1.Nuangong Tiaojing Decoction (暖宫调经汤) Combined with Ginger Moxibustion at Baliao (八髎) Acupoints for Patients with Premature Ovarian Failure of Yang-Deficiency Uterus-Coldeness Syndrome:65 Cases Clinical Observation

ObjectiveTo explore the clinical effectivess of Nuangong Tiaojing Decoction (暖宫调经汤) combined with ginger moxibustion at Baliao acupoints (Eight Bone-Holes) for treating premature ovarian failure (POF) of yang-deficiency uterus-coldeness syndrome, based on conventional western medical treatment. MethodsA total of 130 patients diagnosed with POF of yang-deficiency uterus-coldeness syndrome were divided into a treatment group and a control group, with 65 cases in each group, based on patient preference. The control group received conventional western medicine treatment, while the treatment group was additionally given Nuangong Tiaojing Decoction orally one dose per day and underwent ginger moxibustion at Baliao points once daily. Both groups were treated for three months. Before and after treatment, ovarian function indicators, including ovarian volume, follicle count, ovarian peak systolic velocity (PSV), and endometrial thickness were assessed, and serum sex hormone levels including progesterone (P), estradiol (E2), follicle-stimulating hormone (FSH), luteinizing hormone (LH), prolactin (PRL), and testosterone (T), as well as anti-Müllerian hormone (AMH) levels were also measured. Additionally, traditional Chinese medicine (TCM) syndrome scores were evaluated, covering symptoms such as reduced libido, lumbosacral pain, fear of cold with cold extremities, fatigue, dull complexion, lower abdominal distension and pain, pale tongue, and deep and rough pulse. After treatment, clinical effectiveness and safety were determined. ResultsCompared with the pre-treatment levels within group, both groups showed significant increases in ovarian volume, follicle count, PSV, endometrial thickness, and levels of P, E2, and AMH after treatment, while the levels of FSH, LH, PRL, and T, as well as scores of lumbosacral pain, reduced libido, dull complexion, deep and rough pulse, and total TCM syndrome scores significantly reduced after treatment. Furthermore, the treatment group exhibited higher ovarian volume, follicle count, PSV, endometrial thickness, and levels of P, E2, and AMH compared to the control group, while FSH, LH, PRL, T levels, and all symptom scores, as well as total TCM syndrome scores, were significantly lower in the treatment group than in the control group (P<0.05 or P<0.01). The clinical effectiveness in treatment group was 96.92% (63/65), significantly higher than 81.54% (53/65) in control group (P<0.05). The incidence of adverse reactions in the treatment group was 26.15% (17/65), while in the control group it was 32.31% (21/65), with no statistically significant difference (P>0.05). ConclusionOn the basis of conventional western medicine treament, Nuangong Tiaojing Decoction combined with ginger moxibustion at Baliao acupoints for patients with POF of yang-deficiency uterus-coldeness syndrome can significantly improve ovarian function and clinical symptoms, regulate hormone levels, and thereby enhance clinical effectiveness.
2.Effect of Yifei Huatan Decoction on Relieving Airway Hyperviscosity in Asthmatic Rats with Spleen Deficiency Syndrome and Its Mechanism

