BACKGROUND:
The available evidence regarding the benefits of percutaneous coronary intervention (PCI) on patients receiving dialysis with coronary artery disease (CAD) is limited and inconsistent. This study aimed to evaluate the association between PCI and clinical outcomes as compared with medical therapy alone in patients undergoing dialysis with CAD in China.
METHODS:
This multicenter, retrospective study was conducted in 30 tertiary medical centers across 12 provinces in China from January 2015 to June 2021 to include patients on dialysis with CAD. The primary outcome was major adverse cardiovascular events (MACE), defined as a composite of cardiovascular death, non-fatal myocardial infarction, and non-fatal stroke. Secondary outcomes included all-cause death, the individual components of MACE, and Bleeding Academic Research Consortium criteria types 2, 3, or 5 bleeding. Multivariable Cox proportional hazard models were used to assess the association between PCI and outcomes. Inverse probability of treatment weighting (IPTW) and propensity score matching (PSM) were performed to account for potential between-group differences.
RESULTS:
Of the 1146 patients on dialysis with significant CAD, 821 (71.6%) underwent PCI. After a median follow-up of 23.0 months, PCI was associated with a 43.0% significantly lower risk for MACE (33.9% [ n = 278] vs . 43.7% [ n = 142]; adjusted hazards ratio 0.57, 95% confidence interval 0.45-0.71), along with a slightly increased risk for bleeding outcomes that did not reach statistical significance (11.1% vs . 8.3%; adjusted hazards ratio 1.31, 95% confidence interval, 0.82-2.11). Furthermore, PCI was associated with a significant reduction in all-cause and cardiovascular mortalities. Subgroup analysis did not modify the association of PCI with patient outcomes. These primary findings were consistent across IPTW, PSM, and competing risk analyses.
CONCLUSION
This study indicated that PCI in patients on dialysis with CAD was significantly associated with lower MACE and mortality when comparing with those with medical therapy alone, albeit with a slightly increased risk for bleeding events that did not reach statistical significance.

2.Preparation and in Vitro Evaluation of Matrine Lipid-based Cubic Liquid Crystalline Nanoparticle Gels

ObjectiveTo prepare matrine lipid-based cubic liquid crystalline nanoparticle (MAT-LLCN) gels and investigate its in vitro release and transdermal absorption behavior. MethodTaking entrapment efficiency as the index, the optimal formulation of MAT-LLCN was screened by extreme vertex mixture method based on the optimal ratio of glycerol monooleate (GMO) to poloxamer 407 (P407), and its drug loading was investigated. MAT-LLCN gels was prepared by mixing MAT-LLCN with pre-swelled carbomer 940 as the gel matrix. The structure of MAT-lipid-based cubic liquid crystalline (LLC) was characterized by polarized light microscopy (PLM) and small angle X-ray scattering (SAXS). The in vitro release and transdermal absorption properties of MAT-LLCN gels and MAT ordinary gels were compared by modified Franz diffusion cell method, skin structure changes caused by them were observed by hematoxylin-eosin (HE) staining. ResultThe optimal formulation of MAT-LLCN gels was 5.5% of GMO-P407 (9∶1), 1%-6% of MAT, 0.6% of carbomer 940, adding water to sufficient amount. The prepared MAT-LLC was confirmed as body-centered (Im3m) LLC. The in vitro release behavior of MAT-LLCN gels was in accordance with the Weibull equation (R2=0.954 0), and the release mechanism was the Fick diffusion. In vitro transdermal test showed that all the parameters of MAT-LLCN gels were higher than those of MAT ordinary gels (P<0.05), including cumulative release rate, steady-state release rate and the amount of drug retention in skin. HE staining results showed that MAT-LLCN gels could loose the cellular arrangement of skin stratum corneum, and maintain the stability of the cell structure of the dermis. ConclusionThe prepared MAT-LLCN gels can accelerate the transdermal drug transport and form drug storage in the dermis by rapidly opening the skin stratum corneum barrier, suggesting that LLC has good application prospects in the field of transdermal drug delivery.
3.Protective Effect of Ethanolic Extract of Hemsleya chinensis on HCl/Ethanol-induced Acute Gastric Ulcer in Rats Based on p38 MAPK/NF-κB Signaling Pathway

