1.Relationship between intracranial high-density foci and progressive stroke in patients with acute ischemic stroke after intravascular intervention

Objective:To investigate the relationship between intracerebral high-density foci and progressive stroke (PS) morbidity by using dual-energy CT, which can quantify the intracerebral high-density foci of patients with acute ischemic stroke after endovascular treatment.Methods:Ninety-two patients with acute ischemic stroke who received interventional treatment in Gaozhou People′s Hospital from May 2019 to August 2020, and underwent dual-energy CT scan immediately after intervention, were analyzed. The patients were divided into PS group ( n=35) and non-PS group ( n=57) according to the National Institutes of Health Stroke Scale (NIHSS) score, and the patients whose NIHSS score increased≥4 points within 72 hours of stroke were included in the PS group, while the patients whose NIHSS score increased<4 points were included in the non-PS group. The clinical data, volume of high-density foci and CT values were compared between the 2 groups. Logistic regression analysis was used to adjust for confounding factors and screen for risk factors. The correlations of the admission NIHSS score, presence and volume of high-density lesions, maximum CT (CTmax) value and average CT (CTave) value with the onset of PS were analyzed, and the receiver operating characteristic curve was used to screen predictive indicators of PS. Results:In the PS group, the NIHSS score (18.80±8.50 vs 14.40±9.58, t=2.229, P=0.028), proportion of high-density foci [29/35(82.9%) vs 32/57 (56.1%), χ 2=6.928, P=0.008], high-density focal volume [13.23 (39.33) cm 3vs 0.76 (9.82) cm 3, U=1 440.000, P<0.001], CTmax value [80.00 (92.00) HU vs 65.00 (87.50) HU, U=1 337.000, P=0.005] and CTave value [53.48 (23.79) HU vs 45.94 (55.11) HU, U=1 345.000, P=0.004] were higher than those in the non-PS group. The NIHSS score ( OR=1.054, 95% CI 1.004-1.106, P=0.033; rs=0.255, 95% CI 0.051-0.447, P=0.014), presence of high-density foci ( OR=3.776, 95% CI 1.358-10.503, P=0.011; rs=0.274, 95% CI 0.093-0.460, P=0.008), high-density focal volume ( OR=1.026, 95% CI 1.003-1.049, P=0.027; rs=0.381, 95% CI 0.183-0.560, P<0.001), CTmax value ( OR=1.006, 95% CI 1.001-1.011, P=0.014; rs=0.292, 95% CI 0.088-0.475, P=0.005) and CTave value ( OR=1.021, 95% CI 1.007-1.035, P=0.004; rs=0.299, 95% CI 0.092-0.484, P=0.004) were all risk factors affecting PS morbidity and were positively correlated with PS morbidity. The area under the receiver operating characteristic curve of NIHSS score, high-density lesion volume, CTmax value, and CTave value to predict the onset of PS was 0.652, 0.722, 0.670 and 0.674, respectively. The volume of high-density lesions had moderate predictive value for the onset of PS. Conclusions:For AIS patients, CT examination should be performed immediately after interventional operation. The volume, CTmax value and CTave value of high-density lesions newly appeared in the ischemic area are positively correlated with the onset of PS. Quantifying the volume of high-density lesions can help to predict the onset of PS.
2.Electroacupuncture alleviates hyperalgesia in spared nerve injury mice by regulating sympathetic-sensory coupling

Objective:To observe the effects of electroacupuncture(EA)intervention on norepinephrine(NE)andα2A adrenergic receptors(α2A-R)in the dorsal root ganglion(DRG)of mice with spared nerve injury(SNI).Methods:Male C57BL/6 mice were randomly divided into sham surgery group(Sham),model group(SNI),negative control EA group(SNI+NC-EA),and EA group(SNI+EA).Mechanical and thermal stimuli were used to measure the paw withdrawal mechanical threshold(PWMT)and paw withdrawal thermal latency(PWTL).Immunofluorescence staining was used to detect the sprouting of sympathetic nerve fibers and the co-localization of α2A-R with large-diameter sensory neurons in mouse DRG.Enzyme-linked immunosorbent assay(ELISA)kits were used to measure NE levels in mouse serum and DRG,and Western Blot was used to detect tyrosine hydroxylase(TH)and α2A-R expression levels in DRG.Results:After SNI,the PWMT and PWTL were significantly decreased,and after electroacupuncture treatment,PWMT and PWTL were reversed and increased.Immune fluorescence staining showed that sympathetic ganglion sprouting increased in DRG after SNI,and significantly decreased after electroacupuncture;After SNI,NE,α2A-R,and TH in DRG all significantly increased,and their expression decreased after electroacupuncture intervention,but NE in the serum did not change significantly.Conclusion:In the SNI model,electroacupuncture may regulate the sympathetic-sensory coupling by inhibiting the release of NE and the expression of α2A-R in DRG,thereby producing analgesic effects.
3.Value of TLR/NF-κB signaling axis in predicting bone infection in patients with open fractures

