1.Application and effectiveness of SPD management of consumables and reagents in a public hospital

In the context of high-quality development in medical institutions, the supply-processing-distribution(SPD) management mode has gradually been widely applied. The authors described in detail the procurement, supply, inventory, distribution, and settlement management of medical consumables and in vitro diagnostic reagents in a certain hospital under the SPD mode. It was found that SPD was conducive to strengthening the supervision of medical consumables and in vitro diagnostic reagents in the hospital, ensuring quality and safety of use, reducing hospital operating costs, and improving hospital′s competitiveness. However, attention should be paid to preventing data security risks, strengthening operational management, and improving the cost-benefit analysis of in vitro diagnostic reagents.
2.Stage-by-stage clinical outcomes of percutaneous nephrolithotomy for calculous pyonephrosis:a Meta-analysis

【Objective】 To identify the best surgical treatment for patients with calculous pyonephrosis by evaluating the clinical effectiveness of percutaneous nephrolithotripsy (PCNL) in stageⅠ, percutaneous nephrostomy (PCN) in stageⅠ, and percutaneous nephrolithotripsy in stageⅡ. 【Methods】 For publications published between Jan.2012 and Oct.2022, we thoroughly examined the databases of PubMed, Cochrane Library, Web of Science, EMBASE, CNKI, Wanfang, and VIP.We then chosed the literature based on the inclusion and exclusion criteria.After data were retrieved and literature quality was assessed, Review Manager software (RevMan 5.4.3, Cochrane Collaboration, Oxford, UK) was utilized to Meta-analysis. 【Results】 Out of 688 participants across 105 researches, we chosed 11 trials.Among them, 341 patients received both stageⅠPCN and stage ⅡPCNL (hereinafter referred to as phase ⅡPCNL), while 347 patients received stageⅠPCNL.According to the results of the Meta-analysis, there was no discernible difference between stage Ⅰ PCNL and stage Ⅱ PCNL in terms of stone clearance rate (P=0.95), operation duration (P=0.48), postoperative septic shock (P=0.36), or perirenal effusion infection (P=0.27).There were significant differences between stage Ⅰ PCNL and stage Ⅱ PCNL in fever (P=0.03), indwelling nephrostomy tube time (P=0.01), hospitalization costs (P=0.01), hospitalization time (P=0.01), and postoperative hospitalization time (P=0.02).The following two regimens were comparable in terms of stone clearance rate, operating time, postoperative perirenal effusion infection, and septic shock for patients with calculous pyonephrosis.Despite the fact that there were more patients who developed fever after stage ⅠPCNL, hospitalization costs were lower, indwelling nephrostomy tube time, overall hospitalization time, and postoperative hospitalization time were all reduced. 【Conclusion】 ⅠPCNL was affordable, safe, and successful for treating renal and upper ureteral calculi with pyonephrosis, and it had some promotional value in clinical practice.
3.Study on the assessment of application value of CT equipment from multi-dimension based on real world data

Objective:To assess the values of computed tomography(CT)equipment in whole life cycle and operation status from multi-dimension based on real world data,and to quantify the performance,operation status,and benefit analysis of medical equipment,so as to assist to grasp the operation status of CT equipment for hospital.Methods:Depended on the viewpoint of real-world data,economic benefits,metrology and performance data of 5 pieces of CT equipment,which included type A,type B,type C,type D and type E,used in hospitals in the last 4 years were collected.Type A,type B and type C were all imported CT equipment,and type A and type B were the same model.Both type D and type E were domestic CT equipment.The analytic hierarchy process(AHP)was used to assign the weights of each data,and form effectively multi-dimensional evaluation system.Results:The value of consistency ratio(CR)of the hierarchical weight allocation matrix was 0.042 that was less than 0.1,which met the requirement of consistency test.The scores of the comprehensive values the 5 pieces of CT equipment of type A,type B,type C,type D and type E were respectively 68.10,30.35,39.50,60.99 and 39.76.There was a significant difference between the two pieces of imported CT equipment of type A and type B of the same model in different use environments.The score of type D of domestic CT equipment was higher than those of the type B and type C of imported CT equipment,respectively.Conclusion:Under suitable use environments,domestic CT equipment can replace imported equipment.The AHP method can be used in the assessment of CT equipment value.
4.Effect of silence information regulator 3 on temporomandibular joint osteoarthritis

