1.The relationship between SII,RDW and 25(OH)D levels and frailty index in elderly patients with type 2 diabetes mellitus

Objective To explore the relationship between systemic immune-inflammation index(SII),red blood cell distribution width(RDW),25-hydroxy-vitamin-D[25(OH)D]levels and frailty index in elderly pa-tients with type 2 diabetes mellitus(T2DM).Methods A total of 197 elderly patients with T2DM admitted to the hospital from March 2023 to March 2024 were collected as the research subjects.The patients were divided into the frailty group(106 cases)and the non-frailty group(91 cases)according to the scores of the clinical frailty scale.The clinical data and the levels of SII,RDW and 25(OH)D of the two groups were compared.Pearson correlation analysis was used to analyze the correlations between the levels of SII,RDW and 25(OH)D and the frailty index of elderly patients with T2DM.Logistic regression was used to analyze the influencing factors of frailty in elderly patients with T2DM.Results Compared with the non-frailty group,the proportion of women,the history of falls within 1 year,and the age of the frailty group increased,while the body mass in-dex and the proportion of men decreased,and the differences were statistically significant(P<0.05).The SII and RDW levels in the non-frailty group were lower than those in the frailty group,and the 25(OH)D level was higher than that in the frailty group,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that SII and RDW levels were positively correlated with frailty index,and 25(OH)D level was negatively correlated with frailty index in elderly T2DM patients(P<0.05).Logistic regression analysis showed that female,age ≥ 74.25 years old,SII≥ 938.36,RDW≥ 15.19%,and 25(OH)D≥48.42 nmol/L were independent risk factors for frailty in elderly T2DM patients(P<0.05).Conclusion The levels of SII,RDW and 25(OH)D in elderly patients with T2DM are related to the frailty index.
2.Ultrasound characteristics analysis of thyroid nodules in 306 naval flying personnel

Objective:To provide references for the health management of thyroid nodules in naval flying personnel by analyzing the ultrasound characteristics of naval flying personnel with thyroid nodules.Methods:A total of 306 naval flying personnel, who were diagnosed as thyroid nodules in yearly health examination in the Third Sanatorium of Air Force Healthcare Center for Special Services Hangzhou from January of 2019 to December of 2021, were selected as the research subjects. They were divided into 3 groups by age (20-29 years, 30-39 years, ≥40 years), 2 groups by flying hours (<3 000 h, ≥3 000 h) and 3 groups by aircraft types (helicopter, transporter, fighter). The characteristics of thyroid nodule ultrasound images were retrospectively analyzed and the size, quantity and classification which defined in Chinese-thyroid imaging reporting and data system (C-TIRADS) of thyroid nodule were compared among the groups of different ages, flying hours, and aircraft types.Results:Among 306 naval flying personnel with thyroid noedules, 280 cases (91.50%) were small nodules (diameter <1 cm), 26 cases (8.50%) were large nodules (diameter ≥1 cm). One hundred and fifty-one cases (49.35%) were single nodules and 155 cases (50.65%) were multiple nodules. There were 135 cases (44.12%) with C-TIRADS grade 2 thyroid nodules, 158 cases (51.63%) with C-TIRADS grade 3 nodules, and 13 cases (4.25%) with C-TIRADS grade 4 nodules. There were significant differences in the size, number, classification of composition ratio of thyroid nodules among different age groups ( χ2=8.86, 5.94, 46.20, P=0.013, 0.051,<0.001). There were significant differences in the size and classification of composition ratio of thyroid nodules between different flying hours groups ( χ2=16.91, 24.35, both P<0.001). The proportion of C-TIRADS grade 4 nodules in the flying hours ≥3 000 h group was higher than that in the <3 000 h group, with a significant difference ( χ2=4.77, P=0.029).There were no significant differences in the size, number, classification of composition ratio of thyroid nodules among different aircraft types groups (all P>0.05). Conclusions:Age and flying hours have effects on size and classification of composition ratio of thyroid nodules in flying personnel, and age also has the effect on quantity. A scientific and systematic follow-up mechanism would be helpful to assess the risk of thyroid cancer and give early warning for the timely intervention.
3.Prognostic value of serum levels of human cartilage glycoprotein 39 and miR-499-5p in elderly patients with acute myocardial infarction after percutaneous coronary intervention

