1.Effects of adalimumab treatment on miR-141,miR-335-5p and bone metabolism in patients with rheumatoid arthritis

Objective To analyze the effects of adalimumab treatment on serum micro ribonucleic acid-141(miR-141),micro ribonucleic acid-335-5p(miR-335-5p),and bone metabolism indexes in patients with rheumatoid arthritis(RA),and to provide a basis for the rational development of clinical treatment plans.Methods 82 cases of patients with RA admitted to our hospital from April 2021 to March 2024 were selected as study subjects,and they were divided into the methotrexate group(41 cases)and the adalimumab group(41 cases)by randomized numerical table method.The methotrexate group was treated with methotrexate tablets orally,and the adalimumab group was treated with adalimumab injection subcutaneously on the basis of methotrexate group,and both groups continued the treatment for 3 months.The efficacy after 3 months of treatment,symptom improvement and adverse reactions during the treatment period were counted;enzyme-linked immunosorbent assay detected the levels of inflammatory response,serum ACPA,RF and bone metabolism indexes before and after 3 months of treatment;and fluorescence quantitative PCR assay detected serum miR-141,miR-335-5p before and after 3 months of treatment.Results The total effective rate after 3 months of treatment was higher in the adalimumab group(90.24%)than in the methotrexate group(73.17%)(P<0.05).Compared with before treatment,the duration of morning stiffness was shorter in both groups after 3 months of treatment,and it was shorter in the adalimumab group than in the methotrexate group(P<0.05);pressure pain,number of swollen joints,scores of visual analog score(VAS),28 joints disease activity(DAS28),levels of serum interleukin-22(IL-22),interleukin-17(IL-17),C-reactive protein(CRP),tumor necrosis factor α(TNF-α),miR-141,anti-cyclic citrullinated peptide antibody(ACPA),rheumatoid factor(RF)decreased in both groups,and decreased in the adalimumab group compared with the methotrexate group(P<0.05);levels of serum miR-335-5p,25 hydroxyvitamin D,osteocalcin(BGP),osteocalcin N-terminal intermediate molecular fragment(N-MID),and bone alkaline phosphatase(B-ALP)were elevated in both groups,and increased in the adalimumab group compared with the methotrexate group(P<0.05).The overall incidence of adverse reactions during treatment was lower in the adalimumab group(7.32%)than in the methotrexate group(24.39%)(P<0.05).Conclusion Adalimumab could regulate the levels of inflammatory response indicators,serum miR-141,miR-335-5p,ACPA,RF,and bone metabolism indicators in patients with RA,and promote the improvement of symptoms,which in turn contributes to the improvement of therapeutic efficacy and had a high safety profile.
2.Clinical Study on Correlation of Early Serum Inflammatory Mediator Levels With Disease Severity and Intestinal Dysfunction in Acute Pancreatitis Patients

Background:Acute pancreatitis(AP)is a common clinical gastrointestinal emergency with a high mortality rate.Intestinal dysfunction is one of the common complications of AP and can aggravate the disease condition.Therefore,early and accurate assessment of the disease severity and intestinal dysfunction of AP is of great significance for improving the prognosis of patients.Aims:To investigate the correlation of early serum levels of interleukin-1β(IL-1β),C-reactive protein(CRP),endotoxin(ET),D-lactic acid(D-LA),and diamine oxidase(DAO)with disease severity and intestinal dysfunction in AP patients.Methods:A total of 52 AP patients admitted to the Second Affiliated Hospital of Baotou Medical College from September 2023 to September 2024 were enrolled,and 10 healthy individuals recruited during the same period were served as the control group.AP patients were classified into mild acute pancreatitis(MAP),moderately severe acute pancreatitis(MSAP)and severe acute pancreatitis(SAP)groups according to the severity of the disease.Intestinal function was evaluated using the acute gastrointestinal injury(AGI)grading system.ELISA was used to detect the early serum levels of IL-1β,CRP,ET,D-LA,and DAO,and the correlation of these indicators with the severity of AP and AGI grade were analyzed by Spearman correlation coefficient.Results:Serum levels of IL-1β,CRP,ET,D-LA,and DAO in the MAP,MSAP,and SAP groups were significantly higher than those in the control group(all P<0.05);serum levels of IL-1β,CRP,ET,D-LA,and DAO in the MSAP group were significantly higher than those in the MAP group(all P<0.05);serum levels of IL-1β,CRP,ET,D-LA,and DAO in the SAP group were significantly higher than those in the MAP and MSAP groups(all P<0.05).Compared with AGI grade 0-Ⅰ subjects,serum levels of IL-1β,CRP,ET,D-LA,and DAO in patients with AGI grade Ⅱ,Ⅲ and Ⅳ were significantly increased(all P<0.05).Compared with AGI grade Ⅱ patients,serum D-LA level in patients with AGI grade Ⅲ was significantly increased(P<0.05).Compared with AGI grade Ⅲ patients,serum D-LA level in patients with AGI grade Ⅳ was significantly increased(P<0.05).Serum levels of IL-1β,CRP,ET,D-LA,and DAO were positively correlated with the severity of AP and AGI grade(all P<0.05).Conclusions:Early serum levels of IL-1β,CRP,ET,D-LA,and DAO in AP patients are significantly increased and closely related to the disease severity and intestinal dysfunction.
3.The value of multiparametric MRI in the composition assessment of benign prostatic hyperplasia

