1.Effects and mechanisms of isoginkgetin on osteoclastogenesis

BACKGROUND:During bone remodeling,bone formation and bone resorption are spatially and temporally coordinated,involving intricate interactions between osteoclasts and osteoblasts.Isoginkgetin,a flavonoid found in Ginkgo biloba,has a wide range of anticancer activity and anti-reactive oxygen species activity;however,the effect of isoginkgetin on osteoclast differentiation is unknown.OBJECTIVE:To study the effect and mechanism of action of isoginkgetin on osteoclastogenesis.METHODS:In vitro studies were performed on mouse bone marrow-derived macrophages,and cell counting kit-8 cytotoxicity assay was used to detect the effect of isoginkgetin on cell viability of bone marrow-derived macrophages.Macrophage colony-stimulating factor and receptor activator of nuclear factor kappa-B ligand were used to induce the differentiation of bone marrow-derived macrophages to osteoclasts.Network pharmacology and molecular docking and molecular dynamics simulations were used to predict the processes and targets of the effects of isoginkgetin on the differentiation of osteoclasts.Tartrate-resistant acid phosphatase staining and F-actin staining were used to detect the effects of isoginkgetin on the differentiation and function of osteoclasts.Western blot and RT-PCR were used to detect the effects of isoginkgetin on the expression of genes and proteins related to osteoclast differentiation,reactive oxygen species,and PI3K/AKT pathways.Fluorescent probes were used to detect cellular and mitochondrial reactive oxygen species levels.Flow cytometry technology was used to detect reactive oxygen species levels in cells.RESULTS AND CONCLUSION:(1)Network pharmacology results showed that isoginkgetin affected osteoporosis mainly through the PI3K-AKT pathway and cellular response to drugs and hypoxia,and GSK3β,ESR1,MCL1 and CCNA2 were the key targets.(2)Cell counting kit-8 and tartrate-resistant acid phosphatase staining results showed that isoginkgetin at 8 μmol/L had the most significant inhibitory effect on osteoclastogenesis in vitro,and F-actin results showed that isoginkgetin inhibited osteoclast cytoskeletal actin ring formation in a concentration-dependent manner.(3)Molecular dynamics simulations showed that isoginkgetin bound well to osteoclastogenesis marker proteins(NFATc1,c-Fos,CTSK,and MMP9).Western blot and RT-PCR results indicated that isoginkgetin inhibited the expression of osteoclastogenesis marker proteins and genes(NFATc1,c-Fos,CTSK,and MMP9).(4)Western blot results showed that isoginkgetin inhibited the phosphorylation level of PI3K/AKT/GSK3β and suppressed osteoclastogenesis by activating the PI3K-AKT-GSK3β pathway.(5)The results of reactive oxygen species assay showed that isoginkgetin significantly reduced receptor activator of nuclear factor kappa-B ligand-induced cellular and mitochondrial reactive oxygen species production,and inhibited the differentiation of bone marrow-derived macrophages to osteoclasts.
2.Analysis of sports supplement usage among grade 9 students and associated factors