Objective:To investigate the prognostic value of serum levels of human cartilage glycoprotein 39 (HC-gp39) and microRNA (miR) -499-5p in elderly patients with acute myocardial infarction (AMI) after percutaneous coronary intervention (PCI) .Methods:A total of 175 elderly AMI patients who underwent PCI treated in Qingdao Central Hospital from February 2020 to May 2022 were selected as the observation group, and 54 healthy patients who underwent physical examination in the hospital during the same period were selected as the control group. Serum HC-gp39 and miR-499-5p levels and cardiac function parameters [left ventricular ejection fraction (LVEF) and left ventricular end-diastolic diameter (LVEDD)] of the observation group and control group were compared, and serum HC-gp39 and miR-499-5p levels of patients in the observation group with different cardiac function grades were compared. The correlation between serum HC-gp39 and miR-499-5p levels and cardiac function parameters was analyzed. After 1-year follow-up, elderly AMI patients were divided into occurrence group and non-occurrence group according to whether major adverse cardiovascular events (MACE) occurred after surgery. Clinical data of occurrence group and non-occurrence group were compared to analyze the influencing factors of postoperative MACE in elderly AMI patients. The predictive value of serum HC-gp39 and miR-499-5p levels and their combination on postoperative MACE in elderly patients with AMI was analyzed.Results:The serum levels of HC-gp39, miR-499-5p and LVEDD in the observation group were higher than those in the control group, and LVEF was lower than that in the control group (all P<0.05). The serum levels of HC-gp39 and miR-499-5p in patients with cardiac function classification Ⅲ were higher than those in patients with cardiac function classification Ⅰ and Ⅱ (all P<0.05), and the serum levels of HC-gp39 and miR-499-5p in patients with cardiac function classification Ⅱ were higher than those in patients with cardiac function classification Ⅰ (all P<0.05). Serum HC-gp39 and miR-499-5p levels were positively correlated with LVEDD ( r=0.417, 0.532, all P<0.05), and negatively correlated with LVEF ( r=-0.567, -0.485, all P<0.05). After 1 year of follow-up, 4 cases were lost to follow-up, the follow-up rate was 97.71%(171/175), and 42 cases had MACE. Gensini score, proportion of unrevascularated blood circulation, troponin T, multi-branch ratio of lesion index, serum HC-gp39 and miR-499-5p levels in the occurrence group were higher than those in the non-occurrence group, and hemoglobin and lymphocyte counts were lower than those in the non-occurrence group, with statistical significance (all P<0.05). The results of multifactor stepwise regression analysis showed that troponin T ( OR=2.818, 95% CI: 1.238-6.404), Gensini score ( OR=3.554, 95% CI: 1.562-8.084), serum HC-gp39 ( OR=3.062, 95% CI: 1.346-6.965) and serum miR-499-5p ( OR=4.003, 95% CI: 1.760-9.160) were the influential factors for postoperative MACE in elderly patients with AMI (all P<0.05). The sensitivity of serum HC-gp39, miR-499-5p and their combined prediction of MACE after PCI in elderly AMI patients were 83.33%, 90.48% and 90.48%, respectively, and the specificity was 80.62%, 75.19% and 93.02%, respectively. The area under the curve (AUC) was 0.825, 0.816, 0.901 (all P<0.05), and the AUC of the combination of the two was higher ( P<0.05). Conclusions:Serum HC-gp39 and miR-499-5p levels can be used to predict the prognosis of elderly AMI patients after PCI, and the combination of the two has higher predictive value.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
5.En1 promotes cell proliferation and migration via Hedgehog signaling pathway in esophageal squamous cell carcinoma

