1.Case report and literature review of myocardial infarction caused by myocardial bridge

Medical therapy and surgical intervention are the two primary approaches for treating myocardial bridge.However,there remains controversy regarding the use of coronary artery bypass grafting(CABG)and myocardial bridge unroofing.Here,we report a case of myocardial infarction following CABG in a patient with a myocardial bridge.The patient was admitted to Lanzhou First Peopie's Hospital with persistent chest pain,chest tightness,and shortness of breath lasting 2 hours.Physical examination revealed no significant abnormalities.Electrocardiography(ECG)indicated extensive anterior wall myocardial infarction.Laboratory findings showed myoglobin levels of 140.1 ng/ml and troponin Ⅰ levels of 2.59 ng/ml,with no other significant abnormalities.The initial diagnosis was acute extensive anterior wall myocardial infarction.Emergency coronary angiography revealed a myocardial bridge in the mid-segment of the left anterior descending artery(LAD).Emergency CABG using the left internal mammary artery to the LAD was performed,leading to symptomatic improvement,and the patient was discharged in stable condition.However,the patient experienced a recurrent myocardial infarction seven years post-surgery and received secondary preventive medical therapy.The patient is currently under ongoing follow-up care.CABG is an effective treatment for myocardial bridge.However,based on the case reported in this study,we recommend careful evaluation of whether a patient may benefit from CABG.
2.Correlation of platelet to albumin ratio with occurrence of cerebral infarction after left atrial appendage closure in patients with non-valvular atrial fibrillation

Objective To investigate the correlation between platelet-to-albumin ratio(PAR)and occurrence of cerebral infarction after left atrial appendage closure(LAAC)in patients with non-valvular atrial fibrillation(NVAF).Methods A retrospective study was conducted on 259 NVAF patients undergoing LAAC in our department between 2019 and 2023.According to occurrence of cerebral infarction after LAAC or not,they were divided into a control group(241 cases)and a study group(18 cases).Their general data were collected,and Cox proportional hazards regression model was used to identify the risk factors for cerebral infarction.ROC curve was plotted to assess the predictive value of PAR for cerebral infarction in NVAF patients after LAAC,and the AUC value was calculated.Kaplan-Meier survival curve was drawn to analyze the incidence of cerebral infarction after LAAC in NVAF patients with different PAR values.Results The study group had significantly advanced age,higher SBP at admission,increased WBC,neutrophil,monocyte and platelet counts,longer thrombin time,elevated international normalized ratio(INR)and high-sensitivity C-reactive protein(hs-CRP)level,and higher PAR than the control group(P<0.05,P<0.01).Multivariate Cox regression analysis showed that PAR(HR=2.286,95%CI:1.182-4.420,P<0.05)was an independent risk factor for cerebral infarction in NVAF patients after LAAC.ROC curve indicated that the AUC value of PAR in predicting cerebral infarction after LAAC in NVAF patients was 0.721(95%CI:0.586-0.856,P<0.01),with an optimal cut-off value of 4.137,a sensitivity of 66.39%,and a specificity of 77.78%.Kaplan-Meier survival curve revealed that the higher the PAR value was,the higher the risk of cerebral infarction was(P<0.01).Conclusion PAR is significantly correlated with cerebral infarction in NVAF patients after LAAC.The higher the PAR,the higher the risk of cerebral infarction,demonstrating its predictive value and being worthy of clinical promotion.

Catheter-based percutaneous coronary intervention has become a crucial approach for treating obstructive coronary artery disease and is widely applied in clinical practice.Although the new generation of drug-eluting stents(DES)has significantly reduced the incidence of post-procedur-al restenosis and improved long-term clinical outcomes,in-stent restenosis(ISR)remains one of the key challenges affecting the long-term efficacy of percutaneous coronary intervention.Recent studies have demonstrated that DES still have certain inherent limitations.From the metallic stent platform,drug coating to the polymer carrier,each component may exert varying degrees of biological effects on the vascular intima.Optical coherence tomography(OCT),as a high-resolution intravascular imaging technique,has shown significant value in the mechanism research,etiological diagnosis,and clinical classification of ISR.It can clearly identify the tissue composition and morphological characteristics of ISR,providing a precise basis for the formulation of individualized treatment strategies.This review aimed to systematically search the existing literature,comprehensively review the clinical and patho-logical features of ISR after DES implantation,focus on the OCT-based ISR classification system,and summarize the current treatment strategies and latest advances for ISR.
6.Fusion of Dual-targeting Peptides with MAP30 Promotes the Apoptosis of MDA-MB-231 Breast Cancer Cells

