BACKGROUND:Arthroscopic anchor repair has become the main treatment method for rotator cuff tears at present.Among them, the insertion status of the anchor is a key factor in the success or failure of the operation. However, currently, the impact of the insertion depth of the anchor on the stress of the bone tunnel and the anchor under different bone density conditions remains unclear.OBJECTIVE:To explore the stress distribution of the bone tunnel and the anchor when the insertion depth of the anchor varies under different bone density conditions by using three-dimensional finite element analysis technology.METHODS:The CT image data of the humerus of volunteers were collected, and the models of the humerus and the anchor were constructed by using Mimics,3-Matic,and Solidworks software. In 3-Matic,holes with distances of 0,2,4,6,and 8 mm from the surface of the humerus were respectively created at the same position of the humerus and assembled with the anchor. In Mimics, values were assigned based on the CT gray value to obtain a model with normal bone mass(T value ≥-1.0).The parameters were changed to construct models with reduced bone mass(-2.5<T value<-1.0)and osteoporosis(T value<-2.5).In each model,a 70 N pulling force was applied to the anchor along the direction tangent to the inner edge of the bone tunnel. The stress distribution and magnitude of the bone tunnel and the anchor when inserted at different depths under different bone density conditions were observed.RESULTS AND CONCLUSION:(1)When the insertion depth was the same,as the bone density decreased, the maximum equivalent stress of the anchor increased, while the maximum equivalent stress of the bone tunnel decreased.(2)When the bone density was the same,as the insertion depth of the anchor increased, the maximum equivalent stress of the anchor decreased. When the insertion depth was 4 mm,the stress of the bone tunnel was the smallest and the distribution was relatively uniform. The stress of the anchor was mainly distributed around the lower anchor hole and the proximal thread, and the stress of the bone tunnel was mainly at the part in contact with the proximal thread. The increase in the insertion depth would change the uniformity and pattern of the stress distribution, while the bone density had a relatively small impact on the stress distribution pattern.(3)It is concluded that the bone density of the humerus is crucial for the anchor repair of rotator cuff tears. It is recommended that clinicians measure the bone density of the greater tuberosity of the humerus before the operation.Excessive insertion depth of the anchor does not significantly increase its stability.Clinicians can conduct personalized preoperative assessments by using the finite element analysis method in combination with the actual situation of patients to achieve the best surgical results.

ObjectiveTo analyze the epidemiological characteristics of a local dengue fever cluster outbreak in Qingpu District of Shanghai in 2024, and to provide a reference for subsequent dengue fever prevention and control. MethodsSeven confirmed local dengue fever cases reported through the National Notifiable Infectious Diseases Surveillance System in Qingpu District of Shanghai in 2024 were selected as the research subjects. Descriptive epidemiological methods were used to conduct investigation and analysis from the aspects of onset, medical treatment and reporting, clinical symptoms, travel and contact history within 15 days before onset, and activity trajectories. ResultsA total of 7 cases were identified in this outbreak. None of the cases had a travel history to dengue-endemic areas within 15 days prior to onset, while all had shared exposure environments and mosquito bite histories, indicating a local clustered transmission pattern. The main clinical manifestations included fever (100.00%) and myalgia (42.86%). All 7 cases were positive for dengue virus serotype 2 (DENV-2) by nucleic acid testing. Genetic sequencing showed that the virus strains belonged to the Cosmopolitan genotype and were most closely related to the epidemic DENV strains circulating in southern China in recent years. ConclusionThis outbreak might be a local secondary infection caused by the short-term stay of dengue fever-infected individuals, and the possible source of importation was dengue fever endemic areas in southern China.

ObjectiveTo investigate the effect of Sanet Vision Integrator (SVI)-based visuomotor integration training on the severity of neglect symptoms, upper limb motor function and activities of daily living (ADL) in patients with post-stroke unilateral spatial neglect (USN). MethodsFrom July, 2023 to September, 2025, 41 patients with post-stroke USN in Beijing Bo'ai Hospital were enrolled and randomly assigned to control group (n = 20) and experimental group (n = 21). Both groups received conventional rehabilitation and visual scanning training, while the experimental group received supplementary SVI-based visuomotor integration training, including saccadic, rotation and hand-eye coordination training, for eight weeks. They were assessed with Chinese Behavioral Inattention Test-Hong Kong version (CBIT-HK), Fugl-Meyer Assessment-Upper Extremities (FMA-UE) and modified Barthel Index (MBI) before and after treatment. ResultsAfter treatment, all the scores significantly improved in both groups (|t| > 4.004, P < 0.001), and the total scores of CBIT-HK and the core visual search sub-items (star cancellation, line crossing and letter cancellation), FMA-UE and MBI were better in the experimental group than in the control group (t > 2.096, P < 0.05). ConclusionSVI-based visuomotor training can effectively alleviate the USN symptoms, broaden the span of visual attention, facilitate the functional recovery of the affected upper extremity, and improve ADL in patients after stroke.

