1.Preliminary efficacy and safety analysis of modified SIOPEL-4 protocol for newly diagnosed pediatric hepatoblastoma with lung metastases

Objective:To assess the preliminary efficacy and safety of modified Société Internationale d′Oncologie Pédiatrique Epithelial Liver Tumor Study Group (SIOPEL)-4 protocol for pediatric hepatoblastoma (HB) with lung metastases.Methods:This prospective cohort study enrolled 27 newly diagnosed pediatric HB with lung metastases who received the modified SIOPEL-4 protocol at Shanghai Children′s Medical Center, Shanghai Jiao Tong University School of Medicine, and Shanghai Children′s Hospital between January 2020 to December 2023. Clinical characteristics, lung response rates to induction chemotherapy, treatment outcomes, prognostic factors and sever chemotherapy toxicities at different stages were analyzed. Survival analysis was performed by Kaplan-Meier method. Univariate prognostic analysis was conducted by Log-Rank test.Results:Of the 27 patients, there were 17 males and 10 females, with the age of 21 (15, 33) months. During the follow-up of 31 (12, 45) months for 17 continuous complete remission patients, 4 cases disease progression (2 cases death) and 6 cases relapse were observed. The 2-year event free survival (EFS) and overall survival (OS) rate was (58±11)% and (89±7)%, respectively. All the 27 patients had response to block 1-3 induction chemotherapy (cisplatin+doxorubicin), with 14 cases (52%) achieving complete response and 13 cases (48%) achieving partial response of lung metastatic lesions, the 2-year EFS rate was (81±12)% and (34±14)%, respectively ( χ 2=6.76, P=0.009), the 2-year OS rate was 100% and (79±13)%, respectively ( χ2=2.12, P=0.145). Patients with caudate lobe tumors or ≥10 pulmonary metastatic nodules had significantly lower EFS rates ( χ2=5.36, 7.84, P=0.021, 0.005, respectively). The incidence of grade 3/4 neutropenia after block 1-3 induction chemotherapy, CD (carboplatin+doxorubicin), and VI (vincristine+irinotecan) consolidation chemotherapy was 90% (73/81), 75% (58/77), and 31% (11/35), respectively. The incidence of grade 3/4 thrombocytopenia was 77% (62/81), 69% (53/77), and 14% (5/35), respectively. The incidence of grade 3/4 infections was 64% (52/81), 25% (19/77), and 20% (7/35), respectively. The differences between the groups were statistically significant ( χ2=43.51, 42.69, 33.00, all P<0.001). Two patients (10%) of the 20 evaluable patients for ototoxicity occurred grade 3 and higher hearing impairment, with 1 patient requiring a hearing aid. Conclusions:The modified SIOPEL-4 regimen shows good preliminary efficacy and safety in treating pediatric HB with lung metastases. The prognosis for patients with residual lesions in the lungs after induction chemotherapy needs to be improved. Attention should be given to the ototoxicity induced by high-dose cisplatin chemotherapy.
2.Predictive value of MRI combined with serum lncRNA KCNQ1OT1, miR-204-5p for axillary lymph node metastasis of breast cancer

Objective:To investigate the value of MRI combined with serum long non-coding RNA (lncRNA) KCNQ1OT1 and microRNA-204-5p (miR-204-5p) in predicting axillary lymph node metastasis in breast cancer patients.Methods:A total of 70 patients with breast cancer who were surgically diagnosed in the No.2 Hospital of Baoding, Hebei Province from January 2020 to December 2022 were selected as the study objects. Patients were divided into metastatic group ( n=31) and non-metastatic group ( n=39) according to the status of lymph node metastasis. The MRI features and the levels of serum lncRNA KCNQ1OT1 and miR-204-5p were compared between the two groups. Receiver operator characteristic (ROC) curve was used to evaluate the predictive efficacy of the three detections alone and in combination for axillary lymph node metastasis in breast cancer. Results:There were statistically significant differences in tumor maximum diameter ( χ2=4.28, P=0.039) and Ki-67 expression ( χ2=10.88, P=0.001) between the metastatic and non-metastatic groups. There were statistically significant differences between the breast cancer axillary lymph node metastatic group and the non-metastatic group in the mode of internal reinforcement type ( χ2=6.60, P=0.037), peripheral vessel diameter ( t=4.33, P<0.001), number of peripheral vessel roots ( t=4.38, P<0.001), apparent diffusion coefficient ( t=3.59, P=0.001), and MRI lymph node status ( χ2=29.70, P<0.001). The level of serum lncRNA KCNQ1OT1 in metastatic group was higher than that in non-metastatic group (1.41±0.32 vs. 0.99±0.18, t=6.94, P<0.001), and the level of miR-204-5p was lower than that in non-metastatic group (0.72±0.17 vs. 1.03±0.21, t=6.66, P<0.001). Bioinformatics analysis showed that miR-204-5p may have a targeting relationship with lncRNA KCNQ1OT1. ROC curve analysis showed the area under the curve of MRI features, serum levels of lncRNA KCNQ1OT1, miR-204-5p and their combined detection to predict lymph node metastasis of breast cancer patients were 0.823, 0.858, 0.843, 0.946, respectively. The predictive efficacy of the combined detection was better than that of MRI features and serum lncRNA KCNQ1OT1 and miR-204-5p levels alone ( Z=3.29, P=0.001; Z=2.07, P=0.038; Z=2.23, P=0.026) . Conclusions:Serum lncRNA KCNQ1OT1 level is up-regulated and the level of miR-204-5p is down-regulated in patients with axillary lymph node metastasis of breast cancer. And the combination of MRI features with serum lncRNA KCNQ1OT1 and miR-204-5p has high predictive efficacy in predicting lymph node metastasis in breast cancer patients.
3.Comparison of the diagnostic value of 3.0 T MRI HASTE sequence and high-frequency color Doppler ultrasound for high-risk placenta previa combined with placenta accreta

