1.Prognostic significance of TRIM28 elevation in non-M3 acute myeloid leukemia

ObjectiveTo clarify the expression of TRIM28 in non-M3 acute myeloid leukemia (AML) and its correlation with clinical indicators and prognosis, and to further explore the effect of TRIM28 expression levels on the proliferation and apoptosis of AML cells using small interfering RNA. MethodsThe GSE34577 dataset was analyzed using R software to compare TRIM28 expression between healthy controls and non-M3 acute myeloid leukemia (AML) patients. Clinical samples from non-M3 AML patients were collected, with TRIM28 expression levels measured using real-time quantitative PCR (qPCR). The analysis focused on correlations between TRIM28 expression and various clinical indicators, treatment efficacy, and patient prognosis. Furthermore, small interfering RNA (siRNA) technology was employed to downregulate TRIM28 expression in human primary AML cells (HL60 cell line). The effects on cell proliferation and apoptosis were then assessed through CCK-8 assays and flow cytometry, respectively. ResultsThe results showed that TRIM28 was up-regulated in non-M3 AML of both online database GSE34577 and clinical samples (P<0.000 1), TRIM28 expression of new diagnosis group and relapsed refractory group was higher than iron deficiency anemia group (P<0.01), and there was no significance between different French-American-British classification systems subtype. TRIM28 expression was higher in non-M3 AML patients with a poor genetic prognosis stratified as moderate than in the good prognosis group, and TRIM28 expression was associated with NPM1 combined with the FLT3-ITD mutation, positively correlated with age, bone marrow blast, peripheral blood blast and white blood cell, negatively correlated with hemoglobin. In addition, interference TRIM28 greatly inhibited cell proliferation and promoted cell apoptosis. ConclusionThis study reveals that TRIM28 is highly expressed in non-M3 AML and associated with prognosis, and plays a key role in the proliferation and apoptosis of AML cells, suggesting that TRIM28 may serve as a novel therapeutic target for non-M3 AML.
2.Anorectal dynamics analysis in patients with severe rectocele

Objective To evaluate the results of anorectal dynamics in patients with severe rec-tocele.Methods A retrospective analysis was conducted on the clinical data of 38 patients defini-tively diagnosed with severe rectocele at the pelvic floor center of the anorectal department of Nanjing Hospital of Traditional Chinese Medicine from January 2020 to January 2023.All patients underwent anorectal manometry,and the results of anorectal dynamics were analyzed.Results A total of 15 pa-tients(39.47%)had elevated anal resting pressure(ARP),20(52.63%)had normal ARP,and 3(7.89%)had decreased ARP.Five patients(13.16%)had elevated maximum anal sphincter pressure(MASP),9(23.68%)had normal MASP,and 24(63.16%)had decreased MASP.Nor-mal defecation relaxation reflex was observed in 15 patients(39.47%),and abnormal defecation re-laxation reflex was observed in 23 patients(60.53%).Ten patients(26.32%)had normal rectal defecation pressure,and 28(73.68%)had decreased rectal defecation pressure.Eleven patients(28.95%)had elevated rectal initial sensory threshold(RIST),27(71.05%)had normal RIST.Fifteen patients(39.47%)had elevated rectal defecation sensory threshold,21(55.26%)had normal rectal defecation sensory threshold,and 2(5.26%)had decreased rectal defecation sensory threshold.Three patients(7.89%)had elevated rectal maximum tolerable volume,26(68.42%)had normal rectal maximum tolerable volume,and 9(23.68%)had decreased rectal maximum tolerable vol-ume.ARP was moderately positively correlated with the chronic constipation severity(CSS)score(P=0.007,r=0.429),and abnormal defecation relaxation reflex was moderately negatively correla-ted with the CSS score(P=0.019,r=-0.329).In 3 patients(7.89%),both ARP and MASP were decreased,and both ARP and MASP were elevated in 5 patients(13.16%).Conclusion Pre-operative anorectal dynamics analysis is necessary for patients with severe rectocele to formulate a reasonable individualized surgical plan and postoperative rehabilitation program.
3.Preparation of nanodrug PTX2 NPs and its killing effect on human lung cancer A549 cells