Objective:
To investigate the status and associated factors of sports supplement usage among grade 9 students, so as to provide a scientific basis for targeted supervision and health education regarding sports supplement usage among junior high school students.
Methods:
From June to September 2025, a stratified cluster random sampling method was used to select 2 261 grade 9 students from 10 provinces (autonomous regions, municipalities) in China. A questionnaire survey was conducted on their sports supplement usage and related factors. The Chi square test was used to compare the usage rates of sports supplements among different groups of students, and binary Logistic regression analysis was employed to investigate the related factors of sports supplement usage among grade 9 students.
Results:
Totally 59.7% of the grade 9 students used sports supplements. The usage rate (62.5%) was higher among boys than girls (56.3%), higher among students from rural areas and towns/counties (66.5%, 66.2%) than those from urban districts (52.9%), higher among boarding students (65.2%) than non resident students (54.3%), higher among students whose parents occupations were businessmen and workers (fathers: 65.0%, 63.7%; mothers: 63.6%, 61.1%) than those whose parents were farmers and civil servants (fathers: 57.5%, 54.1%; mothers: 58.8%, 55.7%), higher among students with a monthly family income of 5 000- 10 000 yuan (66.3%) than those in other income groups, and higher among students in the high score zone for the entrance physical examination to senior high school (67.7%) than those in the medium and low score zones ( 56.3% , 56.5%) ( χ 2=8.99, 42.21, 27.98, 20.55, 8.20, 22.74, 24.70, respectively, all P <0.05). Binary Logistic regression analysis showed that boys ( OR =1.26), those from rural areas ( OR =1.59), boarding students ( OR =1.36), those with a monthly family income of 5 000- 10 000 yuan ( OR =1.41), and those in the high score zone for entrance physical examination to senior high school ( OR =1.34) were more likely to use sports supplements during the entrance physical examination to senior high school; the probability of sports supplement usage was lower among students whose fathers were civil servants ( OR =0.74) (all P <0.05).
Conclusions
The usage of sports supplements is relatively common among grade 9 students. Intervention measures should be targeted at specific populations to reduce the risk of misuse.
3.Rapid health technology assessment of carfilzomib in the treatment of multiple myeloma

OBJECTIVE To comprehensively analyze the effectiveness, safety and economic value of carfilzomib in the treatment of multiple myeloma (MM), and provide evidence-based guidance for clinical rational drug use. METHODS A systematic search was performed in PubMed, Embase, the Cochrane Library, Web of Science, CNKI, Wanfang, VIP, and relevant health technology assessment (HTA) websites, from database inception to December 31, 2024. Two researchers independently screened studies based on predefined inclusion and exclusion criteria, extracted data, and assessed study quality. Descriptive statistical analyses were conducted. RESULTS A total of 21 studies were included, comprising 7 systematic reviews/meta-analyses, 5 clinical trials, 5 pharmacoeconomic studies, and 4 HTA reports. Effectiveness analysis indicated that, compared with control groups, carfilzomib significantly improved overall response rate and clinical benefit rate ( P <0.05), prolonged progression-free survival, and enhanced patient’s quality of life ( P <0.05), while its impact on overall survival remains to be further confirmed. Safety analysis showed there was no significant difference in the risk of peripheral neuropathy between carfilzomib and controls, while the risks of cardiotoxicity and hypertension were significantly increased ( P <0.05). Economic analysis suggested that carfilzomib regimens demonstrated certain cost-effectiveness in second-line treatment in the U.S. and some European countries. However, its economic value was influenced by drug price, dosing regimen, and regional differences, and it may lack a cost advantage in some countries or in third-line treatment. CONCLUSIONS Carfilzomib shows definite efficacy and generally acceptable safety in the treatment of MM, but cardiotoxicity and hypertension risks require careful monitoring. Given its high treatment cost, pharmacoeconomic studies in the Chinese population are urgently needed.

5.Treadmill exercise reshaped mitochondrial dynamics and alleviated de-pression-like behaviors in mice

AIM:To explore the molecular mechanism by which treadmill exercise reshapes the homeostasis of mitochondrial dynamics and alleviates depression-like behavior.METHODS:The mouse model of depression-like be-havior was established by electronic foot shock plus restraint stress.The depression-like mice were intervened with tread-mill exercise.The neurobehavior of mice was evaluated using the open field test and elevated plus-maze test,neuronal in-jury was detected by Nissl staining and TUNEL,and the expression of mitochondrial dynamics-related proteins was detect-ed by Western blot and immunofluorescence.RESULTS:Five weeks of electronic foot shock plus restraint stress success-fully established the depression-like mouse model,and neurobehavioral tests showed that the mice exhibited depression-like behaviors.In depression-like mice,neuronal synapses were degraded and neuronal apoptosis increased,while tread-mill exercise reduced neuronal damage and alleviated abnormal neurobehavior in depression-like mice.The mechanism study shows that the expression of mitochondrial fission-related protein 1(Fis1)and dynamin-related protein 1(Drp1)in-volved in mitochondrial division is up-regulated,and the expression of mitofusin 2(MFN2)and optic atrophy 1 protein(OPA1)involved in mitochondrial fusion is down-regulated in the cerebral cortex of depression-like mice.Notably,tread-mill exercise reverses the expression of these mitochondrial dynamics-related proteins and reshapes the homeostasis of mi-tochondrial dynamics.CONCLUSION:Mitochondrial dynamics disorder is involved in the pathological process of neu-robehavioral changes in mice with depression-like behavior.Treadmill exercise reshapes the homeostasis of mitochondrial dynamics and alleviates depression-like behavior.This study provides a scientific reference for the clinical application of exercise therapy as an adjuvant therapy in the intervention of diseases associated with depression-like behavior.
6.A Study on Chinese Medicine Syndrome Typing of Breast Cancer and Interpretability Analysis Based on Multi-parametric MRI Radiomics