Temporomandibular joint osteoarthritis (TMJOA) is a common subclass of temporomandibular disorders, with complicated causes and limited treatment. In addition to inflammatory reaction, structural changes of temporomandibular joint (TMJ) as condylar osteophyte formation, sclerosis, erosion, and subchondral bone cysts will happen at the same time. At present, it is found that epigenetic regulation can affect the growth and development of TMJ and regulate the immune inflammatory reaction, so we can speculate that epigenetic modification is involved in the occurrence and development of TMJOA. This paper reviews the researches of silence information regulator 3 (SIRT3), which is a key factor in epigenetics, in the field of osteoarthritis and rheumatoid arthritis.In order to discuss the effect of SIRT3 on the growth and development of TMJ by mediating acetylation modification, such as bone, cartilage, synovium and blood vessels, as well as alleviating osteoarthritis and synovitis. By doing this, the understanding of TMJOA can be enriched, with the goal of finding a new possibility for the clinical treatment of TMJOA.
5.Application of radar chart in perioperative nursing risk assessment of elderly patients with spinal fractures

Objective:To investigate the application of radar chart in perioperative nursing risk assessment of elderly patients with spinal fractures.Methods:Elderly patients with spinal fractures who were admitted and treated in a hospital from January 2018 to August 2019 were selected and they were divided into the control group ( n=41) and the observation group ( n=46) . In the control group, nursing routine was used for single risk assessment. Patients in the observation group filled in the self-made risk comprehensive assessment and measure sheet and drew the radar chart, which was used as the front page of the assessment in combination with each single assessment sheet. The pain scores, risk prevention and health education effects and satisfaction degree of nursing risk prevention service of patients were compared between two groups. Incidence of pressure injury, pulmonary infection, acute hypotension and complications were recorded and compared between the two groups. Results:The pain score and the incidence of nursing related complications in the observation group were lower than those in the control group while the awareness rate of risk prevention and health education knowledge were higher than those in the control group. The differences were statistically significant ( P<0.05) . Conclusions:The application of radar chart in perioperative nursing risk assessment of elderly patients with spinal fractures can visualize multiple risks assessments, standardize pain management in elderly patients, reduce the occurrence of nursing related complications and improve the risk prevention and health education effects and nursing service satisfaction.

Objective:
To evaluate the effect of the Thyroid Imaging Report and Data System proposed by American Radiological Society (ACR-TIRADS) for differential diagnosis in thyroid nodules, and compare ACR-TIRADS to the TIRADS proposed by Kwak et al.(K-TIRADS) and the ultrasound-based risk stratification system evaluated by American Thyroid Association (ATA-Risk Stratification).
Methods:
The clinical data of 1 760 patients with 1 912 thyroid nodules from 8 hospitals in Jiangsu province were retrospectively analysed. All of them were categorized based on ultrasound-based risk stratification systems. The ROC curve was established to assess and compare the diagnostic value of the systems.
Results:
The area under the ROC curve (AUC) of ACR-TIRADS was 0.830, with high sensitivity and negative predictive value (86.9% and 87.5%, respectively), and relatively low specificity and positive predictive value (64.1% and 62.9%, respectively). The sensitivity and specificity of K-TIRADS were up to 84.9% and 76.1%, respectively. The AUC of ATA-Risk Stratification was 0.852, with relatively high specificity (83.4%), and low sensitivity (79.4%). 7.Effective dosage of sirolimus for seizure treatment of immature C57BL/6 mice induced by kainic acid

