1.Acupoint application of seed of Chinese dodder at Sanyinjiao acupoint improves pregnancy outcome and Th1/Th2 balance at maternal-fetal interface in recurrent abortion rat

Objective:To explore the improvement effect of acupoint application of seed of Chinese dodder at Sanyinjiao acu-point on pregnancy outcome in rats with recurrent abortion,and its influence on the balance of Th1/Th2 at maternal-fetal interface.Methods:SD female pregnant rats were randomly divided into normal pregnant rats group,abortion model group,acupoint application of seed of Chinese dodder group,gavage of seed of Chinese dodder group,acupoint application of starch group,with 15 rats in each group.Except for normal pregnant rats group,pregnant rats of other groups were injected subcutaneously with 0.3 mg/kg bromocriptine on 6~8 days of gestation,for once a day,the abortion model was established,the treatment was given from the first day of pregnancy,and the blood was taken from the abdominal aorta on the 12th day of pregnancy to detect levels of serum prolactin(PRL)and progester-one(P);pregnant rats were sacrificed,the uterus was taken out,the embryo abortion rate was calculated and the uterus weight was weighed to evaluate changes of pregnancy outcome of pregnant rats;the decidua tissue was taken,and the positive expression of pro-gesterone receptor(PR)was detected by immunohistochemical method;Th1 type(TNF-α,IFN-γ)/Th2 type(IL-4,IL-10)cytokines mRNA and protein expressions in decidua tissue were detected by RT-PCR and Western blot.Results:Compared with normal pregnant rats group,abortion model group had an increased embryo abortion rate,and decreased uterine weight,progesterone PRL,P and PR expressions,the decidual tissue Th1 type(TNF-α,IFN-γ)cytokines mRNA and protein expressions were increased,while Th2 type(IL-4,IL-10)cytokines mRNA and protein expressions were decreased(P<0.05).Acupoint application of seed of Chinese dodder and gavage of seed of Chinese dodder could reduce the abortion rate,increase expressions of PRL,P and PR,and correct the Th1/Th2 imbalance,and the therapeutic effect of acupoint application of seed of Chinese dodder is better than that of gavage of seed of Chinese dodder(P<0.05).Conclusion:Application of the seed of Chinese dodder at Sanyinjiao acupoint can improve the adverse pregnancy outcome of abortion model rats,correct Th1/Th2 imbalance,and reduce the abortion rate,and its effect is better than gavage of seed of Chinese dodder.
2.Value of endoscopic retrograde cholangiopancreatography for the diagnosis and treatment of pediatric pancreaticobiliary maljunction

Objective:To investigate the safety and effectiveness of endoscopic retrograde cholangiopancreatography (ERCP) for the diagnosis and treatment of pediatric pancreaticobiliary maljunction (PBM).Methods:Data of 40 pediatric patients under 14 with PBM diagnosed and treated by ERCP at Department of Gastroenterology, Nanjing Drum Tower Hospital, the Affiliated Hospital of Nanjing University Medical School from November 2012 to September 2022 were collected. PBM types, ERCP-related diagnosis and treatment, adverse events and prognosis were retrospectively analyzed.Results:Nineteen cases were P-B type (joining of common bile duct with pancreatic duct), 17 were B-P type (joining of pancreatic duct with common bile duct), and 4 were complex type. Forty children with PBM underwent 50 ERCP-related operations, among which 48 procedures succeeded. One case failed during cannulation of ERCP, replaced by rendezvous-assisted endoscopic retrograde pancreatography (RV-ERP) afterwards. There were no serious postoperative adverse events such as bleeding, perforation or death. Thirty-four patients (85%) were followed up successfully, among which 14 underwent further surgery and 20 continued conservative treatment.Conclusion:ERCP is the golden standard to diagnose pediatric PBM, and it is also safe and effective treatment for PBM.
3.Epidemiological investigation of iron deficiency among preschool children in 10 provinces, autonomous regions, or municipalities in China