[Objective]By integrating multi-parameter magnetic resonance imaging(MRI)radiomics features,an interpretable machine learning model was constructed to accurately predict pre-operative traditional Chinese medicine(TCM)syndromes in breast cancer patients,providing objective and quantitative basis for clinical TCM syndrome types differentiation.[Methods]A total of 315 patients pathologically diagnosed breast cancer from the First Affiliated Hospital of Zhejiang Chinese Medical University between June 2019 to October 2023 were retrospectively included.The preoperative TCM syndromes of the patients were classified into liver depression with phlegm coagulation,chongren dysregulation and positive deficiency blazing toxin type.These patients were randomly allocated to the training(221 cases)and validation sets(94 cases)in a ratio of 7:3.Radiomics features in dynamic contrast-enhanced imaging(DCE),diffusion-weighted imaging(DWI)and apparent diffusion coefficient(ADC)images of each patient were extracted by using Kruskal-Wallis rank sum test,Spearman correlation analysis and least absolute shrinkage and selection operator(LASSO)polynomial Logistic regression screening for radiomics features.Then,light gradient boosting machine(LightGBM)was used to construct the syndrome prediction model.[Results]A total of 2 832 radiomics features were extracted from the DCE,DWI and ADC images of each patient.After a series of feature selection methods,the total numbers of features for DCE,DWI,ADC and three-sequence combination were 3,3,3 and 7 respectively.The machine learning model integrating multi-sequence MRI image information had the best performance,with micro-averaged area under the curve(AUC)of 0.852 and 0.838 for the training and validation sets,and macro-averaged AUC of 0.833 and 0.810 for the training and validation sets respectively.Shapley additive explanation(SHAP)analysis showed that inverse difference moment normalization(Idmn)of the ADC sequence,average gray level(Mean)of the ADC sequence and inverse variance of the DCE sequence were the three features that contributed most to the prediction results.[Conclusion]The interpretable machine learning model constructed in this study can accurately predict pre-operative TCM syndromes in breast cancer patients.SHAP interpretability analysis provides quantitative contribution value information,which can help clinicians understand the model prediction process and provide a new perspective for the application of traditional Chinese medicine in the treatment of breast cancer.
7.Prognostic factors and survival analysis in rectal cancer patients with poor response to neoadjuvant therapy

