ObjectiveThis study aimed to investigate the action mechanism by which Banxia Xiexintang (BXT) inhibits epithelial-mesenchymal transition (EMT) in chronic atrophic gastritis (CAG) rats by regulating the interleukin-17(IL-17)/extracellular regulated protein kinases(ERK)/CCAAT enhancer binding protein β(C/EBPβ)signaling pathway, thereby providing new theoretical evidence for the treatment of CAG with classic traditional Chinese medicine formulas. MethodsA CAG rat model was established by using the combined factor method. After successful modeling, the rats were randomly divided into the model group, low-, medium-, and high-dose groups (0.549, 1.098, 2.196 g·kg-1, respectively) of BXT, and the positive drug group (vitacoenzyme, 0.3 g·kg-1). A normal control group was also set up. After 8 weeks of intervention, the pathological changes of gastric tissue were evaluated. The enzyme-linked immunosorbent assay (ELISA) was used to detect the contents of IL-17, tumor necrosis factor-α (TNF-α), cyclooxygenase-2 (COX-2), and C/EBPβ in serum, as well as the contents of EMT markers in gastric mucosal tissue including E-cadherin, N-cadherin, and vimentin. The immunohistochemistry method was employed to determine the localization and protein expression levels of IL-17, p-ERK, and C/EBPβ in gastric mucosal tissue. Western blot was used to detect the protein expressions of C/EBPβ, ERK, and its phosphorylated form (p)-ERK in gastric mucosa. Real-time polymerase chain reaction (Real-time PCR) was applied to measure the mRNA expression levels of ERK, COX-2, and C/EBPβ in gastric mucosa. ResultsCompared with those in the normal control group, the rats in the model group showed gastric mucosal glandular atrophy and inflammatory cell infiltration. The protein and their related mRNA expressions of C/EBPβ, ERK, and p-ERK in gastric mucosa were significantly increased (P<0.05,P<0.01). The levels of IL-17, TNF-α, COX-2, and C/EBPβ in serum were significantly increased (P<0.01). The contents of N-cadherin and vimentin in gastric mucosal tissue were significantly increased, while the content of E-cadherin was significantly decreased (P<0.01). Compared with the model group, after intervention with different doses of BXT, the pathological damage of the gastric mucosa was improved to varying degrees. The protein and mRNA expressions of C/EBPβ, ERK, and p-ERK in gastric mucosa were significantly reduced (P<0.05,P<0.01). The levels of IL-17, TNF-α, COX-2, and C/EBP β in serum were significantly decreased (P<0.01). The contents of N-cadherin and vimentin in gastric mucosa tissue were decreased, while the content of E-cadherin was increased (P<0.05,P<0.01). ConclusionBXT can effectively improve the pathological damage of gastric mucosal tissue in CAG rats. Its action mechanism may be related to reducing the levels of IL-17 and TNF-α in serum, regulating the IL-17/ERK/C/EBPβ signaling pathway and inhibiting the EMT process.

Objective To investigate the cross-sectional associations between different fatty liver classifications and cardiac structure and function in people undergo health examination. Methods A total of 6 545 adults who underwent health examinations at the Health Management Center of Zhongshan Hospital, Fudan University between January 1 and December 31, 2017, were retrospectively included. Demographic characteristics and laboratory data were collected. The hepatic steatosis was graded by ultrasonography. And patients with fatty liver were further stratified according to alanine aminotransferase (ALT) or aspartate aminotransferase (AST) levels, as well as the degree of liver fibrosis. Cardiac morphology and function were assessed by transthoracic echocardiography, and left ventricular geometric patterns were classified accordingly. Multivariate logistic regression analysis was performed to evaluate the associations between fatty liver classifications and cardiac abnormalities. Results There were 2 795 patients (42.7%) with fatty liver, of whom 832 (29.8%) had significant cardiac structural alterations and 1 500 (53.7%) had diastolic dysfunction. Severe fatty liver was risk factor for concentric remodeling and increased relative wall thickness (RWT), with odds ratios (ORs) of 1.27 (P=0.012) and 1.27 (P=0.009), respectively. Fatty liver accompanied by elevated ALT or AST was risk factor for concentric remodeling (OR=1.45,1.56; P=0.001, 0.001), increased RWT (OR=1.48,1.57; P<0.001, <0.001), and diastolic dysfunction (OR=1.27, 1.32; P=0.035, 0.040), respectively. Fatty liver with liver fibrosis was risk factor for concentric remodeling (OR=1.83, P=0.046) and diastolic dysfunction (OR=2.64, P=0.034). Conclusions Advanced fatty liver, including severe hepatic steatosis, accompanied by elevated liver enzymes or liver fibrosis, could increase risks of cardiac remodeling and diastolic dysfunction, while systolic function is preserved. For patients with fatty liver, it is recommended to undergo regular ultrasonography examination.