Momordica antiviral protein 30 kD(MAP30)is a type Ⅰ ribosome-inactivating protein(RIP)with antibacterial,anti-HIV and antitumor activities but lacks the ability to target tumor cells.To in-crease its tumor-targeting ability,the arginine-glycine-aspartic(RGD)peptide and the epidermal growth factor receptor interference(EGFRi)peptide were fused with MAP30,which was named ELRL-MAP30.The efficiency of targeted therapy for triple-negative breast cancer(TNBC)MDA-MB-231 cells,which lack the expression of estrogen receptor(ER),Progesterone receptor(PgR)and human epidermal growth factor receptor-2(HER2),is limited.In this study,we focus on exploring the effect and mecha-nism of ELRL-MAP30 on TNBC MDA-MB-231 cells.First,we discovered that ELRL-MAP30 significant-ly inhibited the migration and invasion of MDA-MB-231 cells and induced MDA-MB-231 cell apoptosis.Moreover,ELRL-MAP30 treatment resulted in a significant increase in Bax expression and a decrease in Bcl-2 expression.Furthermore,ELRL-MAP30 triggered apoptosis via the Fak/EGFR/Erk and Ilk/Akt signaling pathways.In addition,recombinant ELRL-MAP30 can inhibit chicken embryonic angiogenesis,and also inhibit the tube formation ability of human umbilical vein endothelial cells(HUVECs),indica-ting its potential therapeutic effects on tumor angiogenesis.Collectively,these results indicate that ELRL-MAP30 has significant tumor-targeting properties in MDA-MB-231 cancer cells and reveals potential ther-apeutic effects on angiogenesis.These findings indicate the potential role of ELRL-MAP30 in the targeted treatment of the TNBC cell line MDA-MB-231.
7.Clinical Efficacy and Renal Protective Mechanism of Dan Qi Yishen Prescription in Treating Patients with Diabetic Nephropathy

Objective To observe the clinical efficacy of Dan Qi Yishen Prescription in treating diabetic nephropathy(DN)and explore its renal protective mechanism.Methods A total of 150 patients with DN of qi-yin deficiency complicated with blood stasis syndrome who admitted to Sanya Hospital of Traditional Chinese Medicine Affiliated to Guangzhou University of Chinese Medicine from October 2021 to April 2023 were divided into a control group and an observation group according to the random number table method,with 75 patients in each group.Both groups were given conventional therapies such as dietary treatment,blood glucose control,blood pressure control,lipid-lowering treatment and treatment of complications.Additionally,the control group was given intravenous injection of reduced glutathione,while the observation group was given oral use of the decoction of Dan Qi Yishen Prescription and ionic introduction of Dan Qi Yishen Prescription in renal region.Thirty days constituted one course of treatment,and both groups were treated for three courses.Before and after treatment,the scores of traditional Chinese medicine(TCM)syndrome,and the levels of renal function indicators,renal hemodynamic indicators,coagulation and fibrinolysis indicators,and vascular endothelial function indicators in the two groups were observed.Results(1)After treatment,the scores of TCM symptoms such as fatigue and weakness,palpitations and shortness of breath,dizziness and tinnitus,spontaneous sweating and night sweating,restlessness and insomnia,and thirst with preference for drinks in the two groups were decreased compared with those before treatment(P<0.01),and the decrease in the observation group was significantly superior to that in the control group(P<0.01).(2)After treatment,the levels of renal function indicators such as blood urea nitrogen(BUN),serum creatinine(SCr),collagen Ⅳ(CⅣ),and plasma laminin(LN)of patients in the two groups were improved compared with those before treatment(P<0.01),and the improvement in the observation group was significantly superior to that in the control group(P<0.01).(3)After treatment,the levels of renal hemodynamic indicators such as systolic maximum blood flow velocity(Vsmax)and diastolic minimum blood flow velocity(Vdmin)of the main renal artery(MRA)and interlobar artery(IRA)in the two groups were increased compared with those before treatment(P<0.05 or P<0.01),while the resistance index(RI)and pulsatility index(PI)of MRA and RI of IRA in the two group as well as PI of IRA in the observation group were decreased compared with those before treatment(P<0.05 or P<0.01).The increase of Vsmax and Vdmin of MRA and IRA as well as the decrease of RI and PI of MRA and IRA in the observation group was significantly superior to that in the control group(P<0.05 or P<0.01).(4)After treatment,the levels of coagulation and fibrinolysis indicators such as prothrombin time(PT),activated partial thromboplastin time(APTT),D-dimer(D-D),and fibrinogen(FIB)in the two groups were improved compared with those before treatment(P<0.01),and the improvement in the observation group was significantly superior to that in the control group(P<0.01).(5)The analysis of vascular endothelial function indicators showed that after treatment,the serum vasohibin 1(VASH-1)level in the two groups was increased compared with that before treatment(P<0.01),and the serum vascular endothelial growth factor(VEGF)and endothelin 1(ET-1)levels were decreased compared with those before treatment(P<0.01),and the increase of serum VASH-1 level as well as the decrease of serum VEGF and ET-1 levels in the observation group was significantly superior to that in the control group(P<0.01).Conclusion Dan Qi Yishen Prescription exerts good clinical efficacy in treating patients with DN of qi-yin deficiency complicated with blood stasis syndrome,and it has protective effect on renal function probably by improving the coagulation and fibrinolysis system and vascular endothelial function of the patients.
8.Effect of neutrophil elastase inhibitor sivelestat on cardiac fibrosis in HFpEF mice

