1.Attenuation of esophageal precancerous lesions in mice by Banxia Xiexin Decoction through gut microbiota modulation

Objective:
To investigate the microbial mechanisms of Banxia Xiexin Decoction (半夏泻心汤, BXXXD) in the treatment of esophageal precancerous lesions.
Methods:
A total of 30 specific pathogen-free (SPF) grade female C57BL/6J mice were randomly assigned to a control group (n = 6) and a 4-nitroquinoline 1-oxide (4-NQO)-exposed group (n = 24). Esophageal precancerous lesions were induced by providing the 4-NQO-exposed group with 4-NQO in drinking water (100 μg/mL) for 17 consecutive weeks, whereas control group received sterile drinking water. After model establishment, the mice in 4-NQO-exposed group were further randomized into model group and three BXXXD-treated groups: low-dose (BXXXD-L, 3.7 g/kg), medium-dose (BXXXD-M, 7.4 g/kg), and high-dose (BXXXD-H, 14.8 g/kg) groups (n = 6 per group). During the subsequent intervention period, mice in control and model groups were gavaged with sterile water, while mice in BXXXD groups were gavaged once daily with the corresponding dose of BXXXD aqueous extract for 4 weeks. Histopathological changes in esophageal tissues were observed by hematoxylin and eosin (HE) staining. The fecal and esophageal microbiota were profiled via 16S rDNA high-throughput sequencing to evaluate bacterial diversity, community structure, and co-occurrence networks. BXXXD chemical fingerprints were analyzed using ultra-high-performance liquid chromatography coupled with quadrupole QExactive Orbitrap mass spectrometry (UHPLC-QE-MS). Serum short-chain fatty acids (SCFA) level was quantified by targeted metabolomics using gas chromatography-mass spectrometry (GC-MS). Transcriptomic analysis of esophageal tissues was performed to assess gene expression profiles.
Results:
Compared with model group, BXXXD-M group exhibited reduced mucosal hyperplasia and more orderly epithelial cell arrangement, with superior therapeutic effects in comparison with both BXXXD-L and BXXXD-H groups (P < 0.01). Microbiota analysis revealed that BXXXD increased the abundance of beneficial Enterococcus and reduced pathogenic Escherichia-Shigella in the esophagus. In the gut, BXXXD elevated the relative abundance of beneficial taxa, including Lactobacillus, Dubosiella, Bacteroides, and Faecalibacterium. Targeted metabolomics showed that BXXXD significantly reduced total serum SCFA level (P < 0.01). Transcriptomic analysis indicated that BXXXD downregulated the expression of genes associated with the progression, migration, and invasion of esophageal cancer, which were identified as kallikrein-related peptidase 6 (Klk6), defensin beta 4 (Defb4), family with sequence similarity 3 member B (Fam3b), carboxypeptidase A4 (Cpa4), serum amyloid A1 (Saa1), and chitinase-like 1 (Chil1) (P < 0.05).
Conclusion
BXXXD may reduce the expression levels of esophageal cancer-related genes and improve esophageal precancerous lesions through modulation of the gut microbiota and metabolites.
2.Discovery and preliminary validation of five early gastric cancer biomarkers,including TAGLN2 and CTSD,based on serum proteomics

Background and purpose:Gastric cancer,as one of the most common malignant tumors,requires early diagnosis and treatment to improve patient prognosis.This study,based on serum quantitative proteomics research of early-stage gastric cancer patients and non-gastric cancer patients,aims to identify potential diagnostic biomarkers for early gastric cancer.Methods:Serum samples from primary gastric cancer patients and healthy control individuals were collected from Lanzhou University Second Hospital between June and December 2023,following inclusion and exclusion criteria.A protein spectral library was established using Data-Dependent Acquisition(DDA)mode,and each sample was analyzed using Data-Independent Acquisition(DIA)mode.The STRING database was used to analyze protein-protein interactions of upregulated proteins in gastric cancer serum.Kyoto Encyclopedia of Genes and Genomes(KEGG)and Gene Ontology(GO)were used to analyze the pathways and functional annotations of the corresponding genes.Gene expression levels in gastric cancer and non-gastric cancer tissues were analyzed using GEPIA 2,and overall survival of each gene in gastric cancer was analyzed using Kaplan-Meier Plotter.Differential gene expression in clinical gastric cancer and adjacent tissues was validated by quantitative reverse transcription polymerase chain reaction(qRT-PCR)This study was approved by the Ethics Committee of Lanzhou University Second Hospital(Ethical No.:2023A-459)and was exempt from the informed consent.Results:Finally,serum samples from 30 primary gastric cancer patients,29 healthy control individuals,along with the para-cancerous tissues from 8 patients were collected.A total of 666 intersecting proteins were identified through serum quantitative proteomics.Among them,16 proteins showed upregulated expression and 22 proteins showed downregulated expression in the gastric cancer group(P<0.05,|FC|≥1.5).STRING database analysis showed that 10 upregulated proteins were involved in interaction networks.KEGG and GO analysis indicated that these genes were closely related to the biological processes of cancer occurrence and development.GEPIA 2 and Kaplan-Meier Plotter analysis showed that 6 genes,B2M,TAGLN2,CTSD,HSP90AB1,SH3BGRL3,and CFL1,which were highly expressed in the gastric cancer group(P<0.05)and associated with poor prognosis.Clinical verification by qRT-PCR confirmed that TAGLN2,CTSD,SH3BGRL3,CFL1 and HSP90AB1 were highly expressed in gastric cancer tissues(P<0.05).Conclusion:TAGLN2,CTSD,SH3BGRL3,CFL1,and HSP90AB1 have the potential to serve as clinical early gastric cancer diagnostic serum biomarkers,which may facilitate early diagnosis and treatment of gastric cancer.