Objective To analyze the predictive value of dynamic changes in key factors of the toll-like receptor (TLR)/nuclear factor-κB (NF-κB) signaling axis during the perioperative period for bone infection inpatients with open fractures. Methods A total of 55 patients with open fractures who developed bone infections during the perioperative period were selected as infection group, and 110 patients with open fractures who did not develop infections during the same period were selected as non-infection group. Clinical data, pre-and post-operative serum levels of routine inflammatory markers [C-reactive protein (CRP), interleukin-6 (IL-6) and procalcitonin (PCT)] and key factors of the TLR/NF-κB signaling axis (TLR4, NF-κB) were compared between the two groups. Logistic multivariate regression analysis was used to identify risk factors for bone infection during the perioperative period in patients with open fractures. Receiver operating characteristic (ROC) curves were plotted to evaluate the predictive value of the absolute change (the absolute value of the changes was expressed as △) in the levels of key factors of the TLR/NF-κB signaling axis before and after surgery for bone infection, and these results were compared with the predictive value of routine inflammatory markers. A nomogram prediction model was developed based on the identified risk factors, and its value in predicting perioperative bone infection was analyzed. Results The time from fracture to surgery and the duration of surgery were significantly longer, and the proportion of Gustilo type Ⅲ fractures and wounds with a depth ≥2 cm was significantly higher in the infection group compared to the non-infection group (
4.Characteristics of pruritus in adult patients with psoriasis vulgaris and its effect on quality of life

Objective:To investigate the prevalence and clinical characteristics of pruritus in adult patients with psoriasis vulgaris, and to evaluate the effect of pruritus on sleep and quality of life.Methods:From January 2020 to December 2020, 291 patients with psoriasis vulgaris were enrolled in Lanzhou University Second Hospital, and characteristics of pruritus as well as quality of sleep and life were evaluated by physicians directly or through a questionnaire. Binary logistic regression analysis was performed to analyze risk factors for pruritus, and multivariate linear regression to analyze factors affecting sleep and quality of life.Results:Among the 291 patients with psoriasis vulgaris, 184 (63.23%) were males and 107 (36.77%) were females, with the age ( M [ Q1, Q3]) being 37.00 (28.00, 50.00) years; 258 (88.64%) had the itching symptom, and the severity of itching was usually moderate. The severity of skin lesions was the main risk factor for pruritus ( OR = 1.252, 95% CI: 1.114-1.407, P < 0.001) . Psoriasis area and severity index was positively correlated with pruritus numerical rating scale (NRS) score ( r = 0.65, P < 0.001) , 5-D′S score ( r = 0.65, P < 0.001) , Pittsburgh sleep quality index (PSQI) score ( r = 0.48, P < 0.001) and dermatology life quality index (DLQI) score ( r = 0.63, P < 0.001) , the pruritus NRS score was positively correlated with the DLQI score ( r = 0.61, P < 0.001) and PSQI score ( r = 0.55, P < 0.001) , and the 5-D′S score was positively correlated with the DLQI score ( r = 0.62, P < 0.001) and PSQI score ( r = 0.64, P < 0.001) . Conclusion:Most adult patients with psoriasis vulgaris have moderate pruritus; the severity of disease markedly affects the severity of pruritus, and pruritus exerts considerable impact on sleep and quality of life.