ObjectiveTo explore the association between metabolic score for visceral fat(METS-VF) and cardiometabolic multimorbidity(CMM) as well as cognitive function in the elderly population, revealing the role of METS-VF as an assessment tool in this population, and providing evidence and new perspectives for the early screening and intervention strategies of CMM and cognitive function. MethodsThis study is based on cross-sectional data from the Hubei memory and aging cohort study(HMACS), which includes elderly individuals aged 60 and above. The METS-VF index was calculated and divided into quartiles(Q1-Q4). CMM was assessed through a questionnaire, defining CMM as the coexistence of at least two of the following conditions: diabetes, heart disease, and stroke. Standardized cognitive tests were used to evaluate overall cognition, memory, language, executive function, and attention. Logistic and Linear regression analysis was performed to explore the association between METS-VF and the risk of CMM as well as cognitive function. A restricted cubic spline was used to test the dose-response relationship, and subgroup analysis was conducted to assess the variation of METS-VF in different populations. Additionally, multinomial unordered logistic regression was used to evaluate the performance of METS-VF in predicting CMM morbidity patterns. ResultsA total of 3 790 elderly participants aged 60 and above were included. After adjusting for confounding factors, compared with the lowest quartile(Q1), the Q4 group of METS-VF was associated with a higher risk of CMM [OR=3.00,95%CI (2.18, 4.16)], and with lower scores in overall cognition [b=-0.12, 95%CI (-0.21,-0.04)], attention [b=-0.14, 95%CI (-0.24,-0.05)], and executive function [b=-0.10, 95%CI (-0.20,-0.00)]. Dose-response analysis did not show a nonlinear relationship between METS-VF and either CMM or cognitive function (P for nonlinear > 0.05). Subgroup analysis revealed that the male group with higher METS-VF had lower overall cognitive scores, while the female group showed lower executive function scores (P for interaction < 0.05 for both). Multinomial unordered logistic regression analysis indicated that the METS-VF index was associated with an increased risk of diabetes with coronary heart disease [OR=2.62,95%CI (1.66, 4.15)] or concomitant stroke [OR=2.66,95%CI (1.73, 4.09)], especially in the group with both diabetes and stroke and coronary heart disease, where the association between METS-VF and the risk was strongest. ConclusionsAn increased METS-VF is associated with a higher risk of CMM and lower scores in overall cognition, attention, and executive function in the elderly population. Diabetes may be a key driving factor in the multimorbidity model. The METS-VF index can serve as a dual-purpose tool for screening cardiovascular metabolic risks and assessing cognitive function.

BackgroundDepression is a prevalent emotional problem in adolescents, and parental psychological control is an important predictor of adolescent depression. However, existing research on the acting mechanism between the two is not adequate. ObjectiveTo explore the pathway of negative perfectionism and academic stress between parental psychological control and depressive symptoms among secondary school students, so as to provide references for reducing the incidence risk of depression in such population. MethodsFrom February to April 2023, 1 100 students across 2 middle schools and 2 high schools in Zhongshan city were selected as subjects. The survey was conducted adopting Parental Psychological Control Questionnaire, Chinese Frost Multidimensional Perfectionism Scale (CFMPS), sense of academic stress subscale in Mental Health Inventory of Middle School Student (MMHI-60) and Center for Epidemiological Studies-Depression Scale (CES-D). Spearman correlation analysis was adopted to examine the correlation between scores of all scales above, and Amos 24.0 was used to test the mediating path of negative perfectionism and academic stress between parental psychological control and depressive symptoms among secondary school students. ResultsAmong the 1 009 valid questionnaires withdrew (91.73% of the total), 261 students were detected to have depressive symptoms (25.87%). As the results of Spearman correlation analysis showed, the scores of the Parental Psychological Control Questionnaire, score of negative perfectionism dimension in CFMPS, score of sense of academic stress subscale in MMHI-60 and CES-D score were positively correlated with each other (r=0.323~0.644, P<0.05 or 0.01). The direct effect value of parental psychological control on depressive symptoms in secondary school students was 0.128 (95% CI: 0.061~0.201), accounting for 31.37% of the total effect. Negative perfectionism and academic stress played independently as intermediatory roles between parental psychological control and depressive symptoms in secondary school students, and the indirect effect values were 0.099 (95% CI: 0.068~0.133) and 0.100 (95% CI: 0.060~0.143), accounting for 24.27% and 24.51% of the total effect, respectively. Negative perfectionism and academic stress acted combinedly as the chain effect pathway between parental psychological control and depressve symptoms in secondary school students, with the indirect effect value of 0.081 (95% CI: 0.060~0.106) accounting for 19.85% of the total effect. ConclusionParental psychological control can affect the depressive symptoms among secondary school students directly, and through independent or