AIM:To investigate the effects of the neutrophil elastase(NE)inhibitor sivelestat on the pathologi-cal changes of cardiac tissues in mice with heart failure with preserved ejection fraction(HFpEF).METHODS:Eight-week-old C57BL/6J mice were randomly divided into 3 groups:control group(n=5),model group(n=6),and treatment group(n=18).The HFpEF mouse model was established using a combined approach of high-fat diet and NG-nitro-L-argi-nine methyl ester(L-NAME).The mice in treatment group received intraperitoneal injection of sivelestat at doses of 12.5,25 and 50 mg·kg-1·d-1(n=6),while those in control and model groups were injected with the same volume of nor-mal saline.After 12 weeks,small animal ultrasound was employed to assess changes in cardiac function,and the lung weight-to-tibia length(LW/TL)ratio was calculated to evaluate pulmonary edema.An exercise fatigue test was conducted to assess exercise intolerance.Histological evaluations were performed using HE,wheat germ agglutinin,dihydroethidium and Sirius red staining to examine overall heart morphology,cross-sectional area of cardiomyocytes,levels of oxidative stress,and the extent of cardiac fibrosis.The expression of fibrosis-related proteins was analyzed using Western blot.RE-SULTS:Compared with model group,sivelestat at various concentrations significantly improved cardiac diastolic function in HFpEF mice,indicated by an increased E/A ratio and a decreased E/e'ratio(P<0.05).The LW/TL ratio decreased,alleviating lung congestion and exercise intolerance(P<0.05).The heart weight-to-tibia length(HW/TL)ratio,overall heart cross-sectional area,and cardiomyocyte cross-sectional area all decreased,indicating attenuation in cardiac hypertro-phy(P<0.05).Additionally,cardiac fibrosis and reactive oxygen species levels were significantly reduced(P<0.05).CONCLUSION:Inhibition of NE exerts a protective effect against diastolic dysfunction and cardiac fibrosis induced by HFpEF in mice.
9.Resident training physicians' job preference in Chengdu, China based on the discrete choice experiment

Objective:To explore the job preference and willingness to pay of resident training physicians in Chengdu, China.Methods:The questionnaire was designed by means of the discrete choice experiment. The questionnaire contains the basic information of the survey object and the discrete choice set (including workplace, working intensity, monthly salary, staffing of public institution, and career development, each with several levels). A total of 116 resident training physicians recruited by three standardized residents training centers in Chengdu were selected to conduct the questionnaire survey, and grouped based on their identity and major. The mixed logit model was used to analyze job preferences and willingness to pay of resident training physicians.Results:Except for career development, other four attributes had statistically significant effects on job selection of resident training physicians ( P<0.05), of which working intensity was the most influential attribute, with the highest willingness to pay. The subgroup analysis results showed that staffing of public institution was concerned by clinical postgraduates and resident training physicians in non-scarce majors. Conclusions:Economic and non-economic factors both affect the job preferences of resident training physicians, while working intensity is the most important factor. Resident training policy should focus on adjusting the scale and type of resident physician enrollment, and strengthen humanistic care and career education of resident training physicians.