Objective To compare the value of high-field strength MRI half-Fourier single shot turbo spin-echo(HASTE)sequence with high-frequency color Doppler ultrasound for the diagnosis of placenta previa combined with placenta accreta.Methods A total of 116 pregnant women with suspected high-risk placenta previa and placenta accreta who underwent cesarean section at The No.2 Hospital of Baoding from December 2019 to December 2022 were selected as research objects.All parturients took examinations of ultrasound and high-field MRI before surgery.Pathological diagnosis after cesarean section was used as the gold standard for analysis of the consistency between MRI and ultrasound results and pathological results.The value of HASTE sequence and high-frequency ultrasound in diagnosing placenta accreta was compared.Results Pathological results confirmed that 86 patients had high-risk placenta previa and placenta accreta.The positive predictive values of ultrasound,MRI,and ultrasound+MRI in diagnosis of high-risk placenta previa combined with placenta accreta were 93.51%,97.62%,and 98.81%,respectively.The negative predictive values were 64.10%,87.50%,and 90.63%,respectively.The accuracies were 83.62%,94.83%,and 96.55%,respectively.The sensitivities were 83.72%,95.35%,and 96.51%,respectively.The specificities were 83.33%,93.33%,and 96.67%,respectively.The Kappa values for consistency were 0.611,0.868,and 0.912,respectively.The accuracy of MRI in diagnosis of high-risk placenta previa combined with placenta accreta was higher than that of ultrasound(P<0.05).Conclusion 3.0 T MRI HASTE sequence has greater efficacy and higher accuracy in diagnosis of high-risk placenta previa combined with placenta accreta.
4.Effects of different feeding patterns on growth performance,blood physiological and biochemical indices and intestinal flora of beef calves

The aim of this experiment was to study the effects of different feeding modes on growth performance,blood biochemical indexes and intestinal flora of lactating Holstein male calves.Twenty-four newborn Holstein male calves with body mass of(40.00±1.01)kg and similar day old were selected and randomly divided into four groups of six calves each.The subgroups were low-milk group(LM),high-milk group(HM),high-milk milk replacer feeding group(HMR),and low-milk switching to high-milk milk replacer feeding group(CMR).The results showed that:At 45 d,the body mass of calves in the HM group was significantly higher than that of calves in the other groups(P<0.05),and at 60 d,the body mass of calves in the HM group was significantly higher than that of calves in the LM &.CMR groups(P<0.05).At 90 d,the body mass of calves in the LM group was significantly higher than that of calves in the HM group.Throughout the ex-perimental period,the average daily weight gain and average pellet feed intake of calves in the LM group were significantly higher than that of calves in the HM group(P<0.05).The calf globulin level in the HMR group was significantly higher than that in the LM and HM groups(P<0.05);the plasma immunoglobulin A level of calves in the HM group was significantly lower than that of calves in the LM and HMR groups(P<0.05);and the plasma immunoglobulin M level of calves in the HM group was significantly higher than that of calves in the LM and CMR groups(P<0.05),and HMR group was also significantly higher than that of LM group(P<0.05);plasma glutathione peroxidase level of calves in HMR group was significantly higher than that of LM group(P<0.05);plasma malondialdehyde level of calves in LM group was significantly higher than that of calves in HMR and HM groups(P<0.05),and CMR group was also significantly higher than that of HM group(P<0.05).Relative abundance of Thermodesulfovibrio was higher in the HM group(P<0.05),relative abundance of Bacteroidetes in the LM group was significantly higher than that in the HMR and HM groups(P<0.05),relative abundance of Blautia in the HM group(P<0.05),and relative abundance of Corynebacterium in the CMR group was significantly higher than that in the LM and HM groups(P<0.05).In summary,calves in the LM group had better weaning weights and pellet feed intake;calves in the CMR group could compensate for growth by supplemental feeding of milk replacer to obtain more optimal weaning weights and pel-let feed intake;the HMR group proved that milk-free feeding could ensure stable growth of calves;and calves in the HM group had a better pre-lactation growth performance,lower levels of oxida-tive stress,and a healthier fecal flora.