With the rapid advancement of artificial intelligence, deep learning-based multimodal data fusion technology has found increasing applications in the field of dental and stomatological diseases, and it has demonstrated great potential. This technology constructs deep neural networks to automatically extract and fuse features from multimodal data, such as clinical text information, medical imaging, intraoral and extraoral digital scanning records, and histopathological data. By fully leveraging the complementary information among these diverse data sources, it substantially enhances the accuracy of oral disease diagnosis, the rationality of treatment planning, and the reliability of prognosis prediction. This survey systematically explores the core advantages of deep learning in multimodal data fusion; outlines key main strategies, such as early fusion, intermediate fusion, and late fusion along with their applicable scenarios; and summarizes the specific applications and latest research progress of this technology in oral diseases, including oral cancer, periodontitis, dental caries, and temporomandibular disorders. We also highlight critical challenges in its clinical translation, such as complexity in multimodal data alignment, scarcity of high-quality annotated data, insufficient model interpretability, and privacy and security issues. In the future, this technology is expected to develop toward new learning paradigms, intelligentization, and adaptive fusion while integrating explainable artificial intelligence to enhance clinical credibility. Ultimately, large-scale, multicenter clinical validation systems will be required to ensure its safe and effective deployment in clinical practice.

Polycystic ovary syndrome (PCOS) is a common gynecological endocrine and reproductive disorder, with the main clinical manifestations including ovulation failure, insulin resistance, hyperandrogenism, and obesity. Its occurrence and development are closely related to cellular regulatory mechanisms such as apoptosis, autophagy, oxidative stress, and inflammatory response. Autophagy,as a clearance mechanism that maintains cellular homeostasis, plays a crucial role in maintaining the growth, development, and maturation of oocytes. Exploring the mechanism of autophagy during the occurrence and development of diseases can help develop treatment methods for PCOS by regulating autophagy. Studies have shown that autophagy plays an important role in the pathogenesis of PCOS,and it can affect the occurrence and development of PCOS through multiple pathways, levels, and targets. Traditional Chinese medicine (TCM) regulates autophagy in ovarian granulosa cells or endometrium of patients with PCOS by targeting the expression of autophagy signaling pathways, regulatory factors, and non-coding single-stranded RNA molecules, thereby alleviating inflammation, regulating metabolism disorders, and balancing hormone levels in PCOS. Accordingly,TCM can ameliorate pathological conditions such as insulin resistance, hyperandrogenism, and ovulation failure in PCOS. This article summarizes the TCM formulas and extracts for the treatment of PCOS,as well as the main autophagy pathways and regulatory factors involved, aiming to provide reference and suggestions for the future treatment of PCOS with TCM by regulating autophagy.

