To systematically evaluate the impact of the "group-based " medical assistance program for Xizang, conducted by Peking Union Medical College Hospital, on the implementation outcomes of standardized residency training (SRT) at the Xizang Autonomous Region People′s Hospital, and to provide evidence-based decision-making references and policy optimization recommendations for medical education support models in border ethnic minority regions. Using Donabedian′s quality-of-care theory as the analytical framework, a systematic evaluation was conducted across three dimensions—structure, process, and outcomes. Qualitative research methods and work documentation analysis were comprehensively employed to assess the key interventions, implementation processes, and training outcomes of the "group-based " medical assistance program, centered on the core strategy of "full-time secondment and backbone downward deployment, " implemented by Peking Union Medical College Hospital during the 2025—2026 period at the Xizang Autonomous Region People′s Hospital. Through the core strategy of "full-time secondment and backbone downward deployment, " systematic safeguards for SRT institutional development were achieved, effectively promoting the standardization of teaching activities and the establishment of a multidimensional, full-coverage faculty training system. Concurrently, the establishment of a three-tier teaching supervision mechanism further consolidated the connotation and quality of the SRT program. These measures have played a positive role in enhancing the SRT system construction, improving trainees′ clinical competence, and strengthening faculty teaching capabilities at the Xizang Autonomous Region People′s Hospital. Nevertheless, future SRT implementation in the Xizang region still requires further refinement in precision management, with curriculum design and rotation scheduling more adequately tailored to local circumstances, so as to continuously promote the long-term sustainable development of SRT quality. The "group-based" medical assistance program for Xizang, underpinned by the strategy of "full-time secondment and backbone downward deployment, " has demonstrated a significant facilitating effect on improving SRT quality in the Xizang region. The experiences and outcomes derived from this program may provide a valuable research foundation and policy evidence for future medical education support initiatives in other border ethnic minority regions, including Xinjiang and Xizang.

AIM: To delineate the specific mutation responsible for retinitis pigmentosa(RP)in a Yi pedigree, and to analyze the correlation of RHO gene mutation with clinical phenotype.METHODS:A comprehensive clinical evaluation was conducted on the proband diagnosed with RP and other familial members, complemented by a thorough ophthalmic examination. Peripheral blood samples were obtained from the proband and familial members, from which genomic DNA was extracte. Subsequent whole exome sequencing(WES)was employed to identify the variant genes in the proband. The identified variant gene was validated through Sanger sequencing, then an in-depth analysis of the mutation genes was carried out using genetic databases to ascertain the pathogenic mutation sites. Furthermore, an exhaustive analysis was performed to delineate the genotype and phenotype characteristics.RESULTS:The RP pedigree encompasses 5 generations with 42 members, including 19 males and 23 females. A total of 13 cases of RP were identified, consisting of 4 males and 9 females, which conforms to the autosomal dominant inheritance pattern. The clinical features of this family include an early onset age, rapid progression, and a more severe condition. The patients were found to have night blindness around 6 years old, representing the earliest reported case of night blindness in RP families. The retina was manifested by progressive osteocytoid pigmentation of the fundus, a reduced visual field, and significantly decreased or even vanished a and b amplitudes of ERG. The combined results of WES and Sanger sequencing indicated that the proband had a heterozygous missense mutation of the RHO gene c.1040C>T:p.P347L, where the 1 040 base C of cDNA was replaced by T, causing codon 347 to encode leucine instead of proline. Interestingly, this mutation has not been reported in the Chinese population.CONCLUSION:This study confirmed that the mutant gene of RP in a Yi nationality pedigree was RHO(c.1040C>T). This variant leads to the change of codon 347 from encoding proline to encoding leucine, resulting in a severe clinical phenotype among family members. This study provides a certain molecular, clinical, and genetic basis for genetic counseling and gene diagnosis of RHO.