Objective:To understand the current status of anemia, iron deficiency, and iron-deficiency anemia among preschool children in China.Methods:A cross-sectional study was conducted with a multi-stage stratified sampling method to select 150 streets or townships from 10 Chinese provinces, autonomous regions, or municipalities (East: Jiangsu, Zhejiang, Shandong, and Hainan; Central: Henan; West: Chongqing, Shaanxi, Guizhou, and Xinjiang; Northeast: Liaoning). From May 2022 to April 2023, a total of 21 470 children, including community-based children aged 0.5 to<3.0 years receiving child health care and kindergarten-based children aged 3.0 to<7.0 years, were surveyed. They were divided into 3 age groups: infants (0.5 to<1.0 year), toddlers (1.0 to<3.0 years), and preschoolers (3.0 to<7.0 years). Basic information such as sex and date of birth of the children was collected, and peripheral blood samples were obtained for routine blood tests and serum ferritin measurement. The prevalence rates of anemia, iron deficiency, and iron-deficiency anemia were analyzed, and the prevalence rate differences were compared among different ages, sex, urban and rural areas, and regions using the chi-square test.Results:A total of 21 460 valid responses were collected, including 10 780 boys (50.2%). The number of infants, toddlers, and preschoolers were 2 645 (12.3%), 6 244 (29.1%), and 12 571 (58.6%), respectively. The hemoglobin level was (126.7±14.8) g/L, and the serum ferritin level was 32.3 (18.5, 50.1) μg/L. The overall rates of anemia, iron deficiency, and iron-deficiency anemia were 10.4% (2 230/21 460), 28.3% (6 070/21 460), and 3.9% (845/21 460), respectively. The prevalence rate of anemia was higher for boys than for girls (10.9% (1 173/10 780) vs. 9.9% (1 057/10 680), χ2=5.58, P=0.018), with statistically significant differences in the rates for infants, toddlers and preschoolers (18.0% (475/2 645), 10.6% (662/6 244), and 8.7% (1 093/12 571), respectively, χ2=201.81, P<0.01), and the rate was significantly higher for children in rural than that in urban area (11.8% (1 516/12 883) vs. 8.3% (714/8 577), χ2=65.54, P<0.01), with statistically significant differences in the rates by region ( χ2=126.60, P<0.01), with the highest rate of 15.8% (343/2 173) for children in Central region, and the lowest rate of 5.3% (108/2 053) in Northeastern region. The prevalence rates of iron deficiency were 33.8% (895/2 645), 32.2% (2 011/6 244), and 25.2% (3 164/12 571) in infants, toddlers, and preschoolers, respectively, and 30.0% (3 229/10 780) in boys vs. 26.6% (2 841/10 680) in girls, 21.7% (1 913/8 821), 40.0% (870/2 173), 27.1% (2 283/8 413), 48.9% (1 004/2 053) in Eastern, Central, Western, and Northeastern regions, respectively, and each between-group showed a significant statistical difference ( χ2=147.71, 29.73, 773.02, all P<0.01). The prevalence rate of iron-deficiency anemia showed a significant statistical difference between urban and rural areas, 2.9% (251/8 577) vs. 4.6% (594/12 883) ( χ2=38.62, P<0.01), while the difference in iron deficiency prevalence was not significant ( χ2=0.51, P=0.476). Conclusions:There has been a notable improvement in iron deficiency and iron-deficiency anemia among preschool children in China, but the situation remains concerning. Particular attention should be paid to the prevention and control of iron deficiency and iron-deficiency anemia, especially among infants and children in the Central, Western, and Northeastern regions of China.
4.Progress on associations between intrauterine cigarette smoke exposure and cognitive and behavioral development

Intrauterine cigarette smoke exposure(ICSE)refers to a condition under which pregnant women actively inhale and/or passively inhale cigarettes and smogs mostly consisting of nicotine, tar and carbon monoxide during the first, second and third trimesters of pregnancies.As an adverse environmental factor in early life, ICSE is associated with increased risks of various diseases at childhood and adulthood.ICSE is closely related with children′s cognitive and behavioral developmental disorders.It has been reported that ICSE led to elevated risks of cognitive impairments including disorders in fine motor skills, language and vocabulary, reading comprehension, matrix reasoning, working memory, learning skills, orientation and other capabilities.Additionally, it is associated with increased risks of attention deficit hyperactivity disorder and sleep disorder, but the association with autism spectrum disorder is still controversial.The review focuses on the effects of ICSE on cognitive and behavioral development, and summarizes the underlying mechanisms, providing more clinical ideas for etiological studies of cognitive and behavioral developmental disorders in children.
5.Study on the toxicity of the extract from Miao medicine Wikstroemia indica to zebrafish

