1.Therapeutic Effect and Mechanism of Shentong Zhuyutang Combined with Dilongtang in Treatment of Lumbar Disc Herniation with Qi Stagnation and Blood Stasis Syndrome

ObjectiveTo observe the clinical efficacy of Shentong Zhuyutang combined with Dilongtang in the treatment of lumbar disc herniation (LDH) with Qi stagnation and blood stasis syndrome, and its effect on nucleus pulposus reabsorption and immune-inflammatory factors, exploring its therapeutic mechanism from the perspective of reabsorption. MethodsA total of 120 patients with LDH from the Fourth Clinical Medical College of Guangzhou University of Chinese Medicine, treated between June 2020 and January 2023, were randomly divided into the control group (52 cases, with 8 dropouts) and the observation group (49 cases, with 11 dropouts) according to a random number table. The control group received routine treatment, while the observation group was treated with Shentong Zhuyutang combined with Dilongtang in addition to routine treatment. Visual Analogue Scale (VAS), Oswestry Disability Index (ODI), Japanese Orthopaedic Association (JOA) score, and traditional Chinese medicine (TCM) syndrome score were measured before treatment and after 3 courses of treatment. Venous blood samples were collected for the determination of serological indexes. MR examination was performed during the 6-month follow-up to calculate the absorption rate. ResultsAfter treatment, both groups showed significant reductions in VAS, ODI, TCM syndrome score, serum tumor necrosis factor (TNF)-α, matrix metalloproteinase (MMP)-9, and vascular endothelial growth factor (VEGF) levels, and a significant increase in JOA score compared with pre-treatment values (P<0.05). Compared with the control group, the observation group showed significantly lower VAS, ODI, TCM syndrome score, serum TNF-α, MMP-9, and VEGF levels, and a significantly higher JOA score (P<0.05). The proportion of nucleus pulposus reabsorption in the observation group was 57.14% (28/49), significantly higher than 21.15% (11/52) in the control group (χ2=6.161, P<0.05). ConclusionShentong Zhuyutang combined with Dilongtang can effectively relieve pain, improve lumbar function, and alleviate TCM clinical symptoms in LDH patients with Qi stagnation and blood stasis syndrome. Imaging findings suggest that the treatment promotes the reabsorption of nucleus pulposus protrusion, while laboratory testing shows reduced serum levels of TNF-α, MMP-9, and VEGF, which contribute to the rehabilitation of patients.
2.Quantitative analysis on microvasculature in the optic disc area of patients with unilateral branch retinal vein occlusion

AIM: To observe the changes of retinal nerve fiber layer(RNFL)thickness and radial peripheral capillary(RPC)density in patients with unilateral branch retinal vein occlusion(BRVO), and further analyze the correlation between RPC density and RNFL thickness.METHODS: Observational study. Totally 37 patients with unilateral BRVO diagnosed at the ophthalmology department of First Hospital of Qinhuangdao from October 2020 to January 2022 were selected, the 37 affected eyes were the unilateral BRVO group, and 37 fellow healthy eyes were the contralateral unaffected group, and 35 healthy individuals(35 right eyes were selected)without ocular diseases during the same period were selected as the normal control group. The best corrected visual acuity, intraocular pressure, anterior segment, fundus and optical coherence tomography angiography(OCTA)were examined in both eyes of all BRVO patients and healthy individuals. The central macular thickness(CMT), the RNFL thickness, and the optic disc-AV crossing distance(DAVD)were measured by built-in software of the OCTA equipment. The optimized U-net algorithm was used to eliminate the large blood vessels, and then the RPC density was calculated. The CMT, RNFL thickness and RPC density were compared among the three groups. And the correlations of the RPC density with the CMT, RNFL thickness, and the DAVD were investigated.RESULTS: Compared with the contralateral unaffected group and the normal control group, the CMT and the RNFL thickness were significantly thickened in the unilateral BRVO group(all P<0.05); there were no statistical differences in the CMT and the RNFL thickness between the contralateral unaffected group and the normal control group(all P>0.05). The RPC density in the unilateral BRVO group increased compared with the contralateral unaffected group and decreased compared with the normal control group, but there was no statistically difference(all P>0.05). However, the RPC density in the contralateral unaffected group decreased compared with the normal control group(P<0.05). The RPC density in the unilateral BRVO group was not correlated with the CMT(P=0.960), but positively correlated with the RNFL thickness(r=0.401, P=0.014)and negatively correlated with the DAVD(r=-0.339, P=0.040).CONCLUSION: The RNFL thickened significantly and the RPC density did not change significantly in the optic disc area of BRVO patients. The RPC density is positively correlated with the RNFL thickness, indicating that the RNFL thickness can be used as a monitoring indicator to analyze and study the damage degree of the RPC density.
3.Correlation between change in choroid plexus volume and cognitive function in patients with Parkinson's disease