Objective:To prepare the nanodrug paclitaxel dimer(PTX2)-loaded nanoparticles(NPs)using the block copolymer 1,2-distearoyl-sn-glycero-3-phosphoethanolamine-polyethylene glycol 2000,(DSPE-PEG2000),and to explore the killing effect of PTX2 NPs on the human lung cancer A549 cells and its influence on apop to tis.Methods:The PTX2 NPs were prepared using nanoprecipitation method.Dynamic light scattering(DLS)was employed to determine the particle size distribution,and transmission electron microscope(TEM)was used to observe the ultrastructure of the nanoparticles.After treatment of 0 and 10 mmol·L-1 dithiothreitol(DTT),dialysis method was used to evaluate the in vitro drug release profile of PTX2 NPs.The cell counting kit-8(CCK-8)method was used to assess the survival rates of the A549 cells after treated with PTX2 and PTX2 NPs with different concentrations(0.000 1,0.001 0,0.010 0,0.100 0,and 1.000 0 μmol·L-1).The A549 cells were divided into control group,PTX2 group,and PTX2 NPs group.Live/dead staining method was used to detect the survival of the A549 cells in various groups,and flow cytometry was used to detect the apoptotic rates of the A549 cells in various groups.Results:The mean hydrodynamic diameter of PTX2 NPs was determined to be 144.7nmbyDLS.TheTEM imaging confirmed uniform spherical morphology of PTX2 NPs.In a reductive environment,the PTX2 NPs exhibited continuous drug release with total paclitaxel(PTX)release of 84%within 72 h.The results of CCK-8 method showed that both PTX2 and PTX2 NPs inhibited the proliferation of A549 cells in a dose-dependent manner.When the concentrations of PTX<0.01 μmol·L-1,compared with PTX2 group,the survival rates of A549 cells in PTX2 NPs group were significantly decreased(P<0.01 or P<0.001).The live/dead staining results showed that compared with PTX2 group,the number of red fluorescence-labeled dead cells in PTX2 NPs group was increased.The flow cytometry results demonstrated that compared with control group and PTX2 group,the apoptotic rates of the A549 cells in PTX2 NPs group were significantly increased(P<0.05 orP<0.01).Conclusion:The PTX2-loaded nanoparticles PTX2 NPs are successfully prepared which exhibits responsive drug release and demonstrates a more significant killing effect on the human lung cancer A549 cells compared to PTX2.
4.Investigation of the current preparation status and analysis of the application on first-aid drugs of rescue vehicles in children′s medical institutions in China

Objective:To analyze the current preparation and usage situation of first-aid drugs in the rescue vehicles in children's medical institutions, and provide references for optimizing the list of emergency drugs.Methods:First-aid drug lists in the rescue vehicles of 12 children's medical institutions from 11 provinces and municipalities in China were collected through questionnaire surveys, and the drugs as well as their quantities were compared. The existing problems in the use of first-aid drugs in the 12 medical institutions were investigated by on-site interviews. The usage information of first-aid drugs in the rescue vehicles of the General Surgery and Cardiology Department of Shanxi Children's Hospital from May 2022 to April 2023 was collected through the hospital information center, and usage frequency and dosage per patient of the drugs were calculated. Descriptive statistical analysis was conducted on the collected data.Results:The first-aid drug lists in 12 hospitals were various, including 7 to 22 kinds of drugs and involving a total of 23 drugs. These mainly included vasoactive drugs, cardiotonic drugs, antiarrhythmic drugs, antiangina and anti-ischemic drugs for the heart, antispasmodic drugs, diuretics, dehydrating drugs, sedative-hypnotic drugs, and glucocorticoids, all of which were injections. The drugs that were included in all the lists of 12 hospitals were epinephrine, dopamine, dexamethasone, furosemide, and atropine. The drug lists of different rescue vehicles throughout the hospital were the same in 4 hospitals, while the lists varied among departments based on their specific clinical needs in the other 8 hospitals. None of the 12 hospitals had a first-aid drug usage manual. The on-site interview results showed that, the existing problems about drug preparation and use in rescue vehicles mainly involved the following 6 aspects: drug types, quantities, labels, storage, procurement, and usage. In Shanxi Children's Hospital, the types and quantities of first-aid drugs in rescue vehicles of General Surgery Department and Cardiology Department were the same. There were 6 and 9 kinds of drugs were used in the 2 departments during rescue operations, respectively. The drugs that were never used in either department included promethazine, lidocaine, diazepam, phenobarbital, raceanisodamine, sodium bicarbonate, atropine, glucose, and calcium gluconate.Conclusions:The phenomenon of unreasonable kinds and quantities of first-aid drugs in the rescue vehicles existed in the children's medical institutions, and the drugs provided did not fully match the actual clinical needs. There was an urgent need for preparation guidelines and usage manuals of first-aid drugs that were suitable for children's medical institutions to enhance the scientificity of drug supply and the correctness of usage.

