1.Research advances in vestibular migraine

Vestibular migraine (VM) is the most common cause of recurrent vertigo, and it is an overlapping disease of central vestibular disease and migraine,as well as a syndrome characterized by the simultaneous or alternating occurrence of paroxysmal vestibular symptoms and typical migraine attacks.VM is known as the “chameleon of vestibular system diseases” due to its complex and diverse symptoms, which can easily lead to missed diagnosis and misdiagnosis. In recent years, with the development of neuroimaging technology and the implementation of multi-center clinical research, the pathogenesis of VM has gradually been expanded from the classic trigeminal neurovascular theory to the aspects of neurotransmitter disorder,cortical diffusion inhibition,and abnormal vestibular-cortical pathway integration. In addition, there have been constant improvements in the diagnosis system of VM,and the treatment modality of VM is gradually shifting to the comprehensive treatment paradigm of “symptomatic treatment in the acute stage combined with preventive treatment in the remission stage and lifestyle adjustment”.This article reviews the latest research advances in this field.

To explore the advantages of traditional Chinese medicine (TCM) and integrative TCM-Western medicine approaches in the treatment of diabetic microvascular complications (DMC), refine key pathophysiological insights and treatment principles, and promote academic innovation and strategic research planning in the prevention and treatment of DMC. The 38th session of the Expert Salon on Diseases Responding Specifically to Traditional Chinese Medicine, hosted by the China Association of Chinese Medicine, was held in Beijing, 2024. Experts in TCM, Western medicine, and interdisciplinary fields convened to conduct a systematic discussion on the pathogenesis, diagnostic and treatment challenges, and mechanism research related to DMC, ultimately forming a consensus on key directions. Four major research recommendations were proposed. The first is addressing clinical bottlenecks in the prevention and control of DMC by optimizing TCM-based evidence evaluation systems. The second is refining TCM core pathogenesis across DMC stages and establishing corresponding "disease-pattern-time" framework. The third is innovating mechanism research strategies to facilitate a shift from holistic regulation to targeted intervention in TCM. The fourth is advancing interdisciplinary collaboration to enhance the role of TCM in new drug development, research prioritization, and guideline formulation. TCM and integrative approaches offer distinct advantages in managing DMC. With a focus on the diseases responding specifically to TCM, strengthening evidence-based support and mechanism interpretation and promoting the integration of clinical care and research innovation will provide strong momentum for the modernization of TCM and the advancement of national health strategies.
3.Establishment and Evaluation of New Mouse Model of Rheumatoid Arthritis Combined with Interstitial Lung Disease

ObjectiveTo establish a mouse model of rheumatoid arthritis with interstitial lung disease (RA-ILD) in DBA/1 mice using Porphyromonas gingivalis (Pg) infection combined with collagen-induced arthritis (CIA), and to comprehensively evaluate pathological characteristics in joints, lungs, and serum. MethodsForty DBA/1 mice were randomly divided into four groups, i.e., Control, Pg infection (Pg), CIA, and Pg infection combined with CIA (Pg+CIA), with 10 mice in each group. Arthritis clinical symptoms were evaluated by recording arthritis incidence and clinical scores. Micro-CT scanning was used to assess knee joint pathology. Histopathological changes and collagen deposition in knee joints and lung tissues were analyzed using hematoxylin-eosin (HE) and Masson staining. Immunohistochemistry was performed to detect protein expression of α-smooth muscle actin (α-SMA), typeⅠ collagen (ColⅠ), and fibronectin (FN) in lung tissues. Real-time quantitative polymerase chain reaction(Real-time PCR)was used to measure mRNA expression levels of α-SMA, ColⅠ, FN, tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), and IL-1β in lung tissues. Enzyme-linked immunosorbent assay (ELISA) was used to detect serum levels of Pg, cyclic citrullinated peptide (CCP), and immunoglobulin G (IgG). ResultsJoint lesions: The CIA and Pg+CIA groups showed 100% arthritis incidence, with evident joint redness, swelling, and deformity. The number of affected limbs was 27 and 28, and clinical scores were 68 and 70, respectively. No obvious clinical symptoms were observed in the Pg group. Histopathological and imaging analyses showed severe joint lesions in the CIA and Pg+CIA groups, with significantly increased histopathological scores, bone mineral density, bone volume fraction, trabecular thickness, and trabecular number compared to the Control group (P<0.01). No obvious joint pathology was observed in the Pg group. Lung lesions: The Pg+CIA group exhibited marked alveolar inflammation, interstitial inflammatory cell infiltration, and alveolar wall thickening, with pronounced blue staining of collagen fibers. Histopathological scores and collagen area ratios were significantly higher than those of the Control, Pg, and CIA groups (P<0.05). Lung protein and mRNA expression levels of α-SMA, ColⅠ, and FN were markedly increased, and mRNA levels of IL-6, TNF-α, and IL-1β were significantly elevated compared to the Control group (P<0.05). Serology: The Pg+CIA group showed significantly higher levels of CCP, Pg, and IgG compared with the Control, Pg, and CIA groups (P<0.05). ConclusionDBA/1 mice subjected to Pg infection combined with CIA exhibited pronounced symptoms and pathological features of RA-ILD, along with elevated serum anti-CCP antibody levels. This model represents a novel RA-ILD mouse model, providing a valuable experimental tool for investigating RA-ILD pathogenesis and developing new therapeutics, and serves as a basis for establishing anti-cyclic citrullinated peptide antibody (ACPA)-positive RA-ILD animal models.
4.A Preliminary Discussion on TONG Xiaolin's "Eight-Dimensional Materia Medica Framework" Theory