Objective To investigate the application value of conventional MRI combined with diffusion tensor imaging(DTI)in evaluating the correlation between the texture composition of benign prostatic hyperplasia(BPH)and the International Prostate Symptom Score(IPSS).Methods Seventy patients with BPH confirmed by pathology were retrospectively analyzed and all patients underwent conventional MRI,DTI and IPSS before surgery.Evaluation metrics included:the mean signal intensity of T2WI(mean-SI-T2WI),apparent diffusion coefficient(ADC)and fractional anisotropy(FA)values.Independent samples t-test,partial correlation analysis,and receiver operating characteristic(ROC)curve were used to assess the correlation between the texture parameters of the prostate transition zone and IPSS.Results The mean-SI-T2WI was significantly negatively correlated with IPSS(r=-0.683,P<0.001);the average ADC value was slightly negatively correlated with IPSS(r=-0.467,P<0.001);and the average FA value was slightly positively correlated with IPSS(r=0.419,P<0.001).The predictive value of MRI texture parameters for IPSS in BPH patients,ranked from high to low,mean-SI-T2WI[area under the curve(AUC)=0.734],average ADC value(AUC=0.673),and average FA value(AUC=0.635);However,the combination of mean-SI-T2WI+ADC+FA(AUC=0.791)did not significantly improve the diagnostic efficacy by DeLong's test(P>0.05).Conclusion Mean-SI-T2WI,DWI and DTI can be used to evaluate the composition of the prostate,among which mean-SI-T2WI is the best,and the com-bination of them can not improve the diagnostic efficacy.

7.The influence of cumulative fluid balance on volume status in patients with heart failure

Objective To analyze the correlation between cumulative fluid balance volume and increasing plasma volume and aggravated clinical congestion symptoms in patients with heart failure on admission for 1~7 d,and to explore the application value of cumulative fluid balance volume in predicting volume overload.Methods Using the convenience sampling method,235 heart failure patients hospitalized from October 2022 to February 2023 in a total of 3 tertiary hospitals in Nanjing,Lianyungang,Jiangsu Province,and Zhumadian,Henan Province,were selected and classified into an increasing/decreasing plasma volume group and an aggravated/alleviated clinical congestion symptoms group.General information,clinical characteristics,and 1~7 d cumulative fluid balance volume of the study subjects were collected to compare the differences in cumulative fluid balance volume between the 2 groups.Logistic regression was used to analyze the effect of cumulative fluid balance volume on plasma volume and clinical congestion symptoms.Receiver operating characteristic curves were used to analyze the optimal cutoff value of cumulative fluid balance volume for predicting increasing plasma volume and aggravated clinical congestion symptoms.Results Excluding 15 pat ients who were discharged early and 3 patients with inaccurate records of fluid intake and excretion,a total of 217 patients were included.The incidence of positive fluid balance was lowest on day 1,at 65.90%,with the smallest cumulative volume of(0.235±0.983)L;the highest incidence of positive balance occurred on day 6,at 75.58%,with the largest cumulative volume of(2.444±5.445)L.The cumulative fluid balance at 4~7 d in the increasing plasma volume group was higher than that in the decreasing plasma volume group,and the difference was statistically significant(P<0.05);the cumulative fluid balance at 4~7 d was an independent risk factor for plasma volume elevation,and a cumulative fluid balance of 2.308,3.361,3.518,and 3.702 L at 4~7 d was predictive of a plasma volume elevation,and areas under receiver operating characteristic curve were 0.686,0.721,0.647,and 0.766,respectively.The cumulative fluid balance for 4~7 d in the aggravated clinical congestion symptoms group was higher than that in the alleviated clinical congestion symptoms group,and the difference was statistically significant(P<0.05);the cumulative fluid balance for 4~7 d was an independent risk factor for the aggravated clinical congestion symptoms,and a cumulative fluid balance for 4~7 d of 2.574,3.383,4.995,and 4.235 L predicted aggravated clinical congestion symptoms,and area under receiver operating characteristic curve was 0.640,0.693,0.654,and 0.720,respectively.Conclusion The incidence of cumulative positive fluid balance in heart failure patients is high,and the amount of cumulative fluid balance can better predict the occurrence of volume overload,and the increase of plasma volume precedes the appearance of clinical congestion symptoms.It is suggested that heart failure patients with more than 3 d of cumulative positive balance should be closely monitored,and when the cumulative positive balance exceeds 2.308 L,measures should be taken in time to drain excessive fluid,so as to avoid the incidence of volume overload.

