1.Clinical Application of Harmonizing and Regulating Pivot Method in Pulmonary Fibrosis:Based on the Characteristics of Essence-Attribute-Function

Based on the theory of "shaoyang(少阳) resembling the pivot" and collateral diseases, this article proposes that pulmonary fibrosis (PF) can be divided into three stages including wind bi (痹), constraint bi, and atrophy bi. The core pathogenesis of PF is the obstruction of the pivot and pulmonary collateral obstruction. In terms of treatment, the basic principles are to harmonize and regulate the pivot, and to promote the circulation of the lung collaterals. Depending on the different characteristics of the "essence-attribute-function", treatment methods such as harmonizing and regulating the pivot, resolving phlegm and removing stasis, supplementing deficiency and harmonizing collaterals are suggested. This approach ensures the regulation of the pivot, smooth circulation of qi and blood, unblocking of the lung collaterals and nourishing the lung body, achieving the goals of balancing the ascending and descending of qi, removing phlegm and stasis, and relieving cough and wheezing.
2.Development of a standard for the accreditation of community training teacher in hospice care for general practitioners

Objective:To formulate a community teacher standard for the training of general practitioner program (direction in hospice care).Methods:This was a qualitative study. Twenty-nine experts in general practice and hospice care from various teaching hospitals and community health service centers were invited to participate in two rounds of Delphi consultation from April to August 2022. Based on the results of the consultation, a standard for community faculty training in comprehensive care and end-of-life care for general practitioners was formulated, and the weight coefficients of the indicators were calculated.Results:Of the 29 experts, 11 (37.9%) were male, their age was (52.9±8.9) years, and their working life was (17.0±7.2) years. In the two rounds of expert consultation, the expert positive coefficient was 100.0% (29/29) and 96.6% (28/29), the expert authority coefficient was 0.789 and 0.851, and the expert opinion coordination coefficient W of the importance of all indicators was 0.124 and 0.131, respectively ( χ2=123.01, 131.71, P<0.05). Finally, a set of community-based teacher standard for general practitioner specialty training (direction in hospice care) was established, which consisted of 4 first-level indicators and 30 second-level indicators. The four first-level indicators were basic literacy, clinical qualification and ability, teaching qualification and ability, and scientific research ability, all with the weight coefficients of 0.254, 0.254, 0.252 and 0.240, respectively. Conclusion:The standard of teachers in the community base of general practitioner specialty training (direction in hospice care) established in this study is more scientific, which is conducive to the comprehensive evaluation of teachers in the community training base.
3.Development of accreditation standards for clinical teaching faculty in hospice care training of general practitioners

Objective:To develop accreditation standards for clinical faculty in general practitioner specialized training of hospice care.Methods:This was a qualitative study. Twenty nine experts in fields of general practice and hospice care were invited for two rounds of Delphi consultation from April 2022 to August 2022. Based on the consultation results, the accreditation standards for clinical faculty in general practitioner training bases of hospice care specialty was preliminarily developed.Results:The mean age of the consulting experts was (52.9±8.9) years with a mean working year of (17.0±7.2), and 69.0% (20/29) of them held senior professional titles. In the two rounds of consultation, the expert positive coefficient was 100.0% (29/29) and 96.6% (28/29), the expert authority coefficient was 0.765 and 0.813, and the expert opinion coordination coefficient W for the importance of all indicators was 0.112 and 0.196, respectively. The expert opinions tended to be consistent. Finally, a set of clinical base faculty standards for general specialty training was developed, which consisted of 4 first-level indicators and 24 second-level indicators. The 4 first-level indicators were basic literacy, clinical qualification and ability, teaching qualification and ability, scientific research ability, with the weight coefficients of 0.253, 0.255, 0.254 and 0.241, respectively. Conclusion:The standards for clinical teaching faculty in general practitioner specialty training base of hospice care has been established in this study, which is conducive to the comprehensive quality evaluation for training faculty.
4.Study on the construction of admission evaluation indicators for terminal patients in community hospice wards

