Ⅱ endoleak) and secondary endpoint events (adverse cardiovascular events and clinically relevant bleeding events) between the two groups. Results:The follow-up time was (38.4±11.8) months (range: 30 to 58 months). Among the 133 patients, a total of 25 cases (18.8%) suffered enlargement of the aneurysm sac, including 20 cases in the group receiving aspirin and 5 cases in the group not taking aspirin; 35 cases (26.3%) suffered persistent type Ⅱ endoleak, including 26 cases in the group receiving aspirin and 9 cases in the group not taking aspirin. Adverse cardiovascular events occurred in 11 cases (8.3%) and clinically relevant bleeding events were reported in 5 cases (3.8%). A matched cohort was established after propensity score matching, resulting in 32 cases per group. The survival analysis found that the rate of aneurysm sac enlargement was significantly higher in the group receiving aspirin than that in the group not taking aspirin (Log-rank test: P=0.010), and the incidence of persistent type Ⅱ endoleak was significantly higher than that in the group not taking aspirin (Log-rank test: P=0.019). The incidence of adverse cardiovascular events and clinically relevant bleeding events were not significantly different in two groups (Log-rank test: P=0.061, P=0.286). Conclusions:The risk of aneurysm sac expansion and persistent type Ⅱ endoleak were significantly higher in patients taking long-term aspirin after EVAR than in the group not taking asprin. Therefore, high-risk abdominal aortic aneurysm patients who are prone to aneurysm sac expansion should be evaluated in advance so that the risks and benefits of surgery can be comprehensively evaluated and treatment strategies can be optimized.

8.Analysis of the practice and optimization path of continuing medical education programs in tuberculosis

Objective:To summarize the practice of continuing medical education programs in tuberculosis at Beijing Chest Hospital, and to put forward suggestions and references for improving the level of education and the quality of training.Methods:This paper examined the current situation of continuing medical education programs in tuberculosis at Beijing Chest Hospital. A satisfaction survey was administered to 3 451 trainees in 30 programs using a questionnaire that assessed the four aspects of learning attitude, learning environment and conditions, learning performance, and development status. SPSS 22.0 was used for the χ 2 test and t-test. Python 3.8.1 was used for word frequency analysis. Results:Of the participants, 75.54% (2 607) were generally satisfied and 23.41% (808) were satisfied with the programs. Physicians with different titles, regions, and education levels differed significantly on satisfactory items ( P<0.05). Chief physicians (professors) demonstrated a significantly higher understanding of the training programs compared to other physicians ( P<0.001). Physicians who worked in Beijing exhibited a significantly greater learning uptake than those from regions outside Beijing ( P=0.002). Physicians with doctoral and master's degrees were significantly less satisfied with the teaching materials compared to those with undergraduate or lower degrees ( P=0.001). Conclusions:The overall satisfaction of participants in continuing medical education programs in tuberculosis is relatively high. The managerial staff should improve the programs in multiple dimensions by giving full play to the advantageous disciplines in the hospital, developing brand training programs, and establishing a full-cycle management mechanism. Additionally, it is crucial to standardize high-quality programs, actively expand online distance education, and explore verifiable self-study models.
9.Prevalence and risk analysis of antibiotic resistance genes in municipal solid waste treatment chain

Objective To understand the temporal and spatial distribution of antibiotic resistance genes (ARGs), respiratory exposure risks, and diffusion predictions within a municipal solid waste treatment chain in Shanghai, in order to provide a reference for ensuring the public health safety of the municipal solid waste treatment chain. Methods The PM10 (n=47) and related environmental samples (n=41) collected from a municipal solid waste treatment chain in Shanghai from December 2021 to January 2023 were analyzed by metagenome sequencing. The average daily exposure dose of ARGs was used to assess respiratory exposure risk, and the diffusion of airborne ARGs was predicted by the Hybrid Single-Particle Lagrangian Integrated Trajectory (HYSPLIT) model. Results The most frequently ARGs detected in the air in summer and winter were multidrug-resistant types (57.86%), such as mexT. The relative abundance of total ARG subtypes in the air in summer ranged from 1.03±0.66 copies/m3 to 2.38±1.45 copies/m3, mainly originating from kitchen waste (16.22%-46.62%). Temperature and pressure were the main influencing factors of ARGs in the air in summer (R2=0.81) and winter (R2=0.83), respectively. The average daily exposure doses of ARGs showed a higher respiratory exposure risk in summer (0.10 copies/(kg·d)) compared to winter (0.04 copies/(kg·d)), especially in office buildings. Streptococcus was a potential host of ARGs (bacA) in summer. The diffusion range of airborne ARGs was larger in summer (112.22 km2) than in winter (82.71 km2). Conclusions Compared with winter, the relative abundance and diversity of airborne ARGs in summer were higher, with Streptococcus being the potential host of ARGs. The respiratory exposure risk in the office building was the highest, mainly originating from the original waste. The diffusion range and direction of ARGs pose a greater risk spread of ARGs. This study provides data support for ensuring the public health safety of municipal solid waste treatment chains.
10.Report of 5 gene-edited pig-rhesus monkey heterotopic heart xenotransplantation experiment

Objective:To investigate the changing trends in cardiac function following xenogeneic heterotopic heart transplantation of multi-gene edited pig hearts and assess the impact of recipient immune responses on donor heart, laying experimental groundwork for the clinical application of gene editing technology.Methods:On December 16, 2023, xenogeneic heterotopic heart transplantation was performed between pigs and rhesus monkeys. Functional status of the graft under post-transplantation load conditions and recipient immune indicators were observed.Results:The recipient monkeys survived for 40 days with satisfactory functionality of both donor and recipient hearts, and no hyperacute or acute immune rejection reactions were observed.Conclusion:Multi-gene editing technology provides potential for xenotransplantation, yet further exploration is needed for its clinical application.
