1.The Association between Parental Neglect and Depressive Symptoms among Adolescents:based on Structural Equation Model

Objective This study aimed to explore the underlying mechanisms between parental neglect and depression of adolescents.Methods This study utilized data from the 2020 China Family Panel Studies(CFPS)and included 1902 adolescents aged 10~15.Correlation analysis,multivariable logistic regression,structural equation modeling,and the bootstrap method were employed to examine the relationships between parental neglect,social trust,and depressive symptoms in adolescents.Results This study found that the prevalence of depressive symptoms among adolescents was 10.9%.Parental neglect was associated with an increased risk of depressive symptoms(OR:1.772,95%CI:1.310~2.636),and social trust acted as a mediator between parental neglect and depressive symptoms(mediating effect:17.20%,P<0.001).Additionally,gender moderated the latter half of the mediation model(β:0.127,95%CI:0.016~0.236,P<0.001).Conclusion The experiences of parental neglect are closely related to higher levels of depressive symptoms among adolescents,with social trust serving as a mediating factor.This mediating effect of social trust is particularly significant among females.It is crucial to cultivate a supportive environment and develop a harmonious family atmosphere.
2.Value of LCR combined with FAR in diagnosis of postoperative pneumonia in elderly patients with gastric cancer

Objective To explore the diagnostic value of lymphocyte to C-reactive protein ratio(LCR)combined with fibrinogen to albumin ratio(FAR)for postoperative pneumonia in elderly patients with gastric cancer.Methods A total of 134 elderly patients who underwent radical gastrectomy in Hefei First People's Hospital from September 2017 to March 2024 were selected as study objects.According to whether pneumonia occurred after surgery,patients were divided into uncomplication group(n=109)and complication group(n=25).General data and laboratory results were compared between two groups.Multivariate Logistic regression model was used to explore the influencing factors of postoperative pneumonia in patients with gastric cancer.The receiver operating characteristic(ROC)curve was plotted to calculate the area under the curve(AUC)of different indicators and their combined application in order to predict their value in postoperative pneumonia.Results The body mass index,lymphocyte,albumin and LCR in complication group were significantly lower than those in uncomplication group,while white blood cell,fibrinogen,C-reactive protein,FAR,neutrophil to lymphocyte ratio and platelet to lymphocyte ratio were significantly higher than those in uncomplication group(P<0.05).Multivariate Logistic regression analysis showed that preoperative LCR reduction and FAR increase were independent risk factors for postoperative pneumonia in gastric cancer patients(P<0.05).ROC curve analysis results showed that the AUC of postoperative pneumonia in patients with gastric cancer diagnosed by LCR and FAR was 0.727 and 0.719,respectively,and the AUC of combined diagnosis was 0.790(95%CI:0.702-0.879).Conclusion Preoperative LCR and FAR are independent risk factors for postoperative pneumonia in patients with gastric cancer,and their combination can effectively predict postoperative pneumonia complications in patients with gastric cancer,so as to guide clinical decision-making.

5.Pharmacoeconomic evaluation of fluticasone furoate/umeclidinium/vilanterol powder for inhalation for the treatment of chronic obstructive pulmonary disease

Objective To evaluate the economic value of fluticasone furoate/umeclidinium/vilanterol(FF/UMEC/VI)powder for inhalation in the treatment of symptomatic chronic obstructive pulmonary disease patients with acute exacerbation risk from the perspective of the Chinese health system.Methods Based on subgroup analysis of the China cohort in the IMPACT trial,a four-state lifetime Markov model was established with a 3-month cycle.The model simulation period was 11 years.Clinical efficacy,health benefits,and cost data were obtained through published literature.The health outcomes included quality adjusted life year(QALY).Using 3 times Chinas per capita gross domestic product(GDP)in 2023 as the willingness-to-pay threshold,the cost-utility analysis method was used for analysis the economic viability of FF/UMEC/VI.The scenario analysis,one-way sensitivity analysis and probability sensitivity analysis were used to verify the robustness of the results.Results Compared with fluticasone furoate/vilanterol(FF/VI),FF/UMEC/VI in the treatment of symptomatic chronic obstructive pulmonary disease patients with acute exacerbation risk saved costs 8 118.66 yuan and obtained an additional 0.000 06 QALYs,giving it an economic advantage.Compared with umeclidinium/vilanterol(UMEC/VI),FF/UMEC/VI treatment paid 2 784.41 yuan more and received 0.000 45 QALYs less,making UMEC/VI more cost-utility.The scenario analysis results further confirmed the robustness of the model.The sensitivity analysis results showed that when the drug cost of FF/UMEC/VI per cycle decreases to 637.29 yuan,FF/UMEC/VI had economic benefits under a willingness-to-pay threshold of 3 times China's per capita GDP in 2023.Conclusion For patients with symptomatic chronic obstructive pulmonary disease at risk of acute exacerbation,FF/UMEC/VI is more cost-utility than FF/VI.Compared with UMEC/VI,FF/UMEC/VI has economic viability after price reducing.
6.Clinical Validation Study of Deep Learning-Generated Magnetic Resonance Images