Objective:To construct a set of admission evaluation indicators for terminal patients in community hospice wards.Methods:This qualitative study employed a mixed-methods approach. From January to June 2023, 10 physicians working in community hospice wards in Shanghai participated in one-on-one, semi-structured, in-depth interviews. Based on the interview findings and literature review, a preliminary set of admission evaluation indicators was drafted. Subsequently, from July to December 2023, 18 national experts in hospice/palliative care were selected for a two-round Delphi expert consultation to refine the indicators. The final indicator system was established based on the consultation results, and the weight coefficients for each indicator were determined.Results:Sixteen experts completed both rounds of consultation. The experts had a mean age of (52.0±8.3) years and a mean working experience of (14.4±6.8) years. The response rates for the two rounds were 88.9% and 100.0%, respectively. The authority coefficients were 0.875 and 0.894, and the Kendall′s W coordination coefficients were 0.338 (χ2=471.737, P<0.001) and 0.349 (χ2=398.230, P<0.001), respectively. After two rounds of Delphi consultation, a final admission evaluation indicator system was established, comprising 4 first-level indicators and 63 second-level indicators. The first-level indicators and their weight coefficients were: Underlying Disease (0.256 7), Survival Prognosis (0.256 7), Holistic Needs (0.256 6), and Social Environment (0.240 0). Conclusion:The admission evaluation indicator system for terminal patients in community hospice wards developed in this study facilitates the standardized development of community hospice/palliative care services and contributes to providing high-quality care for patients and their families.
5.Current status and outlooks of acupuncture research driven by machine learning.

The machine learning is used increasingly and widely in acupuncture prescription optimization, intelligent treatment and precision medicine, and has obtained a certain achievement. But, there are still some problems remained to be solved such as the poor interpretability of the model, the inconsistency of data quality of acupuncture research, and the clinical application of constructed models. Researches in future should focus on the acquisition of high-quality clinical and experimental data sets, take various machine learning algorithms as the basis, and construct professional models to solve various problems, so as to drive the high-quality development of acupuncture research.

6.Ruibin Agent versus mainstream large language models: A comparative study on medical literature comprehension with esophageal cancer as a case study

Objective To explore the application value of artificial intelligence in medical research assistance, and analyze the key paths to achieve precise execution of model instructions, improvement of model interpretation completeness, and control of hallucinations. Methods Taking esophageal cancer research as the scenario, five types of literature including research articles, case reports, reviews, editorials, and guidelines were selected for model interpretation tests. The model performance was systematically evaluated from five dimensions: recognition accuracy, format accuracy, instruction execution accuracy, content reliability rate, and content completeness index. The performance differences of Ruibin Agent, GPT-4o, Claude 3.7 Sonnet, DeepSeek V3, and DouBao-pro models in medical literature interpretation tasks were compared. Results A total of 15 studies were included, with 3 studies of each type. The five models collectively conducted 1 875 tests. Due to the poor recognition accuracy of the editorial type, the overall recognition accuracy of Ruibin Agent was significantly lower than other models (92.0% vs. 100.0%, P<0.001). In terms of format accuracy, Ruibin Agent was significantly better than Claude 3.7 Sonnet (98.7% vs. 92.0%, P=0.002) and GPT-4o (98.7% vs. 78.9%, P<0.001). In terms of instruction execution accuracy, Ruibin Agent was better than GPT-4o (97.3% vs. 80.0%, P<0.001). In terms of content reliability rate, Ruibin Agent was significantly lower than Claude 3.7 Sonnet (84.0% vs. 92.0%, P=0.010) and DeepSeek V3 (84.0% vs. 94.7%, P<0.001). In terms of content completeness index, the median scores of Ruibin Agent, GPT-4o, Claude 3.7 Sonnet, DeepSeek V3, and DouBao-pro were 0.71, 0.60, 0.85, 0.74, and 0.77, respectively. Conclusion Ruibin Agent has significant advantages in terms of formatted interpretation of medical literature and instruction execution accuracy. In the future, it is necessary to focus on optimizing the recognition ability of editorial types, strengthening the coverage ability of core elements of various types of literature to improve interpretation completeness, and improving content reliability through optimizing the confidence mechanism to ensure the rigor of medical literature interpretation.
7.Cognitive Disorders Awareness and Associated Risk Factors in Xizang Autonomous Region

To investigate the awareness of cognitive impairment disorders among residents of the Xizang Autonomous Region and its influencing factors, thereby providing a basis for targeted prevention and treatment efforts. From April to December 2024, a questionnaire survey was conducted among permanent residents aged ≥18 years (residing in the Xizang Autonomous Region for 180 days or more). The survey was primarily conducted online, supplemented by QR code distribution during community medical outreach by healthcare workers. Demographic information and data on awareness of cognitive disorders were collected, and an ordered Logistic regression model was used to analyze influencing factors in the overall population and stratified by occupation. A total of 327 questionnaires were collected, with 14 excluded (13 for not meeting residency requirements and 1 for self-reported diagnosis of cognitive impairment), leaving 313 valid questionnaires. The average age of respondents was 42.0±11.9 years; 108 (34.5%) were male, and 205 (65.5%) were female. Most respondents were from Lhasa (78.6%, 246/313); 179 (57.2%) were healthcare workers, and 134 (42.8%) were non-healthcare workers. Regarding awareness of cognitive impairment disorders, 7.3% (23/313) were "unaware", 75.7% (237/313) were "partially aware", and 16.9% (53/313) were "well aware".Ordered Logistic regression analysis revealed that education level of high school or below ( Awareness of cognitive impairment disorders among residents of the Xizang Autonomous Region needs improvement. Educational level, occupation, and prior contact with cognitive impairment patients significantly influence disease awareness. Enhancing overall education levels and using vivid clinical case presentations in health education and public outreach are key strategies to improve public awareness of cognitive impairment disorders.
8.Recognition advantage of metaphorical micro-counseling dialogue texts and the explanatory role of insight