This paper introduces the "eight-dimensional materia medica framework" theory proposed by Professor TONG Xiaolin and, in light of his academic perspectives and clinical experience, explores its inheritance and innovation of the concept of "four pillars of medicinals" by ZHANG Jiebin of Ming-dynasty from multiple angles. Building upon the four foundational medicinals, Renshen (Panax ginseng), Shudihuang (Rehmannia glutinosa Praeparata), Fuzi (Aconitum carmichaelii Praeparata), and Dahuang (Rheum palmatum), as the "four pillars of medicinals", and using eight-principle pattern differentiation framework, this theory expands the representative medicinals to eight. It establishes the "eight-dimensional materia medica framework", i.e. exterior-Mahuang (Ephedra sinica), interior-Rougui (Cinnamomum cassia), cold-Huanglian (Coptis chinensis), heat-Ganjiang (Zingiber officinale), deficiency-Renshen (Panax ginseng), excess-Dahuang (Rheum palmatum), yin-Shudihuang (Rehmannia glutinosa Praeparata), and yang-Fuzi (Aconitum carmichaelii Praeparata). This framework highlights the pivotal roles of these eight medicinals in the treatment of exterior and interior patterns, cold and heat syndromes, deficiency and excess conditions, as well as yin deficiency and yang deficiency. Integrated with the state-target differentiation and treatment theory, this approach enriches the traditional actions of the eight representative medicinals by incorporating findings from modern pharmacological research, achieving a transition from macroscopic regulation of pathological states to microscopic target-oriented intervention. Furthermore, through investigations into dose-effect relationships and the expansion of principles for herbal compatibility, the clinical practicality of the "eight-dimensional materia medica framework" is further demonstrated, promoting the integrative development of Chinese and western medicine at the level of clinical application of Chinese materia medica.

MethodsQualitative studies focusing on the illness experiences and needs of stroke patients in the rehabilitation period were retrieved from the databases of Cochrane Library, PubMed, Embase, Web of Science, CINAHL, CNKI, CBM, Wanfang data and VIP. The search timeframe was from database inception to December, 2024. Methodological quality was evaluated using the Joanna Briggs Institute Qualitative Assessment and Review Instrument. A meta-synthesis method was adopted to categorize and integrate the findings. ResultsA total of ten studies were included. Forty-nine themes were extracted and further grouped into ten categories, which were finally integrated into three overarching themes: negative illness perceptions, multifaceted rehabilitation motivations and unmet multidimensional needs. ConclusionPatients with stroke undergo complex physical and psychological experiences during rehabilitation and present diverse and multidimensional needs.

9.Ultrasound-Guided Microwave Ablation of Papillary Thyroid Cancer in the Context of Hashimoto Thyroiditis