ObjectiveTo explore the effect of Yifei Huatan decoction on relieving airway hyperviscosity in asthmatic rats with spleen deficiency syndrome and its mechanism. MethodFifty-five SPF level SD rats at 8-9 week of age were used to induce asthma with spleen deficiency syndrome by animal modeling of traditional Chinese medicine combined with asthma of western medicine. After successful modeling, the rats were divided into model group, dexamethasone group, low, medium, and high-dose Yifei Huatan decoction groups by random number table method, and 11 clean SD rats at 8-9 week of age were recorded as a normal group. Rats in the dexamethasone group were given 0.087 5 mg kg-1 dexamethasone acetate by gavage. Rats in the low, medium, and high-dose Yifei Huatan decoction groups were given 0.8, 1.6, 3.2 g kg-1 Yifei Huatan decoction liquid extract by gavage, respectively. Rats in the model group and the normal group were given 10 mL kg-1 distilled water. The medicine were given once per day for 8 w, and the general situation of each group was observed. The levels of interleukin-4 (IL-4), IL-13, and interferon-γ (IFN-γ) in bronchoalveolar lavage fluid (BALF) were determined by enzyme-linked immunosorbent assay (ELISA). The pathological changes in lung tissues of rats were observed by hematoxylin-eosin (HE) staining. Alcian blue-periodic acid Schiff (AB-PAS) staining was used to detect the hyperplasia of airway goblet cells and mucus secretion in rats. The mRNA expressions of transforming growth factor-β1 (TGF-β1), Smad2, Smad3, mucin 5AC (MUC5AC), and mucin 5B (MUC5B) in the lung tissues of rats were detected by real-time quantitative polymerase chain reaction (Real-time PCR). The protein expressions of TGF-β1, Smad2, Smad3, MUC5AC, and MUC5B in the lung tissues of rats were detected by Western blot. ResultAs compared with the normal group, rats in the model group showed the symptoms of spleen deficiency syndrome, such as decreased body weight, muscle emaciation, decreased food intake, increased water intake, increased anal temperature, tiredness, and decreased swimming endurance, accompanied by dyspnea symptoms such as wheezing and nodding. As compared with the normal group, IL-4 and IL-13 levels in the BALF of the model group were significantly increased (P<0.01), while the IFN-γ level was significantly decreased (P<0.01). In the model group, a large number of inflammatory cells were observed in the mucosa and submucosa of the airway, and the smooth muscle of the trachea was significantly thickened. The hyperplasia, deformation, and exfoliation of various epithelial cells were observed in the mucosa, and the pathological scores of lung tissue increased significantly (P<0.01) in the model group. A large number of goblet cells were observed in the airway with the formation of plenty of mucous thrombus in the model group, and the positive relative staining area of airway, and mRNA and protein expressions of TGF-β1, Smad2, Smad3, MUC5AC, and MUC5B were significantly increased (P<0.01). As compared with the model group, IL-4 and IL-13 levels in BALF of the dexamethasone group and the Yifei Huatan decoction groups decreased, while the IFN-γ level increased. The inflammatory cell infiltration in airway mucosa and submucosa, the thickening of tracheal smooth muscle, the hyperplasia, deformation, and exfoliation of epithelial cells in mucosa were gradually decreased, and the pathological scores of lung tissues decreased significantly (P<0.01) in the dexamethasone group and the Yifei Huatan decoction groups. Goblet cell proliferation gradually decreased, and the positive relative staining area of airway, and mRNA and protein relative expressions of TGF-β1, Smad2, Smad3, MUC5AC, and MUC5B decreased with statistically significant difference (P<0.05, P<0.01). There was no significant difference in the above indexes in the dexamethasone group and the Yifei Huatan decoction low-dose group. The above indexes were dose-dependent in the low, medium, and high-dose Yifei Huatan decoction groups. ConclusionYifei Huatan decoction reduces airway hyperviscosity in asthmatic rats with spleen deficiency syndrome, which may be related to the inhibition of TGF-β1, Smad2, Smad3, MUC5AC, and MUC5B expressions, down-regulation of IL-4 and IL-13 levels, and up-regulation of IFN-γ level.
3.A comparison of clinicopathological features and prognosis between lymph node dissection extents of pancreatic cancer patients undergoing pancreaticoduodenectomy

Objective:To compare the clinical, pathological features and prognosis of patients who underwent pancreaticoduodenectomy with standard or extended lymph node dissection for pancreatic ductal adenocarcinoma.Methods:A retrospective study was performed on 158 pancreatic head cancer patients who underwent radical resection at the First Affiliated Hospital of Nanjing Medical University from Jul 2017 to Feb 2019. The clinicopathological characteristics and prognosis between the standard dissection group and the extended dissection group were compared. The relationship between the number of examined lymph nodes, positive lymph nodes, and the lymph node ratio, together with their relationship with survival were analyzed.Results:Survival analysis showed no statistical difference in survival between the standard resection group and the extended resection group ( P=0.99). There were statistical differences in gender and age composition between the two group, but no significant differences in operation time, blood loss, or postoperative complications were found. Patients with less examined lymph nodes tended to be of stage N0. examined lymph nodes is positively correlated with positive lymph nodes but is not significantly correlated with lymph node ratio. Positive lymph nodes is strongly correlated with lymph node ratio. The location of lymph node metastasis was not survival-related. Conclusions:There is no prognostic difference between standard lymph node dissection and extended lymph node dissection in pancreatic cancinoma patients after Whipple procedure.
4.Progress of classification and prognosis of diffuse large B-cell lymphoma

