1.Comparison on the treatment of proliferative diabetic retinopathy with ranibizumab and conbercept adjuvant for pars plana vitrectomy

Objective:To compare the outcomes of ranibizumab and conbercept adjunct for pars plana vitrectomy (PPV) in the treatment of proliferative diabetic retinopathy (PDR).Methods:A prospective randomized case-control study. From June 2022 to December 2023, 90 cases (90 eyes) of PDR patients diagnosed through ophthalmic examination at Department of Ophthalmology of Gansu Provincial Hospital were included in the study. All patients underwent the best corrected visual acuity (BCVA), intraocular pressure, B-mode ultrasound, and optical coherence tomography (OCT) examinations. The central macular thickness (CMT) was measured using an OCT instrument. The patients were randomly divided into a intravitreal injection of ranibizumab group (monoclonal-antibody group) and a intravitreal injection of conbercept group (fusion-protein group) using a random number table method, with 45 cases (45 eyes) in each group. Two groups of patients were intravitreal injected with 10 mg/ml ranibizumab or conbercept 0.05 ml, respectively. A standard 23G PPV was performed through the flat part of the ciliary body 3-7 days after intravitreal injection. Relevant examinations were performed using the same equipments and methods as before surgery at postoperative 1 week, 1, 3, 6, and 12 months. The PPV time, intraoperative use of intraocular electrocoagulation, incidence of iatrogenic retinal breaks, and sterile air or silicone oil tamponade rate in the vitreous cavity, the postoperative changes of BCVA and CMT, and incidence of complications were compared between two groups. Independent sample t test was used for inter group comparison. Results:The intraoperative utilization rate of intraocular electrocoagulation in the monoclonal-antibody group was higher than that in the fusion-protein group, and the difference was statistically significant ( χ2=3.876, P<0.05). There were no statistically significant differences in the PPV time ( t=0.152), intraoperative bleeding rate ( χ2=0.800), incidence of iatrogenic retinal breaks ( χ2=1.975), and sterile air and silicone oil tamponade rate in the vitreous cavity ( χ2=1.607, 1.553) between the two groups ( P>0.05). There were no statistically significant differences in early and late postoperative vitreous hemorrhage ( χ2=1.235, 2.355), and re-PPV ( χ2=2.355) between two groups ( P>0.05). The BCVA of the fusion-protein group was significantly better than that of the monoclonal-antibody group at postoperative 3 months, and the difference was statistically significant ( t=2.428, P<0.05). The CMT of the fusion-protein group was lower than that in the monoclonal-antibody group at postoperative 1 week, and the difference was statistically significant ( t=2.739, P<0.05). None of the patients experienced endophthalmitis, retinal artery occlusion, or severe cardiovascular events after surgery. Conclusion:Compared with intravitreal injection of ranibizumab before PPV, intravitreal injection of conbercept before PPV in PDR patients can shorten the surgical time, reduce intraoperative bleeding rate, lower the rate of electrocoagulation and intraocular tamponade, and incidence of iatrogenic retinal breaks, and improve the visual acuity.
2.Metformin upregulates ABCA1 expression via inhibiting ubiquitin-proteasome system

Aim To explore the potential mechanism of metformin on ATP-binding cassette transport A1(ABCA1)expression.Methods J774A.1 macrophages were treated with metformin and cycloheximide,and ABCA1 expression was determined by Western blot.His-tagged ABCA1 and HA-tagged Ub plasmids were co-transferred into HEK293 cells and stimulated with metformin.Co-immunoprecipitation(Co-IP)was used to test the binding ability of ABCA1 and ubiquitin.Candidate E3 ubiquitin-protein ligases(CE3)of ABCA1 were identified through Co-IP-based pro-teomics.The MIB1 plasmid was constructed and transferred into HEK293 cells,and Western blot was used to determine the effect of metformin and MIB1 on ABCA1 expression.Results Metformin increased the expression of ABCA1 in J774A.1 cells(P<0.01),and inhibited ABCA1 degradation(P<0.05).Metformin disrupted the binding of ABCA1 to ubiquitin(P<0.05).The proteins regulated by metformin in ABCA1 expression were primarily enriched in pathways re-lated to cell development,inflammation and immune defense.Metformin may upregulate ABCA1 protein expression via MIB1(P<0.05).Conclusion Metformin inhibits the degradation of ABCA1 by blocking the ubiquitin-proteasome system(UPS),and MIB1 might act as a candidate E3 ubiquitin-protein ligase(CE3)for ABCA1.
3.Ongoing pregnancy rate with intervention and expectant therapy in patients with unexplained recurrent spontaneous abortion: a prospective multicenter cohort study