OBJECTIVE To study the toxicity of the extract from Mi ao medicine Wikstroemia indica to zebrafish . METHODS Zebrafish embryo model was used as the object ,after exposure to W. indica extract (10,20,40 μg/mL),the number of spontaneous twitching within 1 min,heart rate within 10 s,the occurrence of malformation and death were detected and recorded . Zebrafish was used as the object ,after exposure to W. indica extract(10-100 μg/mL)for 24,48 and 72 h,and the median lethal concentration(LC50)of W. indica extract to zebrafish at different time points were calculated . After exposure to low ,medium and high concentration (27,37,51 μg/mL)of W. indica extract,the liver phenotype ,hepatocyte apoptosis and lipid deposition of zebrafish were observed ,and the activities of alanine aminotransferase (ALT),aspartate aminotransferase (AST)and lactate dehydrogenase(LDH)in liver tissue were detected . RESULTS Compared with blank group ,the number of spontaneous twitching , malformation rate (except for 10 μg/mL group )and mortality of embryos increased significantly in 10,20,40 μg/mL groups of W. indica extract,and the heart rate (except for 10,20 μg/mL group )of embryos decreased significantly (P<0.05). LC50 of W. indica extract to zebrafish at 24,48 and 72 h were 39.850,28.300 and 21.490 μg/mL,respectively. After drug treatment ,the transparency of liver area of zebrafish in low ,medium and high concentration groups of W. indica extract reduced and their shape were enlarged;apoptosis and lipid deposition increased ;the activities of ALT ,AST and LDH in liver tissue were significantly increased (P<0.05). CONCLUSIONS Miao medicine W. indica extract had develop mental toxicity to zebrafish embryos and he patotoxicity to zebrafish .
6.The application of peripheral serum interleukin-6 levels in predicting the recurrence of early hepatocellular carcinoma after receiving microwave ablation

Objective To investigate the clinical application of serum interleukin-6 (IL-6)and interleukin-22 (IL-22) levels in predicting the recurrence of hepatitis B virus (HBV)-related early hepatocellular carcinoma (HCC) after receiving microwave ablation (MWA).Methods Preoperative peripheral blood samples were collected in 49 patients with early-stage HBV-related HCC,and serum concentrations of IL-6 and IL-22 were measured by using ELISA.Thirty healthy volunteers were recruited and used as the control group.The xtile software was used to define the best cut-off value,and the IL-6 and IL-22 levels were divided into highlevel group and low-level group.The tumor-free survivals of high-level and low-level groups were analyzed with Kaplan-Meier analysis,log rank test was adopted to determine the difference,and Cox regression model was employed to screen the risk factors affecting HBV-related HCC recurrence.Results The serum IL-6 and IL-22 levels of HCC group were 13.20 pg/ml (11.87-15.79 pg/ml) and 42.18 pg/ml (34.39-57.44 pg/ml) respectively,which were significantly higher than 10.47 pg/ml (9.50-13.82 pg/ml) and 25.45 pg/ml (22.31-30.12 pg/ml) of the control group (P=0.001 and P<0.001 respectively).Kaplan-Meier analysis revealed that preoperative lower IL-6,higher total bilirubin and lower albumin levels indicated a shorter disease-free survival (DFS),and IL-22 levels had no statistically significant effect on the recurrence of HCC.Cox regression multivariate analysis showed that lower serum IL-6 level (≤ 13.2 pg/ml;hazard ratio=3.721;95% CI=1.674-8.272;P=0.001) and lower serum albumin level (≤41.0 g/L;hazard mtio=2.085;95%CI=1.101-3.950;P=0.024) were independent risk factors affecting HBV-related HCC recurrence Conclusion Preoperative serum IL-6 level and serum albumin level can be used as the predictors of HCC recurrence in patients with HBV-related early HCC who are receiving MWA treatment.(J Intervent Radiol,2017,26:232-236)
7. Research advances in the role of mesenchymal stem cells and their exosomes in treatment of liver diseases