Diffuse large B-cell lymphoma (DLBCL) is the most common non-Hodgkin lymphoma, with high clinical and biological heterogeneity. Only 60% of patients can benefit from standard immunochemotherapy. Looking for new clinical parameters and biomarkers to better classify and stratify the prognosis of DLBCL patients has been the focused area in recent years. This article reviews the classifications and their prognostic significances of DLBCL by analyzing the related studies of genome and transcriptome.
5.Relationship between clinicopathological features and prognosis of pancreatic ductal adenocarcinoma

Objective:To investigate the relationship between clinicopathologic features and prognosis of pancreatic ductal adenocarcinoma located in the head of pancreas.Methods:A retrospective study was performed on 169 patients undergoing radical resection for pancreatic head cancer collected in the First Affiliated Hospital with Nanjing Medical University from January 2018 to April 2019. Univariate analysis and multivariate analysis were performed.Results:Patient′s age, tumor differentiation, tumor maximum diameter, resection margin (several resection margins including portal vein groove resection margin, posterior resection margin, and uncinate resection margin), number of positive lymph nodes, number of regional lymph node dissected, and some preoperative and postoperative indicators were associated with prognosis ( P<0.05). Direct tumor invasion of organs and surrounding tissues, perineural and vascular invasion, pathologic variants etc. had no statistical significance for survival time. Patient′s age, maximum tumor diameter, degree of differentiation, uncinate incision margin, number of regional lymph nodes dissected, and preoperative CA19-9 were independent factors affecting prognosis. Patients older than 74 years of age, with tumors larger than 3 cm in diameter, poorly differentiated, less than 7 regional lymph node dissected, positive uncinate margin, and preoperative CA19-9 higher than 1.5×10 5 U/L were independent risk factors in patients with pancreatic head cancer. Conclusions:Old age, tumor lager than 3 cm, poor differentiation, low examined lymph nodes, direct uncinate margin involvement and (or) with preoperative CA19-9 higher than 1.5×10 5 U/L are related to poor prognosis of head pancreatic cancer.
6. Establishment and clinical performance evaluation of 2019 novel coronavirus antibody colloidal gold detection method

Objective:
To establish a colloidal gold technique assay for the rapid detection of immunoglobulin(Ig) M and IgG antibodies against 2019 novel coronavirus (2019-nCoV) and to evaluate its clinical performance.
Methods:
A total of 278 patients who were treated at Wuhan Hankou Hospital and the People's Hospital of Honghu from February 12, 2020 to February 20, 2020 were collected. According to the diagnostic criteria, 89 patients were confirmed with 2019-nCoV nucleic acid positive diagnosis, and 189 were 2019-nCoV nucleic acid-negative suspected patients. A total of 273 medical examiners from Nanfang Hospital, Southern Medical University from 2015 to 2018 were selected as controls. The serum samples of patients were collected. 2019-nCoV nucleic proteins were obtained from prokaryotic expression vectors. Indirect IgM and IgG colloidal gold techniques were established by using recombinant N protein. 2019-nCoV nucleic acid detection by reverse transcription-polymerase chain reaction (RT-PCR) was used as control. Serum specimens were tested for 2019-nCoV IgM and IgG. The specificity and sensitivity of colloidal gold assay were analyzed.
Results:
The sensitivity and specificity of IgM detection reagents were 78.7% and 98.2%, respectively, those of IgG detection reagents were 73.0% and 99.3%, respectively, and those of IgM combined with IgG detection were 87.6% and 98.2%, respectively. For suspected patients with negative 2019-nCoV nucleic acid, the positive rates of IgM and IgG were 59.8% (113/189) and 52.9% (100/189), respectively, and the positive rate of IgM combined with IgG detection was 66.1% (125/189).
Conclusion
This reagent of 2019-nCoV antibodies detection (colloidal gold technique) fulfills the requirement for clinical application with high specificity and sensitivity, which can be served as a supplementary detection method for 2019-nCoV nucleic acid detection by RT-PCR.

