Objective: To develop a prediction model for mild cognitive impairment (MCI) risk among the elderly, so as to provide a tool for MCI early screening. Methods : From July 2022 to September 2024, a multi-stage stratified random cluster sampling method was used to recruit permanent residents aged ≥65 years from the Xinjiang Uygur Autonomous Region as study participants. Data on sociodemographic characteristics, nutritional status, body composition indices, bone mineral density, and handgrip strength were collected through questionnaires and physical examinations. Sarcopenia was defined based on appendicular skeletal muscle index and handgrip strength. MCI was assessed using the Mini-Mental State Examination, with adjustments for educational level. Participants were randomly divided into a training set and a validation set in a 7∶3 ratio. LASSO regression and multivariable logistic regression models were employed to screen for predictors and construct an MCI risk prediction model. The predictive performance of the model was evaluated using receiver operating characteristic (ROC) curve and decision curve analysis (DCA). Results: A total of 1 641 participants were surveyed, including 755 males (46.01%) and 886 females (53.99%). The majority of participants were aged 65-<75 years, comprising 1 154 individuals (70.32%). MCI was detected in 517 participants, corresponding to a detection rate of 31.51%. Resultsfrom LASSO regression and multivariate logistic regression analysis showed that residence (rural, OR = 2.323, 95% CI: 1.682-3.210), age (75-<85 years, OR = 1.405, 95% CI: 1.019-1.937; ≥85 years, OR = 3.655, 95% CI: 1.696-7.875), educational level (primary school, OR = 0.341, 95% CI: 0.247-0.472; junior high school, OR = 0.255, 95% CI: 0.160-0.408; high school, OR = 0.286, 95% CI: 0.154-0.531; bachelor's degree or above, OR = 0.120, 95% CI: 0.041-0.351), history of alcohol consumption (yes, OR = 3.216, 95% CI: 2.164-4.779), risk of malnutrition (yes, OR = 1.464, 95% CI: 1.064-2.014), sarcopenia (yes, OR = 3.197, 95% CI: 2.332-4.385), and waist-to-hip ratio (abnormal, OR = 1.540, 95% CI: 1.159-2.048) were identified as predictive factors for MCI among the elderly. In the training set, the area under the ROC curve, sensitivity, and specificity were 0.788, 0.719, and 0.712, respectively. In the validation set, the corresponding values were 0.784, 0.913, and 0.542, respectively. DCA demonstrated that the model provided a higher clinical net benefit for predicting MCI risk when the risk threshold probability ranged from 0.124 to 0.764. Conclusion The prediction model developed in this study demonstrates good discriminative ability and clinical utility, indicating its substantial value for predicting the MCI risk among the elderly.

Objective To identify risk factors of progressive pulmonary fibrosis(PPF)development in patients with primary Sj?gren's syndrome-associated interstitial lung disease(pSS-ILD),and to develop and validate a clinically applicable nomogram prediction model.Methods Enrolled 168 pSS-ILD patients from Three Gorges Hospital affiliated to Chongqing University from January 2019 to January 2023,randomly allo-cated into training(n=117)and internal validation(n=51)cohorts at 7∶3 ratio.The training cohort was stratified into the PPF group(n=38)and the non-PPF group(n=79)based on one-year radiographic pro-gression.An external validation cohort(n=24)was prospectively enrolled from February 2023 to April 2024.Multivariable analyses incorporated demographic characteristics,clinical manifestations,laboratory parame-ters,pulmonary function tests,and high-resolution CT(HRCT)imaging features.A nomogram was developed using independent predictors identified through logistic regression.Receiver operating characteristic(ROC)curve,area under curve(AUC),calibration curve,Hosmer-Lemeshow test and decision curve were used to e-valuate the prediction efficiency of the model.Results PPF developed in 31.0%(52/168)of patients within one year.Univariate and multivariate logistic regression analysis showed that age,25 hydroxyvitamin D level and high resolution CT fibrosis score were independent risk factors for PPF phenotype in pSS-ILD patients(P＜0.05).Based on the above predictors,a column-line diagram was constructed.Internal verification showed that the column-line differentiation ability was good(AUC=0.88,95%CI:0.79-0.98),and exter-nal verification showed that the predicted probability of the calibration curve was close to the actual probabili-ty(HL test χ2=9.516,P=0.301),indicating that the model had good consistency.Conclusion Age,serum 25 hydroxyvitamin D level and high resolution CT fibrosis score are independent risk factors for PPF phenotype in pSS-ILD patients.The nomogram model constructed according to the above three predictors has good differentiation and consistency,and could provide a reference for the clinical prediction of PPF in pSS-ILD patients.

