1.Construction and Verification of Prediction Model of Qi Deficiency and Blood Stasis Syndrome in Chronic Heart Failure

ObjectiveTo construct and validate a clinical prediction model for Qi deficiency and blood stasis syndrome in chronic heart failure (CHF),aiming to assist clinical diagnosis and provide tools and methods for individualized treatment of CHF. MethodsThe clinical data of patients with chronic heart failure treated at Dongzhimen Hospital of Beijing University of Chinese Medicine from January 2022 to January 2024 were retrospectively collected. The patients were randomly divided into a training group and a validation group with a ratio of 7∶3. First, the least absolute shrinkage and selection operator (LASSO) regression analysis was used to preliminarily screen the predictive factors affecting the diagnosis of Qi deficiency and blood stasis syndrome in CHF. Subsequently, the Logistic regression method was applied to conduct a more in-depth and detailed analysis of these factors. Variables with P<0.05 in the results of the multi-factor Logistic regression were carefully selected and included. Based on the regression coefficients obtained from this analysis, a model was constructed, and a nomogram was accurately drawn. Using R software,the receiver operating characteristic (ROC) curve,calibration curve,and decision curve analysis (DCA) were precisely drawn. These analyses were used to comprehensively evaluate the model from three crucial aspects: discrimination,calibration,and clinical applicability. Additionally, the accuracy,specificity,sensitivity,positive predictive value,and negative predictive value of the model were meticulously calculated to conduct a more all-round and comprehensive assessment. ResultsIn total, 168 cases were successfully obtained in the training group, and 71 cases were included in the validation group. After a thorough comparison, it was found that there were no statistically significant differences in the baseline data between the two groups. After being rigorously screened by the LASSO-multivariate logistic regression method, dark red tongue,smoking history,cardiac troponin I,and N-terminal pro-B-type natriuretic peptide (NT-ProBNP) were identified as the influencing factors for diagnosing patients with the Qi deficiency and blood stasis syndrome in CHF. The constructed model demonstrated an area under the curve (AUC) of 0.812 in the training group and 0.719 in the validation group. The calibration curve showed that the predicted curve of the model was close to the actual observed curve. DCA indicated that the model could provide substantial clinical benefits for patients at the decision thresholds ranging from 0.2 to 0.9. ConclusionThe clinical prediction model for Qi deficiency and blood stasis syndrome in chronic heart failure constructed in this study shows good performance. It has certain application value in clinical practice, which may contribute to the improvement of the diagnosis and treatment of CHF patients with this syndrome.
2.Clinical Observation on Regulating Conception Vessel and Unblocking Governor Vessel Electroacupuncture in the Treatment of Cancer-Related Fatigue in Middle-to-Advanced Stage Cervical Cancer

Objective To observe the clinical efficacy of regulating conception vessel and unblocking governor vessel electroacupuncture in the treatment of cancer-related fatigue(CRF)of middle-to-advanced stage cervical cancer,and to explore its effect on inflammatory factors.Methods A total of 122 patients with a definitive diagnosis of CRF of middle-to-advanced stage cervical cancer admitted to the wards and outpatient clinics of Cangzhou Central Hospital from August 2021 to August 2023 were selected for the study.The patients were randomly divided into the control group and the observation group according to the random number table method,with 61 cases in each group.The patients in both groups were given conventional symptomatic treatments such as gastric protection and anti-vomiting since the day of action of chemotherapy.The control group was treated with Dexamethasone Tablets,and the observation group was treated with regulating conception vessel and unblocking governor vessel electroacupuncture on the basis of the control group,the course of treatment covered 21 days.After three weeks of treatment,the clinical efficacy of the two groups was evaluated.The changes in the traditional Chinese medicine(TCM)syndrome scores,Piper's Fatigue Scale(PFS)scores,Karnofsky Performance Status(KPS)scores,and Functional Assessment of Cancer Therapy-General(FACT-G)scores of the patients in the two groups before and after treatment were observed.The changes in interleukin 6(IL-6),hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor α(TNF-α),and T-lymphocyte subsets indicators[CD3+,CD4+,CD8+,CD4+/CD8+]were compared between the patients in the two groups before and after treatment.Results(1)The total effective rate was 95.08%(58/61)in the observation group and 77.05%(47/61)in the control group.The efficacy of the observation group was superior to that of the control group,the difference being statistically significant(P<0.05).(2)After treatment,the TCM syndrome scores of patients in the two groups were significantly improved(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.05).(3)After treatment,the PFS scores of patients in the two groups were improved significantly(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.05).(4)After treatment,the serum IL-6,hs-CRP,and TNF-α levels of patients in the two groups were significantly improved(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.05).(5)After treatment,the CD3+,CD4+,CD8+,CD4+/CD8+of patients in the two groups were significantly improved(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.05).(6)After treatment,the KPS scores and FACT-G scores of patients in the two groups were significantly improved(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.05).Conclusion Regulating conception vessel and unblocking governor vessel electroacupuncture in the treatment of CRF of middle-to-advanced stage cervical cancer can effectively alleviate the fatigue of patients,inhibit the release of inflammatory factors,regulate the immune function,and improve the physical condition of patients,so as to improve the quality of life of patients.