Objective:To observe the pregnancy outcomes of patients with unexplained recurrent spontaneous abortion (URSA) after interventional treatment or expectant treatment.Methods:This prospective study followed up 398 patients with recurrent spontaneous abortion from March 2017 to September 2022 in seven hospitals. Among them, 267 patients were diagnosed with URSA, including 124 patients who were initially diagnosed in the interventional treatment hospital and 143 patients who were initially diagnosed in the expectant treatment hospital. All URSA patients were followed up for 33 months. Ongoing pregnancy rates were observed as main outcome indicators.Results:A total of 127 patients became pregnant, and 107 of them had sustained pregnancies, the ongoing pregnancy rate was 84.25% (107/127). The ongoing pregnancy rate was 86.11% (31/36) in the interventional treatment group and 83.52% (76/91) in the expectant treatment group, with no significant difference ( P>0.05). During the follow-up, the ongoing pregnancy rates in the interventional treatment hospital and the expectant treatment hospital were 75.71% (53/70) and 94.74% (54/57), respectively, with a significant difference ( P<0.05). The ongoing pregnancy rate after interventional treatment in the interventional treatment hospital was 82.76% (24/29), which was similar to the 94.00% (47/50) after expectant treatment in the expectant treatment hospital ( P>0.05). Conclusion:The ongoing pregnancy rate of interventional treatment for URSA patients has not been significantly improved, suggesting that it may not be necessary to carry out this treatment.

5.Visual analysis of hotspots in prolonged grief disorder research based on Web of Science Core Collection database

Objective:To explore the current research status and hotspots of Prolonged Grief Disorder (PGD), and to provide a reference for conducting PGD-related studies in China.Methods:A literature search was conducted using the Web of Science Core Collection database, covering publications related to PGD from its inception to July 24, 2024. CiteSpace 6.1.R4 and VOSviewer 1.6.19 were used to perform visual analyses of publication countries, institutions, authors, journals, and keywords.Results:A total of 1996-2024 publications showed a fluctuating upward trend in the number of PGD-related studies. The United States was the country with the highest number of publications. Utrecht University had the highest institutional output, and Boelen, Paul A. was the most prolific author. The journal Death Studies had the largest number of PGD-related publications. Keyword clustering revealed that research participants primarily included caregivers of patients receiving palliative care and bereaved children. The relationship between PGD and other psychiatric disorders, the development of PGD assessment tools, and the mechanisms of PGD were identified as research hotspots. Keyword burst detection indicated that psychological interventions for PGD represent the research frontier. Conclusions:Future research should focus on the development of localized assessment tools and intervention programs for PGD, improvement of relevant policies, and the construction of grief care education systems grounded in Chinese cultural context.

9.Visualization of research hotspots in forensic mental health nursing based on Web of Science database

Objective:To analyze research hotspots and trends in forensic mental health nursing internationally.Methods:The Web of Science Core Collection Database was searched for research on forensic mental health nursing, with the search period covering the database's inception to December 31, 2023. CiteSpace 6.1.R4 and VOSviewer 1.6.19 were used to visually analyze research hotspots and trends in forensic mental health nursing.Results:From 1992 to 2023, the volume of publications on forensic mental health nursing steadily increased, with the highest number of publications in 2022 (155 papers). The United Kingdom had the most publications (455 papers), and King's College London was the leading institution (50 papers). The Journal of Forensic Psychiatry was the most frequently published (120 papers). Research hotspots in forensic mental health nursing included forensic mental health care for trauma victims, forensic mental health rehabilitation and forensic mental health nursing management, risk assessment in forensic mental health care, and forensic mental health nursing for schizophrenia. Trauma care for forensic mental health patients became a new research trend. Conclusions:Interest in forensic mental health nursing research is rising internationally. Domestic research can benefit from these trends to promote the development of forensic mental health.
10.Mediating effect of compassion fatigue between organizational support and therapeutic relationships in psychiatric nurses

Objective:To investigate the current status of therapeutic relationships, organizational support, and compassion fatigue among psychiatric nurses, and to analyze the mechanism of action between these variables.Methods:This was a cross-sectional study. From August to September 2023, totally 310 psychiatric nurses were selected from three ClassⅢ Grade A psychiatric hospitals in Qingdao, Jinan, and Linyi by convenience sampling. Data were collected using a general information questionnaire, the Therapeutic Relationship Assessment Scale-Nurse (TRAS-N), the Perceived Organizational Support Scale (POSS), and the Chinese version of Compassion Fatigue short Scale (CCFS). SPSS 22.0 and the Process macro plugin were used for data analysis.Results:A total of 310 questionnaires were distributed, with 282 valid responses, yielding a response rate of 90.97%. The total score for TRAS-N among the 282 psychiatric nurses was (95.59±18.75), the total score for CCFS was 45.50 (24.75, 68.25), and the total score for POSS was (43.66±13.34). Therapeutic relationships were positively correlated with organizational support and negatively correlated with compassion fatigue, while organizational support was negatively correlated with compassion fatigue (all P< 0.01). Organizational support had a direct positive effect on therapeutic relationships (β=0.291, P<0.01) and also had an indirect effect through the mediating effect of compassion fatigue (β=0.212, P<0.01), with the mediation effect accounting for 42.15% of the total effect. Conclusions:Compassion fatigue partially mediates the relationship between organizational support and therapeutic relationships in psychiatric nurses. Nursing managers should enhance organizational support for psychiatric nurses to reduce their levels of compassion fatigue, thereby improving therapeutic relationships.