ObjectiveTo investigate the mechanism of protective effect of ethanol extracts of Hemsleya chinensis (HC-EE) on hydrochloric acid/ethanol (HCl/EtOH)-induced acute gastric ulcer in rats. MethodLipopolysaccharide (LPS)-induced RAW264.7 cells were used to evaluate inhibitory effect of HC-EE on the production of inflammatory mediators in vitro. A rat acute gastric ulcer model induced by HCl/EtOH (60% ethanol in 150 mmol·L-1 HCl) was used to evaluate protective effect of HC-EE on acute gastric ulcer. Rats were divided into five groups, including normal group, model group, HC-EE low dose (HC-EE 30, 30 mg·kg-1) group, HC-EE high dose (HC-EE 60, 60 mg·kg-1) group and ranitidine (35 mg·kg-1) group. For model and drug-treated groups, vehicle solvent or drugs were orally administered twice daily for 7 consecutive days before the rats were subjected to HCl/EtOH to induce acute gastric ulcer. After being anesthetized, ulcer surface of each rat was obtained and recorded using electronic imaging technology, and the ulcer inhibition rate was calculated by ImageJ 1.8.0. Hematoxylin-eosin (HE) and periodic acid-Schiff (PAS) staining were used to observe the pathological histological changes in rats. Content of nitric oxide (NO) in cell culture medium was measured by the Griess method. The levels of interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), IL-6, vascular cell adhesion molecule-1 (VCAM-1) and prostaglandin E2 (PGE2) in rat serum (or cell culture medium) were determined by enzyme linked immunosorbent assay (ELISA). The protein expressions of phosphorylation (p)-p38 mitogen activated protein kinase (MAPK)/p38 MAPK and p-nuclear transcription factor-κB (NF-κB) p65/NF-κB p65 in rat gastric tissue were detected by Western blot. ResultIn vitro assay showed HC-EE could significantly down-regulate the expressions of NO, TNF-α, IL-1β, IL-6 and VCAM-1 in LPS-induced cells (P<0.05, P<0.01). In vivo experimental results showed that, compared with the normal group, gastric tissue of the model group was severely damaged, and the area of gastric ulcer was significantly enlarged, levels of TNF-α, IL-6 were significantly increased (P<0.01), and the level of PGE2 was significantly decreased (P<0.01), the phosphorylation levels of of p38 MAPK, NF-κB p65 in gastric tissue were significantly increased (P<0.01). Compared with the model group, HC-EE dose-dependently improved HCl/EtOH-induced gastric tissue injury and inflammatory cell infiltration, and it could increase ulcer inhibition rate, significantly decreased the release of TNF-α and IL-6 (P<0.01), HC-EE 60 group could increase the content of PGE2 (P<0.05), and significantly inhibit the phosphorylation levels of p38 MAPK and NF-κB p65 (P<0.05, P<0.01). ConclusionHC-EE can exert protective effect on HCl/EtOH-induced acute gastric ulcer in rats, and its mechanism may be related to the inhibition of expression of inflammatory mediators mediated by p38 MAPK/NF-κB signaling pathway.