Objective:To explore the function and mechanism of transcription factor En1 in esophageal squamous cell carcinoma (ESCC).Methods:The correlations of En1 with prognosis were analyzed using the overall survival data of 9 397 pan-cancer patients and progression-free survival data of 4 349 pan-cancer patients from The Cancer Genome Atlas (TCGA) database. The En1 expression data in 53 and 155 cases of ESCC and their paired adjacent tissues were from Gene Expression Omnibus (GEO) database and National Genomics Data Center-Genome Sequence Archive(NGDC-GSA)database. Lentivirus was used to generate En1 stable knockout cell lines KYSE180 and KYSE450. The proliferation ability of the cells was detected by cell counting kit 8 and clone formation assay. The migration ability of the cells was detected by Transwell assay. The effect of En1 on the proliferation of ESCC was detected by xenograft experiment in BALB/c-nu/nu mice. Real-time fluorescence quantitative polymerase chain reaction (RT-qPCR) was used to detect the expressions of En1, glioma-associated oncogene family zinc finger 1 (GLI1), glioma-associated oncogene family zinc finger 2 (GLI2) and smoothened (SMO).Results:Pan-cancer data from TCGA showed that patients with low En1 expression had longer overall survival and progression-free survival than patients with high En1 expression ( P< 0.001). Data from GEO and GSA databases also showed a high expression level of En1 in ESCC tissues compared with paired tissues ( P<0.001). Proliferation was inhibited after knockout of En1 in KYSE180 and KYSE450 cells ( P<0.001). The colony formation numbers decreased. The colony formation numbers of KYSE180 cells in the shEn1#1 group and the shEn1#2 group were 138.33±23.07 and 127.00±19.70, respectively, significantly lower than that of the shNC group 340.67±12.06 ( P<0.001). The colony formation numbers of KYSE450 cells in the shEn1#1 group and the shEn1#2 group were 65.33±2.52 and 9.00±3.00, respectively, significantly lower than that of the shNC group 139.00±13.00 ( P<0.001). The migration numbers was inhibited after knockout of En1 [the Transwell numbers of KYSE180 cells in the shEn1#1 group and the shEn1#2 group were 66.67±12.66 and 71.33±11.02, respectively, significantly lower than that of the shNC group 334.67±16.56 ( P<0.001). The Transwell numbers of KYSE450 cells in the shEn1#1 group and the shEn1#2 group were 112.33±14.57 and 54.33±5.51, respectively, significantly lower than that of the shNC group 253.33±21.03 ( P<0.001)]. Xenograft model showed a slower growth rate of shEn1#1 and shEn1#2 cell lines ( P<0.001). The tumor weights of KYSE450 cells in the shEn1#1 group and the shEn1#2 group were (0.046±0.026)g and (0.047±0.025)g, respectively, significantly lower than that of the shNC group (0.130±0.038)g ( P<0.001). After knockdown of En1, the relative expression levels of GLI1 in KYSE180 cells of the shEn1#1 group and the shEn1#2 group were 0.326±0.162 and 0.322±0.133, and the relative expression levels of GLI1 in KYSE450 cells of the shEn1#1 and shEn1#2 groups were 0.131±0.006 and 0.352±0.050, respectively, which were all lower than that in the shNC group ( P<0.01). After knockdown of En1, overexpression of GLI1 attenuated the inhibitory effect of knockdown of En1 on cell proliferation ( P<0.001), colony formation[the colony formation numbers of the shEn1#1-GLI1 group were 151.00±9.54, higher than 102.33±10.02 ( P=0.004) of the shEn1#1-vector group] and migration [the migration numbers of the shEn1#1-GLI1 group were 193.67±10.07, higher than 109.33±11.50 ( P<0.001) in the shEn1#1-vector group]. In clinical samples of ESCC, major regulatory factors of the Hedgehog pathway were up-regulated and the pathway was activated. Conclusion:En1 promotes the proliferation and migration of ESCC cells by regulating the Hedgehog pathway and can be used as a new potential target for targeted therapy of ESCC.

7.Predictive Value of Serum C18∶1-Cer and LPC18∶0 Levels in Early Pregnancy Women for Gestational Diabetes Mellitus

Objective To explore the value of serum stearoyl sphingosine(C18∶1-Cer)and 1-stearoyl-sn-glycero-3-phospho-choline(LPC 18∶0)levels in pregnant women's serum samples during pregnancy in predicting gestational diabetes mellitus(GDM).Methods The clinical data and laboratory indicators of 126 pregnant women were retrospectively analyzed.The sub-jects were divided into GDM group(n=66)and control group(n=60)according to the GDM diagnosis results.Mass spec-trometry was used to detect the serum C18∶1-Cer and LPC18∶0 levels of the subjects in early and mid pregnancy.Logistic re-gression analysis was used to screen out the risk factors for GDM.Receiver operating characteristic(ROC)curve was used to evaluate the predictive value of C18∶1-Cer,LPC18∶0 and their combination for GDM.Results Compared with the control group,the serum C18∶1-Cer and LPC18∶0 levels of the subjects in the GDM group were significantly increased in early(18.92±2.77ng/ml vs 23.47±4.18ng/ml,41.32±17.55ng/ml vs 88.08±16.02ng/ml)and mid pregnancy(23.14±4.10ng/ml vs 18.76±4.05ng/ml,84.60±14.53ng/ml vs 40.50±17.79ng/ml),and the differences were statistically significant(t=7.127,15.637;-5.984,2.174,all P<0.05)C18∶1-Cer was positively correlated with fasting plasma glucose(FPG),fasting plasma insulin(FPI),homeostasis model assessment of insulin resistance(HOMA-IR),glycated hemoglobin(HbA1c)and triglyceride(TG)(r=0.458,0.209,0.317,0.223,0.219,all P<0.05).LPC18.0 was positively correlated with FPG,FPI,HOMA-IR,HbA1c,total cholesterol(TC)and TG(r= 0.715,0.426,0.580,0.465,0.232,0.372,all P<0.05).Logistic regression analysis results showed that C18∶1-Cer[OR(95%CI):1.522(1.136~2.039),P<0.05]and LPC18:0[OR(95%CI):1.198(1.102~1.302),P<0.001]were independent risk factors for GDM.ROC curve analysis results showed that the area under the curve(AUC)of serum C18∶1-Cer,LPC18∶0 and the combination of the two indicators were 0.819,0.971 and 0.986,respectively.The predictive performance of the combination of the two indicators was better than that of the single detection.Conclusion Serum C18∶1-Cer and LPC18∶0 in early pregnancy were closely related to the occurrence of GDM.C18∶1-Cer combined with LPC 18∶0 has a certain predictive value for the early diagnosis of GDM.