Objective:To investigate the pathological characteristics and the clinical significance of novel coronavirus (2019-nCoV)-infected pneumonia (termed by WHO as coronavirus disease 2019, COVID-19).Methods:Minimally invasive autopsies from lung, heart, kidney, spleen, bone marrow, liver, pancreas, stomach, intestine, thyroid and skin were performed on three patients died of novel coronavirus pneumonia in Chongqing, China. Hematoxylin and eosin staining (HE), transmission electron microcopy, and histochemical staining were performed to investigate the pathological changes of indicated organs or tissues. Immunohistochemical staining was conducted to evaluate the infiltration of immune cells as well as the expression of 2019-nCoV proteins. Real time PCR was carried out to detect the RNA of 2019-nCoV.Results:Various damages were observed in the alveolar structure, with minor serous exudation and fibrin exudation. Hyaline membrane formation was observed in some alveoli. The infiltrated immune cells in alveoli were majorly macrophages and monocytes. Moderate multinucleated giant cells, minimal lymphocytes, eosinophils and neutrophils were also observed. Most of infiltrated lymphocytes were CD4-positive T cells. Significant proliferation of type Ⅱ alveolar epithelia and focal desquamation of alveolar epithelia were also indicated. The blood vessels of alveolar septum were congested, edematous and widened, with modest infiltration of monocytes and lymphocytes. Hyaline thrombi were found in a minority of microvessels. Focal hemorrhage in lung tissue, organization of exudates in some alveolar cavities, and pulmonary interstitial fibrosis were observed. Part of the bronchial epithelia were exfoliated. Coronavirus particles in bronchial mucosal epithelia and type Ⅱ alveolar epithelia were observed under electron microscope. Immunohistochemical staining showed that part of the alveolar epithelia and macrophages were positive for 2019-nCoV antigen. Real time PCR analyses identified positive signals for 2019-nCoV nucleic acid. Decreased numbers of lymphocyte, cell degeneration and necrosis were observed in spleen. Furthermore, degeneration and necrosis of parenchymal cells, formation of hyaline thrombus in small vessels, and pathological changes of chronic diseases were observed in other organs and tissues, while no evidence of coronavirus infection was observed in these organs.Conclusions:The lungs from novel coronavirus pneumonia patients manifest significant pathological lesions, including the alveolar exudative inflammation and interstitial inflammation, alveolar epithelium proliferation and hyaline membrane formation. While the 2019-nCoV is mainly distributed in lung, the infection also involves in the damages of heart, vessels, liver, kidney and other organs. Further studies are warranted to investigate the mechanism underlying pathological changes of this disease.
6. Modularized teaching program on the clinical research training for pathological residents

To meet the requirement of pathological practice and development, we systematically analyzed the situation of pathological residents training and the importance of initiating the clinical scientific research training. Additionally, we proposed the principle and implementation strategy for clinical scientific research training. According to features of pathological practice, we employed the modularized teaching to divide the training courses into several modules: discussion module for clinical pathology, lecture module for advanced research, and training modules for basic scientific theory, technology and writing skill. With these approaches, the systematic and structured system of standardized residents training is implemented to improve the clinical research ability of pathological residents.
7.Efficacy of desloratadine citrate disodium in the treatment of chronic urticaria and its effect on serum interleukin-23, interleukin-33 and PARC/CCL-18

Objective To compare the efficacy of desloratadine citrate disodium versus loratadine in the treatment of chronic urticaria (CU),and to evaluate their effect on serum interleukin (IL)-23,IL-33 and pulmonary and activation-regulated chemokine/CC chemokine ligand 18 (PARC/CCL-18).Methods From January 2013 to December 2016,120 CU patients treated in Department of Dermatology,Wuwei Oncology Hospital were enrolled into this study,and divided into study group and control group by using a random number table.Patients in the study group took oral desloratadine citrate disodium tablets 8.8 mg once a day,and patients in the control group took loratadine tablets 10 mg once a day.The treatment lasted 28 days.The therapeutic effect was compared between the two groups,and changes in serum levels of IL-23,IL-33 and PARC/CCL-18 were compared before and after treatment.Statistical analysis was carried out by using two-sample t test and chi-square test for comparing indices between the two groups.Results The response rate was significantly higher in the study group (88.33%,53/60) than in the control group (61.67% [37/60],x2 =15.352,P < 0.01).After the treatment,the serum levels of IL-23,IL-33 and PARC/CCL-18 in the study group significantly decreased to 87.72 ± 22.16 ng/L,95.94 ± 18.27 ng/L,85.93 ±27.34 μg/L respectively,which were all lower than those in the control group (104.21 ± 32.05 ng/L,106.27 ±20.93 ng/L,95.72 ± 30.28 μg/L,respectively;t =3.264,4.034,3.934,respectively,P =0.020,0.006,0.015,respectively).No significant difference was observed in the incidence of adverse reactions between the study group and control group (P =0.298).Conclusion Desloratadine citrate disodium can markedly improve the clinical symptoms of CU with favorable safety,likely by inhibiting the immune response of the body and reducing the effect of chemokines on the chemotaxis of inflammatory cells.
8.Analysis of risk factors for renal function deterioration in patients with lupus nephritis