Nuanjian Jian(Decoction for Warming Liver Meridian),originating from Jing Yue Quan Shu(Jingyue's Complete Works)written by Zhang Jingyue,is composed of Angelicae Sinensis Radix(Danggui),Lycii Fructus(Gouqizi),Poria(Fuling),Fructus Foeniculi(Xiaohuixiang),Cinnamomi Cortex(Rougui),Linderae Radix(Wuyao),and Aquilariae Lignum Resinatumd(Chenxiang)or Aucklandiae Radix(Muxiang).Nuanjian Jian is a widely-used classical formula for warming the liver and kidneys,promoting qi circulation and alleviating pain.It was included in the Ancient Classical Formulas Catalog(First Batch)published by the National Administration of Traditional Chinese Medicine in 2018,and was primarily used to treat lower abdominal pain,hernia,and other disorders differentiated as the syndrome of cold stagnation in the liver and kidneys.Based on the principles of key information verification for classical formulas,this article systematically reviewed the ancient and modern literature on Nuangan Jian,and conducted a comprehensive textual research and analysis of its origin,composition,dosage,method of making a decoction and taking medicines,processing,modifications,historical and modern applications,and experimental studies.The study concluded that the composition and dosage of this formula are relatively clear.The contemporary single-dose equivalents are calculated as Danggui 9.33 g,Gouqizi 11.19 g,Fuling 7.46 g,Xiaohuixiang 7.46 g,Rougui 5.60 g,Wuyao 7.46 g,and Chenxiang 3.73 g.The decoction is prepared by adding 300 mL of water and 4 g of ginger,boiling until reduced to 140 mL,and taken 2-3 times daily,2 hours after meals.Modern practitioners have expanded the indications of this formula,extending its use from hernia and lower abdominal pain to various disorders in andrology,gynecology,and gastroenterology.The indications have the core pathogenesis of cold stagnation in the liver meridian,and usually affect the liver while also involve the spleen,kidneys,and lower energizer.Diseases of the lower abdomen,genitals,and breasts belonging to the region of the liver-meridian route,are also commonly treated with Nuangan Jian.The findings of this study provide a reference for the further development of this classical formula.

Vascular biomechanics mainly explores how vascular cells perceive mechanical stimuli,how mechanics affects the development of diseases,and the exploitation of various mathematical models to analyze the effects of mechanical factors on diseases.In recent years,researches in the field of vascular biomechanics are developing rapidly,and various research teams have analyzed the mechanical and biological processes of blood vessels from different directions,in order to gain a deeper understanding of the regulatory mechanisms of vascular biomechanical factors affecting the progression of various vascular diseases,and provide a theoretical basis based on the mechanobiology for the prevention and treatment of cardiovascular and cerebrovascular diseases.This article summarizes and discusses the recent research hotspots and emerging trends in the field of vascular mechanobiology based on domestic and foreign expert teams and combined with the work of this research team,thus providing a systematic framework for grasping hotspots and exploring new research directions in the field of vascular mechanobiology.

Objective To investigate the relationship between red blood cell distribution width (RDW) and prognosis in patients with multiple myeloma (MM).Methods The population that studied consisted of 27 patients with multiple myeloma and 30 healthy controls.The RDW was calculated according to the results of blood routine examination and compared between patients and healthy controls.Then,compared the difference between the two groups of RDW.MM patients were treated with international standard staging (ISS),and the differences of RDW in different stages were analyzed.ISS staging was used to draw the receiver operating curve (receiver operating characteristic curve,ROC curve),then take RDW14.65 ％ as the best cut-off point,the MM patients were divided into low RDW group (RDW=14.65 ％) and high RDW group (RDW ＞14.65％).Overall survival (OS) condition were compared between the above two groups.The impacts of RDW on OS were analyzed by Kaplan-Meier and Log-rank test.Results The average RDW value in experimental and controlled were 15.60 ％ ± 2.35 ％ vs 12.72 ％ ±0.61 ％ separately (t=6.201,P＜0.001),with statistical differences.The average RDW value in low ISS(Ⅰ + Ⅱ stage) and high ISS (Ⅲ stage) were 13.99 ％ ± 1.08％ vs 16.55 ％±2.39％ separately (t=3.800,P=0.001).The median survival time of low RDW and high RDW group was 13 months and 8 months respectively,and the difference was statistically significant (x2=6.481,P =0.011).Conclusion RDW increased in patients with MM,the risk stratification higher prognosis is worse.

Ob jective To construct and screen out the RPL 23-siRNA interference fragments ,providing the basis for the following experiments about the correlation with RPL 23 and gastric cancer .Methods The RPL23-siRNA,synthesized chemically through lipofection ,were selected from three target sequences by RNA in-terference and detected by real -time PCR and Western blot .Results Compared with normal cell group and RPL23 control group ,the mRNA and protein expression of RPL 23 in the other 3 interference groups were signifi-cantly decreased(P<0.01).Multiple comparisons showed that the interference efficiency of RPL 23 -siRNA1 group was significantly higher than that of RPL 23-siRNA2 group and RPL23-siRNA3 group(P<0.01).Con-clusion The RPL23-siRNA interference fragment can be successfully constructed and screened out ,which pro-vides the basis for the following experiments .

Ribosomal protein L23 is a new target for gene therapy of cancer.It participates in tumor progression by activating p53,inactivating murine double minute 2,regulating the carcinogenic activity of c-Myc,inducing the multi-drug resistance,and affecting the biologic behaviour of tumors.Generally,it′s con-sidered to be a potential prognostic factor in human cancers.