High-quality development of public hospitals requires cultural construction as an endogenous driving force,ai-ming to build a cultural system that integrates systematic,innovative,and humanistic characteristics.Leveraging its century-old historical legacy,Qilu Hospital of Shandong University has established a cultural construction pathway characterized by"strategic planning,heritage preservation,brand building,exemplary leadership,and communication innovation"through five major sys-tems:improving top-level design,inheriting historical heritage,building distinctive brands,cultivating exemplary models,and expanding communication channels.This approach provides cultural and spiritual impetus for the hospital's high-quality develop-ment.Empirical evidence demonstrates that cultural construction in public hospitals must emphasize systematic top-level design,distinctive cultural identity,synergy between tradition and innovation,and long-term sustainability,ultimately transforming cul-tural"soft power"into"hard support"for development.This practice offers a replicable"Qilu Model"for nationwide public hos-pital cultural development,while injecting new connotations into the implementation of the Healthy China strategy.

The therapeutic potential of heavy ion accelerators in malignant tumors is investigated through the analyses of precision dosimetry,biological optimization and side effects,evaluation of efficacy outcomes across diverse tumor types,and development prospects.By reviewing the recent literatures and clinical researches on heavy ion therapy,the study focuses on analyzing the physical characteristics and biological effects of heavy ion beams,summarizes their application achievements in complex lesions such as head and neck,chest and abdomen,and bone and soft tissue tumors,and explores their optimization strategies in light of current technological advancements.Research analysis has demonstrated that heavy ion therapy has distinct advantages in precise targeting and effective eradication of tumor cells,achieving high local control rate and success rate.In addition,heavy ion therapy significantly reduces the risk of radiation damage to normal tissues and improves the quality of life of patients.Heavy ion accelerator offers an efficient and safe option for malignant tumor radiotherapy.Its superior physical dose distribution and enhanced biological effects make it become a vital tool in modern precision cancer treatment.Through continued optimization of accelerator technology,heavy ion therapy is expected to become a mainstream treatment modality for more cancer patients.
6.Value analysis of management model of data mining in reducing failure rate of medical imaging equipment

Objective:To construct a management model of data mining for medical imaging equipment,so as to improve the quality of managing equipment.Methods:A management model of mining data was constructed to manage medical imaging equipment.A total of twenty imaging equipment that were using at Hainan Hospital of the General Hospital of the People's Liberation Army of China from April 2022 to March 2024 were selected.According to different management methods,the conventional management was adopted to manage them during April 2022 to March 2023,and the management model of mining data(model management)was adopted to manage them during April 2023 to March 2024.A self-developed questionnaire was used to conduct a satisfaction survey for imaging physicians,staffs of operating and maintaining equipment,and technicians who using and managing equipment,and patients who received diagnosis and treatment by using equipment.The failure rate and the imaging effect of equipment,the satisfaction scores of the relative staffs who used equipment,and the growth amplitude of operational benefits of equipment between two management methods were compared.Results:A total of 20 failures occurred in imaging equipment that adopted model management.In them,the failure rates of the self-equipment,improper operation and insufficient professional level were respectively 15%,5%and 5%,all of which were lower than those of the conventional management method.The predicted failure rate of model management was 75%,which was higher than that of the conventional management method,and the differences of the above indicators between two methods were statistically significant(x2=6.547,4.392,5.124,6.701,P<0.05).The scores of image clarity,qualification rate,excellent rate,qualification rate of body position,and the total score of imaging effect of adopting model management method were respectively(22.36±2.01),(23.21±1.54),(22.65±1.87),(23.21±1.52)and(91.43±6.77)points,all of which were higher than those of the conventional management method,and the differences were statistically significant(t=10.662,12.727,15.324,16.333,13.742,P<0.05).The satisfaction scores of imaging physicians,staffs of operation and maintenance,technicians and patients who uses management for adopting the model management method were all higher than those of the conventional management method,and the differences were statistically significant(t=13.586,14.249,17.021,11.006,P<0.05).The average values of growth amplitude of cost and benefit of equipment operation of adopting model management method were higher than those of the conventional management method,and the average value of the growth amplitude of the cost of the troubleshooting of model management method was lower than that of the conventional management method,and the differences of them were statistically significant(t=15.057,19.310,18.336,P<0.05).Conclusion:The application of the management model of data mining for medical imaging equipment can provide warning of equipment failures in advance,and reduce the failure rate of equipment,and improve the quality of management and operation of equipment,and enhance the service level of equipment.
7.Research progress on the mechanism and clinical application of the effective ingredients from Sijunzi decoction in the treatment of gastric cancer