Objective To analyze the risk factors for renal function deterioration in patients with lupus nephritis. Methods Clinical data of patients with lupus nephritis admitted to the Third and the First Affiliated Hospital of Zunyi Medical College from June 2013 to June 2015 were reviewed and retrospectively analyzed. Of them ,62 lupus nephritis patients with the rapid deterioration of renal function were selected as an observation group ,and 62 patients were selected randomly from 200 lupus nephritis patients with normal renal function and were considered as a control group.Risk factors for the rapid deterioration of renal function in patients with lupus nephritis were analyzed by univariate analysis. Furthermore ,multivariate Logistic regression analysis was used to identify the independent risk factors for the rapid deterioration of renal function in patients with lupus nephritis. Results Univariate analysis revealed that infection , lupus activity , anemia , hypoproteinemia and hypertriglyceridemia were risk factors for the rapid deterioration of renal function in patients with lupus nephritis (P < 0.05).Multivariate logistic regression analysis showed that infection ,lupus activity and hypoproteinemia were the independent risk factors for the rapid deterioration of renal function in patients with lupus nephritis(OR=3.109 ,3.669 and 2.691 ,P=0.003 ,0.004 and 0.033 , respectively ). Conclusions Infection ,lupus activity and hypoproteinemia are the independent risk factors for the rapid deterioration of renal function in patients with lupus nephritis. Early prevention and intervention can improve clinical outcome and quality of life in lupus nephritis patients.
9.Preparation and Characterization of Scutellarin Nanosuspension

Objective: To prepare scutellarin nanosuspension ( SCU-NS) and study the main influencing factors in the prepara-tion. Methods:The technology parameters were determined, and then the influencing factors of SCU-NS were studied. The optimal formula was confirmed by orthogonal design with zeta potential as the evaluation index. Results: The optimal formula process was as follows:drug amount was 0. 5 g, Pluronic? F68 amount was 0. 1 g, phospholipid amount was 0. 2 g, SDS amount was 0. 05 g and HPMC E5 amount was 0. 05 g. The average particle size and the zeta potential of SCU-NS was (122 ± 4) nm and ( -25. 5 ± 0. 6) mV, respectively. The result of transmission electron microscope showed that SCU-NS was spherical and uniform, and the dissolution of SCU-NS in 30 min was more than 90%. Conclusion:Nanosuspension can significantly enhance the dissolution of SCU.
10.Effects of daily average temperature on the daily number of outpatient visits for eczema in two third-grade class-A hospitals in Lanzhou city

Objective To evaluate effects of the daily average temperature on the daily number of outpatient visits for eczema in Lanzhou city.Methods Clinical data were obtained from outpatients with eczema in the Department of Dermatology of 2 third-grade class-A hospitals in Lanzhou city from January 1st 2007 to December 31st 2015,and meteorological data during this period were also collected.Controlling for confounding factors like long-term trends and day of the week,a distributed lag non-linear model (DLNM) fitted with quasi-Poisson link function was used to assess the effects of daily average temperature on the daily number of outpatient visits for eczema,and the analysis was stratified by season,age and gender.Results The exposure-response relationship between the daily average temperature and daily number of outpatient visits for eczema could be roughly described by a W-shaped curve.Stratification analysis showed that the effect of the daily average temperature on outpatient visits for eczema was strongest in autumn and winter,followed by summer,and weakest in spring.Low temperature may have lagged,cumulative and persistent effects on the daily number of outpatient visits for eczema,with the maximum relative risk (RR) value (1.12 [95% CI:1.03-1.22]) observed at-9 ℃ on lag day 14.With a 1 ℃decrease in the temperature,16% (RR =1.16,95% CI:1.00-1.03),14% (RR =1.14,95% CI:1.02-1.26) and 13% (RR =1.13,95% CI:1.02-1.25) increases in the daily number of outpatient visits for eczema were observed in men,teenagers and middle-aged adults respectively (P < 0.05).However,low temperature had no significant effects on outpatient visits for eczema among women or the elderly (P >0.05).The effect of high temperature usually occurred following exposure without lag periods,and was gradually weakened over lag time (P > 0.05).Conclusions In Lanzhou,the effect of daily average temperature on outpatient visits for eczema was strongest in autumn and winter.Changes of the daily temperature may be one of risk factors for eczema.Low temperature had lagged effects on the daily number of outpatient visits for eczema,and the effects were strongest on lag day 14.