6.Finite element analysis of osteoporosis in proximal femur after cannulated screw fixation for femoral neck fracture

BACKGROUND:After the internal fixation of cannulated screws in femoral neck fractures,because the affected limb is often unable to bear weight in the short term and the implants with high stiffness have a stress shielding effect on the fracture end,it is easy to cause osteoporosis of the affected limb and changes in the biomechanical distribution of the proximal femur,the incidence of osteonecrosis of the femoral head is high after surgery.At present,few studies have been conducted on the biomechanical effects of osteoporosis at the proximal end of the femur occurring after femoral neck fracture surgery on femoral neck fracture treated with cannulated screws. OBJECTIVE:Using finite element analysis,to investigate the biomechanical effects of osteoporosis occurring after femoral neck fracture surgery on femoral neck fracture treated with cannulated screws and explore the role of biomechanical factors in osteonecrosis of the femoral head. METHODS:Based on the obtained CT scan data of the femur in a patient with a femoral neck fracture,a proximal femoral model for internal fixation for femoral neck fracture was established by Mimics 19.0,3-Matic,UG 11.0,Hypermesh 14.0,and Abaqus software.One finite element model of the proximal femur without osteoporosis and three finite element models of the proximal femur with osteoporosis were analyzed using Abaqus software.The stress,contact pressure,displacement peak and cloud map under different components of the four models were measured and analyzed,and the internal stress changes and distribution of the femoral head were compared and analyzed. RESULTS AND CONCLUSION:The stresses and contact pressures of the femoral head and lower anterior cannulated screws varied more with the degree of osteoporosis.The peak displacement of the four models increased slowly with the degree of osteoporosis.By one-way analysis of variance,there was no significant effect of the degree of osteoporosis on the peak stress,contact pressure,and displacement of the different components.The internal stress distribution of the femoral head changed with the degree of osteoporosis.Changes in the biomechanical environment of the proximal femur have an important impact on osteonecrosis of the femoral head.
7.Clinical characteristics and prognosis analysis of acute myeloid leukemia patients with PTPN11 gene mutation

Objective To investigate the clinical characteristics and prognosis of acute myeloid leukemia(AML)patients with PTPN11 gene mutation.Methods Total 115 adult AML patients who underwent initial diagnosis,treatment,and second-generation sequencing(NGS)detecting at hospital were recruited in this study.Clinical da-ta included disease characteristics,treatment efficacy,long-term prognosis,immune cell subpopulations,and leu-kemia stem cells were collected to analyze the clinical characteristics and prognosis of AML patients with PTPN11 gene mutation.Results PTPN11 gene mutation rate in newly diagnosed adult AML was 9.57%,and the mutation site mainly occurred in exon 3 region with all mutation type being point mutation.Compared with PTPN11 wild-type group,PTPN11 gene mutation group had a higher early mortality rate(18.18%vs 4.00%,P=0.048),a lower complete response rate(33.33%vs 67.71%,P=0.039),a higher recurrence rate(83.33%vs 42.31%,P=0.043),a shorter median overall survival time(9 months vs 20 months,P=0.026),a lower proportion of ef-fector T cells[(1.39±0.12)%vs(3.56±0.46)%,P=0.038],and a higher proportion of leukemia stem cells[(13.82±3.66)%vs(3.87±1.40)%,P=0.021].Conclusion PTPN11 gene mutation is a poor prognostic marker for AML.Those patients have a high early mortality rate,low complete remission rate,high recurrence rate,short median overall survival time,a low proportion of effector T cells,and a high proportion of leukemia stem cells.
8.Meta-analysis on the effects of exercise training-based respiratory rehabilitation therapy in pneumoconiosis patients

Objective To comprehensively analyze the effectiveness of exercise training-based respiratory rehabilitation therapy on patients with occupational pneumoconiosis (hereinafter referred to as "pneumoconiosis"). MethodsLiterature on randomized controlled trials of exercise training-based respiratory rehabilitation therapy for pneumoconiosis patients published from the establishment of the database to July 2023 was retrieved from academic systems such as the China National Knowledge Infrastructure, Wanfang Data Knowledge Service Platform, VIP Database, and China Biology Medicine using bibliometrics method. The RevMan 5.4 software was used for meta-analysis of the selected literatures. Subgroup analysis was conducted to explore the source of study heterogeneity. The funnel plot method was used to test publication bias. Results A total of 55 articles were included, involving 2 436 pneumoconiosis patients in the experimental group and 2 405 pneumoconiosis patients in the control group. The result of random or fixed effect model showed that the six minutes walking distance, the total score of Short from Health Survey-36, forced vital capacity (FVC), forced expiratory volume in one second (FEV1), FEV1/FVC, arterial partial pressure of oxygen of pneumoconiosis patients increased after respiratory rehabilitation therapy (all P<0.05), while the total score of the St. George's Respiratory Questionnaire and arterial partial pressure of carbon dioxide decreased compared with the conventional treatment (all P<0.05). 9.Analysis of information related to child-prohibition in drug labels in China