4.Rehabilitation effects based on immersive virtual reality as a treatment for peripheral vestibular dysfunction:a Meta-analysis

Objective To systematically evaluate the rehabilitation effect of immersive virtual reality technology for patients with peripheral vestibular dysfunction,and to provide theoretical basis and practical guidance for the design of efficient rehabilitation training programs.Methods PubMed,Embase,Cochrane Library,Web of Science,CINAHL,CNKI,Wanfang Database,VIP Database,and China Biomedical Literature Database were searched for randomized controlled trials on the intervention effect of immersive virtual reality technology in patients with peripheral vestibular dysfunction from inception to February 2025.RevMan5.4 software was used to perform meta-analysis of the literature that met the quality standards.Results A total of 10 papers with 491 patients with peripheral vestibular dysfunction were included.Meta-analysis showed that immersive virtual reality technology improved vertigo symptoms in patients with peripheral vestibular dysfunction compared with conventional vestibular rehabilitation care[SMD=-1.41,95%CI(-1.58,-0.70),P<0.001],and subgroup analysis showed that the frequency of intervention was<5 times/week[SMD=-1.31,95%CI=(-1.96,-0.66),P<0.001],single intervention duration<30 min[SMD=-1.45,95%CI=(-2.21,-0.70),P<0.001],and intervention period≤7 weeks[SMD=-1.49,95%CI(-2.04,-0.94),P<0.001]were more significant.Immersive virtual reality technology reduced the risk of falls[MD=4.66,95%CI(3.84,5.48),P<0.001],and improved anxiety and depression[SMD=2.65,95%CI(1.98,3.33),P<0.001].Conclusion Immer-sive virtual reality technology improves vertigo symptoms,reduces the risk of falls,and improves anxiety and depres-sion symptoms in patients with peripheral vestibular dysfunction,in which the intervention period of ≤7 weeks,<5 training sessions per week,and each training session of<30 min are better for improving vertigo symptoms in patients with peripheral vestibular dysfunction.
5.Prognostic and immunotherapeutic significance of ARV1 in colorectal cancer

Objective:To investigate the prognostic and immunotherapeutic significance of androgen receptor V1 (ARV1) in colorectal cancer (CRC) and to explore its mechanism in CRC progression.Methods:The relationship between ARV1 expression and CRC prognosis was analyzed using data from The Cancer Genome Atlas Program (TCGA). Gene ontology (GO) analysis was performed to identify potential mechanisms through which ARV1 regulates CRC progression. Multiple public databases were used to analyze the correlation between ARV1 and immune cell infiltration and to predict the sensitivity of ARV1 to immunotherapy and chemotherapeutic drugs. Immunohistochemical validation was conducted using postoperative specimens from 199 CRC patients, and clinical correlations were analyzed.Results:ARV1 expression was significantly lower in CRC tissues compared to normal tissues ( P<0.001). Patients with high ARV1 expression exhibited better overall survival than those with low expression ( P=0.016). Clinical analysis indicated that ARV1 serves as an independent prognostic factor in CRC, and its expression was associated with age and clinical stage (all P<0.01). GO analysis revealed that ARV1 influenced multiple pathways in CRC. Immune-related analysis demonstrated that ARV1 participated in regulating immune cell infiltration in CRC. Drug sensitivity analysis showed differences in responses to various chemotherapeutic agents between high and low ARV1 expression groups ( P<0.01). In immunotherapy, significant differences in Immunophenotype Score (IPS) were observed between high and low ARV1 expression groups in PD1-negative/CTLA4-negative patients ( P<0.05). Immunohistochemical results from 199 CRC patients confirmed that low ARV1 expression was associated with poorer prognosis ( P<0.001). Conclusions:ARV1 expression affects CRC prognosis and may serve as a potential novel biomarker for immunotherapy in CRC.
6.Effect of hyperthermic intraperitoneal chemotherapy (HIPEC) on postoperative immune function and tumor marker levels in patients with advanced gastric adenocarcinoma