Objective: To detect the different proteomic characteristics of microvesicles (MVs) and exosomes (EXOs) released from apheresis platelets during storage, and to explore their role in mediating platelet storage damage lesion (PSL). Methods: Apheresis platelets were collected from the retention bag on the third day of storage. MVs and EXOs were isolated using differential centrifugation. Platelet, MVs and EXOs protein samples were extracted respectively, and the differentially expressed proteins were detected by quantitative proteomics technology. Further, the co-incubation model of MVs, EXOs and fresh platelets was adopted to evaluate the effect of extracellular vesicles on PSL. The aggregation response of platelets to collagen agonizers and the changes in ATP release rate were evaluated by optical turbidimetry. Flow cytometry was used to evaluate the changes of platelet early activation indicators (P-selectin and PAC-1) and mitochondrial membrane potentia. Western blot was used to detect the changes in the expression of key proteins for platelet activation and apoptosis (P-selectin, Integrin β3 and Bcl-xl). Results: Proteomic analysis revealed a significantly separation in protein expression profiles of platelet, MVs and EXOs samples within the latent variable space. Energy metabolization-related proteins such as mitochondrial respiratory chain complex and oxidative phosphorylation were enriched specifically, in MVs while EXOs were enriched with inflammation-related proteins. Co-incubation experiments confirmed that extracellular vesicles could significantly induce platelet responses to agonists (the maximum aggregation rate in the MVs group increased by 187.36%, P<0.001; 71.26%, in the EXOs group P=0.002). The maximum ATP release rate of platelets also increased (275.44% in the MVs group, P<0.001; 70.18% in the EXOs group, P=0.015). The expression of P-selectin increased (119.33% in the MVs group, P<0.001; 25.61% in the EXOs group, P=0.013), as detected by flow cytometry. The binding rate of PAC-1 increased (132.18% in MVs group, P<0.001; 21.41% in EXOs group, P=0.043), and the mitochondrial membrane potential decreased (20.49% in MVs group, P<0.001; 9.73% in EXOs group, P=0.044). In the MVs group, platelet P-selectin and Integrin β3 expression were significantly increased (100.83% and 395.64%, P<0.001), while Bcl-xl expression was lower than that in the control group (83.94%, P<0.001). Compared with the control group, P-selectin and Integrin β3 expression were also increased (27.89% and 181.91%, P=0.007和P=0.002), while Bcl-xl was decreased in the EXOs group (36.52%, P<0.001). Conclusion: MVs and EXOs derived from stored platelets show different proteomic characteristics. Compared with EXOs, MVs exhibits a stronger effect in inducing mitochondrial dysfunction. Mvs also promots PSL responses including platelet activation and apoptosis.

Platelet lipid rafts are dynamic nanodomains enriched in cholesterol and sphingolipids within the plasma membrane. As pivotal hubs for signal transduction, membrane trafficking, and intercellular interactions, they have garnered significant attention in recent years for their roles in regulating platelet function and related disease mechanisms. This article reviews the structural characteristics and molecular composition of platelet lipid rafts, as well as their central roles in signal transduction and cytoskeletal reorganization. It particularly focuses on the functional abnormalities and pathological contributions of lipid rafts in diseases such as atherosclerosis, antiphospholipid antibody syndrome, and ANCA-associated vasculitis. Research indicates that lipid rafts coordinate platelet activation, inflammatory responses, and immunomodulatory processes by integrating receptor clustering, enrichment of signaling proteins, and regulation of metabolites. Finally, the article discusses the future prospects of lipid raft-targeted nanotherapeutic strategies and addresses challenges in translational research, providing a theoretical foundation and novel perspectives for understanding platelet biology and intervening in thrombo-inflammatory diseases.

Acta Universitatis Medicinalis Anhui 2025;60(1):123-129
Objective :
To explore the effect of the addition of N-acetylcysteine ( NAC) on the abnormal increase of reactive oxygen species ( ROS) during platelet cold storage, and to clarify its function of preventing the platelets from being recognized and cleared by macrophages and hepatocytes.
Methods :
Platelets Concentrates were divided into room temperature group ( 22C) ,cold storage group ( 4C) and NAC addition group. In NAC addition group, the platelets were supplemented with 5 mmol / L ( N5) and 25 mmol / L ( N25) NAC.After 5-day storage, the levels of ROS,activation markers and other index of platelets in each group were detected by flow cytometry. Platelet phagocytosis was detected by PMA-activated THP-1 cells or by primary cultured HepG2 cells.
Results :
After 5 days of storage,ROS increased significantly in 4C group than those in 22C group ( P<0. 05) ,and after NAC addi- tion,ROS level reduced significantly ( P<0. 05) .The expression of CD62P and PS and the exposure of β-GlcNAc on platelets in N5 group significantly decreased compared with those in 4C group ( P<0. 05) .The platelet phago- cytosis by THP-1 and HepG2 cells were also significantly lower in N5 group than those in 4C group ( P<0. 05) . However, the addition of 5 mmol / L NAC did not significantly affect the count,pH,CD42b expression,β-Gal ex- posure and coagulation function of cold-stored platelet after 5-day storage.
Conclusion
The addition of NAC to re- frigerated platelets can significantly decrease the level of platelet ROS and effectively reduce their phagocytosis by cells, suggesting that NAC addition may protect refrigerated platelets from being cleared by phagocytosis after trans- fusion.