The migration numbers was inhibited after knockout of En1 [the Transwell numbers of KYSE180 cells in the shEn1#1 group and the shEn1#2 group were 66.67±12.66 and 71.33±11.02, respectively, significantly lower than that of the shNC group 334.67±16.56 ( P<0.001). The Transwell numbers of KYSE450 cells in the shEn1#1 group and the shEn1#2 group were 112.33±14.57 and 54.33±5.51, respectively, significantly lower than that of the shNC group 253.33±21.03 ( P<0.001)]. Xenograft model showed a slower growth rate of shEn1#1 and shEn1#2 cell lines ( P<0.001). The tumor weights of KYSE450 cells in the shEn1#1 group and the shEn1#2 group were (0.046±0.026)g and (0.047±0.025)g, respectively, significantly lower than that of the shNC group (0.130±0.038)g ( P<0.001). After knockdown of En1, the relative expression levels of GLI1 in KYSE180 cells of the shEn1#1 group and the shEn1#2 group were 0.326±0.162 and 0.322±0.133, and the relative expression levels of GLI1 in KYSE450 cells of the shEn1#1 and shEn1#2 groups were 0.131±0.006 and 0.352±0.050, respectively, which were all lower than that in the shNC group ( P<0.01). After knockdown of En1, overexpression of GLI1 attenuated the inhibitory effect of knockdown of En1 on cell proliferation ( P<0.001), colony formation[the colony formation numbers of the shEn1#1-GLI1 group were 151.00±9.54, higher than 102.33±10.02 ( P=0.004) of the shEn1#1-vector group] and migration [the migration numbers of the shEn1#1-GLI1 group were 193.67±10.07, higher than 109.33±11.50 ( P<0.001) in the shEn1#1-vector group]. In clinical samples of ESCC, major regulatory factors of the Hedgehog pathway were up-regulated and the pathway was activated. Conclusion:En1 promotes the proliferation and migration of ESCC cells by regulating the Hedgehog pathway and can be used as a new potential target for targeted therapy of ESCC.
9.Ultrasound characteristics analysis of thyroid nodules in 306 naval flying personnel

Objective:To provide references for the health management of thyroid nodules in naval flying personnel by analyzing the ultrasound characteristics of naval flying personnel with thyroid nodules.Methods:A total of 306 naval flying personnel, who were diagnosed as thyroid nodules in yearly health examination in the Third Sanatorium of Air Force Healthcare Center for Special Services Hangzhou from January of 2019 to December of 2021, were selected as the research subjects. They were divided into 3 groups by age (20-29 years, 30-39 years, ≥40 years), 2 groups by flying hours (<3 000 h, ≥3 000 h) and 3 groups by aircraft types (helicopter, transporter, fighter). The characteristics of thyroid nodule ultrasound images were retrospectively analyzed and the size, quantity and classification which defined in Chinese-thyroid imaging reporting and data system (C-TIRADS) of thyroid nodule were compared among the groups of different ages, flying hours, and aircraft types.Results:Among 306 naval flying personnel with thyroid noedules, 280 cases (91.50%) were small nodules (diameter <1 cm), 26 cases (8.50%) were large nodules (diameter ≥1 cm). One hundred and fifty-one cases (49.35%) were single nodules and 155 cases (50.65%) were multiple nodules. There were 135 cases (44.12%) with C-TIRADS grade 2 thyroid nodules, 158 cases (51.63%) with C-TIRADS grade 3 nodules, and 13 cases (4.25%) with C-TIRADS grade 4 nodules. There were significant differences in the size, number, classification of composition ratio of thyroid nodules among different age groups ( χ2=8.86, 5.94, 46.20, P=0.013, 0.051,<0.001). There were significant differences in the size and classification of composition ratio of thyroid nodules between different flying hours groups ( χ2=16.91, 24.35, both P<0.001). The proportion of C-TIRADS grade 4 nodules in the flying hours ≥3 000 h group was higher than that in the <3 000 h group, with a significant difference ( χ2=4.77, P=0.029).There were no significant differences in the size, number, classification of composition ratio of thyroid nodules among different aircraft types groups (all P>0.05). Conclusions:Age and flying hours have effects on size and classification of composition ratio of thyroid nodules in flying personnel, and age also has the effect on quantity. A scientific and systematic follow-up mechanism would be helpful to assess the risk of thyroid cancer and give early warning for the timely intervention.
10.Prognostic value of serum levels of human cartilage glycoprotein 39 and miR-499-5p in elderly patients with acute myocardial infarction after percutaneous coronary intervention