Objective To investigate the change in choroid plexus(CP)volume in Parkinson's disease(PD)patients with different cognitive states and its correlation with structural volumes of other brain regions.Methods A cross-sectional study was conducted on 48 PD patients admitted in Department of Neurosurgery of the First Affiliated Hospital of Army Medical University between May 2023 and April 2024,and on 35 healthy controls(HC)recruited through a physical exam center.According to the results of Mini-Mental State Examination(MMSE),the patients were divided into PD with cognitive impairment(PD-CI)group(n=27)and PD with normal cognitive function(PD-NC)group(n=21).3.0T magnetic resonance imaging(MRI)was performed using MPRAGE sequences,and CP volume and volumes of other brain regions were obtained using FreeSurfer 6.0 software.The CP volume was adjusted by calculating the ratio of its volume to estimated total intracranial volume(eTIV).After controlling for confounders,partial correlation analysis was used to assess the relationship between the CPV/eTIV ratio and the volumes of other brain regions as well as cognitive scale scores.Additionally,multiple linear regression analysis was performed to further explore the relationship between CPV and cognitive function in the PD-CI group.Results Compared to the HC group,the CPV in the PD-CI group was significantly larger(P=0.029).In the PD-CI group,the CPV/eTIV ratio showed significant positive correlations with the volume of the lateral ventricles(r=0.689,P=0.001),the volume of the third ventricle(r=0.592,P=0.006),the volume of cerebrospinal fluid(CSF)(r=0.508,P=0.022),and white matter hyperintensities(WMH)(r=0.486,P=0.030),but was negatively correlated with the volume of the caudate nucleus(r=-0.530,P=0.016),the volume of the thalamus(r=-0.477,P=0.033),and the MMSE scores(r=-0.483,P=0.031).But in the PD-NC group,the CPV/eTIV ratio was only positively correlated with CSF volume(r=0.571,P=0.021).Multiple linear regression analysis indicated that the CPV/eTIV ratio and MMSE scores remained significantly negatively correlated in the PD-CI group(β=-0.388,P=0.046).Conclusion Cognitive impairment in PD patients may be closely associated with the change in CP volume,suggesting that the volume can serve as a potential imaging marker in assessment of cognitive impairment in PD patients.
4.Armored IL15 or IL15 receptor adaptors enhance proliferation and anti-tumor activity of CD19-targeted CAR-NK cells

Objective To construct CD19-targeted chimeric antigen receptor natural killer(CAR-NK)cells armored with interleukin15(IL15)or IL15 receptor-linker(RLI)and preliminarily validate their proliferative capacity and anti-tumor activity in vitro.Methods Natural killer cells(NK92 and NK92MI)from patients with human malignant non-Hodgkin lymphoma were cultured,and CD19-targeted CAR-NK cells armored with IL15 or RLI were prepared using retroviral vector particles.IL15 secretion was measured by ELISA,and proliferative capacity was assessed via CFSE dilution assays.Human B-lymphocytic leukemia cells(Nalm6-GFP-Luc)and human colon cancer cells overexpressing CD19(hCD19-SW620-GFP-Luc)were cultured,with surface CD19 expression confirmed(>99%positivity for both).Anti-tumor activity was evaluated by measuring cytotoxicity at effector-to-target(E:T)ratios using luciferase-based assays(4/12 h),detecting surface CD107a expression via flow cytometry,and quantifying cytokine release using CBA assays.Results NK92/NK92MI-CD19 CAR cells armored with IL15 or RLI were successfully generated.IL15 secretion was significantly higher in armored groups versus non-armored controls(P<0.01).Without IL-2 stimulation,IL15/RLI enhanced CAR-NK proliferation(P<0.05).Both armored designs significantly increased tumor-killing efficiency(P<0.05)and CD107a degranulation.IL15/RLI-armored NK92-CD19 CAR cells exhibited elevated release of IL2,IL10,IL6,TNF-α,sFas,IFN-γ,and Granulysin(P<0.05),while armored NK92MI-CD19 CAR cells showed additional increases in Granzyme A,Granzyme B,and Perforin(P<0.05).Conclusion IL15/RLI-armored NK92/NK92MI-CD19 CAR cells demonstrate potent anti-tumor activity,supporting their combined clinical therapeutic potential for tumors.
5.The relationship between white matter injury and sleepiness,sleep disorders,and cognitive decline in patients with obstructive sleep apnea