Objective:To compare the impact of different treatment strategies on the survival outcomes in rectal cancer patients with poor response to neoadjuvant therapy, and to explore the survival-related influencing factors.Methods:A retrospective cohort study was conducted. Between January 2018 and November 2022, the clinical, pathological, and follow-up data of 106 rectal cancer patients who received neoadjuvant therapy and were evaluated as grade 4 or 5 based on the Magnetic Resonance Tumor Regression Grade (mrTRG) from the rectal cancer database at Peking Union Medical College Hospital were retrospectively collected. Based on the post-neoadjuvant therapy assessment, patients were classified into three groups: the chemotherapy-radiotherapy group (23 patients), the consolidation therapy group (18 patients), and the standard treatment group (65 patients). General condition, pathological findings, selection of neoadjuvant therapy, comorbidities, as well as 3-year expected DMFS and OS were observed in the three groups.Results:All 106 patients were followed up, with a median follow-up time of 28 (21, 38) months. The overall 3-year DMFS rate was 60%, and the 3-year OS rate was 74%. The 3-year DMFS in the standard treatment and consolidation therapy groups were 74% and 72%, respectively; the 3-year OS were 84%, 81%, respectively. The Log-rank test showed that there was no significant difference in the 3-year expected DMFS and OS between the standard treatment group and the consolidation therapy group (both P>0.05), but both groups had better survival outcomes than the chemotherapy-radiotherapy group (10% and 39%, respectively; all P<0.001). Multivariate Cox regression analysis indicated that the chemotherapy-radiotherapy only regimen was an independent risk factor for DMFS (HR=12.425, 95% CI: 4.436–34.594, P<0.001), and the independent risk factors for OS were chemotherapy-radiotherapy only regimen (HR=8.991, 95%CI:2.220–36.403, P=0.002) and age≥65 years (HR=3.495, 95%CI: 1.017–12.009, P=0.047). Stratified analysis showed that chemotherapy-radiotherapy only regimen was the independent risk factors for DMFS and OS in patients with extramural vascular invasion (EMVI) positive ( n=66) and mesorectal fascial invasion (MRF) positive (n=56) (all P<0.05). Whether consolidation therapy was added to the standard neoadjuvant treatment regimen was not an independent factor affecting 3-year expected DMFS or OS in rectal cancer patients with poor response to neoadjuvant therapy. Further comparisons between the standard neoadjuvant treatment and consolidation therapy groups showed no statistically significant differences in spincter-preservation rate or postoperative complication rates (both P>0.05). However, the consolidation therapy group had a longer interval between the end of radiotherapy and surgery [80.1 (50.8, 109.4) days vs. 61.8 (48.8, 74.8) days, P<0.001], and a higher incidence of chemotherapy-related adverse effects ([10/18] vs. 26.2% [17/65], P=0.018). Conclusion:In rectal cancer patients with poor response to neoadjuvant therapy and clear adverse prognostic features before surgery (locally advanced stage, MRF positive or EMVI positive), the addition of short- or long-course chemotherapy-based systemic therapy does not provide short- or long-term survival benefits. Moreover, an extended chemotherapy duration increases the incidence of chemotherapy-related adverse effects.
8.Probiotics empower postoperative intestinal function recovery after colorectal surgery: mechanisms and clinical progress

Gastrointestinal dysfunction is a common and significant complication in colorectal surgical practice, which is associated with gut microbiota dysbiosis caused by various perioperative interventions. Currently, enhanced recovery after surgery (ERAS) protocols have been increasingly adopted in clinical practice, greatly accelerating the recovery of postoperative intestinal function. However, there are still no effective interventions in the ERAS protocols to target surgery-induced gut microbiota dysbiosis. Probiotics, as a key treatment method targeting the gut microbiota, can stimulate intestinal motility, inhibit the colonization of pathogenic bacteria, enhance intestinal barrier function, among other effects. Based on these effects of probiotics, they are expected to resolve the neglected gut microbiota dysbiosis, further accelerating the postoperative recovery of intestinal function after colorectal surgery. This article reviews the mechanisms and clinical progress in postoperative bowel functional recovery after colorectal surgery.

Osteoarthritis(OA)is a common joint disease in clinical practice,and cartilage damage is a typical pathological change.The pathogenesis of OA is complex,and various adverse factors can lead to the occurrence of OA.Mitochondria are im-portant organelles within cells and play important roles in cellular physiological and pathological activ-ities.Mitochondrial quality control is an important regulatory mechanism in the body to maintain nor-mal mitochondrial structure and function,mainly including mitochondrial biogenesis,mitochondrial dynamics,mitochondrial autophagy,mitochondrial oxidative stress,and other forms.The imbalance of mitochondrial quality control in chondrocytes is closely related to the occurrence and development of osteoarthritis,and regulating the balance of mi-tochondrial quality control is a potential therapeu-tic point for osteoarthritis.The author reviewed rel-evant research literature in recent years to provide a review of the relationship between mitochondrial quality control and the occurrence and develop-ment of osteoarthritis,in order to provide new ideas and directions for the research and diagnosis and treatment strategies of osteoarthritis.