This research utilizes a deep learning-based image generation algorithm to generate pseudo-sagittal STIR sequences from sagittal T1WI and T2WI MR images.The evaluations include both subjective assessments by two physicians and objective analyses,measuring image quality through SNR and CNR in ROIs of five different tissues.Further analyses,including MAE,PSNR,SSIM,and COR,establish a strong correlation between the generated STIR sequences and the gold standard,with Bland-Altman analysis indicating pixel consistency.The findings indicate that the deep learning-generated STIR sequences not only align with but potentially surpass the gold standard in terms of image quality and clinical diagnostic capabilities.Moreover,the approach demonstrates promise for clinical implementation,offering reduced scan time and enhanced imaging efficiency.

8.Application effect of nutritional management scheme based on modified Subjective Global Assessment in hemodialysis patients

Objective:To explore effects of establishing a modified Subjective Global Assessment (SGA) -based nutritional management scheme in undergoing maintenance hemodialysis (MHD) .Methods:Using the convenient sampling method, a total of 208 patients who received MHD treatment in Jinhua Hospital of Traditional Chinese Medicine from September 2019 to December 2020 were selected as the research subjects. They were divided into the control group and the observation group according to random number table method, with 104 cases in each group. The control group received routine nutrition intervention, while the observation group received nutritional management scheme based on modified SGA at basis of the control group. The nutritional indicators, anthropometric indicators, renal function indicators before and after 6 months of intervention and the total incidence of complications after 6 months of intervention were compared between the two groups.Results:After 6 months of intervention, the nutritional indexes and anthropometric indexes of the observation group were higher than those of the control group and the renal function index were lower than those of the control group, and the differences were statistically significant ( P<0.05) . The total incidence of complications in the observation group was 55.77% (58/104) , which was lower than 80.77% (84/104) in the control group, and the difference was statistically significant ( P<0.05) . Conclusions:The establishment of the modified SGA-based nutritional management scheme in MHD patients can ensure reasonable nutritional intake, improve nutritional index levels, reduce inflammatory response, improve immune function, maintain stable renal function, and reduce the occurrence of complications.
9.Identification of potential pathogenic genes for perioperative neurocognitive disorder in patients with digestive system tumors

Objective:To identify the potential pathogenic genes for perioperative neurocognitive disorder (PND) in the patients with digestive system tumors.Methods:The gene expression data of esophageal cancer, gastric cancer, colon cancer, rectal cancer and liver cancer in The Cancer Genome Atlas database were analyzed by bioinformatics analysis method, and the differentially expressed genes in tumor tissues in above-mentioned disease samples were identified compared with para-carcinoma tissues.Secretory proteome differential genes with the same expression trend in digestive system tumors were obtained by comparing with human secretory proteome genes.The correlation between secretomics and PND was determined by comparing with the GeneCards database.Hub genes were identified through PPI network construction and calculation, and the functions and signaling pathways of the above-mentioned differential genes were identified through GO and KEGG enrichment analysis.Results:Compared with para-carcinoma tissues, the expression of 2 640 genes was significantly up-regulated and the expression of 1 423 genes was down-regulated in esophageal cancer tissues; the expression of 3 748 genes was up-regulated and the expression of 908 genes was down-regulated in gastric cancer samples; the expression of 2 684 genes was up-regulated and the expression of 2 678 genes was down-regulated in colon cancer samples; the expression of 2 876 genes was up-regulated and the expression of 2 945 genes was down-regulated in rectal cancer samples; the expression of 1 484 genes was up-regulated and the expression of 723 genes was down-regulated in hepatocellular carcinoma samples.Among them, the expression of the encoding genes of 53 secreted proteins was uniformly up-regulated and the expression of the encoding genes of 20 secreted proteins was uniformly down-regulated in the above tumors.Twenty up-regulated genes and 3 down-regulated genes were associated with PND.PPI network analysis showed that MMP9 was the hub gene.The results of GO and KEGG analysis suggested that differentially expressed genes were mainly related to receptor-ligand activity, cytokine activity and chemokine activity, and were mainly enriched in signaling pathways related to cell cycle and cellular senescence.Conclusions:About 23 differentially expressed genes in digestive system tumors are potentially related to PND, of which MMP9 and other genes may be the hub genes, mainly acting on receptor-ligand binding, regulation of cytokine and chemokine activity, cell cycle, cellular senescence and other related signaling pathways.
