1.Evaluation of the quality of Jingangteng capsules based on UPLC fingerprinting combined with multi-component content determination

OBJECTIVE To establish the UPLC fingerprint and the method for multi-component content determination in Jingangteng capsules, and to evaluate its quality by combining chemical pattern recognition analysis. METHODS An UPLC method was established. Separation was performed on a Zorbax SB-C 18 Rapid Resolution HD column, with acetonitrile-0.1% formic acid as the mobile phase for gradient elution.Using the Similarity Evaluation System for Chromatographic Fingerprints of Traditional Chinese Medicines (2012 edition), UPLC fi ngerprints were established for 10 batches of Jingangteng capsules, and similarity was evaluated. SPSS 22.0 and SIMCA 14.1 software were used to perform hierarchial-cluster analysis and orthogonal partial least squares discriminant analysis (OPLS-DA), respectively. The same UPLC method was employed to determine the contents of chlorogenic acid, 3,5-dihydroxy-2-methylbenzoic acid-3- O -glucoside (M1), caffeic acid, astilbin, oxyresveratrol, quercitrin and resveratrol in the 10 batches of samples. RESULTS A total of 17 common peaks were identified in UPLC fingerprints of the 10 batches of samples, of which 7 were identified as chlorogenic acid, M1, caffeic acid, astilbin, oxyresveratrol, quercitrin, and resveratrol. The similarities of 10 batches of samples ranged from 0.820 to 0.985. The results of hierarchial-cluster analysis showed that 10 batches of samples were grouped into four categories: S1-S4 formed one group, S5 and S6 formed another, S7, S8 and S10 formed a third, and S9 formed a fourth, consistent with the OPLS-DA results; the variable importance projection values for peaks 7, 10, 2, 16 (resveratrol), 13 (oxyresveratrol), 11, 6 (caffeic acid), 5 (M1) and 15 (quercitrin) were >1. Quantitative analysis results showed that the contents of chlorogenic acid, M1, caffeic acid, astilbin, oxyresveratrol, quercitrin, and resveratrol were 1.650 8-4.213 7, 0.636 2-2.161 7, 0.031 0-0.086 5, 0.239 1-1.069 3, 0.211 9-1.104 0, 0.488 8-2.399 2, and 0.164 0-0.699 8 mg/g, respectively. CONCLUSIONS UPLC fingerprint and content determination methods established in this study are simple to operate, accurate, reliable and reproducible; when combined with chemical pattern recognition analysis, they can be used to evaluate the quality of Jingangteng capsules. Nine components, such as resveratrol, oxyresveratrol, caffeic acid, M1 and quercitrin, may serve as markers of quality variation.

Functional dyspepsia (FD), characterized by persistent or recurrent dyspeptic symptoms without identifiable organic, systemic or metabolic causes, is an increasingly recognized global health issue. The objective of this guideline is to equip clinicians and nursing professionals with evidence-based strategies for the management and treatment of adult patients with FD using traditional Chinese medicine (TCM). The Guideline Development Group consulted existing TCM consensus documents on FD and convened a panel of 35 clinicians to generate initial clinical queries. To address these queries, a systematic literature search was conducted across PubMed, EMBASE, the Cochrane Library, China National Knowledge Infrastructure (CNKI), VIP Database, China Biology Medicine (SinoMed) Database, Wanfang Database, Traditional Medicine Research Data Expanded (TMRDE), and the Traditional Chinese Medical Literature Analysis and Retrieval System (TCMLARS). The evidence from the literature was critically appraised using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach. The strength of the recommendations was ascertained through a consensus-building process involving TCM and allopathic medicine experts, methodologists, pharmacologists, nursing specialists, and health economists, leveraging their collective expertise and empirical knowledge. The guideline comprises a total of 43 evidence-informed recommendations that span a range of clinical aspects, including the pathogenesis according to TCM, diagnostic approaches, therapeutic interventions, efficacy assessments, and prognostic considerations. 3.Research on the comprehensive evaluation of the operation status of tertiary public hospitals based on extensible cloud model