Objective To investigate the serum lactate dehydrogenase (LDH),total bilirubin (TBIL)and indirect bilirubin (IBIL)and homocysteine (Hcy)levels change in megaloblastic anemia (MA)and myelodysplastic syndrome (MDS)in the differential diagnosis of the clinical application value.Methods Hitachi 7 600 automatic biochemical analyzer was applied to the determination of 40 patients with MA,30 with MDS patients and 40 healthy physical examination (normal control group)of serum LDH,TBIL,IBIL and Hcy levels,and application SPSS16.0 statistical software for data processing were retrospectively analyzed.Results Ma group of LDH,TBIL,IBIL and Hcy four biochemical index level was significantly higher than that of MDS group and normal control group (P<0.01).MDS group of LDH levels higher than normal control group (P<0.01).MDS group of TBIL,IBIL and Hcy,there was no statistically significant difference compared with normal control group (P>0.05).MA group of LDH,TBIL,IBIL,Hcy higher positive rate higher than that of MDS group,LDH and Hcy was most significant,four biochemical indexes between the two groups compared to increase the positive rate of difference was statistically significant (P<0.01).Conclusion Joint detection of LDH,TBIL,IBIL and Hcy four biochemical index level of the differential diagnosis of MA and MDS has certain clinical value.

Objective To analyze health status and characters of health service demand and utilization among farmers and herdsmen in Kangxiwa and Saitula border areas from the altitude of 3700m to 4300m for offered scientific advices to the health management institutions for optimizing the allocation of health resources.Methods A total of 51 families in the Kangxiwa and Saitula areas in Karakorum-Kunlun Mountains were randomly chosen with stratified sampling method for the family health survey.Physical and biochemical examination were carried out for each herdsman.Results Among the participants,the most disease the two-week medical consultation rate was 1.74％.the hospitalization rate was 0.98％.The two-week medical consultation and hospitalization rate of herds were lower than the national and Xinjiang level for rural area.Herdsmen's doctor poor accessibility and low-income may be the main reasons for the underutilization of health services ;The difficult situation of herdsmen has been initially improved after participating in the new rural cooperative medical care.Conclusion The health status of native herdsmen in Kara Kunlun mountains was poor,and the provision of health services cannot meet the demand for health service of native herdsmen.It's necessary to improve efficiency of utilization of health service and optimize the allocation of health resources.

BACKGROUND:Now researches on psychiatric disease and molecular heredity are becoming more and more gradually because of the development of molecular genetics,but conclusions are controversial,and for the time being there are no reports about whether there are regional differences and ethnical differences in the distribution of genotype frequency of 5-hydroxytryptamine 2A receptor gene T102C.OBJECTIVE:Researches on the geographic distribution of genotype frequency of 5-hydroxytryptamine 2A receptor gene T102C of schizophrenics people.DESIGN:Sample survey and observation with schizophrenics selected as subjects.SETTING: Mental Department, Changzhou Peace Hospital.PARTICIPANTS: The experiment was completed in The No.102 Hospital of PLA from January 1999 to August 2003.Totally 177 people of Han nationality (Age: 18-45; Duration of illness: 1 month-20 years) from different provinces and autonomous regions of China in compliance with diagnostic standards of Psychiatric Disease Classification and Diagnosis Standards of China (The 3rd version) were selected as the subjects.METHODS:The blood samples from 117 schizophrenics were given test of polymerase chain reaction(PCR),and the distribution differences of genotype frequency of 5-hydroxytryptamine receptor gene of normal control population from different provinces and autonomous regions were compared. DNA were abstracted by means of phenol chloroform, amplification and cataphoresis test of polymerase chain reaction of target DNA,primer sequence of gene segment amplification of 5-hydroxytryptamine (5-HT) 2A receptor gene T102C: 5'-TCT GCT ACA AGT TCT GGC TT-3', 5 '-CTG CAG CTT TTT CTC TAG GG-3'; Reaction system of 50 μL, DNA 0.5 μg,Primer 50 pmol,TagDNA enzyme 2U were added with dNTP to the final concentration of 200 μmol/L.MAINOUTCOMEMEASURES: DNA cataphores is test of schiophrenicanddistributionofgenotypefrequencyof5-hydroxytryptamine 2A receptor gene.RESULTS: The distribution of genotype frequency of 5-hydroxytryptamine(5-HT) 2A receptor gene: A1A1, 0.07-0.03; A1A2, 0.50-0.72; A2A2,0.17-0.29. Correlation analysis of genotype frequency in different regions:A1A1, x2=4.44, P=0.617 1, P ＞ 0.05;A1A2, x2=1.14, P=0.942 2, P ＞ 0.05;A2A2, x2=0.93, P=0.985 7, P ＞ 0.05.CONCLUSION: The geographic distributions of genotype frequency of 5-hydroxytryptamine 2A receptor gene T102C of schizophrenics of the Chinese Han nationality people were comparatively even.
