1.Research on Building a Performance Evaluation Index System for the Specialty Cooperation Alliance in Hospital-Community

Objective To construct the performance evaluation index system of specialist cooperation alliance in hos-pital-community in order to scientifically evaluate the effect of the construction of the specialist cooperation alliance.Methods By searching literature related to the construction of specialized league,sort out national and local policy documents,and the"structure-process-result"model was adopted to initially construct the index system.The ex-pert letter consultation form of the indicator system was formed through the discussion of the special group,two rounds of Delphi expert consultation were carried out to determine the evaluation indicator system,and the weight coefficient of each indicator was determined by the simultaneous application of analytic hierarchy process.Results The effective recovery rates of the two rounds of questionnaire were 84.62%and 100.00%,respectively.The positive de-gree of experts was high,and the average authority degree of experts was 0.864.The importance,operability,sen-sitivity Kendall's W coefficient and x2 test value of all indexes were P<0.05.Finally,an evaluation index system covering 9 first-level indicators and 25 second-level indicators was established.Conclusion The constructed perfor-mance evaluation index is scientific and reliable.The practical application shows that it can be used as a tool to evalu-ate the effect of integrated,continuous and collaborative health service delivery in hospital-community.
2.Dynamic network analysis of postoperative core symptoms in hepatocellular carcinoma patients and nursing insights

Objective This study conducted a dynamic network analysis of the symptoms in patients with hepatocellular carcinoma(HCC)at different postoperative time points,aiming to explore evolving core symptoms and provide a basis for developing precise clinical symptom management programs.Methods A longitudinal study was carried out involving 370 HCC patients who underwent surgical treatment at a hospital in Kunming city between February and December 2024.Questionnaires were administered on postoperative day 2(T1)and day 7(T2),using the Chinese version of the Memory Symptom Assessment Scale.Symptom networks of 18 symptoms were constructed for both time points using R software to explore core symptoms and their interconnections.A network comparison test was also performed to assess differences between the 2 time points.Results All 370 patients completed the questionnaire at T1,and 357 valid responses were collected at T2,yielding a response rate of 96.49%.At T1,"feeling nervous"had the highest centrality(rs=1.29),while at T2,"feeling sad"was most central(rs=1.11).Overall strength of the symptom networks showed that notable changes were observed in the edge weights between symptoms(P=0.01).Conclusion While the overall structure of early postoperative symptoms in HCC patients remained relatively stable over time,the core symptoms shifted."Feeling nervous"emerged as the central symptom at T1,whereas"feeling sad"became predominant.Digestive-related symptoms are progressively more closely linked over time.Healthcare professionals should dynamically assess patients' early postoperative psychological symptoms and digestive system related symptoms,and focus on intervening with symptoms of tension and sadness.
3.Research on the comprehensive evaluation of the operation status of tertiary public hospitals based on extensible cloud model

Objective:To explore an evaluation method for the operational status of tertiary public hospitals, identify strengths and weaknesses in hospital operations, and provide decision-making support for hospital managers.Methods:In 2023, an evaluation index system for the operation of tertiary public hospitals was constructed based on literature analysis and the Delphi method. The combined weights of the evaluation indexes were determined using the cooperative game theory method. An evaluation model for hospital operational status based on the extensible cloud model was established, with the determination of evaluation index thresholds and standards based on the operational data of a tertiary public hospital in Liaoning Province from 2020 to 2022. The operational status of the hospital was then comprehensively evaluated using the evaluation model based on the hospital′s operational data from 2023.Results:The constructed evaluation index system for the operation of tertiary public hospitals included five dimensions and 14 indicators: financial health, service efficiency, medical quality, satisfaction, and sustainable development. The indicators with higher weight values were net cash flow (0.135), average appointment rate for outpatient patients (0.088), and budget savings from government procurement (0.087). The overall operational status evaluation score of the sample hospital was 4.48, with an evaluation grade of level V. The scores for the dimensions of financial health, medical quality, and sustainable development were 4.99, 4.74, and 5.00, respectively, all reaching level V. The scores for service efficiency and satisfaction were 4.00 and 3.70, respectively, both at level Ⅳ.Conclusions:The evaluation index system and the determined weights constructed in this study were reasonable to a certain extent. The hospital operational status evaluation model based on the extensible cloud model can objectively reflect the operational status of the hospital, and the evaluation results can provide decision-making support for managers.
4.A mixed study of latent profile analysis and influencing factors of psychological responses to trauma in ICU patients

Objective To explore the characteristics of the potential categories of traumatic psychological reactions in ICU patients and analyze the influencing factors of different potential categories,in order to provide ideas for the implementation of scientific and effective personalized intervention in the future.Methods A convenience sample of 300 patients treated in the ICU of a hospital from May 2023 to February 2024 was surveyed.The used instruments included a general information questionnaire,the Social Support Rating Scale,the Brief Resilience Scale,the Acute Stress Disorder Scale,and the Simplified Chinese version of the Post-Traumatic Growth Inventory.Data were analyzed using multivariate Logistic regression.Additionally,a purposive sampling method was used to conduct semi-structured interviews with 12 ICU patients,and the interview data were analyzed by interpretative phenomenological analysis.Results Quantitative research results showed that the traumatic psychological responses of ICU patients were categorized into 4 latent classes,including Trauma-Dependent Type(17.3%),Smooth-Transition Type(36.0%),External-Support Growth Type(25.3%),and Internal-Rooted Growth Type(21.3%).Factors influencing these responses included age,educational level,social support,psychological resilience,household income,and the number of used hormone medications(P<0.05).Qualitative research results showed that 4 core themes were identified,including immersion in psychological trauma,rational cognition and acceptance of reality,seeking external support for personal growth,and independent exploration of inner self.Influencing factors included age,income level,history of major illness or ICU treatment,level of disease awareness,presence of underage children,and individual capacity for rumination.The quantitative and qualitative results were mutually corroborative and complemented each other.Conclusion The acute stress response of ICU patients is obvious,and there are multiple characteristics in the potential categories of traumatic psychological response,suggesting that medical and nursing staff can identify the characteristics of various types and influencing factors early,formulate personalized intervention programs,and promote their post-traumatic growth.
5.Development and initial implementation of a neonatal home skin care guidance scheme

