This paper introduces a real-world cohort research model for community-acquired pneumonia (CAP) based on the Jiangsu Traditional Chinese Medicine (TCM) Dominant Diseases Diagnosis and Treatment Data Platform. Firstly, data cleaning is performed by standardizing diagnosis, symptoms, treatment and imaging, intelligently extracting unstructured information, and cleaning and constructing a standardized database. Secondly, for cohort establishment, CAP patients across the province are screened in accordance with CAP diagnostic criteria to build a high-quality disease-specific cohort. Lastly, in terms of protocol design, the characteristics of TCM research and the CAP disease profile are considered to determine appropriate inclusion and exclusion criteria, estimate sample size, define interventions, outcomes and economic evaluations, providing a reference for real-world TCM research on CAP.

3.Study on the mechanism of cordycepin inhibiting ferroptosis via the Nrf2/HO-1 pathway to delay the transformation from AKI to CKD

OBJECTIVE To investigate the mechanism of cordycepin in delaying the transformation from acute kidney injury (AKI) to chronic kidney disease (CKD) (short for “AKI-CKD”). METHODS Network pharmacology and bioinformatics were used to analyze and predict the signaling pathways of cordycepin in delaying AKI-CKD progression and its relationship with the ferroptosis pathway. Cell experiments were performed to verify the predicted results. Human renal tubular epithelial HK-2 cells were divided into blank group, cordycepin group, model group, and H 2 O 2 +cordycepin group. Except for the blank group and cordycepin group, all other groups were treated with 150 μmol/L H 2 O 2 for 72 h to induce persistent oxidative stress injury in cells. After 72 h of cordycepin (40 μmol/L) intervention, the protein and mRNA expression levels of glutathione peroxidase 4 (GPX4), long-chain acyl-CoA synthetase 4 (ACSL4),nuclear factor-erythroid 2-related factor 2(Nrf2), and heme oxygenase-1 (HO-1) in cells were detected. Furthermore, the Nrf2 inhibitor ML385 was added on the basis of H 2 O 2 +cordycepin to verify the role of the Nrf2/HO-1 pathway. RESULTS Network pharmacology and bioinformatics analysis showed that there were 42 overlapping genes related to AKI-CKD and ferroptosis that interact with cordycepin ferroptosis, among which 38 were annotated as ferroptosis-related genes. HO-1 and Nrf2 might be important targets for cordycepin to inhibit ferroptosis. The binding energies of cordycepin with Nrf2 and HO-1 proteins were -8.5 and -6.7 kcal/mol, respectively. Cell experiments showed that compared with the model group, the protein and mRNA expression levels of GPX4, Nrf2 and HO-1 were significantly increased ( P <0.05),while the protein and mRNA expression levels of ACSL4 were significantly decrease d ( P <0.05) in the H 2 O 2 +cordycepin group. After the addition of the Nrf2 inhibitor ML385, the effects of cordycepin on the above proteins and mRNA were significantly reversed ( P <0.05). CONCLUSIONS Cordycepin can inhibit ferroptosis by activating the Nrf2/HO-1 pathway, reduce persistent oxidative stress injury of renal tubular epithelial cells, and delay the progression of AKI-CKD.
4.A study on the practical effect of critical value management based on the goal and key result approach

Objective To explore the effectiveness of the OKR method in improving the efficiency of the whole process of critical value management,and to provide new ideas for the implementation of medical quality improvement in other medical insti-tutions.Methods A hospital in Tianjin was selected as the study object,which used the OKR method to reform the management of critical value since January 2024.The core goal of"improving the effectiveness of critical value management"was set,and the goal was broken down into quantitative key results such as"the rate of receiving the critical value system is over 95%"and"the rate of completing the standardised writing of medical records is up to 85%".Results After the reform,several quantitative key results,such as the completion rate of critical care medical record writing,the rate of standardised medical record writing,and the rate of overtime acceptance,were all better than before.Conclusion Through the OKR method to unify the whole hospital's strategic objectives,and the dynamic adjustment of the program based on data review,the hospital's critical value management efficiency has been significantly improved,effectively guaranteeing the safety of patients,and providing new perspectives and methods for the management of critical value in other medical institutions.
5.Silencing KRT17 inhibits proliferation of human esophageal squamous cell line KYSE-150