OBJECTIVE To explore the relationship between white matter damage and sleepiness,decreased sleep quality,and cognitive decline in patients with OSA.METHODS There were 55 confirmed cases of OSA diagnosed by polysomnography(PSG)from January 2018 to May 2023 were selected,with 29 non-OSA controls also diagnosed by PSG.DKI scanning and Epworth sleepiness scale(ESS),Pittsburgh sleep quality index(PSQI),and Montreal cognitive assessment(MoCA)scores were performed for all subjects.Differences in kurtosis fractional anisotropy(KFA)of various brain regions were compared between the two groups to identify differential brain regions,and pairwise correlations were analyzed between KFA reduction and apnea-hypopnea index(AHI),lowest oxygen saturation(LSaO2)and various scale scores in OSA patients.RESULTS The KFA values in right external capsule,bilateral corona radiata,bilateral superior longitudinal fasciculus,corpus callosum,posterior cingulate gyrus of OSA group were lower than control group(P<0.05).For the OSA group:The correlation between ESS scores and KFA values of right posterior corona radiata,left superior longitudinal fasciculus,body of corpus callosum,splenium of corpus callosum are all negative(r=-0.287,-0.286,-0.276,-0.449,P<0.05).The correlation between PSQI scores and KFA values of right posterior corona radiata,left posterior corona radiata,splenium of corpus callosum are all negative(r=-0.390,-0.274,-0.348,P<0.05).The correlation between MoCA scores and KFA values of right posterior corona radiata,right superior longitudinal fasciculus,left anterior corona radiata,left posterior corona radiata,left superior corona radiata,left superior longitudinal fasciculus,genu of corpus callosum,body of corpus callosum,are all positive(r=0.290,0.389,0.298,0.278,0.340,0.473,0.344,0.344,P<0.05).The correlation between visuospatial and executive function scores and AHI,LSaO2,ESS scores and KFA values of right posterior corona radiata,left superior corona radiata,left superior longitudinal fasciculus and splenium of corpus callosum are all significant(r=-0.350,0.470,-0.343,0.401,0.284,0.387,0.274,P<0.05).CONCLUSION Patients with OSA exhibit damage to the white matter in certain brain regions.The damage to the corpus callosum,posterior corona radiata,and superior longitudinal fasciculus has the greatest impact on patients'sleepiness,reduced sleep quality,and cognitive impairment.In particular,the impairment in visuospatial and executive function is closely associated with white matter damage in the corona radiata and superior longitudinal fasciculus.
6.Analysis of Efficacy of Self-Formulated Bushen Qianyang Decoction Combined with Alendronate Sodium and Vitamin D3 in Treating Osteoporosis and Its Effect on Bone Metabolism and TMAO-NF-κB/NFATc1 Pathway