ObjectiveTo evaluate the efficacy of Shuanghua drink in treating primary dysmenorrhea in the rat model and explore its mechanism of action. MethodsAn oxytocin-induced writhing mouse model was established to evaluate the analgesic effect of Shuanghua drink. Forty-eight non-pregnant female institute of cancer research (ICR) mice were randomly divided into six groups, including a blank group, a model group, an ibuprofen group (85.00 mg·kg-1), a low-dose group of Shuanghua drink (7.14 mL·kg-1), a medium-dose group of Shuanghua drink (14.28 mL·kg-1), and a high-dose group of Shuanghua drink (28.57 mL·kg-1). Each group consisted of eight mice. All treatment groups received daily intragastric administration at corresponding doses for 10 consecutive days. One hour after the final administration, 2 U of oxytocin was intraperitoneally injected per mouse. The writhing latency and number of writhing within 20 minutes were recorded. A primary dysmenorrhea rat model was established by using estradiol benzoate and oxytocin to evaluate the inhibitory effect of Shuanghua drink on the contraction of uterine smooth muscle. Forty-eight non-pregnant female Sprague-Dawley (SD) rats were divided into six groups, including a blank group, a model group, an ibuprofen group (51.00 mg·kg-1), a low-dose group of Shuanghua drink (4.28 mL·kg-1), a medium-dose group of Shuanghua drink (8.57 mL·kg-1), and a high-dose group of Shuanghua drink (17.10 mL·kg-1). Each group consisted of eight rats. Rats received subcutaneous injections of estradiol benzoate for 10 consecutive days to enhance uterine sensitivity. On the eleventh day, oxytocin (2 U/rat) was intraperitoneally administered to induce abnormal uterine contractions for establishing the primary dysmenorrhea model. All treatment groups received daily intragastric administration from the second day of modeling for 10 days. The effects of Shuanghua drink were evaluated by using parameters including uterine motility and the variation rate of uterine motility. The mechanism of action was investigated in rats with primary dysmenorrhea. The content of prostaglandin F2α (PGF2α), prostaglandin E2 (PGE2), thromboxane B2 (TXB2), prostacyclin metabolite (6-keto-PGF1α), and β-endorphin (β-EP) in uterine tissue of rats was detected by using enzyme-linked immunosorbent assay (ELISA). The changes in the content of nitric oxide (NO) and inducible nitric oxide synthase (iNOS) were analyzed via colorimetric assay. Western blot was performed to determine the content of phosphorylated inhibitor of kappa B kinase beta (p-IKKβ)/IKKβ, phosphorylated inhibitor of kappa B alpha (p-IκBα), IκBα, phosphorylated p65 (p-p65), p65, and cyclooxygenase-2 (COX-2) proteins in uterine tissue of rats. ResultsIn the oxytocin-induced writhing mouse model, the model group exhibited significantly shortened writhing latency and increased writhing frequency compared to the control group (P<0.01). Both the ibuprofen group and the high-dose group of Shuanghua drink displayed prolonged writhing latency (P<0.05), while the ibuprofen group and the low-dose, medium-dose, and high-dose groups of Shuanghua drink exhibited reduced writhing frequency (P<0.01). In the primary dysmenorrhea rat model, the uterine motility and its variation rate in the model group were significantly higher than those in the blank group (P<0.01). These parameters were markedly suppressed by ibuprofen and Shuanghua drink at all tested doses (P<0.01). For the mechanism of action, the model group showed significantly increased PGF2α/PGE2, TXB2/6-keto-PGF1α, NO, and iNOS in uterine tissue (P<0.05, P<0.01) and significantly decreased β-EP (P<0.01). These parameters were significantly attenuated in the ibuprofen group and the low-dose, medium-dose, and high-dose groups of Shuanghua drink. The PGF2α/PGE2 (P<0.01), TXB2/6-keto-PGF1α (P<0.01), NO (medium-dose group P<0.05), and iNOS (P<0.01) were reduced, and the β-EP (medium-dose group P<0.05) was up-regulated. Compared to the model group, the ibuprofen group and medium-dose group of Shuanghua drink showed significantly increased content of β-EP in the serum of rats (P<0.05). Compared to the blank group, the model group showed significantly elevated expressions of COX-2, p-IKKβ/IKKβ, p-IκBα/IκBα, and p-p65/p65 proteins (P<0.01) and significantly reduced anti-inflammatory protein IκBα (P<0.05). Compared to the model group, the ibuprofen group and the low-dose, medium-dose, and high-dose groups of Shuanghua drink showed significantly reduced expressions of COX-2 (P<0.01), p-IKKβ/IKKβ (P<0.01), p-IκBα/IκBα (P<0.05, P<0.01), and p-p65/p65(P<0.01) and up-regulated expression of IκBα protein (P<0.05, P<0.01). ConclusionShuanghua drink effectively alleviates primary dysmenorrhea through analgesia and suppression of abnormal contractions of uterine smooth muscle. Its mechanism may be mediated by reduced levels of PGF2α/PGE2, TXB2/6-keto-PGF1α, iNOS, and NO, elevated β-EP level, and inhibited COX-2/NF-κB signaling pathway.