Objective To explore the effect and mechanism of keratin 17(KRT17)on proliferation of human esophageal squamous cell line KYSE-150.Methods The correlation of KRT17 expression with the disease stage and survival of patients with esophageal squamous cell carcinoma was analyzed by GEPIA2 website.Human esophageal squamous cell line KYSE-150 was divided into control group,si-NC group,si-KRT17 group and activator group.Small interfering RNA of si-NC and si-KRT17 were transfected into cells of si-NC group and si-KRT17 group,re-spectively.Cells in the activator group were transfected with si-KRT17 and treated with 740 Y-P(PI3K/AKT/mTOR pathway activator)with a final concentration of 30 μmol/L in the medium.The cell proliferation was detec-ted by CCK-8 assay.The clonal formation was detected by clonal formation experiment.The apoptosis was detected by TUNEL staining.The cell cycle was detected by flow cytometry.The cell migration and invasion were detected by Transwell assay.The contents of glucose,lactic acid and pyruvate in cell supernatant were detected by commercially available kits.The expression of KRT17 mRNA was detected by qRT-PCR.And the expression of KRT17,GLUT1,PDK1 and LDHA,p-PI3K,PI3K,p-AKT,AKT,p-mTOR and mTOR protein were detected by Western blot.Results The expression level of KRT17 in esophageal squamous cell carcinoma tissues was significantly higher than that in normal tissues(P<0.05).There was a statistically significant correlation between the expression level of KRT17 and the stage of esophageal squamous cell carcinoma(P<0.05).The survival prognosis of patients with low KRT17 expression was better than that of patients with high KRT17 expression(P<0.05).Compared with control group or si-NC group,the mRNA and protein expression of KRT17 in si-KRT17 group were decreased(P<0.05),and the cell proliferation activity,number of clone formation,migration and invasion cells were de-creased(P<0.05).And the lactic acid content,protein expression levels of GLUT1,PDK1 and LDHA were de-creased(P<0.05),values of p-PI3K/PI3K,p-Akt/AKT and p-mTOR/mTOR were decreased(P<0.05).The proportion of cells in G0/G1 phase,TUNEL positive rate,and contents of glucose and pyruvate were increased(P<0.05).Compared with si-KRT17 group,the mRNA and protein expression levels of KRT17 in activator group were increased(P<0.05),and the cell proliferation activity,number of clone formation,migration and in-vasion cells were increased(P<0.05).And the lactic acid content,protein expression levels of GLUT1,PDK1 and LDHA were increased(P<0.05),the values of p-PI3K/PI3K,p-Akt/AKT and p-mTOR/mTOR were in-creased(P<0.05).The proportion of cells in G0/G1 phase,TUNEL positive rate,and contents of glucose and pyruvate were decreased(P<0.05).Conclusions KRT17 is highly expressed in esophageal squamous cell car-cinoma tissues and cells.Silencing KRT17 inhibits proliferation,migration and invasion of human esophageal squamous cell line KYSE-150,and promotes apoptosis and cell cycle arrest.
6.Clinical characteristics and their correlations with systemic inflammatory and serological indicators in 235 hospitalized patients with pemphigus

Objective:To summarize the clinical and related characteristics of hospitalized patients with pemphigus, and to analyze their correlations with systemic inflammatory and serological indicators.Methods:A retrospective analysis was conducted on the clinical data from pemphigus patients hospitalized in the Department of Dermatology, Wuhan No.1 Hospital from January 2021 to December 2023. Spearman correlation analysis was performed to assess the correlations between the Pemphigus Disease Area Index (PDAI) scores and systemic immune-inflammation index (SII) , pan-immune-inflammation value (PIV) , serum albumin levels, anti-desmoglein 1/3 (Dsg-1/3) antibody levels, and C-reactive protein (CRP) levels. Linear regression models were used to evaluate the associations of systemic inflammatory and serological indicators with the length of hospital stay and treatment costs. Logistic regression analysis was conducted to analyze the effect of these indicators on the risk of infection in pemphigus patients.Results:A total of 235 pemphigus patients were included (112 males and 123 females) , with ages of 58.12 ± 16.47 years. Among them, 73 patients (31.06%) had pemphigus alone, while 162 (68.94%) had comorbidities including tumors, infections, or hypoalbuminemia. PDAI scores showed significantly positive correlations with SII, PIV, and CRP levels ( r = 0.62, 0.58, 0.50, respectively, all P<0.001) . According to PDAI scores, 164 cases (69.79%) were classified as mild pemphigus, 57 (24.26%) as moderate pemphigus, and 14 (5.96%) as severe pemphigus; compared with the patients with mild pemphigus, those with moderate-to-severe pemphigus had significantly increased SII, PIV, anti-Dsg-1 antibody and CRP levels, but significantly decreased serum albumin levels (all P < 0.05) . Among the 235 patients, 213 were diagnosed with pemphigus vulgaris, 9 with pemphigus erythematosus, 10 with pemphigus foliaceus, and 3 with paraneoplastic pemphigus; serum albumin levels and anti-Dsg-1/3 antibody levels differed significantly among patients with different subtypes of pemphigus (all P < 0.05) . The serum albumin level was significantly associated with the length of hospital stay and treatment costs ( β [95% CI]: -0.729 [-0.946 - -0.512], -0.266 [-0.362 - -0.171], respectively, both P < 0.001) ; furthermore, the serum albumin level was identified as a relevant factor for infections in pemphigus patients ( OR = 0.938, 95% CI: 0.883 - 0.995, P = 0.036) . Conclusion:SII, PIV, CRP, serum albumin, and anti-Dsg-1 antibody levels could reflect the severity of pemphigus to some extent, and the serum albumin level was significantly associated with comorbid infections, length of hospital stay, and treatment costs in hospitalized patients with pemphigus.