Objective To develop a home skin care guidance scheme for newborns,and initially implement it to reducing the risk of skin-related issues.Methods Searching both domestic and international databases,we identified relevant literature.Following a quality evaluation,evidence integration,and group discussions,we developed the con-tent of the home skin care guidance for newborns.Subsequently,we refined this content through 2 rounds of expert consultations to finalize the scheme.To initially implement the scheme,we conveniently selected 20 healthy new-borns born between August 1st and 15th,2024,at a tertiary-level comprehensive hospital in Fujian Province as an experimental group.Their parents received skin care guidance based on our scheme via a WeChat platform.In contrast,we selected another group of 20 healthy newborns delivered between July 1st and 15th,2024,at the same hospital as a control group;their parents were provided with conventional skin care guidance.We compared the in-cidence rates of diaper dermatitis and eczema between these 2 groups of newborns at a month of age.Results A total of 33 experts from 30 tertiary hospitals across 19 provinces(including autonomous regions and municipalities)were invited to participate in a questionnaire survey.The response rates for both rounds of expert questionnaires reached 100%.The authority coefficients for the experts were recorded at 0.78 and 0.83,while the Kendall concor-dance coefficients were found to be 0.188 and 0.142(all P<0.001).The final newborn home skin care guidance scheme consists of 6 first-level items,41 second-level items,such as the selection of newborn care products,methods for neonatal bathing and prevention of neonatal diaper dermatitis,and so on.Preliminary application results indicated that the incidence of diaper dermatitis in the experimental group was significantly lower than that observed in the control group,with a statistically significant difference noted(P=0.047).There was no significant difference in the incidence of eczema between the 2 groups at the age of a month(P=0.201).Conclusion The Neonatal Home Skin Care Guidance Scheme for newborns has been demonstrated to be scientific,reliable and feasible.The implementa-tion of this scheme has proven beneficial in reducing the incidence of diaper dermatitis at a month of age.Howev-er,the sample size needs to be expanded to further verify its implementation effect.
6.Network analysis of the relationship between self-management level and supportive care demand in maintenance hemodialysis patients

Objective:To clarify the complex network structure between self-management levels and supportive care needs of patients undergoing maintenance hemodialysis, utilizing network analysis methods to identify the core and bridging nodes among the variables, thereby defining targets for nursing interventions to implement more precise care strategies.Methods:A cross-sectional survey was conducted from May to November 2023 among 302 outpatient maintenance hemodialysis patients at the hemodialysis centers of two healthcare institutions in Harbin (the Second Affiliated Hospital of Harbin Medical University and Heilongjiang Changjiang Nephrology Specialist Hospital). This involved the use of general information questionnaires, the Hemodialysis Patient Self-Management Scale, and the Supportive Care Needs Scale for Hemodialysis Patients. R language was employed for the network analysis.Results:A total of 300 valid questionnaires were collected, including 186 males and 114 females, with an age range of 23 to 88 years (mean age 55.00 ± 13.78 years). The scores for the dimensions of self-management among dialysis patients were as follows: problem-solving (3.38 ± 0.63), execution of self-care (3.16 ± 0.52), partnership (2.56 ± 0.69), and emotional processing (1.89 ± 0.63). The scores for the dimensions of supportive care needs among dialysis patients were: physiological needs (2.82 ± 1.08), psychological needs (1.51 ± 1.02), social needs (1.97 ± 1.07), emotional needs (1.67 ± 1.12), spiritual needs (2.22 ± 0.77), informational needs (2.83 ± 1.08), and practical needs (2.82 ± 1.03). In network analysis, the strongest intensity was found in the execution of self-care (1.753), and the highest closeness was in psychological needs (0.017). The top three dimensions ranked by bridge strength were social needs (1.463), partnership (1.462), and execution of self-care (1.384). The root mean square error was lowest for psychological needs (0.518) and emotional needs (0.538). The stability and accuracy of the network structure were found to be good.Conclusions:The key intervention targets for nursing care in maintenance hemodialysis patients were identified as executing self-care, psychological needs, emotional needs, and social needs. Among these, executing self-care served as the core intervention focus, while psychological and emotional needs had dominant influences, and social needs exhibited the strongest bridging role. Nursing staff should prioritize these key targets and tailor personalized comprehensive nursing intervention plans to enhance patients′self-management levels and fully meet their care needs.
7.Clinical nursing practice guideline for central venous access device associated skin impairment in cancer patients (2024)

Central venous access device associated skin impairment is a common complication of indwelling central venous catheters in cancer patients. To further enhance the standardization of nursing staff's practice of central venous access device associated skin impairment, a clinical nursing practice guideline for central venous access device associated skin impairment in cancer patients was developed through the Delphi expert consultation and expert meeting methods in accordance with the methodology for developing evidence-based nursing practice guidelines. Recommendations cover four aspects of management requirements, assessment, prevention, and management of central venous access device associated skin impairment, providing a practical basis for clinical healthcare professionals to make scientific decisions on central venous access device associated skin impairment.