chain paths of negative perfectionism and academic stress indirectly. Objective:To retrospectively analyze and compare the clinical effects of rigid neuroendoscopic hematoma removal and drilling irrigation drainage in the treatment of chronic subdural hematomas with mixed density on head CT, and explore the appropriate surgical methods for chronic subdural hematomas with mixed density. Methods:A retrospective case-control study was conducted to analyze the clinical data of 80 patients with CSDH with mixed density CT findings admitted to the Department of Neurosurgery of Guizhou Medical University Affiliated Hospital from January 2021 to November 2023. There were 57 males and 23 females. According to the surgical method, patients were divided into endoscopic group ( n=36) and drilling group ( n=44). Patients in the endoscopic group underwent hard neuroendoscopic hematoma removal surgery, while patients in the drilling group underwent drilling flushing and drainage surgery. Compared the surgical time, drainage time, hematoma clearance rate, length of hospital stay, markwalder neurological function grading, and activities of daily living (ADL) score between two groups 30 days after surgery. Followed up for 3 months to record the recurrence situation.Measurement data with a normal distribution were expressed as mean±standard deviation ( ± s), and t-test was used for inter-group comparison. Measurement data with a non-normal distribution were expressed as M( Q1, Q3), and rank sum test was used for inter-group comparison. Chi-square test or Fisher′s exact test was used for inter-group comparison of count data. Rank sum test was used for inter-group comparison of ordinal data. Results:The operation time, postoperative drainage time, 24-hour hematoma clearance rate, midline deviation distance and hospital stay in the endoscopic group respectively were (77.50±8.15) min, 1.00(1.00, 2, 00) d, (95.00±2.66)%, 1.00(0.00, 2.00) mm, (9.47±2.52) d. The drilling group were (44.77±6.56) min, 3.00(2.25, 3.00) d, (87.86±3.43)%, 3.00(2.00, 3.00) mm, (11.84±3.28) d, the difference was statistically significant between the two groups ( P<0.05). Comparison of long-term efficacy in the endoscopic group, the hematoma clearance rate at 30 days after operation, the ADL score at 30 days after operation, and the number of recurrence cases at 3 months after operation respectively were 99.00(97.25, 100.00)%, (88.06±7.86) points, and 1 case. The drilling group were 93.50(91.25, 95.75)%, (83.29±9.58) points and 10 cases, with statistical difference between the two groups( P<0.05). 30 day postoperative Markwalder neurological functional grading, there were 27 cases grades 0, 9 cases of grade I, and 0 cases of grade II in endoscopic surgery. In the drilling group, there were 24 cases, 15 cases, and 5 cases, respectively. The difference between the two groups was statistically significant ( P<0.05). Conclusions:Compared with drilling and drainage irrigation surgery, neuroendoscopic treatment of mixed density chronic subdural hematoma takes a relatively long time, but the hematoma clearance rate is higher, the hospitalization time is shorter, the postoperative recovery is faster and the recurrence rate is lower. Neuroendoscopic therapy has unique advantages and may be more suitable for the treatment of mixed density CSDH.

Salvia miltiorrhiza (S. miltiorrhiza) represents a crucial component of traditional Chinese medicine, demonstrating effects on blood circulation activation and stasis removal, and has been widely utilized in asthma treatment. This study isolated a novel phenolic acid (S1) from S. miltiorrhiza and investigated its anti-asthmatic activity and underlying mechanisms for the first time. An allergic asthma (AA) model was established using ovalbumin (OVA). The mechanism of S1's effects on AA was investigated using multi-factor joint analysis, flow cytometry, and co-culture systems to facilitate clinical asthma treatment. S1 (10 or 20 mg·kg-1) was administered daily to mice with OVA-induced AA (OVA-AA) during days 21-25. The study examined airway responsiveness, lung damage, inflammation, and levels of immunoglobulin E (IgE), PGD2, interleukins (IL-4, 5, 10, 13, 17A), tumor necrosis factor α (TNF-α), GM-CSF, CXCL1, CCL11, and mMCP-1. Additionally, mast cell (MC) activation and degranulation were explored, along with T helper type 17 (Th17)/Treg immune cells and TLR4 pathway biomarkers. The antagonistic activity of that specific antagonist of TLR4 (TAK-242) (1 µmol·L-1), a specific TLR4 blocker, against S1 (10 µmol·L-1) was examined in co-cultured 16HBE cells and bone marrow-derived cells (BMDCs) or splenic lymphocytes (SLs) induced with LPS (1 µg·mL-1) to elucidate the TLR4 pathway's mediating role. S1 demonstrated reduced airway responsiveness, lung damage, and inflammation, with downregulation of IgE, PGD2, interleukins, TNF-α, GM-CSF, CXCL1, CCL11, and mMCP-1. It also impeded MC activation and degranulation, upregulated IL-10, and influenced Th17/Treg immune cell transformation following OVA challenge. Furthermore, S1 inhibited the TLR4 pathway in OVA-AA mice, and TLR4 antagonism enhanced S1's positive effects. Analysis using an OVA-AA mouse model demonstrated that S1 alleviates AA clinical symptoms, restores lung function, and inhibits airway response. S1's therapeutic effects occur through regulation of Th17/Treg immune cells and inflammation, attributable at least partially to the TLR4 pathway. This study provides molecular justification for S1 in AA treatment.