7.Screening of quantitative preparation quality markers of Zhibao Sanbian Wan

On the basis of the qualitative preparation quality markers of Zhibao Sanbian Wan (ZBSBW), we screened out the quantitative markers and evaluated the content consistency of ZBSBW. A method capable of simultaneously determining 34 compounds in ZBSBW was established based on HPLC-MS/MS, and 16 batches of ZBSBW were simultaneously analyzed by this method. Furthermore, we explored a general strategy for analyzing the component migration in preparation, plasma, brain tissue and cerebrospinal fluid. The methodological investigation was confirmed by linear range, recovery (85.10%-105.07%), precision (RSD: 1.37%-4.58%), stability, and repeatability (3.00%-12.45%), the established method was suitable for the detection and quantification of the compounds in ZBSBW. The contents of compounds in ZBSBW were all lower than 1 mg·g-1, and the contents and daily dose of nystose were the highest, followed by echinacoside, paeoniflorin, osthole and paeonol. The results of systematic clustering showed that the contents were consistent for ordinary preparations of ZBSBW. The principal component analysis showed that the components of berberine, ginsenoside Re, ginsenoside Rg1, pinoresinol diglucoside and tenuifolin had large variation, which contributed significantly to the grouping. 8.The clinical efficacy of Salvia Miltiorrhiza Injection on prevention splenectomy surgery of deep vein thrombosis

Objective To study the clinical efficacy of Salvia Miltiorrhiza Injection on prevention splenectomy surgery of deep vein thrombosis (DVT). Methods 132 patients who would received splenectomy were selected as an object of study from June 2011 to June 2013. They were divided into the treatment group and the control group by randomizing. The two groups were surgery treated with the same methods and other postoperative. The treatment group received intravenous drip of Salvia Miltiorrhiza Injection after 6 hours once daily for 10 consecutive days. The control group received subcutaneous low molecular weight heparin like the time of the treatment group. Results There was a difference in the incidence of DVT between the two groups (P < 0.05). Conclusion It is effective that Salvia Miltiorrhiza injection in preventing deep vein thrombosis after splenectomy.
9.Chemical constituents of flowers from Polygonum orientale.

OBJECTIVETo study the chemical constituents in the active portion from the flowers of Polygonum oriental.
METHODChromatographic techniques were employed for isolation and purification of the constituents and their structures were determined by spectral analysis and chemical evidence.
RESULTNine compounds were obtained and identified as alphitonin (1), methyl 3, 4-dihydroxybenzoate (2), apocynin (3), kaempferol-3-O-beta-D-glucoside (4), 1,3,5-trihydroxybenzene (5), 3,3'-dimethoxyellagic-acid-4-O-beta-D-glucoside (6), kaempferol-3-O-alpha-L-rhamnoside (7), quercetin-3-O-alpha-L-rhamnoside (8), kaempferol (9).
CONCLUSIONCompounds 2, 4, 5 were isolated from P. oriental for the first time and compounds 1, 3 were firstly obtained from the genus Polygonum.

10.Determination of catalpol in dried rehmannia root and Taohong Siwu Decoction with high performance liquid chromatography

OBJECTIVE: To determinate the catalpol contents in dried rehmannia root and Taohong Siwu Decoction containing rehmannia root with high performance liquid chromatography (HPLC). METHODS: Catalpol was separated on a YWG-C18 column using water-acetonitrile (99.4:0.6) as mobile phase and detective wavelength at 210 nm. RESULTS: The linear curve of tested catalpol concentration within the range of 0.0536-5.3600 microg/microl was ideal (n=5, r=0.999 7). The average recovery rate of the dried rehmannia root and Taohong Siwu decoction was 98.7% (RSD=0.48%) and 98.2% (RSD=1.29%) respectively. CONCLUSION: HPLC method is accurate and valuable for the quality control of Radix Rehmanniae and Taohong Siwu Decoction.