Objective To analyze the efficacy of self-formulated Bushen Qianyang Decoction combined with alendronate sodium and vitamin D3 in treating osteoporosis(OP)and to observe its effect on bone metabolism and trimethylamine oxide(TMAO)-nuclear factor κB(NF-κB)/activated T nuclear factor c1(NFATc1)pathway.Methods A total of 102 cases of patients with OP of spleen and kidney yang deficiency type who admitted to Taizhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2021 to January 2023 were randomly divided into an observation group and a western medicine group according to the random number table method,51 cases in each group.The western medicine group was treated with alendronate sodium and vitamin D,and the observation group was treated with self-formulated Bushen Qianyang Decoction on the basis of treatment for the western medicine group,and the course of treatment covered for 12 weeks.The changes of traditional Chinese medicine(TCM)syndrome score,Oswestry Disability Index(ODI)score,pain visual analogue scale(VAS)score,bone metabolism indicators,and levels of TMAO-NF-κB/NFATc1 pathway related factors before and after treatment in the two groups were observed.Moreover,the clinical efficacy and medication safety of the two groups were evaluated.Results(1)After 12 weeks of treatment,the total effective rate of the observation group was 96.08%(49/51),and that of the western medicine group was 78.43%(40/51).The intergroup comparison(tested by chi-square test)showed that the efficacy of the observation group was significantly superior to that of the western medicine group(P<0.01).(2)After treatment,the two groups presented the decreased scores of TCM symptoms such as cold pain in the lower back,soreness and fatigue,shortness of breath and reluctance to talk,vertigo and dizziness when compared with those before treatment(P<0.05),and the decrease of TCM syndrome scores in the observation group was significantly superior to that in the western medicine group(P<0.01).(3)After treatment,the ODI scores and pain VAS scores of patients in the two groups were decreased when compared with those before treatment(P<0.05),and the decrease in the observation group was significantly superior to that in the western medicine group(P<0.01).(4)After treatment,the serum osteocalcium(BGP)level in the two groups of patients was increased when compared with that before treatment(P<0.05),and the serum total procollagen type 1 intact N(amino)-terminal propeptide(PINP)and β-collagen degradation product(β-CTX)levels were decreased when compared with those before treatment(P<0.05),and the increase of the serum BGP level and the decrease of the serum PINP and β-CTX levels in the observation group were significantly superior to those in the western medicine group(P<0.01).(5)After treatment,the levels of serum TMAO,NF-κB,and NFATc1 in the two groups of patients were all decreased when compared with those before treatment(P<0.05),and the decrease of the above indicators in the observation group was significantly superior to that in the western medicine group(P<0.01).(6)The total incidence of adverse reactions in the observation group was 5.88%(3/51)and that in the western medicine group was 9.80%(5/51),and the intergroup comparison showed that the difference was not statistically significant between the two groups(P>0.05).Conclusion Self-formulated Bushen Qianyang Decoction combined with alendronate sodium and vitamin D3 exerts certain efficacy in treating OP patients with spleen and kidney yang deficiency type,which can effectively alleviate the symptoms such as cold pain in the lower back,alleviate the degree of dysfunction,improve bone metabolism,and inhibit the high-expressed TMAO,NF-κB,and NFATc1 in the serum.Moreover,the combined therapy does not cause serious adverse reactions,with high safety.
7.Exploration on the Congenital Circuit-Qi Endowment Features and Treatment Strategies for Patients with Chronic Atrophic Gastritis Based on the Five-Circuit and Six-Qi Theory

Objective To explore the distribution of circuit-qi indicators at birth in the patients with chronic atrophic gastritis(CAG)based on five-circuit and six-qitheory,and to summarize their congenital circuit-qi endowment features,so as to explore treatment strategies for CAG accordingly.Methods From January 2018 to December 2023,a total of 2 046 cases of patients with CAG admitted to Shenzhen Bao'an Traditional Chinese Medicine Hospital Affiliated to Guangzhou University of Chinese Medicine(i.e.,Shenzhen Bao'an Traditional Chinese Medicine Hospital)were selected,and the distribution of the five-circuit and six-qiindicators at birth in the patients was analyzed by using the SPSS 24.0 statistical software.The features of the congenital circuit-qi endowment were summarized,and then the corresponding treatment strategies were put forward.Results The CAG patients had the highest proportions of circuit-qiendowment features at birth,showing as the yearly circuit being deficiency fire circuit,the dominant circuit being metal circuit,the dominant qibeing yangmingdryness-gold,the guest qibeing taiyangcold-water,and sitianbeing shaoyangminister fire and zaiquanbeing j ueyinwind-wood,and the differences in the distribution of the dominant circuit,dominant qi,and guest qiwere statistically significant(P<0.05 or P<0.01),but differences in the distribution of the yearly circuit and sitian/zaiquanwere not statistically significant(P>0.05).The analysis of the combined circuit-qifeatures at birth showed that the proportion of patients with CAG born in the year of celestial restriction,mildly-rebellious year and disharmony year was the highest,and the proportion of those born in the convergent year and iso-convergent year was the lowest,with statistically significant differences(P<0.05).Conclusion The congenital circuit-qiendowment of patients with CAG is characterized by liver depression and qistagnation,dryness and coldness.Therefore,the treatment for CAG should be from the perspective of the liver,with the method of soothing liver and harmonizing stomach,supplemented by nourishing yin to moisten dryness,and protecting yang qiof the spleen and stomach.The combined use of acup-moxibustion and Chinese medicines is recommended.
8.Progress in the application of deep learning in the auxiliary diagnosis and prognostic evaluation of left ventricular hypertrophy