Objective:To explore an evaluation method for the operational status of tertiary public hospitals, identify strengths and weaknesses in hospital operations, and provide decision-making support for hospital managers.Methods:In 2023, an evaluation index system for the operation of tertiary public hospitals was constructed based on literature analysis and the Delphi method. The combined weights of the evaluation indexes were determined using the cooperative game theory method. An evaluation model for hospital operational status based on the extensible cloud model was established, with the determination of evaluation index thresholds and standards based on the operational data of a tertiary public hospital in Liaoning Province from 2020 to 2022. The operational status of the hospital was then comprehensively evaluated using the evaluation model based on the hospital′s operational data from 2023.Results:The constructed evaluation index system for the operation of tertiary public hospitals included five dimensions and 14 indicators: financial health, service efficiency, medical quality, satisfaction, and sustainable development. The indicators with higher weight values were net cash flow (0.135), average appointment rate for outpatient patients (0.088), and budget savings from government procurement (0.087). The overall operational status evaluation score of the sample hospital was 4.48, with an evaluation grade of level V. The scores for the dimensions of financial health, medical quality, and sustainable development were 4.99, 4.74, and 5.00, respectively, all reaching level V. The scores for service efficiency and satisfaction were 4.00 and 3.70, respectively, both at level Ⅳ.Conclusions:The evaluation index system and the determined weights constructed in this study were reasonable to a certain extent. The hospital operational status evaluation model based on the extensible cloud model can objectively reflect the operational status of the hospital, and the evaluation results can provide decision-making support for managers.

Helicobacter pylori(H.pylori)infection triggers gastric mucosal inflammatory responses and spasmolytic polypeptide-expressing metaplasia(SPEM).These pathological conditions can escalate the severity of chronic gas-tritis,gastric ulcers and even cause gastric cancer.SPEM is frequently viewed as an early sign of gastric mucosal injury and the onset of carcinogenesis.A comprehensive analysis of the genesis and molecular regulation of SPEM cells in the context of H.pylori infection further has enlightened the pathogenesis of gastric mucosal diseases and provide new ideas and targets for diagnosing and treatment of H.pylori-related gastric mucosal diseases.This paper reviews a variety of molecular biomarkers associated with SPEM,encompassing TFF2,CD44v9,and AQP5,and delineates their pivotal regulatory functions in H.pylori infection and SPEM.This paper also reviews the origination of SPEM cells and pertinent molecular regulatory mechanisms.

6.Effect of Shenfu Yixin Granules on Mitochondrial Dynamics in Rats with Heart Failure After Myocardial Infarction

ObjectiveTo prepare a rat model of heart failure after myocardial infarction by ligation of the anterior descending branch of the left coronary artery, and to observe the effect of Shenfu Yixin granules on the mitochondrial dynamics of rats with heart failure. MethodFifty SD male rats were randomly taken ten as the sham operation group and the rest as modeling group. The rat model of heart failure after myocardial infarction was prepared by ligation of anterior descending branch of left coronary artery. According to the left ventricular ejection fraction(LVEF) on the 28th day after operation, the model rats were randomly divided into the model group, Shenfu Yixin granule low-dose and high-dose groups(3.011, 15.055 g·kg-1) and sacubitril valsartan sodium group(20.83 mg·kg-1). Each administration group was gavaged daily with the corresponding dose of drug solution, while the sham operation group and model group were given the same amount of normal saline once a day for 28 days, with 6 rats in each group. Ultrasound was used to detect the cardiac function parameters, rat heart mass and body mass were weighed to calculate the cardiac mass index, enzyme linked immunosorbent assay(ELISA) was used to detect serum brain natriuretic peptide(BNP) and soluble growth stimulation expressed gene 2 protein(sST2) levels. Hematoxylin-eosin(HE) staining was used to observe the pathological morphology of the myocardium. Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR) and Western blot were used to detect the mRNA and protein expression of mitochondrial fusion protein 1/2(Mfn1/2), optic atrophy protein 1(Opa1), dynamin-related protein 1(Drp1) and fission protein 1(Fis1). ResultCompared with the sham operation group, the mRNA and protein expression of LVEF, Mfn1, Mfn2, Opal in the model group decreased(P<0.05), while BNP, sST2, cardiac mass index, Drp1, Fis1 mRNA and protein levels increased(P<0.05). Compared with the model group, the expression of LVEF, Mfn1, Mfn2, Opal mRNA and protein increased in Shenfu Yixin granule high-dose and sacubitril valsartan sodium groups(P<0.05), while BNP, sST2, cardiac mass index, Drp1, Fis1 mRNA and protein levels decreased(P<0.05). Pathological observation showed that compared with the sham operation group, the model group had disordered arrangement of myocardial cells, inflammatory cell infiltration and myocardial fibrosis. Compared with the model group, the degree of inflammatory cell infiltration, myocardial or interstitial fibrosis was improved and alleviated in all administered groups. ConclusionShenfu Yixin granules can resist heart failure, reduce cardiac mass index, decrease BNP and sST2 contents, and improve cardiac function. Its mechanism may be related to the adjustment of mitochondrial dynamics.
7.Consistency between bioelectrical impedance analysis and dual-energy X-ray absorptiometry in body composition measurement in children aged 6-17 years in China