Objective:To explore the relapse-related genes and their clinicopathological connections of diffuse large B cell lymphoma (DLBCL).Methods:Targeted panel sequencing was conducted on 32 eligible DLBCL samples; the patients were diagnosed, treated, and went into complete remission at the First Affiliated Hospital of Nanjing Medical University from January 2015 to December 2019, including 14 cases with recurrence (relapsed group) and 18 cases with long-term complete remission of over five years (remission group). Clinical and pathological data were further reviewed. Fisher′s exact test was employed to compare the differences in clinicopathological characteristics and mutation patterns between the two groups. Results:Among the 32 patients, there were 18 males and 14 females, with a male to female ratio of 1.3∶1.0 and a median age of 53 (45.5, 67.0) years. In the relapsed group, PIM1 (11/14), KMT2D (7/14), PRDM1 (6/14), MYD88 (6/14), DTX1 (6/14) emerged as the most frequently mutated genes. In the remission group, while recurrent PIM1, KMT2D and MYD88 mutations were also observed, the TP53 gene exhibited the highest mutation frequency (6/18). Compared to the remission group, relapsed group showed elevated mutation frequencies of PIM1 ( P=0.013) and FAT4 ( P=0.010), alongside a reduced incidence of TP53 mutations. In all 32 patients, DLBCL with CD79B, CCND3, DTX1, KMT2D and PRDM1 mutations demonstrated a propensity towards advanced clinicopathologic stage. Conclusions:Relapsed DLBCL has distinctive clinicopathological and genetic features. PIM1 and FAT4 may be served as potential biomarkers for screening relapsed DLBCL-NOS and as targets for novel therapeutic strategies.

Objective:To explore the influencing factors of the subjective well-being of elderly individuals and improve the accuracy of well-being predictions. Methods:A multi-stage stratified sampling method was conducted in Guangdong province and Guangxi Zhuang autonomous region from August 2019 to October 2020. The health status and subjective well-being of older adults were assessed using a self-administered multidimensional health questionnaire, while overall well-being was evaluated with a single-item 5-point scale. The statistical analysis was performed using R software (version 4.3.2), with analytical methods including χ2 tests, univariate analysis, quantile regression and random forest model. Results:A total of 2 620 participants were recruited, and 70.9% of whom reported feeling happy. The depression symptom score of the elderly was 3.00(1.00, 6.00), emotional status score was (28.26±4.14), activities of daily living (ADL) score was 69.00(52.00, 81.00), and the body mass index(BMI) was (22.68±3.81)kg/m 2.The quantile regression results indicated that marital status (married)(Q 10: β=0.049; Q 50: β=0.014; Q 90: β=0.005) and depressive symptoms (no depression, depressive tendency)(Q 10: β=1.000; Q 50: β=1.000; Q 90: β=0.025) had varying effects on older adults' subjective well-being across three quantiles (Q 10, Q 50, Q 90)(all P<0.05). In contrast, social activity (no social activity)(Q 10: β=-0.072; Q 50: β=-0.011; Q 90: β=-0.006), ADL (severe disability, moderate disability, mild disability)(Q 10: β=-0.103, Q 50: β=-0.006, Q 90: β=-0.048; Q 10: β=-0.063, Q 50: β=-0.012, Q 90: β=-0.005; Q 10: β=-0.078, Q 50: β=-0.016, Q 90: β=-0.009), and emotional status (poor emotion, fair emotion)(Q 10: β=-0.261, Q 50: β=-0.009, Q 90: β=-0.025; Q 10: β=-0.140, Q 50: β=-0.017, Q 90: β=-0.029) had a negative impact on elderly subjective well-being at the three quantile points(all P<0.05). Random forest results indicated that depressive symptoms, ADL, emotional status, household monthly income per capita, BMI, social activity, multimorbidity status, and marital status were the most important determinants of subjective well-being. Conclusion:Depressive symptoms and emotional status have a significant impact on the subjective well-being of older adults. Additionally, ADL, household income per capita, BMI, and social engagement also play important roles in influencing well-being, while the emotional support provided by marriage should not be overlooked.