Objective:To analyze the information related to child-prohibition in drug labels in China.Methods:The existing drug labels for chemical drugs and biological products included in the China Pharmacopoeia 2020 (Ch. P) were collected through searching websites such as "Yaozhiwang" "Dingxiangyuan" and "Yimaitong", and information related to child-prohibition was extracted. The expression and existing problems of information about child-prohibition were analyzed.Results:A total of 1 741 and 149 chemical and biological products were included in the Ch.P, respectively, of which 411 (23.6%) and 6 (4.0%) products involved information of child-prohibition. Information of child-prohibition was expressed in 18 ways, such as "prohibited" "not recommended" and "try not to", etc. Issues of child-prohibition information in drug labels of the same drug from different manufacturers were inconsistent text descriptions, inconsistent age range, and incomplete information on whether there was benzyl alcohol; issues in the same drug label was different age ranges for child-prohibition in different chapters.Conclusions:There are inconsistencies and non-standard issues in information related to child-prohibition in the existing drug instructions in China. Drug manufacturers should conduct full lifecycle management of drug labels in accordance with the "Technical Guidelines for Writing Information on Children′s Medication in the Instructions of Chemical Drugs and Therapeutic Biological Products (trial)", and continuously standardize and improve the information of child-prohibition.
10.Analysis of self-assessment results on medication safety in 10 pediatric medical institutions

Objective:To understand preliminarily the current status of medication safety management in pediatric medical institutions in China.Methods:Self-assessment of medication safety situation were conducted in 10 pediatric medical institutions of the Pharmacy Professional Committee of Futang Children′s Medical Development Research Center using the "optimized version of 2011 Medication Safety Self Assessment ? for Hospitals (161 assessment items)" (self-assessment criteria). The self-assessment criteria included 10 key elements, 20 core characteristics, and 161 assessment items. The self-assessment results on medication safety among participating institutions were summarized, and the percentage scores and implementation proportions of the key elements, core characteristics, and assessment items were descriptively analyzed. Results:All 10 participating medical institutions completed the questionnaires within the specified time. The results showed that the overall percentage score of medication safety self-assessment was 74.72%. The element Ⅲ (communication of medication orders and other medicines information, 83.89%) and element Ⅹ (quality process and risk management, 67.84%) were with the highest and lowest percentage scores, respectively. The element Ⅲ (66.67%) and element Ⅸ (patient education, 40.00%) were with the highest and lowest percentage of full implementation, respectively. The element Ⅸ (46.25%) and element Ⅲ (26.67%) were with the highest and lowest percentage of partial implementation, respectively. The element Ⅹ (quality process and risk management, 18.38%) and element Ⅵ (procurement, use, and monitoring maintenance of drug treatment equipment, 6.25%) were with the highest and lowest percentage of non-implementation, respectively. Among the 20 core characteristics, characteristic 10 (guaranteeing the isolation of non-pharmaceutical chemicals from patients and avoiding harm to patients due to misuse, 96.25%) and characteristic 7 (adopting measures such as developing clinical pathways and establishing standardized administration time to ensure patients receive standardized diagnosis and treatment, 57.50%) were with the highest and lowest percentage scores, respectively. The characteristic 20 (following hospital infection control measures during drug storage, formulation, preparation, and administration, 90.00%) and characteristic 17 (establishing a fair and just drug safety culture within the hospital, encouraging and advocating safe behavior, rather than punitive measures for medical personnel related to medication errors, 25.00%) were with the highest and lowest percentage of complete implementation, respectively. The characteristic 7 (55.00%) and characteristic 20 (5.00%) were with the highest and lowest percentage of partial implementation, respectively. The characteristic 17 (22.86%) and characteristic 10 (0) were the highest and lowest percentage of non-implementation, respectively.Conclusions:The 10 pediatric medical institutions participating in the self-assessment have a higher level of management in the communication of medication orders and other drug information, as well as in the storage and distribution of drugs. However, there are shortcomings in the construction of a fair culture, proactive prevention of medication risks, and improvement of information systems.