OBJECTIVE To explore the safe and effective dose of sirolimus (Rapamycin,Sir) and its effect on seizure comorbidities. METHODS Immature C57BL/6 mice at postnatal 10 d of age were administered with kainic acid(KA) 12.0 mg · kg-1 intraperitoneally by a single injection to induce acute seizure. Sir 0.3, 1.0 and 3.0 mg · kg-1 was injected 24 h after seizure every other day until 3 d, 1 week, 3 weeks, 5 weeks and 6 weeks. Western blotting analysis was used to detect the expression and phos?phorylation level of S6 protein and to determine the minimum effective dose of Sir. Effect of the mini?mum effective dose of Sir on cognitive function and body growth was observed by several evaluations. Immunofluorescent intensity of Doublecortin (DCX) immunofluorescent staining was conducted to evaluate the development of neurons in the hippocampus. Morris water maze was used to assess the cognitive function. Tail suspension test, O maze and new object recognition test were used to study the anxiety-like behaviors of mice. RESULTS The result of Western blotting showed that Sir 0.3 mg · kg-1 had no significant effect on the phosphorylation of S6 protein in normal mice or KA mice, whereas 1.0 and 3.0 mg · kg- 1 could significantly inhibit the phosphorylation of S6 protein in KA mice (P<0.05). Sir 1.0 mg·kg-1 had no obvious effect on DCX-positive cells or body wass. Morris water maze showed that KA-induced seizure resulted in prolonged escape latency and swimming length (P<0.05), and a decreased crossing number of target quadrant (P<0.05). Sir 1.0 mg·kg-1 significantly reversed the deficit of cognitive function of KA-induced seizure mice (P<0.05), whereas no significant difference was found between Sir group and normal control group. Compared with normal control group, model group showed increased freezing time in tail suspension test (P<0.05), decreased migration length and reten?tion time in open arms in O maze (P<0.05), decreased retention time and touch frequency with new objects, migration length and average speed in new object recognition test (P<0.05). Sir 1.0 mg · kg-1 significantly reversed the above anxiety and depression status, whereas no significant difference was found between sirolimus group and normal control group. CONCLUSION Sir 1.0 mg · kg-1 inhibits the abnormal activation of mTOR pathway and the formation of epilepsy comorbidity in immature mice. Along with its mild side effect in development, Sir 1.0 mg · kg-1 will be an ideal dose to be used in the treatment of seizure in immature mice.

Objective:To study effects of neuroligin (NLG) on the proliferation and apoptosis in human neuroblastoma SH-SY5Y cells.Methods:The SH-SY5Y cells were cultured in vitro for 24 hours,and then transfected with NLG siRNA at dose of 50,100,200 μmol/L,respectively.MTT procedure was used to detect the cell proliferation,and expression levels of apoptosis gene including Bax or Bcl-2 and Bcl-xl were measured by RT-PCR.Results:Compared to control groups proliferation of SH-SY5Y cells were distinctly inhibited after NLG siRNA transfection accompany with a dose-dependent,which was caused by activation of apoptosis.Conclusions:NLG protect neuron by inhibiting apoptosis.
9.The condition of small airway function and its related influence factors in controlled bronchial asthmatic children

Objective To investigate the condition and the related influence factors of small airway function in controlled bronchial asthmatic children.Methods Children diagnosed with bronchial asthma controlled,aged 5-14 years old without gender limitation were consecutively enrolled.Survey questionnaire including asthma symptoms in the past one month and medication usage were conducted.Physical condition and lung function were examined.Results A total of 255 patients were included in this study,and 224 patients had normal small airway function in which the level of forced expiratory flow at 50% of forced vital capacity(FVC) exhaled (FEF50),forced expiratory flow at 75% of FVC exhaled(FEF75),maximal mid-expiratory flow(MMEF) was 87.9% (77.0%,97.2%),73.8% (65.6%,93.5%),and 84.9% (76.4%,97.5%) of the predicted value,respectively.Thirty-one patients had small airway dysfunction,accounting for 12.2% (31/255 cases),and the level of FEF50,FEF75,MMEF were 62.8% (59.9%,65.5%),51.9% (46.6%,55.5%),and 62.7% (57.2%,64.4%) of the predicted value,in which FEF75 had shown more decrease than others.In small airway dysfunction group,10 patients had conducted bronchodilation testing,and improvement rate of FEF50,FEF75 and MMEF were 41.1%,47.5% and 41.3%,and all 10 cases (100.0%) returned to normal level.No significant difference was found in gender,body mass index,atopy,disease duration,drug usage between 2 groups (all P > 0.05).Conclusion In 5-14 controlled asthmatic children,12.2% have small airway dysfunction with a certain degree of reversibility.Gender,body mass index,atopy,disease duration,drug usage are not influencing factors.