As an intermediate phenotype for multiple cardiovascular diseases, left ventricular hypertrophy (LVH) benefits from early diagnosis, which allows for timely intervention to prevent worsening of the condition, mitigate severe complications like heart failure and arrhythmias, and consequently improve patient outcomes. Preliminary advances have been made using deep learning for the early diagnosis and identification of etiology in LVH. This paper reviews the pathophysiology, causes, and diagnostic standards for LVH, discusses the strengths and weaknesses of applying deep learning to diagnostic tools such as echocardiography, cardiac magnetic resonance imaging, and electrocardiogram, examines its use in prognostic evaluation, and concludes by summarizing current achievements and suggesting future research avenues.
9.Electroacupuncture at Baihui and Shenting Regulates BDNF/TrkB-Mediated Microglial Polarization to Improve Insomnia in Rats

Objective To investigate the therapeutic effects and mechanisms of electroacupuncture(EA)at"Baihui"(GV20)and"Shenting"(GV24)acupoints in a rat model of insomnia.Methods An insomnia model was established by intraperitoneal injection of para-chlorophenylalanine(PCPA).Thirty-six rats were randomly divided into four groups(n=9 each):normal control group,EA-treated normal,model,and EA-treated model group.After successful modeling,EA intervention was administered for 7 days.Sleep duration and quality were monitored using an animal sleep-phase analysis system.Hypothalamic levels of serotonin(5-HT),dopamine(DA),tumor necrosis factor α(TNF-α),interleukin 6(IL-6),inducible nitric oxide synthase(iNOS),mannose receptor(CD206),interleukin 10(IL-10),and arginase 1(ARG1)were measured by ELISA.Hypothalamic histopathology was assessed via hematoxylin-eosin(HE)staining.Western Blot analyzed brain-derived neurotrophic factor(BDNF)and its receptor tropomyosin kinase B(TrkB)protein expression.Results Compared with the normal group,rats in the model group exhibited hypothalamic tissue damage,accompanied by reduced 5-HT levels,elevated DA concentrations,and significantly impaired sleep duration and sleep quality scores(P<0.05).Concurrently,the model group showed increased levels of M1 microglial markers(TNF-α,IL-6,iNOS)but decreased levels of M2 markers(CD206,IL-10,ARG1),alongside diminished protein expression of both BDNF and TrkB in the hypothalamus,the difference being statistically significant(P<0.05).In contrast,the EA-treated model group demonstrated attenuated hypothalamic lesions relative to the untreated model group.These rats exhibited elevated 5-HT levels,reduced DA concentrations,and significantly improved sleep duration and quality(P<0.05).Furthermore,EA treatment decreased expression of M1 markers(TNF-α,IL-6,iNOS)while increasing M2 markers(CD206,IL-10,ARG1),with concomitant upregulation of BDNF and TrkB protein expression,the difference being statistically significant(P<0.05).Conclusion EA at"Baihui"and"Shenting"alleviates insomnia symptoms by promoting microglial M1-to-M2 polarization,potentially via BDNF/TrkB pathway activation.
10.Effect of 3D rotational digital subtraction angiography-guided drug balloon predilation bare-metal stent in patients with vertebral artery origin stenosis

Objective To investigate the effect of 3D rotational digital subtraction angiography(DSA)-guided drug balloon predilation bare-metal stents on vertebral artery origin stenosis(VAOS).Methods A total of 63 VAOS patients who were admitted to Hai'an People's Hospital from May 2020 to December 2022 were selected and divided into two groups according to different treatment methods.There were 16 males and 14 females with a mean age of(52.10±3.62)years in the observation group.There were 19 males and 14 females with a mean age of(52.47±3.85)years in the control group.There was no statistical difference in the basic data between the two groups(P>0.05).The observation group was treated with 3D rotational DSA-guided drug balloon predilation bare-metal stent,while the control group was treated with conventional drug balloon predilation bare-metal stent.The efficacy of the two groups was compared.Results The success rate of operation was 100%(30/30)in the observation group,which was higher than that in the control group(96.97%[32/33],χ2=0.924,P=0.336).There was no significant difference in the stenosis rate between the two groups either before surgery or 3 months after surgery(P>0.05).The hemodynamic level of the initial segment of vertebral artery in the observation group was lower than that in the control group 3 months after surgery(P<0.05).The scores of neurological function and quality of life at 3 months after surgery and the incidence of recurrent stroke at 6 months after surgery in the observation group were lower than those in the control group(P<0.05).Restenosis occurred in 2 cases(6.67%)in the observation group and 9 cases(27.27%)in the control group,and there was significant difference in the incidence of restenosis between the two groups(χ2=4.630,P=0.031).Conclusion 3D rotational DSA-guided drug balloon predilation bare-metal stent can effectively reduce the stenosis rate and the incidence of restenosis,and improve the nerve function and the quality of life of VAOS patients.