Objective:To assess the clinical effectiveness and safety of Omalizumab for treating pediatric allergic asthma in real world in China.Methods:The clinical data of children aged 6 to 11 years with allergic asthma who received Omalizumab treatment in 17 hospitals in China between July 6, 2018 and September 30, 2020 were retrospectively analyzed.Such information as the demographic characteristics, allergic history, family history, total immunoglobulin E (IgE) levels, specific IgE levels, skin prick test, exhaled nitric oxide (FeNO) levels, eosinophil (EOS) counts, and comorbidities at baseline were collected.Descriptive analysis of the Omalizumab treatment mode was made, and the difference in the first dose, injection frequency and course of treatment between the Omalizumab treatment mode and the mode recommended in the instruction was investigated.Global Evaluation of Treatment Effectiveness (GETE) analysis was made after Omalizumab treatment.The moderate-to-severe asthma exacerbation rate, inhaled corticosteroid (ICS) dose, lung functions were compared before and after Omalizumab treatment.Changes in the Childhood Asthma Control Test (C-ACT) and Pediatric Asthma Quality of Life Questionnaire (PAQLQ) results from baseline to 4, 8, 12, 16, 24, and 52 weeks after Omalizumab treatment were studied.The commodity improvement was assessed.The adverse event (AE) and serious adverse event (SAE) were analyzed for the evaluation of Omalizumab treatment safety.The difference in the annual rate of moderate-to-severe asthma exacerbation and ICS reduction was investigated by using t test.The significance level was set to 0.05.Other parameters were all subject to descriptive analysis.A total of 200 allergic asthma patients were enrolled, including 75.5% ( n=151) males and 24.5% ( n=49) females.The patients aged (8.20±1.81) years. Results:The median total IgE level of the 200 patients was 513.5 (24.4-11 600.0) IU/mL.Their median treatment time with Omalizumab was 112 (1-666) days.Their first dose of Omalizumab was 300 (150-600) mg.Of the 200 cases, 114 cases (57.0%) followed the first Omalizumab dosage recommended in the instruction.After 4-6 months of Omalizumab treatment, 88.5% of the patients enrolled ( n=117) responded to Omalizumab.After 4 weeks of treatment with Omalizumab, asthma was well-controlled, with an increased C-ACT score [from (22.70±3.70) points to (18.90±3.74) points at baseline]. Four-six months after Omalizumab administration, the annual rate of moderate-to-severe asthma exacerbation had a reduction of (2.00±5.68) per patient year( t=4.702 5, P<0.001), the median ICS daily dose was lowered [0 (0-240) μg vs. 160 (50-4 000) μg at baseline] ( P<0.001), the PAQLQ score was improved [(154.90±8.57) points vs. (122.80±27.15) points at baseline], and the forced expiratory volume in one second % predicted (FEV 1%pred) was increased [(92.80±10.50)% vs. (89.70±18.17)% at baseline]. In patients with available evaluations for comorbidities, including allergic rhinitis, atopic dermatitis or eczema, urticaria, allergic conjunctivitis and sinusitis, 92.8%-100.0% showed improved symptoms.A total of 124 AE were reported in 58 (29.0%) of the 200 patients, and the annual incidence was 0(0-15.1) per patient year.In 53 patients who suffered AE, 44 patients (83.0%) and 9 patients (17.0%) reported mild and moderate AE, respectively.No severe AE were observed in patients.The annual incidence of SAE was 0(0-1.9) per patient year.Most common drug-related AE were abdominal pain (2 patients, 1.0%) and fever (2 patients, 1.0%). No patient withdrew Omalizumab due to AE. Conclusions:Omalizumab shows good effectiveness and safety for the treatment of asthma in children.It can reduce the moderate-to-severe asthma exacerbation rate, reduce the ICS dose, improve asthma control levels, and improve lung functions and quality of life of patients.

The cultivation and production of cucumber are seriously affected by downy mildew caused by Pseudoperonospora cubensis. Downy mildew damages leaves, stems and inflorescences, and then reduces the yield and quality of cucumber. This review summarized the research advances in cucumber downy mildew, including pathogen detection and defense pathways, regulatory factors, mining of pathogens-resistant candidate genes, proteomic and genomic analysis, and development of QTL remarks. This review may facilitate clarifying the resistance mechanisms of cucumber to downy mildew.

Objective:To investigate the knowledge, attitudes, and practices (KAP) of pulse oximetry among pediatric healthcare providers in China and analyze the factor influencing the KAP.Methods:A self-developed questionnaire was used for an online research on the KAP of 11 849 pediatric healthcare providers from 31 provinces, autonomous regions, and municipalities of China from March 11 to 14, 2022.The factors influencing the KAP of pulse oximetry among pediatric healthcare providers were examined by Logistic regression. Results:The scores of KAP, of pulse oximetry were 5.57±0.96, 11.24±1.25 and 11.19±4.54, respectively.The corresponding scoring rates were 69.61%, 74.95%, and 55.99%, respectively. Logistic regression results showed that the gender and working years of pediatric healthcare providers, the region they were located, and whether their medical institution was equipped with pulse oximeters were the main factors affecting the knowledge score (all P<0.05). Main factors influencing the attitude score of pediatric healthcare providers included their knowledge score, gender, educational background, working years, region, medical institution level, and whether the medical institution was equipped with pulse oximeters (all P<0.05). For the practice score, the main influencing factors were the knowledge score, gender, age, and whether the medi-cal institution was equipped with pulse oximeters (all P<0.05). Conclusions:Chinese pediatric healthcare providers need to further improve their knowledge about and attitudes towards pulse oximetry.Pulse oximeters are evidently under-used.It is urgent to formulate policies or guidelines, strengthen education and training, improve knowledge and attitudes, equip more institutions with pulse oximeters, and popularize their application in medical institutions.
7.Current Development Status and Consideration for Rare Hemorrhagic Disease Drugs