9.Relationship of frailty with sarcopenia and subjective social isolation among elderly inpatients

Objective:To understand the status of frailty among elderly inpatients,analyze its influencing factors,and explore the relationship of frailty with sarcopenia,and subjective social isolation in elderly inpatients.Methods:A total of 518 elderly inpatients from a tertiary hospital in Wuhu were selected as the research subjects.General information questionnaire,Frail Scale,SARC-F Scale,and Subjective Social Isolation Scale were used for investigation.Logistic regression analysis was used to analyze the influencing factors of frailty among elderly inpatients.Results:The incidence of frailty in 518 elderly inpatients was 52.5%(272/518).Logistic regression analysis revealed that recent falls(OR=3.458,95%CI:1.454-8.229),sarcopenia(OR=5.622,95%CI:2.494-12.677),subjective social isolation(OR=181.165,95%CI:57.307-572.721),polypharmacy(OR=2.409,95%CI:1.336-4.346),and 80-89 years old(OR=8.982,95%CI:0.640-2.357)were risk factors for frailty in elderly inpatients(P<0.05).Conclusions:The prevalence of frailty is high among elderly inpatients.Healthcare professionals should pay attention to the assessment of frailty in elderly inpatients,and promptly implement targeted interventions to slow down or prevent the progression of frailty.
10.Construction of a carbon dioxide laser-induced mouse corneal injury model and observation of corneal wound healing

Objective To construct a carbon dioxide(CO2)laser-induced corneal injury model in mice and observe the process of corneal wound healing.Methods Twenty eyes of ten C57BL/6J mice were divided into 4 groups.The cen-tral corneas in each group were irradiated by CO2 laser with a wavelength of 10.6 μm,spot diameter of 2 mm and power of 0.94 W.The exposure doses were 3.0 J·cm-2,4.5 J·cm-2,7.5 J·cm-2 and 10.5 J·cm-2,with corresponding expo-sure durations of 0.10 s,0.15 s,0.25 s and 0.35 s.Corneal injury severity was assessed using a slit lamp microscope,opti-cal coherence tomography and histopathological evaluation at 1 day after the exposure to determine the proper exposure dose for constructing a corneal injury model.Subsequently,the corneal injury model was constructed and the same meth-ods were used to monitor wound healing before,and 0 hours to 6 months after the exposure.Results No obvious corne-al injury was observed at an exposure dose of 3.0 J·cm-2.At an exposure dose of 4.5 J·cm-2,an off-white injury area appeared on the central cornea with loss of epithelium and endothelium.At an exposure dose of 7.5 J·cm-2 or 10.5 J·cm-2,the injury area became porcelain white,the cornea was thickened,and the iris was seen adhering to the margin of the cornea.Therefore,4.5 J·cm-2 CO2 laser was selected to construct a corneal injury model.At this exposure dose,the cornea swelled and thickened rapidly after injury,reached the maximum thickness 1 day after the exposure,and then grad-ually recovered,returning to normal by 14 days after the exposure.In the early stage(0 hours to 3 days after the expo-sure),the cornea showed shedding of injured epithelium and endothelium,migration of new epithelium and endothelium,and infiltration and regression of inflammatory cells.At the late stage(7 days to 6 months after the exposure),the cornea gradually returned to a normal physiological state,but some of the injured cornea exhibited stromal hyperplasia.Conclu-sion A CO2 laser with an exposure dose of 4.5 J·cm-2 can be used to construct a corneal injury model in mice.The a-cute phase of corneal injury primarily occurs within 3 days after the exposure.The cornea tends to restore its original physi-ological structure,but the corneal transparency cannot return to a normal state.