Objective:To analyze and compare the health education efforts of respiratory infectious diseases at home and abroad.Methods:The literature related to health education and popular science of respiratory infectious diseases included in the databases of China National Knowledge Infrastructure (CNKI) and Web of Science (WOS) from January 1, 2003 to December 31, 2023 was searched. A total of 4 686 articles were retrieved in CNKI, 1 540 articles unrelated to the theme were excluded, and 3 146 Chinese articles were obtained. In the WOS database, 7 724 articles were retrieved, 3 685 articles about the clinical mechanism of diseases, pharmacology, and other research topics were excluded, and 4 039 English articles were obtained. The information of annual publications, institutions, authors and keywords was analyzed by using CiteSpace visualization software, and the publication status, research hotspots and development trends of health education related to respiratory infectious diseases at home and abroad were analyzed.Results:Since 2003, the number of publications in health education on respiratory infectious diseases at home and abroad had shown a fluctuating growth trend, and in 2020, the field showed a sharp growth trend at home and abroad. There was no core author group in this field in China, and the network density of domestic authors was 0.006 5, and the network density of foreign authors was 0.009 6. The domestic institutions were mainly the Center for Disease Control and Prevention and medical institutions, including Guangzhou Chest Hospital (29 articles) and the Chinese Center for Disease Control and Prevention (10 articles); Foreign research institutions were mainly higher education institutions, including the University of London (91 articles) and Harvard University (67 articles). The network density of domestic was 0.001 3, and the network density of foreign publishing institutions was 0.026 3, the network density was greater than that of Chinese publishing institutions. The emergence of "COVID-19" "Avian influenza" "Knowledge, Attitude, Practice" and "mental health" in China had strong burst (burst intensity: 46.41, 12.12, 10.33, 8.5); "Coronavirus" "coverage" "Avian influenza" and "COVID-19 vaccine" in foreign countries had strong burst (burst intensity: 14.34, 11.06, 10.73, 10.02).Conclusions:At present, the health education of respiratory infectious diseases at home and abroad has received great attention. But the cooperation between domestic authors and research institutions is loose, and the close collaboration needs to be strengthened. There are differences in the research focus of health education on respiratory infectious diseases at home and abroad.

Among them, the open avulsion injury has the highest incidence and is often accompanied by multiple tissue injuries. Therefore, improper diagnosis and treatment may cause skin and soft tissue defects of the lower leg combined with bone exposure or even bone defects, seriously impairing patients′ life and health. In order to have a better understanding of the open skin avulsion injury of the lower leg, achieve precise diagnosis and treatment and improve prognosis, the Chinese Society of Traumatology of Chinese Medical Association and the Chinese Association of Microsurgeons of Chinese Medical Doctor Association organized experts in the related fields to formulate Clinical guideline for the diagnosis and treatment of open skin avulsion injuries of the lower leg ( version 2024) based on evidence-based medicine principles. A total of 16 recommendations were proposed on the diagnosis, treatment, postoperative rehabilitation of open lower leg skin avulsion injury, so as to provide a reference for its diagnosis and treatment.