Purpose To evaluate the safety and efficacy of microwave ablation(MWA)treatment for papillary thyroid cancer(PTC)in patients with Hashimoto thyroiditis(HT).Materials and Methods A retrospective analysis was conducted on clinical data from April 2020 to May 2022,involving 181 patients who underwent MWA at the China-Japan Friendship Hospital.All patients were divided into experimental group(n=89 cases)and control group(n=92 cases).The technical success rate,tumor volume reduction rate(VRR),disease progression and incidence of complications were compared between the two groups,respectively.Results Both groups achieved a technical success rate of 100%,with a median follow-up time of(16.90±11.43)months.At 3,6,12,and 18 months post-ablation,the VRR in the experimental group was significantly lower than that in the control group(3 months:-405.10(-778.57,-119.64)%vs.-190.00(-525.62,0)%;6 months:-50.00(-318.00,45.52)%vs.52.75(-93.33,97.13)%;12 months:83.33(17.70,100.00)%vs.100.00(64.88,100.00)%;18 months:100.00(96.05,100.00)%vs.100.00(100.00,100.00)%,Z=-2.77,-3.70,-2.41,-2.18,all P<0.05).At the end of follow-up,there was no significant difference in the disease progression rates and the incidence of hoarseness between the two groups(5.61%vs.4.34%,2.24%vs.4.34%,both P>0.05).Conclusion Patients with HT who received MWA treatment demonstrate safety and effectiveness,with no significant differences in postoperative disease progression rates compared to the controls.
10.HDAC2-mediated H3K27 acetylation promotes the proliferation and migration of hepatocellular carcinoma cells

Objective:To explore the specific mechanism of histone deacetylase 2(HDAC2)mediated histone H3 lysine 27 acetylation(H3K27ac)modification in promoting the proliferation and migration of hepatocellular carcinoma cells.Methods:Samples of 40 cases of hepatocellular carcinoma and paracan-cerous tissues resected from January 2021 to January 2023 were collected.The expressions of HDAC2 and H3K27ac in hepatocellular carcinoma,paracancerous tissues and cell lines were detected by immunohis-tochemistry and Western blotting.The correlation between the expression levels of HDAC2 and H3K27ac and the relationship between HDAC2 expression and clinicopathological characteristics of patients with hepatocellular carcinoma were analyzed.The proliferation,migration and invasion of Hep3B and HepG2 cells were determined by MTS,clone formation,scratch and Transwell experiments.The acetylation of H3K27 mediated by HDAC2 was verified by Western blotting,real-time fluorescence quantitative PCR(qRT-PCR)and chromatin immunoprecipitation high-throughput sequencing(ChIP-seq).In vivo xeno-transplantation experiment,the tumorigenicity of cells in each group was measured,and the expression of proteins related to phosphoinositide 3-kinases/phosphatase and tensin homolog deleted on chromosome ten/protein kinase B/mammalian target of rapamycin(PI3K/PTEN/AKT/mTOR)signal pathway was detected.Results:High expression of HDAC2 and low expression of H3K27ac were found in hepatocel-lular carcinoma tissues and cell lines(P<0.05),and there was a negative correlation between them(r=-0.477,P=0.002).The expression of HDAC2 was related to tumor size,hepatitis B virus infec-tion,TNM stage and portal vein tumor thrombus(P<0.05).Compared with the sh-NC group of Hep3B and HepG2 cells,the proliferation,clone formation,migration and invasion ability of sh-HDAC2 group were decreased(P<0.05).Compared with the Empty group,the HDAC2 group exhibited increased ex-pression levels and activity of HDAC2,as well as enhanced cell proliferation,clone formation,migra-tion,invasion ability,tumor volume and mass in vivo,and elevated expression levels of p-PI3K,p-AKT,and p-mTOR(P<0.05).Conversely,the enrichment and expression levels of H3K27ac,along with the expression level of PTEN,were decreased(P<0.05).In the iHDAC2 group,the expression levels and activity of HDAC2,as well as the proliferation,clone formation,migration,invasion ability,tumor volume and mass in vivo,and expression levels of p-PI3K,p-AKT,and p-mTOR were reduced(P<0.05).Additionally,the expression levels of H3K27ac and PTEN were increased(P<0.05).To validate the involvement of the PI3K/PTEN/AKT/mTOR signaling pathway in HDAC2-mediated regula-tion of malignant behaviors in liver cancer cells through H3K27ac,the PI3K activator 740Y-P was intro-duced.Compared with the iHDAC2 group,the iHDAC2+740Y-P group exhibited increased prolifera-tion,clone formation,migration,invasion ability,tumor volume and mass in vivo,and elevated expres-sion levels of p-PI3K,p-AKT,and p-mTOR(P<0.05).Conversely,the expression level of PTEN was decreased(P<0.05).Conclusion:HDAC2 initiates PI3K/PTEN/AKT/mTOR signal pathway by me-diating H3K27 acetylation,which promotes the occurrence and development of hepatocellular carcinoma.