Objective:To evaluate the consistency between bioelectrical impedance analysis (BIA) and dual-energy X-ray absorptiometry (DXA) in the measurement of body composition in children aged 6-17 years in China.Methods:Fat-free mass, fat mass and fat mass percentage were measured by both BIA and DXA in 1 161 children. t-test or Wilcoxon paired test was used to evaluate the different outcome of the two methods. The correlation and consistency between the methods were evaluated by Spearman correlation coefficients ( r) and Bland-Altman analysis. Results:Body compositions measured by BIA was positively correlated with those measured by DXA (fat mass r=0.95, fat-free mass r=0.98, fat mass percentage r=0.86, all P<0.05). Comparing with DXA, BIA underestimate children's fat mass [the mean difference is -3.15 kg, and the SD is 2.35 kg, 95% limits of agreement (LoA): -7.74-1.45 kg] and fat mass percentage (the mean difference is -8.45%, and the SD is 4.63%, 95% LoA: -17.53%-0.64%). Conclusions:Body compositions measured by BIA was highly positively correlated with those measured by DXA. BIA has certain application value in the measurement of body fat mass and fat-free mass of children aged 6-17 years.
8.Effects of recombinant human metallothionein-Ⅲ combined with wound dressing on wound healing of full-thickness skin defects in mice

Objective:To investigate the effects of recombinant human metallothionein-Ⅲ (rh-MT-Ⅲ) combined with wound dressing on wound healing of full-thickness skin defects in mice.Methods:This study was an experimental study. Twenty-four half male and half female 6 weeks old Institute of Cancer Research mice were taken, and two symmetrical round full-thickness skin defect wounds were prepared on the back of each mouse. The mice were stratified and randomly divided into normal saline group, dressing group, rh-MT-Ⅲgroup (applying the corresponding solution on the wounds), and combined treatment group (applying a mixture of rh-MT-Ⅲ and wound dressing on the wounds) according to sex and body weight, with 6 mice in each group. From 1 to 7 d after injury, all mice were observed daily for changes in activity, diet, and fur growth, their body weights and wound areas were recorded, and the relative wound area percentages were calculated. On 7 d after injury, the wound tissue of mice was taken for hematoxylin-eosin staining to observe the newborn granulation tissue, for Masson staining to observe collagen fiber formation, and for immunofluorescence staining to detect cell proliferation (denoted as Ki67 relative fluorescence intensity) and cell apoptosis (denoted as TdT-mediated dUTP nick end labeling (TUNEL) relative fluorescence intensity). The sample size in the above experiments was 6.Results:There were no abnormalities in activity, diet, or fur growth in the 4 groups of mice within 7 d after injury. There were no statistically significant differences in the overall comparison of the body weights of mice in the 4 groups from 1 to 7 d after injury ( P>0.05). The relative wound area percentages of mice in combined treatment group were significantly lower than those in normal saline group and rh-MT-Ⅲ group on 2, 3, 4, 5, 6, and 7 d after injury ( P<0.05), and the relative wound area percentages of mice in combined treatment group were significantly lower than those in dressing group on 3, 4, 5, 6, and 7 d after injury ( P<0.05). The relative wound area percentages of mice in dressing group on 6 and 7 d after injury and in rh-MT-Ⅲ group on 7 d after injury were significantly lower than those in normal saline group ( P<0.05). On 7 d after injury, a large number of capillaries and fibroblasts could be seen in wound tissue of mice in combined treatment group, and the growth of new epithelial tissue at the upper edge of the wound was better than that of the other three groups; the collagen fibers in the wound tissue of mice in combined treatment group had higher degree of density and arrangement in a more orderly manner than those of the other three groups. On 7 d after injury, the Ki67 relative fluorescence intensity in the wound tissue of mice in dressing group, rh-MT-Ⅲ group, and combined treatment group was (289±35)%, (197±17)%, and (389±56)%, which was significantly higher than (100±15)% in normal saline group, respectively ( P<0.05), and the Ki67 relative fluorescence intensity in the wound tissue of mice in combined treatment group was significantly higher than that in dressing group and rh-MT-Ⅲ group, respectively (with both P values <0.05). On 7 d after injury, the TUNEL relative fluorescence intensity in the wound tissue of mice in dressing group, rh-MT-Ⅲ group, and combined treatment group was (55.5±5.7)%, (66.7±8.0)%, and (20.0±2.2)%, which was significantly lower than (100.0±12.9)% in normal saline group, respectively ( P<0.05), and the TUNEL relative fluorescence intensity in the wound tissue of mice in combined treatment group was significantly lower than that in dressing group and rh-MT-Ⅲ group, respectively (with both P values <0.05). Conclusions:rh-MT-Ⅲ combined with wound dressing can promote the growth of granulation tissue around the wound as well as collagen deposition, increase the cell proliferation vitality, reduce cell apoptosis, and promote the re-epithelialization of skin at the edge of the wounds, thus accelerating the healing of full-thickness skin defect wounds in mice.
9.Clinical retrospective analysis of coagulation abnormalities caused by tigecycline