Gastric cancer is a common malignant tumor of the digestive tract and can be classified as “fullness of the stomach”, “epigastric pain”, “noise” and other categories in the field of traditional Chinese medicine. Sijunzi decoction is composed of Panax ginseng, Poria cocos, Atractylodes macrocephala, and honey-fried Glycyrrhiza uralensis, and it has the effect of tonifying qi and strengthening the spleen. This article summarizes the active ingredients, mechanism of action, and clinical application research progress of Sijunzi decoction in treating gastric cancer. The results show that the main active ingredients of Sijunzi decoction include ginsenosides, atractylenolide, pachymic acid, glycyrrhizic acid, etc.; Sijunzi decoction and its effective ingredients can play an anti-gastric cancer role by inhibiting the proliferation of gastric cancer cell, inducing apoptosis of gastric cancer cell, enhancing gastric cancer cell chemotherapy sensitivity, and inhibiting invasion and metastasis of gastric cancer cell. In addition, Sijunzi decoction can enhance the efficacy of chemotherapy drugs, strengthen the immune function of the body and lower serum cancer marker levels during the clinical treatment of gastric cancer.
8.Mediating effect of frailty on social isolation and cognitive function among the elderly

Objective:
To examine the mediating effect of frailty on social isolation and cognitive function among the elderly.
Methods:
Demographic information, smoking, alcohol consumption and cognitive function of the elderly at ages of 60 years and older were collected from the China Health and Retirement Longitudinal Study 2020. Social isolation and frailty were evaluated using social isolation index and frailty index, respectively. The mediating effect of frailty on social isolation and cognitive function was analyzed using the Process program, and the significance of the mediating role was tested using the Bootstrap test.
Results:
A total of 2 822 individuals were enrolled, including 1 483 males (52.55%) and 1 339 females (47.45%). There were 2 497 (88.48%) and 325 (11.52%) individuals at ages of 60-<75 years and ≥75 years, respectively. The median cognitive function score was 14 (interquartile range, 16) points. There were 432 cases with social isolation (15.31%), with a median social isolation index of 10 (interquartile range, 5) points. The median frailty index was 0.11 (interquartile range, 0.15). There were 1 111 individuals without frailty, accounting for 39.37%; 1 214 individuals with pre-frailty, accounting for 43.02%; and 497 individuals with frailty, accounting for 17.61%. Mediating effect analysis showed that social isolation affected cognitive function directly and negatively with the effect value of -0.773 (95%CI: -0.899 to -0.647), and also affected cognitive function by frailty indirectly and negatively with the effect value of -0.147 (95%CI: -0.188 to -0.110), with the mediating effect contributed 15.98% of the total effect.
Conclusion
Frailty can directly or indirectly affect cognitive function among elderly through social isolation.
9.Current Status, Challenges, and Prospects of Clinical Research on Rare Tumors in China

Although the incidence of each individual type of rare tumor is relatively low, their cumulative impact affects a vast patient population, and their survival prognosis is significantly worse than that of common tumors. In recent years, China has made remarkable progress in clinical research on rare tumors, with a rapid increase in the number of studies and a diversified exploration of treatment modalities. However, clinical research on rare tumors still faces numerous challenges, such as uneven distribution of medical resources, the absence of a foundational registration system, and insufficient motivation for original research and development. Looking ahead, key measures including specialized development, a national collaborative group model, full utilization of real-world data, application of novel clinical research designs, and artificial intelligence technologies will strongly drive comprehensive advancements in China′s rare tumor prevention and treatment system.
10.Research Progress in the Diagnosis and Treatment of Pancreatic Acinar Cell Carcinoma

Pancreatic acinar cell carcinoma (PACC) is a rare exocrine tumor of the pancreas with distinct clinical and pathological features. In recent years, advancements in molecular biology techniques have led to a deeper understanding of the molecular mechanisms underlying PACC. Progress in imaging, endoscopic, and molecular diagnostic technologies has improved the early detection rate of PACC. The primary treatment modalities for PACC include surgical resection, chemotherapy, targeted therapy and immunotherapy; however, the therapeutic efficacy still requires further improvement. This article reviews the current research status of PACC, covering its epidemiology, pathological characteristics, molecular alterations, diagnostic methods, and treatment strategies, and discusses the controversies and future directions in PACC research.