Objective:To explore the effect of hyperthermic intraperitoneal chemotherapy (HIPEC) on postoperative immune function and tumor marker levels in patients with advanced gastric adenocarcinoma.Methods:A prospective cohort study was conducted, enrolling 72 patients with stage ⅢA-ⅢC gastric adenocarcinoma admitted to Jinjiang Hospital from August 2022 to December 2023. Patients were divided into the HIPEC group ( n=36, radical resection+ HIPEC within 3 days after surgery) and the control group ( n=36, radical resection alone) using the random number table method. The HIPEC protocol was perfusion with raltitrexed (4 mg in 4, 000 ml normal saline, at 43 ℃) for 60 minutes. Peripheral blood immune cells (CD3 +, CD4 +, CD8 +, CD4 + /CD8 + ratio) before treatment and 7 days after treatment, and serum levels of carcinoembryonic antigen (CEA), carbohydrate antigen 19-9 (CA19-9), and carbohydrate antigen 72-4 (CA72-4) 30 days after treatment were compared between the two groups. Results:Before treatment, there were no statistically significant differences in CD3 +, CD4 +, CD8 +, or CD4 + /CD8 + levels between the two groups (all P>0.05). Seven days after treatment, the HIPEC group had higher levels of CD3 +, CD4 +, and CD4 + /CD8 + ratio, and a lower CD8 + level than the control group, with statistically significant differences (all P<0.05). Before treatment, there were no significant differences in CEA, CA19-9, or CA72-4 levels between the two groups (all P>0.05). Thirty days after treatment, the HIPEC group had lower levels of CEA, CA19-9, and CA72-4 than the control group, with statistically significant differences (all P<0.05). Conclusions:HIPEC can significantly improve the postoperative immune function of patients with advanced gastric adenocarcinoma (by increasing the CD4 + /CD8 + ratio and immune cell activity) and effectively reduce tumor marker levels, which may provide a new strategy for preventing postoperative recurrence.

Objective To construct a self-service traditional Chinese medicine(TCM)syndrome differentiation-based home sleep scenario and establish a home-based TCM sleep aid environment.Methods Based on medical principles and modern sleep-related technologies,this study divided the home sleep scenario into four dimensions:Perception,syndrome differentiation,intervention,and evaluation.It proposed a construction plan for a self-service TCM sleep aid home scenario that integrated these four dimensions.Results The proposed TCM sleep aid scenario solution enables collaborative collection data from various devices in a home setting.This involved the setup of hardware at the perception level,the development of software at the syndrome differentiation level,the integration of hardware at the intervention level,and data cleaning,fusion,and analysis at the evaluation level.These four components are interconnected,allowing for the extraction of physiological data that is valuable for TCM-based population classification and intervention,which can be used to update algorithms and improve accuracy through iterative upgrades.Conclusion The construction of a home sleep scenario based on TCM syndrome differentiation is feasible and can provide a reference for the promotion and application of appropriate TCM techniques in home settings.
8.Early identification of high-risk children who develop systemic adverse reactions related to dust mite al-lergy subcutaneous immunotherapy

Objective To identify children with high-risk systemic adverse reactions(SARs)associated with dust mite subcutaneous immunotherapy prior to immunotherapy.Methods An analysis was conducted on the high-risk factors in patients who experienced SARs prior to initial immunotherapy.A total of 40 pediatric patients with dust mite allergic rhinitis who underwent treatment with Dermatophagoides pteronyssinus(Dp)extracts at our hospital's pediatric department from April 2021 to January 2025 and developed SARs were selected as the observa-tion group,while 54 patients who did not experience SARs served as the control group.Differences in gender,age,disease duration,eosinophil count,polysensitization,multisystem involvement,total IgE(tIgE),and dust mite-specific IgE(sIgE)were evaluated.Results Significant differences were observed between the observation and control groups in polysensitization,multisystem involvement,and tIgE levels.The polysensitization rates were 70%vs.48.1%(P=0.034),multisystem involvement 55%vs.22.2%(P=0.001),and high tIgE levels(tIgE≥327.8 IU/mL)82.5%vs.62.1%(P=0.025).Multivariate logistic regression identified multisystem involvement[OR(95%CI)=4.278(1.749~10.461)]as an independent risk factor for SARs during allergen immunotherapy(AIT).Conclusion Patients with dust mite allergic rhinitis accompanied by multi-system damage exhibit poorer conventional prevention efficacy against SARs when undergoing AIT.They remain highly susceptible to SARs occur-rence,necessitating more robust preventive measures against SARs to enhance the safety of AIT.

Objective To provide a scoping review of studies on the application of intelligent healthcare for continuity management in liver transplant patients,in order to provide a reference for the future development of intelligent liver transplant management.Methods A systematic search of relevant studies was conducted in PubMed,Embase,CINAHL,Web of Science,Cochrane Library,Scopus,CNKI,Wanfang Database,VIP Database,and CBM from the establishment of the databases to 30 September 2024.Following this search,the included literature was screened,summarized,and analyzed.Results A total of 19 papers were included.The intervention forms included application,intelligent medical device,and remote rehabilitation platform.The content elements included transplant health management,rehabilitation program planning,health data monitoring,telemedicine consultation,psychosocial support,and follow-up supervision.The evaluation indicators included physiological indicators,self-management ability,health-related quality of life,transplantation outcomes,patient satisfaction,and feasibility indicators.Conclusion The application of intelligent healthcare in the continuity management of liver transplantation patients is feasible and effective,and it is recommended that healthcare professionals integrate the diverse practice forms of intelligent healthcare,deepen the content elements and refine the evaluation indicator system based on the actual clinical situation,in order to improve the quality of the continuity management of liver transplantation patients.
