Objective To construct a home care readiness program for caregivers of patients with chronic heart failure based on the McMaster Family Functioning Model and explore its application effects.Methods The pro-gram was developed through literature analysis,expert correspondence and pre-testing.86 pairs of patients with chronic heart failure and their caregivers who were admitted to the Department of Cardiology of a tertiary-level hos-pital in Bengbu City were conveniently selected from December 2023 to April 2024 as the study subjects.Using the coin-flip method,the patients were randomly divided into 43 pairs in an experimental group and 43 pairs in a control group on a ward-by-ward basis.The experimental group implemented the program on the basis of routine care,and the control group was given routine care.Differences in caregiver home care readiness,caregiver caregiv-ing ability and patients' quality of life were compared between the 2 groups at the time of admission,at the time of discharge,and 1 and 3 months after discharge.Results A total of 81 pairs of patients and their caregivers completed the intervention,with 41 in the experimental group and 40 in the control group.Repeated-measures ANOVA showed that there was an interaction effect between time and group for caregiver home care readiness,caregiver caregiving ability,and patient quality of life in both groups(F=320.995,P＜0.001;F=83.303,P＜0.001;F=113.283,P＜0.001).Simple effect analyses showed that caregiver home care readiness and caregiver caregiving com-petence scores were higher in the experimental group than those in the control group at the time of discharge and at 1 and 3 months post-discharge(P＜0.001);quality of life scores of patients in the experimental group were lower than it in the control group at 1 and 3 months post-discharge(P＜0.001).Conclusion The implemen-tation of the program can effectively improve the home care readiness and caregiving ability of caregivers of pa-tients with chronic heart failure,and improve the quality of life of patients.

Objective MiR-375 is lowly expressed in esophageal squamous cancer cells and the downstream target gene of miR-375 remains unclear .This paper discusses the role of miR-375 in regulating the expression of short stature homobox 2 ( SHOX2) in human esophageal squamous cancer cells . Methods The bioinformatics software TargetScan , miRanda, PicTar, miRTarget2, and PITA were used to predict the assumptive targets of miR-375 in SHOX2.Then, two recombinant luciferase gene report plasmids containing wild pSHOX2 3′UTR ( pSHOX2-375-WT ) and mutant pSHOX2 3′UTR ( pSHOX2-375-mut ) were constructed , sequenced , and identified.Human esophageal squamous cancer cells were co-transfected with miR-375 mimics and pSHOX2-375-WT or pSHOX2-375-mut, respectively , and divided into 7 groups: pmiR, pSHOX2-375-WT, pSHOX2-375-WT +miR-375, pSHOX2-375-WT +miR-NC, pSHOX2-375-mut, pSHOX2-375-mut+miR-375, and pSHOX2-375-mut+miR-NC, each subjected to the measurement of luciferase activity .The expressions of SHOX 2 mRNA and protein were de-termined after transfection of the esophageal squamous cancer cells with miR-375 mimics, and so were the expressions of miR-375 and SHOX2 in the esophageal squamous carcinoma tissue samples obtained postoperatively . Results Prediction with the five software showed only one conserved function site of miR-375 in SHOX2 3′UTR at 1156-1170 bp.Luciferase activity was significantly lower in the pSHOX2-375-WT+miR-375 group (0.261 ±0.036) than in the pmiR (1.818 ±0.061), pSHOX2-375-WT (1.820 ±0.086), pSHOX2-375-WT+miR-NC (1.851 ±0.094), pSHOX2-375-mut (1.861 ±0.059), pSHOX2-375-mut +miR-375 (1.896 ± 0.048), and pSHOX2-375-mut+miR-NC group (1.760 ±0.062) ( P<0.01).SHOX2 mRNA and protein expressions were sup-pressed by the overexpression of miR-375 in the EC9706 cells.The expression of miR-375 was decreased, while that of SHOX2 in-creased in the esophageal squamous carcinoma tissue as compared with the normal esophageal tissue . Conclusion MiR-375 regu-lates the expression of the SHOX 2 gen in esophageal squamous cancer cells .

Objective To investigate the diagnostic value of multiplanar reformation (MPR)reconstruction for the detection of traumatic diaphragmatic rupture (TDR) in multi-slice CT examination.Methods Thirty six cases with thoracoabdominal trauma, including 21 cases with and 15 cases without TDR confirmed by surgery, received multi-slice CT examination. They were enrolled in this study. Three experienced radiologists retrospectively analyzed the axial and MPR images. The diagnostic criteria for TDR included abnormally elevated hemidiaphragm, diaphragmatic discontinuity, the "collar sign" or "dependent viscera "sign. Referenced to surgical results, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of axial and MPR images in detection of TDR were calculated. The McNemar was used to investigate the differences between axial and MPR images in the detection of diaphragmatic discontinuity and "collar sign", and the differences between axial and MPR images of these two signs in TDR diagnosis. Results The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of axial images in detection of TDR were 71% ( 15/21 ), 80% ( 12/15 ), 83% ( 15/18 ),67% ( 12/18 ) and 75% ( 27/36 ), respectively; of MPR images, they were 86% ( 18/21 ), 93%(14/15), 95% ( 18/19 ), 82% ( 14/17 ) and 89% ( 32/36), respectively. By axial images, twelve diaphragmatic defects or interrupts were identified in nine cases, and "collar sign" was identified in six cases. By MPR, 20 diaphragmatic defects or interrupts were identified in 15 cases ( P = 0.125 ), and "collar sign" was identified in 14 cases (P =0.021 ). The sensitivity and specificity of diaphragmatic defects or interrupts for TDR diagnosis in axial images were 43% (9/21) and 80% ( 12/15 ), respectively;in MPRimages, they were71% (15/21) (P=0.125)and93% (14/15) (P=0.500), respectively.The sensitivity and specificity of "collar sign" for TDR diagnosis in axial images were 29% (6/21) and 100% ( 15/15), respectively; in MPR images, they were 67% ( 14/21 ) (P =0. 021 ) and 100% (15/15)( P = 1.000), respectively. Conclusions MSCT presented good sensitivity, specificity and accuracy for the diagnosis of TDR. MPR images were useful supplements for axial images in TDR diagnosis which improved the diagnosis.