Hemophilia is the only rare hereditary hemorrhagic disorder included in the First Rare Diseases catalogue. However, rare bleeding diseases identified in the clinic are far more common than hemophilia. Most other rare hemorrhagic disorders have less effective treatment than hemophilia. Hemophilia has a history of successful drug development in rare hemorrhagic diseases, and the cycle between clinical research and drug development has been gradually realized. Drug research and pharmaceutical companies can refer to the drug research and development process in the field of hemophilia, learn from the experience of hemophilia drug research and develop treatments. The industry can increase drug development by strengthening basic research, focusing on the value of natural history research, the application of quantitative pharmacological tools and improving the efficiency of drug development to meet the urgent unmet medical needs of patients with rare hemorrhagic diseases.
8.Acute promyelocytic leukemia combined with hereditary fibrinogen deficiency: report of one case and review of literature

Objective:To improve the clinical recognition of hereditary fibrinogen deficiency.Methods:The diagnosis and treatment process of a patient with acute promyelocytic leukemia (APL) complicated with hereditary fibrinogen deficiency who was admitted to the second Affiliated Hospital of Anhui Medical University in December 2018 was retrospectively analyzed, and the relevant literature was reviewed.Results:The patient was initially diagnosed as APL, and the complete remission was obtained after dual-induction therapy of all-trans retinoid acid and arsenous acid. During the first consolidation treatment, repeated reviews of fibrinogen fluctuated between 1.0-1.5g/L, and further improving the fibrinogen gene sequencing to diagnose APL combined with hereditary fibrinogen deficiency.Conclusion:For APL patients in remission who have decreased fibrinogen for many times and patients with hereditary fibrinogen deficiency who have significantly decreased fibrinogen in a short period, bone marrow biopsy and genetic testing should be further conducted to determine the pathogenesis.

Flowering is a critical transitional stage during plant growth and development, and is closely related to seed production and crop yield. The flowering transition is regulated by complex genetic networks, whereas many flowering-related genes generate multiple transcripts through alternative splicing to regulate flowering time. This paper summarizes the molecular mechanisms of alternative splicing in regulating plant flowering from several perspectives, future research directions are also envisioned.

10.Diagnostic and prognostic assessment value of serum interlukin-35 combined with Klotho protein in acute kidney injury with sepsis

Objective:To investigate the diagnostic and prognostic assessment value of serum interlukin(IL)-35 combined with Klotho protein in acute kidney injury(AKI) with sepsis.Methods:One hundred and seventy-five patients with sepsis admitted to Affliated Dongfeng Hospital, Hubei University of Medicine from January 2017 to May 2019 were enrolled and divide into AKI group(86 cases) and non-AKI group (89 cases) according to the standard of Kidney Disease Improving Global Outcomes (KIDGO) and groups were divided based on the prognosis of AKI clinical outcome. The levels of serum creatine (Scr), serum IL-35 and Klotho protein of different groups were compared. The receiver operating characteristic curve (ROC curve) of serum IL-35, Klotho protein and IL-35 combined with Klotho protein were drawn, and the value of serum IL-35, Klotho protein and IL-35 combined with Klotho protein in the diagnosis and prognosis prediction of sepsis AKI were evaluated.Results:The level of Scr in the AKI group was higher than that in the non-AKI group [(264.43 ± 67.86) μmol/L vs. (64.76 ± 22.41) μmol/L], while the levels of IL-35 and Klotho protein were lower than those in the non-AKI group [(69.37 ± 28.15)ng/L vs. (94.74 ± 35.32) ng/L, (142.31 ± 32.55) ng/L vs. (256.23 ± 36.25) ng/L], and the differences were statistically significant ( P<0.05). In the patients that had AKI, those who were in AKI phase one, without continuous renal dialysis or whose renal functions recovered at the end point had relatively lower Scr level and higher IL-35 and Klotho protein levels, and the differences were statistically significant ( P<0.05). The area under the curve (AUC) of AKI and staged AKI predicted by IL-35 combined with Klotho protein levels were 0.918 and 0.899 respectively, and the area under the curve (AUC) of continuous renal replacement therapy needed in the AKI patients and their renal function recovery predicted by IL-35 combined with Klotho protein levels were 0.954 and 0.950 respectively, all higher than the single indicator, and the differences were statistically significant ( P<0.05). Conclusions:IL-35 and Klotho protein levels are good biomarkers in diagnosing the patients′ sepsis complicated by AKI as well as the prognosis. The combination of the two index will further improve the prediction effect.