Objective:To investigate the recognition advantage of metaphorical micro-counseling dialogue texts in mental health contexts,and to explore a potential explanatory role of insight.Methods:A total of 104 col-lege students,recruited through a network announcement,were randomly assigned to three groups.In the learning phase,participants read negative cognitions paired with metaphorical or literal solutions and rated the insightfulness of each solution.In the recognition phase,previously unseen solutions were intermixed,and participants judged whether each item had been presented before and rated their confidence.Recognition was assessed after 15 minutes(n=37),1 day(n=33),or 7 days(n=34).Reaction time was recorded,and recognition sensitivity and criterion was calculated using signal detection theory.Results:At three intervals,the scores of insightfulness,recognition sen-sitivity,recognition criterion,and recognition confidence of metaphorical solutions were both higher than those of literal solutions(Ps<0.001),and the recognition reaction time of metaphorical solutions were all shorter than those of literal solutions(P<0.001).The insightfulness scores of the solutions were positively correlated with the scores of recognition sensitivity,recognition criterion,recognition confidence,and recognition reaction time(β=0.67,0.13,0.18,-0.39).Conclusion:This study suggests that the use of metaphors in mental health contexts may en-hance long-term memory,with insight playing a significant mediating role in this process.
9.Temporal dynamics of strategy monitoring in individuals with varying degrees of media multitasking

Objective:To investigate the temporal dynamics of strategy monitoring during prospective memory testin individuals with varying degrees of media multitasking.Methods:Twenty heavy media multitaskers and twen-ty light media multitaskers were identified through the media use questionnaire.Using a dual-task paradigm and e-vent-related potential technique,the two groups' accuracy,response time,and the amplitudes of frontal positive components reflecting strategy monitoring were compared across the early,middle,and late stages of the prospective memory test.Results:Heavy media multitaskers exhibited significantly faster response times compared to light media multitaskers(P<0.001),with no statistical difference in accuracy(P>0.05).Moreover,the amplitude of frontal positive components in heavy media multitaskers was greater than that in light media multitaskers(P<0.05)across all three stages,and the amplitude increased gradually with the passing of time in heavy media multi-taskers(P<0.05).Conclusion:Heavy media multitaskers tend to allocate more cognitive resources to strategic mo-nitoring over time,while light media multitaskers maintain a relatively consistent level of strategic monitoring.
10.Translation of the Quality of Life Tool for Patients with Aplastic Anaemia and the test of its reliability and validity

Objective:To translate the quality of life tool for patients with aplastic anaemia and paroxysmal nocturnal haemoglobinuria (QLQ-AA/PNH) into Chinese, and to test its reliability and validity.Methods:According to the scale translation principle, the Chinese version of QLQ-AA/PNH was formed through translation, back translation and cross-cultural adaptation. A cross-sectional survey method was used to conveniently select 58 patients with aplastic anemia who were treated in the hematology department of the Second Affiliated Hospital of Dalian Medical University from January 2018 to September 2023 for investigation, and to evaluate the reliability and validity of the scale.Results:The Chinese version of QLQ-AA/PNH retains 36 items, and 5 common factors (psychological status dimension, life burden dimension, physical condition dimension, illness anxiety dimension and other symptom dimension) were extracted through exploratory factor analysis. The cumulative variance contribution rate reached 71.33%, and the factor load of each entry was greater than 0.5 on corresponding common factors. The Cronbach α coefficient of the scale as a whole was 0.971, the broken half reliability coefficient was 0.985, the Cronbach α coefficient of each common factor was 0.637 to 0.954, and the broken half reliability coefficient was 0.637 to 0.930. Conclusions:The Chinese version of QLQ-AA/PNH has been proved to be valid and reliable. It is a valuable tool for evaluating the quality of life among patients with aplastic anaemia.