At present, liver transplantation remains the most effective treatment for acute liver failure and advanced cirrhosis, but its use and promotion are limited by insufficient organ donors, financial consideration, and requirements for lifelong immunosuppression. In recent years, stem cell transplantation has been recommended as an effective substitutive therapy for liver disease. Mesenchymal stem cells, also known as pluripotent interstitial stromal cells, are self-renewing cells that can be found in almost all postnatal organs and tissues, including the liver. Their potential to differentiate into hepatocytes and immunomodulatory properties provide new insights into the use of mesenchymal stem cells in the treatment of acute and chronic liver diseases. This article reviews the characteristics of mesenchymal stem cells, their mechanisms in the treatment of acute and chronic liver diseases, and related risks.
8.Research on diagnosis and surgical treatment of congenital coronary artery fistula

Objective To summarize and analyze clinical diagnosis and surgical treatment methods of 11 cases with congenital coronary arterial fistula (CCAF). Methods The clinical data of 11 patients who were definited by ultrasonic cardiogram, CT angiography (CTA) and coronary angiography were analyzed retrospectively. Four cases were simple CCAF, 7 cases coexisted with other heart abnormalities. Six cases were given surgical closure of fistula without cardiopulmonary bypass. One of the cases adopted coronary artery under the tangent cotton-padded mattress suture, and 4 cases fistula arterial were ligatured directly. Six cases were given surgical closure of fistula under cardiopulmonary bypass. The right coronary arterial was opened in three of the cases with right coronary artery aneurysm to close fistula. The chambers of heart in the others were opened to close fistula. Results All patients received surgical treatment successfully, and no death happened during the operation. The ultrasonic cardiography showed that all patients recovered well. Follow-up was conducted on 10 patients with the time period ranging from 3 months to 5 years. There was no death and no complication. Conclusions Combined application of ultrasonic cardiogram, coronary angiography and CTA increases the accuracy rate of diagnosis greatly and offers visual bases to formulate operation plan. Surgical operation is quite effective for congenital coronary arterial fistula after definite diagnosis. Operator should try to reserve the expanded coronary arterial, strengthen the anticoagulant after opeation to prevent thrombosis.
9.Roles of regulatory T cells and T-helper cells in hepatitis B virus-related liver diseases

In recent years, the role of immune cells in hepatitis B virus (HBV)-related liver diseases has attracted more and more attention. This article reviews the roles of regulatory T cells (Treg) and T-helper cell type 17 (Th17) in the development, progression, and prognosis of HBV-related liver diseases, especially HBV-related liver fibrosis and liver failure, as well as hepatocellular carcinoma. It is pointed out that the dynamic changes of Treg and Th17 cells in different stages of HBV infection and their roles in hepatocellular carcinoma need further investigation, and future studies on the regulatory effects of various signaling pathways on Treg or Th17 cells are needed to provide a more precise direction for the treatment of HBV-related diseases.

Objective To summarize the individualized cavity Single branch stent grafting through rebuilding the aortic arch surgery in 26 cases of the application of the Stanford type A aortic dissection. Methods From 2010 January to 2014 October, 26 patients received Stanford type A aortic dissection surgery, 26 patients received individualized cavity single branch stent grafting to rebuild the aortic arch surgery , together with improved myocardial protection fluid. Results In the present study, 26 cases with aortic dissection that were treated with single branch stent grafting for the reconstruction of aortic arch under DHCA and selective cerebral perfusion. Twenty-six patients received individualized cavity single branch stent grafting reconstruction of aortic arch surgery alone, and were stopped by using deep cryogenic loop (DHCA) plus selective cerebral perfusion surgical treatment. One patient suffered from permanent nerve dysfunction iand give up treatment. Conclusion The sexua branch stent grafting in reconstruction of aortic arch operation could simplify the operation procedures , shorten the operation time, and reduce the amount of blood transfusion and postoperative drainage.