Objective To investigate the effect of tigecycline on coagulation function and to provide a reference for the clinical rational use of tigecycline.Methods The data of patients treated with tigecycline in Shanghai Tenth People's Hospital between June 2019 and December 2023 by retrospective analysis.Statistical analysis was performed by collecting data on patients'basic information,routine coagulation parameters and thromboelastogram(TEG)parameters before and after the use of tigecycline.Results Activated partial thromboplastin time,prothrombin time and thrombin time were prolonged and fibrinogen levels were decreased with the use of tigecycline in 41 patients,the differences were statistically significant(P<0.05).There was no significant difference in levels of coagulation factor activation time,clot formation rate parameter,maximum angle of tangency,maximum amplitude of elastography and coagulation index after treatment with tigecycline(P>0.05).Conclusion For patients with suspected coagulation abnormalities after tigecycline administration,a comprehensive assessment of coagulation should be made by combining routine coagulation indexes with TEG.
10.Nursing experience of extracorporeal membrane oxygenation combined with inhaled nitric oxide in the treatment of morbid obesity with severe acute respiratory distress syndrome

Objective To summarize the successful experience of extracorporeal membrane oxygenation (ECMO) combined with inhaled nitric oxide (iNO) in the treatment of severe acute respiratory distress syndrome (ARDS) after empyema surgery in a morbid obesity patient,and to explore the nursing points. Methods On July 3,2023,a patient was admitted to the department of intensive care unit (ICU) of Guangdong Provincial People's Hospital (Guangdong Academy of Medical Sciences),Southern Medical University following the failure of closed thoracic drainage and catheterization at another hospital. Using the rapid integration of the WeChat group,a multidisciplinary team (MDT) model was built. This approach enabled the professional and standardized integration of clinical data and the implementation of targeted treatments,significantly reducing response times and optimizing the overall nursing process. Results A 27-year-old male patient with morbid obesity was admitted to the hospital due to dyspnea and chest pain for more than 7 days. ① Treatment process:on July 4,the patient underwent video-assisted thoracic surgery (VATS),including right chest exploration,pleural adhesion release,and empyema,performed under general anesthesia. Two thoracic drainage tubes were retained and water-sealed bottles were connected. The drainage fluid was purulent. After the operation,the patient was short of breath and the condition was aggravated and transferred to ICU. On admission,the patient's bedside chest X-ray showed that more moist rales were heard in both lungs,especially on the right side. At 18:30 on July 10,the pluse oxygen saturation (SpO2) was 0.75-0.80,and fiberoptic bronchoscopy was performed immediately. The ventilator parameters were up-regulated,the position was changed,and 20 mg of furosemide injection was injected intravenously,the effect was not good. Attempted to perform prone position ventilation,SpO2 did not improve. At 21:30,the SpO2 gradually decreased to 0.60,and the extracorporeal circulation was immediately decided. After veno-venous ECMO (VV-ECMO) at 2:30 on July 11,the SpO2 was 0.90,and the blood gas was stable after multiple reexaminations. During July 12,there was still shortness of breath and poor oxygenation index. According to the MDT consultation opinion,during the emergency treatment combined with iNO treatment,oxygenation improved rapidly to 172 mmHg (1 mmHg≈0.133 kPa) and 190 mmHg after 1 hour and 2 hours,respectively. After 6 days,the oxygenation index stabilized between 222-285 mmHg. On July 17,the iNO support was gradually reduced and successfully removed. On July 21,a chest X-ray showed that the patient's lung lesions were significantly improved,and ECMO support parameters were gradually reduced until ECMO treatment was successfully discontinued. On August 3,the patient's consciousness returned to normal,and the indicators returned to normal. The ventilator-assisted breathing was stopped,and the high-flow oxygen therapy was observed after extubation. He was transferred from the ICU on August 8 and was discharged on August 15. ② Nursing points:we focus on personalized analgesia and sedation,and adjust the types and doses of sedative drugs in stages to reduce oxygen consumption and reduce complications. For the treatment of ARDS with ECMO combined with iNO to improve oxygenation,close monitoring and supportive care were carried out. Special attention was paid to the fixation of ECMO pipeline and tracheal intubation in patients with morbid obesity and individualized fine skin care was implemented. Actively prevent the potential complications of ICU acquired myasthenia,and carry out phased psychological nursing to establish rehabilitation confidence. Conclusion ECMO combined with iNO treatment requires professional teamwork,close observation,effective nursing and perfect monitoring technology to ensure the safety of patients with severe ARDS morbid obesity,reduce complications and improve prognosis,which has important reference significance for relevant medical practice.