Diffuse large B-cell lymphoma (DLBCL) is an aggressive B-cell lymphoma, and its etiology and molecular mechanism has not been fully elucidated. Recently, increasing evidence has indicated that long non-coding RNA (LncRNA) participates in the occurrence, development, invasion and metastasis of DLBCL. This article reviews lncRNA-related genes and signaling pathways as well as the molecular mechanism of DLBCL.
9.Clinicopathological features in relapsed diffuse large B-cell lymphoma

Objective:To study the clinical pathological features of patients with relapsed diffuse large B-celllymphoma (DLBCL) and to provide evidence for early clinical screening of recurrent cases.Methods:The clinical and pathological data of the 20 patients, who had relapsed DLBCL (relapsed group) and were admitted to the First Affiliated Hospital of Nanjing Medical University from January 2015 to December 2019, were included. Meanwhile, other 34 patients with DLBCL who had achieved complete response (CR) for 36 months or more (CR group) were used as controls.Statistical methods were used to retrospectively analyze the differences in general conditions, clinical characteristics, lab resultsand pathological features between the two groups.Results:Clinically, there were 6 males and 14 females with a median age of 55.5 (33-85) years in the relapsed group and 14 males and 20 females with a median age of 53 (15-89) years in the CR group. The relapsed and CR groups had significant difference in Ann Arbor stage ( P=0.001), International Prognostic Index score ( P=0.006), primary lesions ( P=0.003), extranodal involvement ( P=0.002), and hepatitis B viral infection ( P=0.046), β2-MG level ( P=0.029), LDH level ( P=0.005) and CRP level ( P=0.006), while the age ( P=0.732), gender ( P=0.416), ECOG score ( P=0.248), B symptoms ( P=0.511), the presence of hypoalbuminemia ( P=0.279), anemia ( P=0.983) and A/G( P=0.416) showed no statistical difference.Pathologically, compared with the CR group, the relapsed group was mostly non-GCB type (85% vs. 59%, P=0.048), with a higher CD5 positive rate (25% vs.3%, P=0.014) and a lower bcl-6 positive rate (60% vs. 88%, P=0.017), while the expression of Ki-67, CD10, bcl-2, MUM1, CD20 and PAX5 was not different between the two groups. Conclusion:Most of the patients with relapsed DLBCL are non-GCB type. The patients with CD5 positivity, stage III-IV, International Prognostic Index score 3-5, nodal origin, often involving>1 extranodal organ, abnormally elevated LDH, CRP and β2-MG level, and HBV infection are more likely to relapse.
10.Identification of pathogenic mutation in a Chinese pedigree affected with split hand/split foot malformation.

OBJECTIVE:
To detect potential mutation in a Chinese pedigree affected with split hand/split foot malformation (SHFM).
METHODS:
The patients were screened for genome-wide copy number variations with single nucleotide polymorphism (SNP) microarray. Copy number variations were verified by real-time fluorescence quantitative PCR.
RESULTS:
There were 3 SHFM patients from three generations, which conformed to an autosomal dominant inheritance. SNP microarray assay revealed that all patients have carried a 0.34 Mb duplication in 10q24.31-q24.32 (102 993 649-103 333 271) encompassing the BTRC and DPCD genes. The result was verified by real-time fluorescence quantitative PCR, confirming that the duplication has co-segregated with the SHFM phenotype in the pedigree.
CONCLUSION
The 10q24.31-q24.32 duplication probably underlies the pathogenesis of SHFM in this pedigree. Tiny copy number variations can result in diseases featuring autosomal dominant inheritance.