Objective:To investigate the prognostic value of serum levels of human cartilage glycoprotein 39 (HC-gp39) and microRNA (miR) -499-5p in elderly patients with acute myocardial infarction (AMI) after percutaneous coronary intervention (PCI) .Methods:A total of 175 elderly AMI patients who underwent PCI treated in Qingdao Central Hospital from February 2020 to May 2022 were selected as the observation group, and 54 healthy patients who underwent physical examination in the hospital during the same period were selected as the control group. Serum HC-gp39 and miR-499-5p levels and cardiac function parameters [left ventricular ejection fraction (LVEF) and left ventricular end-diastolic diameter (LVEDD)] of the observation group and control group were compared, and serum HC-gp39 and miR-499-5p levels of patients in the observation group with different cardiac function grades were compared. The correlation between serum HC-gp39 and miR-499-5p levels and cardiac function parameters was analyzed. After 1-year follow-up, elderly AMI patients were divided into occurrence group and non-occurrence group according to whether major adverse cardiovascular events (MACE) occurred after surgery. Clinical data of occurrence group and non-occurrence group were compared to analyze the influencing factors of postoperative MACE in elderly AMI patients. The predictive value of serum HC-gp39 and miR-499-5p levels and their combination on postoperative MACE in elderly patients with AMI was analyzed.Results:The serum levels of HC-gp39, miR-499-5p and LVEDD in the observation group were higher than those in the control group, and LVEF was lower than that in the control group (all P<0.05). The serum levels of HC-gp39 and miR-499-5p in patients with cardiac function classification Ⅲ were higher than those in patients with cardiac function classification Ⅰ and Ⅱ (all P<0.05), and the serum levels of HC-gp39 and miR-499-5p in patients with cardiac function classification Ⅱ were higher than those in patients with cardiac function classification Ⅰ (all P<0.05). Serum HC-gp39 and miR-499-5p levels were positively correlated with LVEDD ( r=0.417, 0.532, all P<0.05), and negatively correlated with LVEF ( r=-0.567, -0.485, all P<0.05). After 1 year of follow-up, 4 cases were lost to follow-up, the follow-up rate was 97.71%(171/175), and 42 cases had MACE. Gensini score, proportion of unrevascularated blood circulation, troponin T, multi-branch ratio of lesion index, serum HC-gp39 and miR-499-5p levels in the occurrence group were higher than those in the non-occurrence group, and hemoglobin and lymphocyte counts were lower than those in the non-occurrence group, with statistical significance (all P<0.05). The results of multifactor stepwise regression analysis showed that troponin T ( OR=2.818, 95% CI: 1.238-6.404), Gensini score ( OR=3.554, 95% CI: 1.562-8.084), serum HC-gp39 ( OR=3.062, 95% CI: 1.346-6.965) and serum miR-499-5p ( OR=4.003, 95% CI: 1.760-9.160) were the influential factors for postoperative MACE in elderly patients with AMI (all P<0.05). The sensitivity of serum HC-gp39, miR-499-5p and their combined prediction of MACE after PCI in elderly AMI patients were 83.33%, 90.48% and 90.48%, respectively, and the specificity was 80.62%, 75.19% and 93.02%, respectively. The area under the curve (AUC) was 0.825, 0.816, 0.901 (all P<0.05), and the AUC of the combination of the two was higher ( P<0.05). Conclusions:Serum HC-gp39 and miR-499-5p levels can be used to predict the prognosis of elderly AMI patients after PCI, and the combination of the two has higher predictive value.
